Johnson, Ph.D. is an Assistant Professor in the Department of Health, Sport and Exercise Sciences at the University of Kansas and is the Assistant Director of the Jayhawk Athletic Performance Laboratory. He received his degrees from Midland University and Oklahoma State University. Dr. Johnson's research interests focus on optimizing athletic performance, developing physiological profiles to support training and return to play approaches, long-term athletic development, and training load monitoring. These interests have led to professional roles within high school, collegiate, professional, and private athletic settings. In addition to his scholarly activity, Dr. Johnson has been an active strength and conditioning coach working with athletes from professional to peewee levels of sport. To add, he currently serves as the Associate Director of the Youth Exercise and Sport Center and Program Coordinator for Jayhawk Performance housed within the Dept. of Health, Sport and Exercise Sciences. Dr. Johnson is passionate about developing and creating opportunities for the next generation of exercise science professionals.Johnson is an active member of the National Strength and Conditioning Association (NSCA), Certified Strength and Conditioning Coaches Association (CSCCa), and American College of Sports Medicine (ACSM). Previous episodes: 158, 100 (sexercise), 95, We all know exercise is good for us, but how do we prescribe it like we do medications? How can we tailor exercise recommendations to our patients' needs and lifestyles? To help us unpack this, I'm joined today by Dr. Ranbir Sandhu, and Medical Student, Wessam Daoud, who has a passion for preventive medicine. Welcome to the show!Ranbir: Thanks, Dr. Arreaza! We're excited to be here and to discuss something so fundamental to health.Segment 1: Understanding Exercise PrescriptionArreaza: Let's start with the basics. In medicine, we prescribe medications with precise instructions—dosage, frequency, duration. But how do we apply this concept to exercise?Ranbir: Great question! Before we prescribe exercise, we have to make sure that it is not contraindicated. We can use a system to stratify our patients based on risk factors, such as older age, smoking, baseline level of activity, etc. For example, a patient who had a heart attack within the last 6 weeks should not exercise yet, a person with heart failure exacerbation, asthma exacerbation, uncontrolled heart arrhythmia, etc. Wes: Exercise prescription follows a structured approach, similar to medications. We use the FITTE mnemonics to guide recommendations: Frequency – How often?Intensity – How hard should the patient work?Time – How long should each session last?Type – What kind of exercise is best?Enjoyment – Does the patient enjoy this activity?By adjusting these components, we can tailor exercise to each patient's needs, whether it's improving cardiovascular health, managing chronic disease, or building strength.Segment 2: How Much Exercise Do Adults Need?Arreaza: Now, when we talk about exercise, there's a lot of conflicting advice out there. What do the official guidelines say about how much adults should exercise?Ranbir: The American College of Sports Medicine (ACSM) and CDC provide clear guidelines:Aerobic Exercise: At least 150 to 300 minutes of moderate-intensity exercise per week, OR 75 to 150 minutes of vigorous-intensity exercise (or a mix of both).Muscle Strengthening: At least two days per week of resistance training targeting major muscle groups.Balance & Flexibility: Particularly important for older adults to reduce fall risk.These guidelines are adaptable, meaning patients can break them into shorter sessions throughout the week.Arreaza: For weight regain, you may need to exercise a little bit more, about 300 minutes/week, and >2 days of resistance activity.Segment 3: Choosing the Right Type of ExerciseArreaza: With so many options—cardio, strength training, yoga—how do you guide patients in choosing the right type of exercise for them?Wes: It depends on the patient's goals, health conditions, and personal preferences. Here's how we might break it down:For cardiovascular health: Activities like brisk walking, jogging, cycling, or swimming.For strength and bone health: Resistance exercises, bodyweight exercises, or weightlifting.Ranbir: For flexibility and balance: