Dr Abraham shares her experience and insight into the complexities of managing IBD. Bincy P. Abraham is the Distinguished Professor and Director of the Fondren Inflammatory Bowel Disease Program at the Underwood Digestive Diseases Center of Houston Methodist Hospital. She is also the Program Director for the Gastroenterology Fellowship at Houston Methodist Hospital. She earned her medical degree from University of Texas Medical Branch, Galveston, TX, where she continued with residency training in internal medicine and fellowship in gastroenterology. During fellowship, she received specialized training in inflammatory bowel disease and earned her degree in Masters of Clinical Investigation. She also completed a National Visiting Fellow Inflammatory Bowel Disease Rotation program through the Crohn's and Colitis Foundation in Cedars Sinai Hospital in Los Angeles. She has chaired the Southern Regional chapter of the CCFA Medical Advisory Committee, served as president of the Texas Gulf Coast Gastroenterology Society, is involved in national committees for and is a Fellow of the American College of Gastroenterology, American Gastroenterology Association, American Society of Gastrointestinal Endoscopy, and Crohn's and Colitis Foundation. She is trained in and passionate about intestinal ultrasound in IBD patient care. Dr. Abraham discusses the importance of nutrition and emphasizes the need for patient empowerment. She also addresses common patient concerns regarding treatments and nutritional deficiencies. A big takeaway for us is: We don't have to suffer as IBD patients. On this episode, we are joined by April O'Connell to discuss exercise dosing for patients who have sustained injuries of their upper extremity. She shares with us factors that should be considered when designing an exercise program for a patient and how to progress them through their plan of care to achieve their goals and beyond. Guest Bio: April O'Connell, OTR/L, CHT, ACSM, is an occupational therapist at Cedars Sinai Hospital in Beverly Hills, CA. She works with a variety of sports teams including the NFL, MLB and NBA and recently spoke at ASHT and AAHT on Rehabilitation of the Overhead Athlete. She is one of the main contributors to the fellowship program for hand and upper extremity rehabilitation as well as the co-director for Cedars Sinai Hand Symposium. April got her start as the Clinical Specialist in the Hand and Upper Extremity Therapy Department at NYU Langone Health. She has lectured to orthopedic surgeons and therapists nationally and internationally in current concepts of orthopedic rehabilitation of the upper extremity. Some of her most recent lectures include being a guest speaker for Dr. Radio on Sirius Satellite on topics for rehabilitation of the hand and upper extremity as well as injuries to the overhead athlete. In addition, she has presented on Flexor Tendon Repair Advancement at AAOS in 2015 and was later asked to speak at the ASSH national conference in 2016 and ASHT in 2017. She recently presented her latest research at the ASHT National Conference on flexor tendon rehabilitation in 2019. April is a founding member of the NYU Langone Golf and Pitching Labs and was the head hand therapist for the first successful bilateral hand and face allotransplantation. She earned her Occupational Therapy degree at Boston University, is a Certified Hand Therapist and is certified by the American Council of Sports Medicine (ACSM) as a Clinical Exercise Specialist.

The average rate of cesarean in the United States in 2022 is 32.1% and the World Health Organization's statement that cesarean rates over 20% lead to worse outcomes for moms and babies. Los Angeles hospital, Cedars-Sinai Hospital, does not disclose their rate of cesarean and that is likely due to a higher than average rate. One of the attorneys will be here to explain why they are going after social media firms and firearm manufacturers.  The Department of Justice launches an investigation into Fulton County, Georgia, Jail where a black man, LaShawn Thompson, died covered in insects and filth. Civil Rights attorney, Ben Crump, will be here to tell us more.  The DOJ files to become a part of the NAACP's lawsuit against the state of Mississippi, trying to create a separate legal system for the state's Capitol City.  And One of Los Angeles' largest hospitals, Cedars-Sinai Hospital, is facing a federal investigation over how they treatment of black women, especially those having babies.  The family of a black Indiana man who died after he was tased and handcuffed while naked during a mental health crisis will be here with their attorneys to talk about their civil lawsuit and how they are preparing for the trial of the cops who killed their son.  We have a special report on the disproportionate use of menthol products in Black communities. In this episode of The Mommy Labor Nurse Podcast, I am joined by Charles Johnson, the founder of 4Kira4Moms.  