Episode Summary:In this episode of the Modern Pain Podcast, host Mark Kargela is joined by Brett Neilson, a physical therapist and professor at Hawaii Pacific University. Recorded live at the Pain Science in Motion conference in Las Vegas, the episode delves into the crucial intersection between sleep and chronic pain management. Brett shares insights from his scoping review on the integration of sleep measures in chronic low back pain research, highlighting a significant gap in current practices. The conversation covers the impact of sleep on pain, the role of wearables, and practical advice for clinicians on how to incorporate sleep assessments and interventions into their practice. Brett emphasizes the importance of understanding individual sleep needs and using tools like the CBTI Coach app to help patients improve their sleep quality.CBT-i Coach App Scoping Review TIMESTAMPS:00:00:00 Episode begins00:01:43 Interview Start00:02:53 Brett's Work on Sleep00:05:04 What did they find in their systematic reveiw?00:08:25 Clinical pearls gained in pre-conference course00:10:43 What apps or tools are out there for clinicians?00:13:46 Can tracking sleep excessively become anxiety generating?00:14:57 How do we determine the right amount of sleep for the individual?00:19:47 Mindulness or meditation for sleep00:23:12 How do we get this in curriculum?00:26:20 How do we get this as every day practice?00:31:59 Sleep as a part of the big picture in painKey Takeaways:Sleep and Chronic Pain Connection: There is a strong link between poor sleep and chronic pain. Addressing sleep issues can potentially alleviate pain symptoms.Research Findings: A scoping review revealed that less than 10% of chronic low back pain trials include sleep measures, highlighting an area for improvement in research practices.Role of Wearables: While wearables can provide useful sleep data, they should be used cautiously to avoid causing anxiety. It's best to view sleep data in the context of long-term trends rather than daily fluctuations.Screening and Interventions: Simple questions about sleep quality can help identify patients who may benefit from further sleep assessment. Tools like the CBTI Coach app, developed by the VA, can assist clinicians in guiding patients through cognitive behavioral therapy for insomnia.Individualized Approach: Quality of sleep is more important than quantity. Clinicians should tailor their recommendations based on the individual needs and experiences of their patients.Practical Tips for Clinicians: Incorporate basic sleep questions into patient assessments, use validated sleep measures, and educate patients on sleep hygiene practices. Encourage patients to find relaxation strategies that work for them, whether it's mindfulness, reading, or other calming activities.Educational Integration: Teaching about sleep and its impact on health should be integrated into PT education to better prepare future clinicians.*********************************************************************

Wisdom realizing emptiness is the realization that leads directly to enlightenment. This wisdom is also the direct remedy for all the misguided notions we harbor about our identity and the true nature of the world. As long as we carry the weight of these misconceptions, we find ourselves ensnared within a realm of self-imposed illusions, doomed to endlessly wander in the cycle of discontent we have forged. The Buddhist concept of "empty of inherent existence" is a fundamental teaching that all phenomena, including ourselves and the world around us, lack inherent or independent existence. This means that nothing possesses an essential, fixed, or unchanging nature. According to Buddhism, everything is interdependent and arises due to various causes and conditions. Objects, people, and experiences are not isolated entities with inherent qualities, but rather temporary and contingent manifestations. They are composed of multiple parts and are influenced by countless factors. The concept of emptiness challenges our ordinary way of perceiving and conceptualizing reality. It invites us to recognize the illusory nature of inherent existence and to transcend our attachment to fixed ideas and identities. By understanding that things lack inherent existence, we can cultivate wisdom, alleviate suffering, and develop a more accurate and compassionate view of the world “Not coincidentally, he had gone from an inner-hell-realm experience to that of a pure realm (the Tibetan equivalent of heaven), even though his outer circumstances hadn't changed. The Buddha taught that to live from our small sense of self (ego, meaning “I” in Latin) is a recipe for misery. To live with a vast sense of Self that includes all sentient beings is a recipe for happiness.” Anyone who doesn't cherish as “mine” Anything of body-and-mind And who doesn't grieve for that which doesn't exist, Is indeed called a bhikkhu. In this episode, we turn back the clock 170 years, and revisit what is perhaps THE great foundational text of magick of the 19th and 20th century - Transcendental Magick, by the French magus Éliphas Lévi. Éliphas Lévi Zahed, born Alphonse Louis Constant (8 February 1810 – 31 May 1875), was a French esotericist, poet, and author of more than 20 books on magic, Kabbalah, alchemical studies, and occultism. He pursued an ecclesiastical career in the Catholic Church until, after great personal struggle, at the age of 26, he abandoned the Roman Catholic priesthood. At the age of 40, he began professing knowledge of the occult, and becoming a reputed ceremonial magician. The pen name "Éliphas Lévi", was