Yoga, Pilates, or tai chi, especially for older adults.For chronic disease management: Customized plans—e.g., low-impact options for arthritis or supervised exercise for heart disease.The key is finding something they enjoy, because sustainability is the most important factor.Arreaza: We can use our physical therapy friends to design an appropriate plan for our patients.Segment 4: Overcoming Common Barriers to ExerciseArreaza: I hear this all the time in the clinic—patients want to exercise but struggle to stay consistent. What are the biggest barriers, and how do we help patients overcome those barriers?Wes: Absolutely. Some common barriers include:Lack of time: Patients think they need hours at the gym, but even short bouts of 10 minutes throughout the day add up.Low motivation: Encouraging goal setting and accountability, such as a workout buddy or an activity tracker, helps.Arreaza: Instagram post from Ranbir: Go to the gym even if you don't want to go. Wes: Pain or chronic illness: We can adapt exercises—low-impact options like swimming or chair exercises work well.No access to a gym: Many exercises require no equipment—walking, stair climbing, bodyweight exercises.Ranbir: As physicians, we need to normalize movement as part of daily life rather than an all-or-nothing approach.Segment 5: The Role of Healthcare Providers in Exercise CounselingArreaza: We often focus on medications and procedures, but exercise is one of the best treatments we have. What role should physicians play in promoting physical activity?Ranbir: Our role is critical! Exercise is preventive medicine and can reduce the risk of heart disease, diabetes, obesity, and even depression. As physicians, we can:-Ask about exercise levels at routine visits.-Provide specific, personalized exercise prescriptions rather than just saying 'you should exercise more.'-Address patient concerns by modifying recommendations to their abilities.-Follow up and reinforce progress like we would with any other treatment.-Even a brief conversation about physical activity can significantly impact patient motivation and adherence.Arreaza: Exercise vs physical activity. Ask your patients as a routine. Closing Thoughts & Call to ActionArreaza: Ranbir andWessam, this has been a fantastic discussion. Any final thoughts for our listeners?Ranbir: My biggest takeaway is that any movement is better than none. Exercise doesn't have to be perfect—it just has to be consistent. On this episode, we are joined by April O'Connell to discuss exercise dosing for patients who have sustained injuries of their upper extremity. She shares with us factors that should be considered when designing an exercise program for a patient and how to progress them through their plan of care to achieve their goals and beyond. Guest Bio: April O'Connell, OTR/L, CHT, ACSM, is an occupational therapist at Cedars Sinai Hospital in Beverly Hills, CA. She works with a variety of sports teams including the NFL, MLB and NBA and recently spoke at ASHT and AAHT on Rehabilitation of the Overhead Athlete. She is one of the main contributors to the fellowship program for hand and upper extremity rehabilitation as well as the co-director for Cedars Sinai Hand Symposium. April got her start as the Clinical Specialist in the Hand and Upper Extremity Therapy Department at NYU Langone Health. She has lectured to orthopedic surgeons and therapists nationally and internationally in current concepts of orthopedic rehabilitation of the upper extremity. Some of her most recent lectures include being a guest speaker for Dr. Radio on Sirius Satellite on topics for rehabilitation of the hand and upper extremity as well as injuries to the overhead athlete. In addition, she has presented on Flexor Tendon Repair Advancement at AAOS in 2015 and was later asked to speak at the ASSH national conference in 2016 and ASHT in 2017. She recently presented her latest research at the ASHT National Conference on flexor tendon rehabilitation in 2019. April is a founding member of the NYU Langone Golf and Pitching Labs and was the head hand therapist for the first successful bilateral hand and face allotransplantation. She earned her Occupational Therapy degree at Boston University, is a Certified Hand Therapist and is certified by the American Council of Sports Medicine (ACSM) as a Clinical Exercise Specialist.