Charles tragically lost his wife Kira during a routine C-section at Cedars-Sinai Hospital in Los Angeles, California in 2016. This experience led him to create 4Kira4moms in 2017. The goal of 4Kira4Moms is to be a voice for other mothers and families facing unnecessary maternal loss and to help put an end to the black maternal mortality health crisis. It is a sad reality that in the US, black women are four times more likely to die during pregnancy and childbirth than white women. This is due to implicit bias and systemic racism within the healthcare system, as well as a lack of access to high-quality maternal healthcare and resources. Inside the episode, Charles and I discuss this tragic epidemic and talk about what platforms like 4Kira4Moms are doing to spark change and shed light on the issue. After experiencing the unimaginable, Charles harnessed his pain and used it to pursue the highest level of good. He channeled his grief into founding 4Kira4Moms, a nonprofit organization dedicated to improving birth outcomes in America. Charles knows childbirth should be empowering and free of fear for all mothers. 4Kira4Moms advocates for improved maternal health policies and regulations, educates about the impact of maternal mortality in our communities, provides support to victims' loved ones, and approaches maternal mortality as a human rights issue. Mothers are the very framework of our society. They teach us how to love unconditionally and are willing to protect the most vulnerable among us. The loss of even one at the hands of an unjust system robs our country of what it needs most. Charles has used his voice and story to advocate for change at countless events and media appearances all over the country. He is a board member of March for Moms and serves as Chairman of the Fairness of Injured Patients Act, which seeks to restore the rights of victims of medical negligence in California. Charles has testified twice before Congress and helped pass key legislation including: the Preventing Maternal Death Act of 2018, the Protecting Moms Who Served Act of 2021, and the California Momnibus. He is currently working with Congress to pass the Black Maternal Health Momnibus Act, an unprecedented set of bills addressing every aspect of the maternal health crisis happening in America. I have the honour of speaking with Colleen Murphy about the journey following a severe accident that turned this family's life upside down.  On the morning of April 19, 2013, while out for a typical morning run, Cooleen's daughter Lauren was hit by a car and suffered a severe Traumatic Brain Injury.  Lauren was admitted to Cedars- Sinai Hospital in West Hollywood and remained in extremely critical condition for several weeks. Lauren had lived in New York at the time of the accident and was in LA on business. She did not have identification on her and remained a Jane Doe for several hours. Her prognosis was not good. Upon arriving in LA, her parents were told that she had a very slim chance of surviving. Once she got past the extremely critical stage it had been explained that her brain damage was so severe that she would most likely not be able to feed herself, walk, talk or understand spoken words. Thankfully, Lauren has never been typical and she has spent the last 5 years proving all of the experts wrong.  Her recovery has been far from easy, she has travelled all over the United States working with experts trying to get better.  She works extremely hard and continues to struggle each and every day to regain as much of her former life as possible.  Lauren has become the embodiment of the Murphy family saying, “Murphys Don't Quit” and this has become her calling card. Today she lives with her parents in O'Fallon MO and continues to focus on her recovery.  She maintains a very busy schedule of speech therapy, physical therapy and the all too often retail therapy trips with her Mother.  Lauren enjoys getting out in the community, meeting new people and sharing her story. She remains at high risk for seizure activity due to the extent of her brain damage. That said, a good guideline to follow would be, as a first step: give the problem some crutches to lean on. Eliminating the most obvious food sensitivities like nuts, eggs, dairy, wheat, processed foods, nightshades, etc. Once that's dialed in, go a level up and consider removing high-histamine foods that could be perpetuating the inflammatory cycle. And then, once a little more stable, support detoxification by getting in a light daily sweat, and eating steamed foods that assist the liver, like dandelion, artichoke, beets, cruciferous vegetables, garlic, and gluten-free oats. And all the while, prioritize meditation, sunlight, and lymphatic drainage. A light rebounding session on a Bellicon, or even just doing some jumping jacks on some carpeting can be a big help. To dive deep into all matters colitis, and IBD, someone who'd been a well-kept secret here in LA for several years, but is now out of LA, and entering the online world in a bigger way, is a personal friend of mine, Jordan Kravitz. Jordan was diagnosed with Crohn's Disease in 2015, and after getting handed the pamphlets about all the drugs he'd need from the head of the IBD clinic here at Cedars-Sinai Hospital, Jordan laughed at him and told him to keep his drugs. What are the different possible triggers for how people get Parkinson’s? Find out in this episode as I try to narrow down how the