CardioNerds (Dr. Jessie Holtzman, Chair for the CardioNerds Women's Heart Disease Committee, and Dr. Naima Maqsood, Chair for the CardioNerds Electrophysiology Committee) join Dr. Ritika Gadodia, Dr. Namratha Meda, and Dr. Tsion Aberra from the Medstar Washington Hospital Center/Georgetown University Program for the National Cherry Blossom Festival. They discuss involving a patient with Chagas cardiomyopathy. Dr. Rachel Marcus provides the Expert CardioNerd Perspectives & Review segment for this episode. Episode audio was edited by Dr. Diane Masket. A 79-year-old male with a history of cardiomyopathy presented with recurrent ventricular tachycardia (VT) post-CRT-D placement. On arrival, the patient was in cardiogenic shock. Initial treatment with amiodarone and milrinone failed, necessitating the addition of mexiletine. Imaging was suggestive of a left ventricular ejection fraction of 20-25% with severe global hypokinesis. Prior coronary angiogram had shown nonobstructive coronary artery disease. Further non-ischemic cardiomyopathy evaluation was unrevealing. Given his El Salvadorian origins, Chagas serology results revealed Chronic Chagas Cardiomyopathy (CCM) confirmed by CDC testing. This case underscores the importance of suspecting CCM in patients with risk factors. An early diagnosis of CCM, can prevent catastrophic events (heart blocks, ventricular arrhythmias, thromboembolic events). In summary, this case takes the learner through the journey of a patient with non-ischemic cardiomyopathy and emphasizes the importance of approaching it with a wide range of differentials. Case Media Pearls - Chronic Chagas Cardiomyopathy with Recurrent Ventricular Tachyarrhythmia Always consider Chagas cardiomyopathy when you have a patient from Latin America who presents with non-ischemic cardiomyopathy. Chagas cardiomyopathy is associated with an unfavorable prognosis and serves as an independent predictor of mortality. Chagas cardiomyopathy is arrhythmogenic and requires consideration for ICD and, when appropriate, catheter based ventricular tachycardia ablation. It is crucial to treat patients with nifurtimox and benznidazole when appropriate. Provide screening for first-degree family members or close relatives who may have lived in the same environment. Show Notes - Chronic Chagas Cardiomyopathy with Recurrent Ventricular Tachyarrhythmia What is the disease progression in Chagas disease5? Acute Stage:Initial infection occurs through contact with infected triatomine bug feces or contaminated blood products.Symptoms may be mild or absent but can include fever, fatigue, body aches, and swelling at the injection site (chagoma). Parasitemia is high during this stage. Intermediate/Indeterminate Stage:The infection becomes chronic if left untreated. Many individuals enter this stage with no noticeable symptoms.Parasitemia levels decrease, but the parasite remains in the body, mainly in muscle and cardiac tissue. This stage can last for years to decades. Chronic Stage:Some individuals will remain asymptomatic throughout their lives.Cardiac complications (chronic Chagas cardiomyopathy) can lead to arrhythmias, congestive heart failure, and sudden death. Digestive complications can result in enlarged esophagus (megaesophagus) and colon (megacolon), leading to difficulties in swallowing and digestion. When do we suspect, and who do we screen, for Chagas disease? The seroprevalence of CCM in the USA is as high as 19%16. Among patients with LVEF

Thousands of Japanese cherry trees are blooming on the National Mall in Washington, D.C. When the trees reach peak bloom, the United States' capital city is awash in pink and white. In the 1920s, the average peak bloom date was April 5. That moved up to March 31 in recent years. And the 2023 peak arrived yet a week earlier, the National Park Service said last week. “I'm feeling like this is going to be the trend” because of climate change, said Matthew Morrison. He is the park service's urban forester charged with overseeing the trees' care. The yearly National Cherry Blossom Festival will run through April 16 this year. It celebrates Japan's 1912 gift of 3,020 cherry trees to the city of Washington. Two of those trees were planted by First Lady Helen Herron Taft and Viscountess Chinda, wife of the Japanese Ambassador to the United States, with just a few onlookers present. The average life of a Japanese cherry tree is between 30 and 40 years. Yet the two trees planted by Taft