In this episode, we speak with Dr. Eneida Nemecek, a pediatric oncologist specializing in bone marrow transplants and cellular therapies. Dr. Nemecek provides an in-depth look at the science behind these treatments, the process patients go through, and the challenges faced by both patients and caregivers. She emphasizes the importance of caregiver well-being, particularly sleep, and discusses a PCORI study on stress management for caregivers. The episode concludes with a rapid-fire Q&A session covering key terms and concepts in the field. This episode was supported by the Patient Centered Outcomes Research Institute (PCORI) and features these PCORI studies (Study #1 & Study #2) by Mark Laudenslager, PhD. Key Highlights: 1. Bone marrow transplant involves replacing a patient's immune system with a healthier one, either from a donor or the patient's own modified cells. 2. Caregiver quality of life, especially sleep, is crucial for patient outcomes in bone marrow transplant cases. 3. CAR-T therapy is a form of cellular therapy where white blood cells are engineered to attack specific targets, offering new treatment options for certain cancers. About our guest: Dr. Eneida Nemecek is a Professor of Pediatrics and Medical Oncology and Associate Director of Clinical Research at the Knight Cancer Institute-Oregon Health & Science University (OHSU) in Portland, Oregon. Native from Puerto Rico, she completed her Pediatric residency at Case Western Reserve University in Cleveland, OH and Pediatric Hematology/Oncology fellowship at the Fred Hutchinson Cancer Research Center in Seattle, Washington. She has also held elected leadership positions as director, trustee or committee chair in multiple professional organizations. The Your Cancer GPS™ platform is coming! Step-by-step subway maps that guide you through the entire cancer experience. Learn more here! Key Moments: At 14:40 “Sleep is healthy. If you are the one caregiver of a patient and you get sick because you're stressed and not getting enough rest, then you get affected and your patient gets affected too. It's a very important part of our health. Eat, sleep, get some time for yourself. Those are all things that just need to happen. The way that we're designing medicine today, sometimes we forget that the caregiver is kind of a patient. If we lose that person, we are in serious trouble. I can tell you multiple examples about when that has happened and how difficult it is for the medical team and for the family.” At 28:03 “I think it's really important to remind ourselves that the brain, our psychosocial life, is part of our health. I think sleep, exercise, diet studies, anything that can improve the life of people should be studied in a very organized setting, just like you study drugs, if we're going to do this well.” Disclaimer: All content and information provided in connection with Manta Cares is solely intended for informational and educational purposes only. Erick Turner is an Associate Professor in the Department of Psychiatry at the Oregon Health and Science University (OHSU). He is also a senior scholar with OHSU's Center for Ethics and Health Care. Dr. Turner has been an FDA reviewer and has dedicated his work and life to improving research transparency. He's well known for his work on publication bias and antidepressant trials, but his findings show that psychotherapy research is also riddled with problems. What happens when those we trust with knowledge in our society betray us? In today's interview, we discuss how dubious research practices are not simply the work of a few bad apples but instead built into the way we produce knowledge. We further explore the consequences of these practices on patients and the dangerous tradition of journal worship before exploring how many of these problems can be solved. *** Thank you for being with us to listen to the podcast and read our articles this year. What You Need to Know is Biden forgives student loans. What you're seeing is crony government at its best. President Biden is canceling student loan debt for about half a billion students — Biden cancels all remaining student loan debt from Corinthian Colleges. This will wipe out about five billion dollars worth of student debt. When adults make decisions about taking out a loan, they need to be responsible for paying it back. John Schlafly delivers the #SchlaflyReport: Where's the Praise of Heroic Border Patrol? John shares how last week it was the border patrol agents who went into Ulvade school and stopped the shooter. Now they are getting lost in the shuffle and we need to make sure they are honored for their heroic actions last week. Find all of John's work with his brother Andy Schlafly at PhyllisSchlafly.com. Buddy Ullman, retired Professor of Biochemistry and Molecular Biology from The Oregon Health and Science University (OHSU), discusses how 50 years ago Title IX was created to prevent discrimination, today it's used to hunt down dissenters of wokeness. OHA: Find a COVID-19 vaccine OHA: COVID-19 vaccine information for parents, guardians and people under 18 OHA: COVID-19 vaccine boosters and third doses OHA: Press release about lifting mask requirements: Statement by Governor Kate Brown OHA: Mask recommendations and requirements OHA Facebook Safe+Strong website Ask a Black Doctor on The Numberz REACH web page and REACH Facebook Current Masking Mandates and What This Means for Us Q1: What is the latest about mask requirements for indoor settings? OHA announced yesterday that it will lift indoor mask requirements in schools and for the general public on March 12, 2022 because hospitalizations and cases are rapidly declining. Earlier this month, OHA had announced that the indoor mask requirement would be lifted by March 31, with the option of lifting it sooner