Dr Scott discusses the correct way of tapering for runners and alot more, with our host, Vikas Singh. Dr. Scott Trappe is a renowned expert in human bioenergetics and muscle physiology, currently serving as the John and Janice Fisher Endowed Professor of Human Bioenergetics and the Director of the Human Performance Laboratory at Ball State University in Muncie, IN. With over 25 years of experience, Dr. Trappe has made significant contributions to the fields of exercise science, aging, and space medicine. He earned his undergraduate degree at the University of Northern Iowa, where he was also the captain of the swim team. He completed his graduate studies (MS) at the University of Colorado while working with US Swimming at the Olympic Training Center in Colorado Springs. He received his PhD from Ball State University, studying under the esteemed Dr. David Costill, and completed post-doctoral training in muscle physiology with Dr. Robert Fitts at Marquette University. For over two decades, Dr. Trappe has collaborated with NASA to optimize exercise protocols for astronauts, ensuring their health and performance during space missions. He has held several administrative positions including Chair for the Department of Exercise Science, Associate Dean for Research at the Arnold School of Public Health, and Vice Provost for Health Sciences. Dr. Pate is an exercise physiologist with interests in physical activity and physical fitness in children and the health implications of physical activity. His research has been supported by NIH, CDC, the American Heart Association, and several private foundations and corporations. He coordinated the effort that led to the development of the recommendation on Physical Activity and Public Health of the CDC and the American College of Sports Medicine (ACSM) (1995). He served on the 2005 Dietary Guidelines Advisory Committee, the 2008 Physical Activity Guidelines Advisory Committee, and an Institute of Medicine panel that developed guidelines on prevention of childhood obesity. He currently serves as Chair of the National Physical Activity Plan Alliance. Shelby Baez, Ph.D., ATC, is an Assistant Professor in the Department of Exercise and Sport Science. Dr. Baez is a certified and licensed athletic trainer with an expertise in psychology of sport injury and an approved non-certified mentor for aspiring CMPCs through the Association for Applied Sport Psychology (AASP). Dr. Baez's research examines the impact of psychosocial factors on health outcomes after sport-related injury, with a focus on patients after anterior cruciate ligament reconstruction. Her lab examines the impact of psychosocial factors on biological outcomes, such as neurocognitive function, biomechanical alterations, and secondary injury risk. Her lab also seeks to identify clinically feasible and novel psychosocial interventions to improve health outcomes. If you're sedentary and begin to exercise, you get a dose-dependent decrease in mortality, diabetes, depression, high blood pressure, coronary disease, osteoporosis, sarcopenia, falls and more People who are doing the highest volume of vigorous exercise start losing longevity benefits. If you're doing full distance triathlons when you're in your 40s and 50s, your risk of atrial fibrillation increases by 500% to 800% In the case of moderate exercise — loosely defined as exercising to the point where you're slightly winded but can still carry on a conversation — there's clear evidence that more IS better and cannot be overdone Every 1,000 steps you get on average per day reduces your mortality by 10% to 15%. Benefits plateau around 12,000 steps (6 miles) a day Strength training adds another 19% reduction in all-cause mortality on top of the 45% reduction that you get from one hour of moderate exercise per day. However, benefits cease once you go beyond one hour per week. The sweet spot is 20 to 40 minutes of strength training, two to three times a week. Above 60 minutes per week, the benefits of strength training are nullified, and you're worse off than if you did no resistance training at all.Regular moderate exercise is highly beneficial for overall health and longevity, while excessive vigorous exercise may have detrimental effects. Key Points: Sedentary individuals who start exercising experience significant health improvements, including reduced risk of mortality, diabetes, depression, high blood pressure, coronary disease, osteoporosis, sarcopenia, and falls. She holds a Ph.D. in Curriculum and Instruction specializing in Sport Science with a cognate in Public Health.  Her Ph.D. was centered around the merging of Health Science and Exercise Physiology, and she gained a deeper understanding of sexual health when writing her dissertation, “Sex-Positive Curricula: An Investigation of the Relationship Between Physical Fitness, Self-Concept, and Sexual Functioning.”She is currently an Associate Professor in Exercise Science at Nova Southeastern University. Today, my guest is someone I met through supporting bodybuilding shows. I had a client competing at the Showtime Classic in Ponte Vedra, Florida, where I had the opportunity to meet and talk with Jacob Pomroy. As he shared some of his stories with me, I knew I wanted to get him on the show to share with others. Jacob is a Full Time Online Coach who champions Grand Mal Seizures, now known as Tonic-Clonic Seizures. According to https://www.hopkinsmedicine.org/health/conditions-and-diseases/epilepsy/tonic-clonic-grand-mal-seizures, here are some things to know about these types of seizures: they involve a tonic (stiffening) phase and a clonic (twitching or jerking) phase. Jacob points out that if you are with someone with one of these types of seizures, you do NOT want to place anything in their mouth. You want to turn them to the side, clear space, and time the seizure. You must let it play out. Jacob served in the US Army National Guard as a Mechanized Infantryman from 2014 to 2017, when he was medically discharged. He has competed in bodybuilding since 2016 and recently competed in 2022. he graduated from the University of Wisconsin - River Falls with a Bachelor's Degree in Exercise & Sports Science. Our host, Vikas, and Dr. Aashish discuss all about the human heart, the risk factors, and various preventive