and Chinda and several others are still standing 111 years later. “That defies science,” said Morrison. He credited the tree's long life to “a little bit of magic” and the year-round care his team provides. Many of the 3,700 trees blooming at the park today come from cuttings from the first trees, Morrison said. Others are regularly donated by the Casey Trees group of Washington. Morrison oversees a team of just three arborists. They are charged with caring for the cherry trees and keeping a “personal health record” for each. Three years ago, Morrison introduced the practice of mulching trees with large amounts of wood chips to help their growth. As the chips break down, he said, they release fungi and bacteria to help the tree's roots. The wood chips also serve as a barrier between the grass and the trees. This prevents accidental cuts caused by lawn equipment. Morrison suggests the same practice for homeowners growing trees in their own yards. When wood chips are applied over the root areas, he said, “it would never need any fertilizer, amendments” or more water. “Even on the hottest days of summer when it hasn't rained, I dig down in the wood chips and find moisture,” he said. Morrison said most of the National Mall's cherry blossom trees are Yoshino. “It's one of the best growers and the best to flower,” he said. “Kwanzan cherry also is a good grower and gets a little bigger than Yoshino.” As for the National Cherry Blossom Festival, there are still several events throughout Washington and its suburbs. They include a Japanese street festival, running events, fireworks, and food festivals. If you cannot make it, there is a Bloom Cam to look at the cherry blossoms.

Public relations is not only practiced with news releases and press conferences. More than a century ago, Japan delivered a powerful message of friendship with a gift of cherry trees that today continues to speak to millions of spring-time visitors to Washington, D.C.’s Tidal Basin. With 40 events and a world fixed on the annual arrival of the cherry blooms, the non-profit National Cherry Blossom Festival communications team has its hands full all year long. Krystle Fleetwood and Nora Strumpf are about to watch their hard work come to life, starting with a kick-off media event on March 6th. But they still made time to talk with Robert about handling the PR for one of America’s most treasured springtime routines. Diana Mayhew on the National Cherry Blossom Festival ~ ". . .the festival's first and foremost the Washington D.C. springtime celebration that makes Washington the destination to come in Spring, but we always remember the roots and we celebrate the culture of Japan, because that's where it all started." Diana Mayhew - President, National Cherry Blossom Festival A Ockershausen: This is Andy Ockershausen and this is Our Town. I have the distinct, wonderful pleasure to talk to an old friend. This is not only the busiest time of the year, but inviting her into the studio right now is like kidnapping Santa Clause at Christmas Eve, but she's here with us for Our Town. Over the past 20 years, she's grown the Cherry Blossom Festival into an internationally recognized extravaganza, generating over 100 million dollars for Our Town. She's usually wearing pink in honor of her role as one of the most important people in the city of the Spring. Please welcome Diana Mayhew, Director of Events at Downtown BID, and President of the National Cherry Blossom Festival. Diana, welcome to Our Town. Diana Mayhew: Hi, thank you, nice to see you again. A Ockershausen: We go back a long way, long before there were cherry blossoms, there was you. All of the things you have done ... one of the things you are probably too young to know or to remember, but WMAL and WMAL TV, we were involved in the Cherry Blossom Festival up to our necks. A couple of the young people here were just asking me stories about what did we do, and I said, "Well, we used boats then. We televised the parade on Channel 7. We did a lot in radio and our talent was always involved, Harden and Weaver, and Bill Mayhugh, Tom Gauger. We did shows with the Cherry Blossom Festival." But you have put the Cherry Blossom Festival at a new level, Diane. I know that because I've lived through it with you. Diana Mayhew: Oh, thank you. A Ockershausen: How's it been? It's just been such a great experience to know all about what has happened, where they came from, how they started. I mean, the cherry blossom