When crises hit, a host of questions arise, among them: Who needs humanitarian aid? How much? Who delivers it? And who has the power to make all of those decisions? How aid agencies and the media choose to frame this information doesn't always help. For the last year, researchers at ODI's Humanitarian Policy Group (HPG) have been trying to understand narratives and the role they play in humanitarian response and policy. What they're finding so far is that human stories are more powerful than data when it comes to influencing change in the sector, and yet humanitarians don't take their role as storytellers seriously enough. In this bonus episode, we get a snapshot of HPG's ongoing exploration of humanitarian narratives from one of its main researchers, and we bring together a local organisation founder, a researcher, and a journalist to discuss the power humanitarians have to shape the stories that affect crisis response. CHRISTINA PREVETTHello, everyone, and welcome to the PT on Ice Daily Show. My name is Christina Prevett. I am one of the lead faculty in our pelvic divisions. And I am coming to you from a hotel room. I'm about to get back after teaching MMOA Live here this weekend. So you got my hotel version of today's podcast. Today we're going to be talking about advocating for birth control. And so this might be a bit of a hot take hot topic, because in the allied health or birth provider space, there has been a lot of anti birth control messaging. And so I want to kind of play devil's advocate a little bit. and speak to some of the potential pros of birth control, and then really try and loop this into why it is so important, especially as healthcare providers, that we become more nuanced in our approach, right? It is so easy with social media for us to be thinking in 30 to 60 second snippets. But one of the reasons why I love the podcast is that we're able to kind of dive into nuance a little bit more. So firstly, the development of the oral contraceptive pill was one of the big revolutionary medical marvels that allowed women to have reproductive choice in a lot of ways, right? The idea behind oral contraceptives was that females could have some, you know, obviously when they're having intercourse, but like they were able to prevent unwanted pregnancies and that gave them some sense of control in a lot of ways. So the development of oral contraceptives outside of barrier methods was truly such an amazing medical advancement that paved the way for a lot in reproductive healthcare. With the use of exogenous hormones, what we have also seen with the use of oral contraceptives is that it has been used in the management of different gynecological conditions. So here's where we get to messaging numero uno. When people take birth control, they aren't actually balancing their hormones. Something is doing it for them and it is a band-aid and it's making all your sex hormones go down. This is the messaging. So we shouldn't be giving people birth control because it's not fixing the problem. So let's talk about an argument where that works, and let's talk about an argument where it doesn't. Okay, so in our pelvic division, we talk a lot about relative energy deficiency in sport. This is for individuals with primary or secondary amenorrhea, where because they are not fueling their body appropriately, their body goes into battery saver mode, which means that they are not doing any bodily processes that require excesses of energy out like energy out because they don't have enough energy coming in, which can include pregnancy. And so we suppress the HPT access to prevent ourselves from ovulating because right now we're not taking in enough fuel for our body to function. We're definitely not taking in enough fuel to support a pregnancy. In those circumstances where individuals are not getting their period because of under fueling, sometimes birth control can be recommended and The argument can be made that. we're not getting at the root cause for the hormonal imbalance, because you need to have that fuel to the root cause, and we should see a hormonal re-regulation, and reds from the literature that we have right now is reversible, right? So that makes sense, right? If individuals are highly active, they're in low energy availability, and they're not screening for root causes of issues with hormone status, and we give birth control as a knee-jerk reaction without doing the proper investigations, I can see where that argument of it's exogenously balancing your hormones would work. But here's where it doesn't. Okay, here's where it doesn't. So birth control is also used as a frontline treatment for a lot of fertility-impacting conditions or gynecological conditions, such as PCOS, endometriosis, and fibroids, right? PCOS, is a androgen excess and it is a chronic disease. It is a chronic disease. It is a chronic disease that has no cure. So there is no cure to be able to balance your hormones naturally with PCOS. Does health promotion potentially help with becoming more regular with your menstrual cycle? Does it help with bringing you to a more regular cycle where you may be more ovulatory with PCOS? Yes. Are you going to change to a, within normal levels, your androgen access? Probably not. So guess what? The birth control pill is being used to bring androgen load down, right? And that is how we treat chronic diseases, right? I don't give a person, oh, I'm not, I'm not a physician, but physicians don't give a person a blood pressure med and we get mad at the physician for giving them a blood pressure med because they're treating the symptom of the high blood pressure, but they're not getting to the root cause of the issue, which is cardiovascular disease, right? These medications are given specifically to manage the symptoms. which is the exact same logic that we are seeing with