degenerative brain disorder was unleashed on me. After Niki and I chat about the “how,” my wife ,Rebecca and I begin to explore the “why?” If you have Parkinson’s, you may want to play along at home while you listen. Here’s a handy check list to see what factors potentially triggered your onset of PD. Check ALL that apply: I am male. I am older than 60 years old. I have parents or siblings who have Parkinson’s disease. Genetic tests show I carry a gene that is associated with the onset of Parkinson’s disease. Genetic tests show I carry a gene that is associated with brain degeneration. I play or played a high contact sport (Football, Hockey, Rugby, Boxing, Martial Arts) I have played or played high contact sports for more than eight years. I have experienced several concussions. I live or have lived near a major, busy road. I have worked or lived near a chemical plant. I have suffered Agent Orange exposure. I have been exposed, over a long period time or at extremely high levels to pesticides. I have been described as a workaholic, a Type-A personality, or stressed-out. I do not exercise. I have had a stroke. I have injected the MPTP strain of synthetic Heroin and suffered spontaneous Parkinson’s. I was born with damaged dopamine-producing brain cells. I died. During my autopsy, Alpha-Synuclein was discovered clumping in my brain (Lewy Bodies). I did not realize how much I was asking myself, “What did I do to get Parkinson’s?” until I started putting together this episode. The recent study by Cedars-Sinai Hospital in California that we discuss at the end of the episode was a real revelation for me. It suggests that people with Young Onset Parkinson’s may be born with malfunctioning dopamine producing brain cells, which leads to the clumping of the protein Alpha-Synulcein (Lewy bodies) and ultimately leads to the onset of Parkinson’s disease. In the episode, we hear from many qualified professionals about different reasons how Parkinson’s onsets. Sohini Dhowdhury, Deputy CEO of The Michael J. Fox Foundation, discusses why she is so interested in Alpha Synuclein research. “We know it’s the hallmark of Parkinson’s. Research and therapies targeting this protein gets to the root biological process,” Chowdhury said.   At UCLA, Dr. Jeff Bronstein, the head of the movement disorder clinic there and Dr Beata Ritz, professor of epidemiology at Fielding School of Public Health, collaborate on environmental factors that could trigger the onset of PD, like pesticides, both for the home and industrial use. Dr. Ritz has little doubt when she speaks on this topic, “Some pesticides are neurotoxic in a way that causes Parkinson’s disease when you are exposed over a very long time or very high levels.” In the episode, we examine the herbicide Paraquat, which is banned in many countries around the world, yet remains one of the most widely used chemicals to protect crops. There are many ways people can trigger Parkinson’s and many reasons why someone is diagnosed with it. Dr. Bronstein believes every case is unique, “I think of it as a humungous Venn diagram in which there and many, many different factors.” In the end, how I got Parkinson’s may not matter to anyone else, but me. But, for me, I’d be more comfortable knowing it was hibernating within me until I was ready for it, instead of me doing something to unnaturally cause it. In reality, I’ll likely never know what combination of things unlocked this disease. A 30-year physician and two-time mayor of Beverly Hills, Dr. Julian Gold gained empathy and gratitude for community through his trial with cancer.   Dr. Julian Gold was 56-years-old when he was diagnosed with a deadly form of cancer, Acute Myeloid Leukemia. In 1950, medical information doubled every 50 years.  It’s been estimated that, in 2020, it will take only 73 days for medical knowledge to double!  This is not surprising given that a new biomedical journal article is published every 26 seconds!  It is clear that we will need tools to manage and synthesize all of this data, and that’s where the clinical decision support (CDS) comes in.  Our guest today is Dr. Scott Weingarten.  Dr. Weingarten recently joined Premier Inc., retaining his role as CEO of Stanson Health - a clinical decision support company he founded approximately 6 years ago.  Prior to joining Premier Inc., Scott was the Senior Vice President and Chief Clinical Transformation Officer at Cedars-Sinai Hospital.  In addition to his long-standing tenure as a practicing physician and executive, Scott is also a serial entrepreneur and inventor - holding three software patents.  