a transliteration of his given names "Alphonse Louis" into the Hebrew. Levi gained renown as an original thinker and writer. His works attracted the attention of the heterogeneous ambiences of the era in Paris and London; from esotericists to artists of romantic or symbolist inspiration. He also expressed his independence by leaving the Masonic lodge of the "Great Orient", believing that it was a form of modern secularization, where knowledge of the original meanings of symbols and rituals was lost. "I ceased being a freemason, at once, because the Freemasons, excommunicated by the Pope, did not believe in tolerating Catholicism." Many authors influenced Levi's political, occultic and literary development, such as the French monarchist Joseph de Maistre, whom he quotes in many parts of his Dogme et Rituel de la Haute Magie, Paracelsus, Robert Fludd, Swedenborg, Fabre d'Olivet, the Rosicrucians, Plato, Raymond Lull, and other esoterics. Mindfulness is a type of meditation in which you focus on being intensely aware of what you're sensing and feeling in the moment, without interpretation or judgment. Mindfulness isn't something that only certain people can use. It is something that everyone should use and incorporate into their life on a regular basis. With mindfulness it helps to ensure your less distracted and improves your overall focus. It minimizes stress and helps you become your best self. Mindfulness is the energy that helps us recognize the conditions of happiness that are already present in our lives. Mindfulness doesn't have to be overwhelming or boring. It can actually be something that we incorporate in every area of our life which in turn can change our life. In this episode, I share how you can add more mindfulness into your daily actions with ease and excitement.

What is mindfulness?· It’s a state of active, and open attention to the present. · Observing one’s thoughts and feelings without judging them as good or bad. · To live in the moment and reawaken to the present, rather than dwelling on the past or looking forward to the future. Basically being “in the moment.” Ways to practice mindfulness: · Meditation· Listening wholeheartedly· Observing your breathing, eating mindfully, walking mindfully, showering mindfully · Get lost in the flow of doing things you love · Setting an amount of time without devices/electronics, etc. Benefits of mindfulness · Lowers stress, bolsters immune system· Restores emotional balance, reduces anxiety, reduces depression· Increases resilience· Reduces physical pain, reduces fatigue · Improves sleep quality· Improves concentration, uncovers your own blind spots · “Among its many benefits, mindfulness meditation has actually been proven to increase telomerase, the ‘caps’ at the end of our genes, which, in turn, can reduce cell damage and lengthen our lives.” (Firestone, Lisa “Benefits of Mindulness.”) Tips for Listeners for Improving Mindfulness: · Take a minute to quiet your mind, if it’s not working, you need to practice some of the methods listed above. o Keep a mindfulness log to track progress (can add it to your gratitude journal as well) · Try focusing on one thing at a time, or slowing down (put the devices down) · Meditate, or just take 5 minutes to sit quietly and pay attention to your breathing· Spend time in nature· For work: Try following the 20/20/20 rule. o Every 20 minutes, walk 20 feet away from your desk, for 20 seconds§ If you can’t do every 20 minutes, then do it for 60 minutes and walk 20-60 feet away from your desk for 60 seconds. § Take time to stretch out during this time, this will also help your muscles and bones recover from sitting all day. Dr. Edward Creagan, Professor of Medical Oncology, Mayo Clinic College of Medicine, John and Roma Rouse, Professor of humanism, Palliative care consultant, Past President of Mayo Clinic faculty and distinguished Mayo Clinic's Highest Honor will discuss Grit, Resilience and Survivors. Did you ever wonder how someone survived a monumental challenge or life threatening disease? Was it in their DNA or some other ingredient? Was it courage, their attitude, or their inner faith? Did that person focus in on the rainbow beyond the clouds? Did you know that the Beatles were turned down by a record label because they had a guitar band? What happened to Michael Jordan? Did you know that JK Rowling was a single mom and destitute? Now she is one of the wealthiest women in the United Kingdom. What did all of these superstars have? Who was Angela Duckworth? She was an inner city school teacher who encouraged children despite their diverse adversity. Dr. Creagan tells host Carol Olivia that all of these mentioned people had gritty defiance or gritty determination. Is it the inner drive of a person or is it a spiritual inner voice, or an angel? Why do they go the extra mile? They had no fear of rejection or intimidation. Children learned from Angela Duckworth that the quality of nurturing was their key motivation to achieve their goals. Dr. Creagan stresses that it doesn't matter if it's a genetic member of a family that nurtures the child. It could be a school guard, a grandmother, or anyone that enters that child's life that supplies that child with a good heart. It is the belief system that works. Remember 'I have a Dream' by Martin Luther King? What does AA and Al Anon have in common? How did AA start? Was it divine inspiration? They both validate the importance of inner faith. Dr. Creagan believes that suffering souls look for a life preserver. Listen to his stories on The Matter of Heart podcast and travel on your road much more lightly.