measures for runners.Dr. Contractor has been the head of the department of preventive cardiology and rehabilitation at the Asian Heart Institute from its inception in 2002, till June 2014. He had the distinction of being part of the Asian Heart team which treated the Prime Minister of India, Dr. Manmohan Singh, after his redo bypass surgery in January 2009, and was in charge of his post-surgery cardiac rehabilitation. He is widely recognized as the pioneer of cardiac rehabilitation in India and set up the first telemetry monitored center in the country in Mumbai in 2001. On the academic front, Dr. Contractor has worked and trained in the United States, under the leading experts in his field. He completed his medical training at the TN Medical College, Mumbai and post-graduate training at the University of Virginia, USA. He has several international scientific publications, both in textbooks as well as journals. His current research interests include mobile healthcare delivery (m-Health) in cardiovascular medicine as well as the role of genetics in the prevention and treatment of disease.He is the Vice-Chairman of the International Council of Cardiovascular Prevention and Rehabilitation, which includes 21 countries around the world. He is a visiting lecturer at the University of Chester, United Kingdom, and serves as the Certification Director of the American College of Sports Medicine (ACSM) in India, since November 2003. Dr. Contractor has been the Medical Director for the Standard Chartered Mumbai Marathon, since its inception in 2004. Among student athletes, however, approximately 30% of women and 25% of men report having anxiety. There are very few people (particularly listeners of the Wellness Paradox) that would dispute the notion that exercise is medicine. On many levels this is the rallying cry of our profession. While it may be that potent message, we need more than a strong rallying cry to make exercise is medicine a reality within the healthcare community. Operationalizing exercise is medicine into clinical practice has been a significant strategic emphasis of the American College of Sports Medicine (ACSM), through their Exercise is Medicine® (EIM) initiative. The ACSM's EIM program seeks to make physical activity assessment and promotion a standard of clinical care around the globe.  In episode 67 we talk with Robyn Stuhr, the Vice President of Exercise is Medicine® at the ACSM. Our discussion will focus on the important pillars needed to make EIM a reality, clinically. First, we'll discuss the newly revamped Exercise is Medicine® curriculum. This specialty certificate, offered by ACSM, provides fitness professionals with the knowledge, skills, and abilities to work with individuals who have common chronic diseases. Furthermore, it also provides an understanding of the healthcare landscape and health coaching principles for lasting behavior change. Overall, this revamp is an exciting iteration of the previous curriculum. Our conversation also expanded to the other levers that must be pulled to operationalize the exercise is medicine objectives of assessment and promotion. Achieving a health and fitness goal is hard, maintaining that achievement is even harder. If you've worked with clients for any length of time, you know how true this is. Many fitness professionals grow frustrated by their clients lack of long-term progress. It seems like the same cycle perpetuates itself. The client makes some positive change, only to fall back into old behaviors, feel bad, get motivated, and repeat the cycle over and over again. As frustrating as it is for the trainer, this can be down right demoralizing for clients, causing many to eventually give up entirely, assuming “fitness isn't for them.” There is a way to break that cycle of frustration and demoralization, and that is exactly the topic we'll cover in episode 60 with Dr. Renee Rogers. Dr. Rogers is an expert in bio-behavioral healthy lifestyle intervention and platform design with a focus on using technology, media, and relevant engagement strategies to promote long-term adherence.  Our discussion will explore the role of client-centered coaching in helping individuals initiate new health behaviors and work towards maintaining those behaviors long-term. As Renee points out, this is more than merely following the guidelines purposed by the ACSM and the CDC. It has a lot more to do with listening intently to your client and creating relevance that results in lasting motivation and desire to change.  Dr. Rogers brings unique insights into this conversation, as she's worked as a practitioner, researcher, and consultant during her professional career. She blends her background in theater and production with her formal academic training in Exercise Physiology, Behavior Change, and Weight Management to develop and implement innovative health promotion, physical activity, and weight management initiatives that encourage well-being, reduce sedentary behavior, and increase physical activity engagement. Playing many professional roles that include working as a practitioner, clinician, program director, researcher, and consultant shapes her translational approach to platform and programmatic design. Dr. Rogers has a strong foundation in clinical research with expertise in the areas of developing and implementing lifestyle behavior-based weight management trials, and physical activity interventions. She has also been developing technology-based interventions using media and video since 2010 creating successful video interventions related to physical activity with progressive strategies for engaging different population groups including older adults, young adults/weight loss, and participants learning yoga and other modes of physical activity. She has even adapted a media intervention into Italian for a project that ran in the US and in Italy.Dr. Rogers is currently working as an independent healthy lifestyle, fitness and wellness consultant where she develops programming platforms and interventions (research and corporate) that integrate the latest research and best practices for consumer engagement. Dr. Rogers received her B.S. in Exercise Science from Youngstown State University and her M.S. and Ph.D. in Exercise Physiology from the University of Pittsburgh.