story, I think, is fabulous. How Diana Mayhew Came to Lead National Cherry Blossom Festival Diana Mayhew: My background is hospitality industry here in Washington. A Ockershausen: This is your home town, correct? Diana Mayhew: Yes, I love- A Ockershausen: It's your hometown. Diana Mayhew: I love Washington D.C., I love showcasing it and just really this National Cherry Blossom Festival had the opportunity many years ago when it was run all by volunteers, just 16 years ago, to really get upgraded, to have somebody pay attention to it and not thinking I would be sitting here. I was trying to be a proponent out there for the different groups in Washington to take it under its ... A Ockershausen: It became bigger than anybody thought it would be, Diane, but it was real big at one time and then it went down. I remember going to the dinners and we would bring talent in as an expense. The president of our company then was deeply involved with the convention and visitors bureau, board of trade, which essentially expired. But we'd bring in talent from Hollywood to participate in the festival and the parade, and then it slopped off until you brought it back. Diana Mayhew: Well, thank you. A Ockershausen: Which you did, Diana. Diana Mayhew: It wasn't just me by myself. Obviously it was a vision of several organizations in the city. The Downtown Business Improvement District actually was the one 16 years ago who said, "Let's go ahead and fund an executive director for three years." They donated the salary of an executive director for three years to start it ... getting strong again. There's so many volunteers over the years that have kept this festival together. Back in the days that you know, very, very strong, but if the volunteers sort of went away- A Ockershausen: Changed. Diana Mayhew: If they changed, there's no consistency, and that's what makes it a little weaker than we...

Read the full article with photos at: https://www.otsuka.co.jp/en/company/globalnews/detail.php?id=217&date=2015-04-05 Otsuka is proud to be the lead supporter of the National Conference of State Societies’ (NCSS) 67th Cherry Blossom Festival events, including their flagship Cherry Blossom Princess Program. Otsuka has pledged its recurring support to the NCSS Cherry Blossom Events for three consecutive years, to demonstrate the company’s commitment to empowering young women to aspire to accomplish their goals and become the leaders of the next generation. The National Cherry Blossom Festival is held in Washington, DC each spring, commemorating the gift of the cherry blossom trees, which symbolizes the friendship between the peoples of the United States and Japan. The cherry trees were first gifted to Washington, DC in 1912 from Mayor Yukio Ozaki of Tokyo. 2015 marks the 100th anniversary of the United States’ reciprocal gift of flowering dogwood trees to the people of Japan. For Otsuka, as a Japanese company with operations in the United States, supporting an event that commemorates this enduring friendship is especially appropriate. William M. Christian, the President of NCSS complimented Otsuka, noting, “NCSS is proud to have continuously administered the Cherry Blossom Princess program, including the US Cherry Blossom Queen, since 1948. We are particularly grateful for the support of our chief sponsor, Otsuka America Pharmaceuticals, Inc. With our shared ties to the Japanese culture, Otsuka is a natural partner for the NCSS, and this collaboration portends the continued success of our iconic program of empowering young women and promoting cultural exchange." The Cherry Blossom Princess Program has been held annually since 1948, with the mission of “Aspiring Leadership and Empowering Young Women.” The program offers young women from across the US and around the world the opportunity to participate in community outreach, personal development, educational, leadership, cultural and networking activities. Otsuka’s support to the NCSS’ Cherry Blossom Festival events inspires the many activities for the Princesses, which promote personal growth and foster leadership. Former Cherry Blossom Princesses include US Senator Lisa Murkowski of Alaska, US Senator Shelley Moore Caputo of West Virginia, and Lynda Johnson Robb, the former First Lady of Virginia and daughter of former US President Lyndon B. Johnson. These accomplished women are a testament to the program’s success in encouraging young women to achieve the highest caliber of success.