if conditions improved enough. Conditions have improved. A recent modeling report by Oregon Health & Science University (OHSU) predicted Oregon will have 400 or fewer people in the hospital with COVID-19 by March 20. Daily COVID-19 hospitalizations have declined 48% since peaking in January. Over the past two weeks, hospitalizations have fallen by an average of more than 30 a day. Reported COVID-19 infections also have dropped in recent weeks. Over the past month, new infections have declined by more than 80%. The seven-day moving average for new cases is 84% lower than at the peak of the Omicron surge. We hope the March 12 date gives communities and schools time to prepare for the transition. Q2: I'm a business owner. Am I allowed to require employees or patrons to wear face masks after the general public indoor mask requirements are lifted? Once requirements are lifted, employers and businesses may establish their own mask requirements to protect employees and customers. Each business owner is allowed to enforce restrictions in their own business. Q3: Will masks be required in spectator sports and indoor concerts? It depends. Practice and play for indoor competitive sports don't require masks or face coverings. However, venues may continue to require masks or face coverings for spectators as part of their business policy. Q4: Since the indoor mask requirements will be lifted on March 12, does that mean face masks no longer help prevent COVID-19 transmission? No. Health experts still strongly recommend high-risk individuals, and the people who live with them continue wearing masks. People at high risk of COVID-19 infection include people who are: Unvaccinated Immunocompromised At high risk of COVID-19 hospitalizations, including those with underlying health conditions 65 and older Those who live with someone in any of these conditions Q5: Does lifting the mask requirement in public settings also apply to non-public settings, such as businesses? The answer depends on the business. Masks will still be required in some places, such as health care settings. Health care settings include places like hospitals, doctor offices, dentist offices, urgent care and dialysis centers. You will also still have to wear a mask on public transportation, and in airports and on airplanes. Q6: Is there anyone who should not wear a mask, face covering or face shield? Yes. The following individuals should not wear a mask, face covering or face shield: Children under the age of two (2). Anyone who cannot safely wear a mask, face covering or face shield because of a disability or medical condition, in which case an accommodation should be requested. Anyone who is having trouble breathing, is unconscious, or unable to put on the face covering, take it off or remove it without help. Anyone who is sleeping. In addition to wearing a mask, there are other things you can do to protect yourself from COVID. Getting vaccinated and getting a booster can help keep you safe. Source: https://sharedsystems.dhsoha.state.or.us/DHSForms/Served/le3818.pdf Q7: Is OHA pausing quarantining? Yes. Starting on March 12, OHA will no longer require you to quarantine if you have been exposed to someone who has COVID. This applies to the general public, K-12 schools and childcare settings. Why is OHA pausing quarantining? For a couple of reasons: A significant proportion of our state's population has been vaccinated or has developed immunity following a COVID infection. At the same time, the tools we previously used, such as contact tracing, are no longer as effective because transmission occurs quickly, many infected people have no symptoms, or mild symptoms and many infections are not reported to public health. It's important to know that: Schools will continue to exclude individuals who have symptoms or a known COVID-19 infection, and Isolation will remain at 5 days after a positive test result + additional 5 days of masking. Document accessibility: For individuals with disabilities or individuals who speak a language other than English, OHA can provide information in alternate formats such as translations, large print, or braille. Joe Biden on the Vaccines and more on today's CrossPolitic Daily News Brief This is Toby Sumpter. Today is Tuesday, August 31, 2021. Oregon Law Prohibits Vaccine Mandates https://www.kgw.com/article/news/health/coronavirus/oregon-hospitals-cannot-mandate-vaccines-employees/283-1c73f5fc-ffcf-4f10-af20-7aabeb74d3c5 Hospitals in Oregon cannot require doctors, nurses or any of their employees to be vaccinated for anything from the flu to COVID-19. It's been this way since 1989, when a law was passed stating "an employee shall not be required as a condition of work to be immunized under this section." The Oregon Association of Hospitals and Health Systems (OAHHS) said it's long overdue for a rule change. "We believe that it's important for hospitals to have that tool during a pandemic. Not that they all would, but just to have that option to require staff, to be vaccinated during a pandemic, we just think it's really important to have that local option," said OAHHS spokesman Dave Northfield. "We believe that the state could write a rule that gives individual hospitals local control to institute a mandate that falls short of a statewide mandate," said Northfield. OHA said the change that would allow hospitals to decide would have to come from the legislature. OHA Communication Director Robb Cowie said the only rule they could pass would be a statewide mandate. One group against a law change is the Oregon Nurses Association (ONA). ONA Communication Director Scott Palmer released a statement that said in part: "We are not currently seeking a change in Oregon's statutes regarding workplace immunizations. We believe there is much more work