We discuss practical ways to apply the latest research to support your health and fitness.   Abbie Smith-Ryan, PhD is a professor of exercise physiology and sports nutrition at UNC Chapel Hill. She directs the Applied Physiology Lab and is a fellow of several professional organizations including the International Society of Sports Nutrition (ISSN), the American College of Sports Medicine (ACSM), and the National Strength & Conditioning Association (NSCA). Dr. Smith-Ryan has received numerous awards including Nutrition Researcher of the year and Young Investigator of the Year for the NCSA.   Her research focuses on exercise and nutrition interventions to modify body composition, cardiovascular health, and metabolic function. She is extremely passionate about women's health and has recently been studying the menopause transition.   In this episode, we discuss: Body composition: Why the number on the scale is not the best predictor of health; how body composition is affected by menopause and aging. We are joined by the wonderful Dr. Kelly Kennedy, PhD, CSCS, a subjective matter expert in law enforcement and strength coach. We discuss the evolution of HPO in law enforcement since the beginning, why law enforcement falls under tactical, the importance of giving your client's the opportunity to be successful as well as locking in their drive by narrowing down what makes them feel proud. Kelly Kennedy, PhD is one of the nation's leading experts in fitness training for police. While working for the largest Police Department in the Southeast United States, she has physically trained and tested over 3,700 police recruits and officers since 1999. During this time she has lectured on physical fitness to classes for Police, Corrections and specialized units such as the Special Response Team, Rapid Deployment Force (RDF), and Defensive Tactics Instructor. She lead the design of a large tactical obstacle course, and was a part of a committee to create a pretest physical fitness screening for the members of the RDF. Additionally, she created a physical screening standard for readiness for the Aviation Unit, Marine Patrol Unit and served as the project manager of a team of physiologists in the development of the minimum standards for fitness for the job of Police officer, K-9 unit and Special Response Team.In 2003 she started an education and consulting company called Fit-to-Enforce that takes her all over the country teaching and consulting law enforcement. She has worked as a Subject Matter Expert in physical fitness for the Florida Department of Law Enforcement and has served on the editorial review for the Tactical Strength and Conditioning Report.Kelly is certified with the National Strength and Conditioning Association as a Certified Strength and Conditioning Specialist (CSCS), with the American College of Sports Medicine (ACSM) as a Certified Exercise Physiologist, and Certified from the International Society of Sports Nutrition (CISSN). She is a past certified Level 1 Sport Performance Coach from USAW, for 10 years was a Level 1 CrossFit Instructor, currently Certified as a Defensive Tactics Instructor, member of the International Law Enforcement Educators and Trainers Association, and earned a PhD in Global Leadership from Lynn University.

Today's guest is Injury Rehabilitation and Prevention Specialist Dorota Lewandowski. Dorota takes us through her mission & love for expanding others potential through recovery and movement. Dorota has a Master's of Science in Kinesiology, concentrated in Cardiopulmonary Rehabilitation from CUNY Brooklyn College, a Double Bachelors of Science in Biology and Physical Education from St. Francis College in Brooklyn, NY, and an Associate Degree of Applied Science in Polysomnography from Oregon Institute of Technology. She is a certified Exercise Physiologist through the American College of Sports Medicine (ACSM) and is a board certified Polysomnographic Technologist, or Sleep Disorder Specialist.In this episode she takes us through her own injury recovery journey of breaking her neck while surfing. She shares with us how this experience has allowed her to develop an emotional understanding of recovery and rehabilitation to integrate with her intellectual understanding of it.