Prior to forming Stanson Health, Scott co-founded Zynx Health, a highly successful leader in ‘order sets’ and ‘care plans’ for electronic health records.In this interview Scott will share:The demonstrable positive impact of CDS on organizational quality and cost - resulting from marked improvements in appropriate utilization of lab tests, imaging studies and medication prescribing.The sophistication of today’s CDS technology that results in the provision of real-time, patient-specific, evidence-based recommendations.The vast breadth of clinical situations and conditions for which Stanson Health has created evidence-based clinical decision supports.The specific benefits for practicing clinicians: offering providers performance data on how they compare to their colleagues and to evidence-based standards.The highly sophisticated analytic approach Stanson Health takes to streamline and optimize clinical alerts embedded in the electronic medical record - removing a substantial percentage of “low value alerts” - what Scott calls “pruning the tree”.I went into this interview understanding CDS as a tool to enhance clinical practice - improving providers’ and organizations’ ability to meet quality goals, as well as optimize utilization of tests and treatments.  I came out of this interview believing that CDS is not only a basic necessity if we are going to practice safe, high quality medicine; but, that it's also one of the most profound technologies shaping medical practice - fundamentally changing the way providers & patients will experience clinical encounters in the future.  Imagine when a care provider will be able to dialogue with a patient, supported by natural language processing software and sentiment analysis, feeding the artificial intelligence that will be making real-time, evidence-based, customized recommendations for that individual patient.  This reality does not seem that far off, and I suspect Scott and Stanson Health are moving rapidly toward that day.  Imagine how liberating and humanizing that would be - allowing providers and patients to really communicate and engage with one another, with technology aiding that human-to-human interaction rather than interfering with it.  The take-home point is that Clinical Decision Support is not just about meeting quality metrics or reducing inappropriate testing and prescribing.  CDS is not just about assisting providers in consistently delivering up-to-date, evidence-based medical care.  CDS is not just about reducing unnecessary costs of care.  The purpose of CDS, as I understand it now, is to make healthcare more accessible, affordable, effective, personalized and relationship-oriented.  The highly informed perspective, and the picture that Dr. Scott Weingarten paints for the future of healthcare is, indeed, a bright one.  His is a hopeful and realistic message, and one that is well worth listening to.

What Up, Get Up Nation? My name is Ben Biddick, the Host of the Get Up Nation Podcast and Co-Author of Get Up: The Art of Perseverance with former Major League Baseball player and CEO of Lurong Living, Adam Greenberg. Recently I had the honor and privilege of speaking with Alison Crowley and Cyndi Tomlinson who have joined forces to create a non-profit organization that supports and empowers people through their cancer experience called Bolster & Bridge: The Foundation. Alison is a yoga therapist specializing in cancer since 2007. She was inspired to become a yoga therapist after witnessing her mother’s experience dealing with multiple chronic illnesses. She and her sisters spent more than 25 years navigating the medical world on behalf of their mother. During these experiences, Alison recognized a need to serve the whole person through challenging medical circumstances. Cyndi is a business executive and cancer survivor. Cyndi experienced her first bout with cancer at the age of 22. She faced a diagnosis of Hodgkin’s Lymphoma while a single mother with a 2 year old daughter. Cyndi underwent treatment while working to support her daughter with the love and help of her family. Twenty years later, Cyndi was diagnosed with breast cancer. This time, she knew she needed additional support to care for herself and her family as she worked full time and raised her second daughter, age 13 at the time. After attending a six-week program with Alison at Cedars-Sinai Hospital in Los Angeles, Cyndi responded to the mind-body work with such enthusiasm that she couldn’t wait to come home after each class to share the practices with her daughters. Dr. Jason Fung grew up in Toronto, Ontario, Canada..... I went to the University of Toronto at age 17 to begin studies in Biochemistry. By 23, I completed medical school at the U of T, and began my Internal Medicine residency there. Finishing my specialty of Internal Medicine, I chose Nephrology (kidney disease) as my sub-specialty. Each field of internal medicine draws its own personalities. Nephrologists had the reputation of being a ‘thinkers’ specialty. There are a lot of intricacies of fluid and electrolytes, and I enjoyed these puzzles. I studied Nephrology at the University of California, Los Angeles mostly at Cedars-Sinai Hospital and the VA Wadsworth. Looking back, I realize that it must have been a little disconcerting to patients in the hospital to be treated by a doctor who looked about 18 years old. I returned to Toronto in 2001 to start my career in Nephrology, where I still have both an office and hospital practice. Type 2 diabetes is by far and away the leading cause of kidney disease, and I treat many hundreds of patients with this disease. Many also have obesity. By the early 2010s my interest in nutrition, combined with my professional focus on obesity and T2D had led me directly to the diabesity puzzle. More details HERE.