corporations can do to protect workers and increase vaccination rates before they begin firing working Oregonians during a pandemic." KGW Investigates could not find any other state with a law that prohibits hospitals from requiring vaccines for their employees. https://www.kgw.com/article/news/local/hospitals-vaccine-requirements-oregon-law/283-a734746a-49d4-4c15-9e3f-904d4556f81e — Kaiser Permanente, PeaceHealth and Oregon Health & Science University (OHSU) will require employees to be vaccinated against COVID-19, despite a 1989 Oregon law that prohibits hospitals from doing so as a condition of employment. "I think that law is certainly a barrier. I think we're looking at this as a public health emergency. We're hoping to have some support from our government officials...we are calling this a requirement, there are ways for there to be a medical exemption," said Dr. Koekkoek. The deadline for employees to get vaccinated varies by organization. PeaceHealth is requiring all caregivers to be fully vaccinated by Aug. 31. OHSU is targeting Sept. 1 for its staff and students and Kaiser Permanente hopes to have its workforce fully vaccinated by Sept. 30. It's unclear what action, if any, hospitals will take against employees who are not vaccinated or exempt by the proposed deadlines. High School in Washington State Using Ankle Monitors https://www.iheart.com/content/2021-08-25-high-school-uses-ankle-monitors-to-track-social-distancing-of-athletes/ A high school in Washington state is facing criticism after equipping student-athletes with ankle monitors to track social distancing during practices. The students only have to wear them during practice and leave them in the locker room when they go home. [They are only prisoners when they are playing the game.] The monitors are being used as part of the district's contact tracing program. They record how close students get to each other and for how long. In the event of a positive COVID-19 test among students, health officials can quickly identify who had close contact with the infected individual. Officials with the Eatonville School District have defended using the "proximity monitors" against backlash from parents and other people online. "The monitors are for both staff (coaches) and students on the field, regardless if they are vaccinated or unvaccinated. If a student or coach tests positive, we will have immediate information regarding athletes' and coaches' contacts, so we can more tightly determine who might need to quarantine," the district said in a statement. [This is called Totalitarian grooming, people. Getting you used to be monitored at all times.] One parent said he was reluctant about letting the school put an ankle monitor on his son but didn't want to keep him from playing football. [Reluctant?!!] "I was notified if I didn't sign it, they couldn't play," Jason Ostendorf told the Tacoma News Tribune. "My son has played football since he was in third grade. He's passionate about the sport. ... I signed it reluctantly. It's either that or he doesn't play. ... It's not optional. If you don't sign the waiver, they don't get to play. You have no choice in the matter." "It's just one more thing they're doing to the kids through this whole covid thing," he added. "The vaccine, now be tracked when you're at practice. Where does this end? I feel like this is an experiment on our kids to see how much we can put them through before they start breaking." Another parent said he had no problem with the school using the ankle monitors. "It's a continuation of doing what we have to do to let the boys play a sport that they love," Jason Lewis told KCPQ. "If my son would have told me, "Dad, I don't want to wear it. I'm not going to wear it,' He wouldn't be playing football. That's his choice. I mean, but he wanted to play football. He would have worn three of them if you asked him to." And… there are your gods, America. Doing what we have to do to let the boys play a sport they love…. Ah, but what is a boy anyway? What is sport? And what is love? Leading sportswear brand Nike has repurposed its shoe and apparel materials to produce full-face shields and other protective medical equipment for healthcare workers in the United States. As part of Nike's pandemic response, the company has worked with medical experts from the Oregon Health & Science University (OHSU) to manufacture Personal Protective Equipment (PPE). Nike patterned its output from the face shields currently used at OHSU. OHSU health professionals tested the prototypes and approved the final product for mass production. Nike made use of thermoplastic polyurethane (TPU), collar padding, and cords to create the face shields. These materials were previously used for making Nike Air soles, shoes, and apparel, respectively. In a press release, Nike said it wanted to not only produce high-quality face shields but also develop an efficient production process. People from Nike's Innovation Teams and Air Manufacturing Innovation (Air MI) facilities collaborated to streamline the process. Air MI's decades of experience in creating different kinds of TPU products made it easier to seamlessly adapt to manufacturing PPE while also complying with government guidelines. Miko Enomoto, an associate professor from the OHSU School of Medicine, explained the importance of Nike's initiative. She said the lack of protective gear compromises the health of frontline medical workers, which could potentially put the healthcare workforce under strain in the coming months. She added that full-face shields can prolong the usability of surgical or N95 masks. Nike shipped its first batch of full-face shields and other protective medical equipment to OHSU in April, and it will continue sending PPE to health facilities across the United States.