individuals with gynecological conditions. We are not giving oral contraceptives in order to balance their hormones because they are chronic diseases, right? Outside of excision for endometriosis and fibroids, where we may see a reduction in symptoms, that is not a guarantee. And the only known cure for true 100% cure for endometriosis and fibroids is a hysterectomy. So if we have individuals with a high amount of symptom burden, heck yes, we are going to treat the symptoms, right? And so we can use oral contraceptives to treat those symptoms, right? If I wanted to pull this into our physiotherapy logic, that would be like saying, well, this person has a disc bulge on MRI. If we can't fix the disc bulge and get it back in that spinal alignment, then all of our interventions for pain don't matter because we're not fixing the root cause, right? So, but, PT we say you are not your image like we're not just going to treat you mechanically we're gonna treat how you're feeling within your own body and yet we flip that in our health care providers spaces when we talk about birth control and we make women with heavy menstrual bleeding with heavy periods with individuals who are suffering from fatigue and lethargy because they have anemia we have cyclical pain that could be treated with oral contraceptives and we make them feel bad that they're using it or make them feel fear that they shouldn't be using this because they should be able to balance their hormones regularly and so inadvertently in an attempt to help we're kind of gaslighting them, right? And, and I, I mean this in a very, like, I want to have a fruitful conversation about this because I have seen this messaging over and over and over again. And when individuals have gynecological conditions, birth control can be a management strategy. Should it be a knee-jerk reaction for everybody without the need for further investigation or evaluation? No. Are individuals oftentimes dismissed with birth control because they're not actively trying to get pregnant? Yes. Do some people not tolerate certain types of oral contraceptives or different types of birth control methods? Absolutely. But it is a trial of treatment that has some evidence to back it up. and it can be helpful in some circumstances with some individuals. So having this knee-jerk reaction and saying, well, it's not getting to the root cause or it's not balancing our hormones in the background of a chronic disease with no cure, we are missing the mark on our messaging. And so many of our clients come to us as pelvic PTs and they trust our opinions. And we are trying to lock shields with physicians, not battle with swords. And we need to be mindful of that, that by being very dismissive or not getting to the nuanced approach to contraceptive care or using birth control methods, we are not doing ourselves any favors and we're not helping our clients by not getting into the nuance of it. So the first argument that we see a lot is you aren't balancing your hormones, like it's doing something for you. It's taking your HPG access and bringing it down to nothing, right? That's not always the case and not always the method of oral contraceptives. It can blunt the HPG access, but it doesn't make it go down to zero. And then the secondary piece that individuals have fear on when thinking about oral contraceptives is future fertility. So, There was a cross-sectional study that said that almost 70% of females surveyed were worried about long-term fertility because of oral contraceptive use. We do not have evidence. We actually have multiple systematic reviews and meta-analyses that actually demonstrate that there are no changes in fertility upon cessation of long-term birth control utilization. All right, let me repeat. We do not have evidence that being on birth control negatively impacts future fertility. It does not. What we see is that using hormonal, non-hormonal IUDs, oral contraceptives and patches, the rates of live pregnancy or positive pregnancy rate for contraceptive versus non-contraceptive users in age-matched cohorts appears to be the same. where we can kind of get into this bias, this selection bias, is based on the reason for individuals going on birth control. So if you were a person who went on oral contraceptives in order to prevent pregnancy, but you did not have any fertility related concerns, and that wasn't a factor in your prescription, once you stop taking oral contraceptives, maybe after a couple months things will kind of re-regulate, you should have no future impacts on your fertility. Where you can have downstream fertility related issues is based on the reason for being on those oral contraceptives. So if you are on oral contraceptives for heavy bleeding or cyclical related pain, or hirsutism or clinical androgenism as a consequence of PCOS, we know that PCOS, endometriosis and fibroids can negatively impact your fertility and increase your chance of infertility. So in those situations, because we were treating the symptoms of your condition, we do not have the capacity outside of excision and endometriosis and fibroids to cure these conditions, that downstream fertility consequence is still going to be present upon removing your birth control method or upon removing oral contraceptive use. So it is not the pill itself, it is some of the reasons why you were on the pill that can negatively impact future fertility. And so I have now been talking for about 11 or 12 minutes on the nuance of birth control. The final thing that I will say is it is hysterical to me that the clinicians who are absolutely adamant against birth control for reproductive age individuals, are big advocates for using topical estrogens and hormone replacement therapies, menopausal hormone therapies, for individuals going through the menopausal window, because they are treating the symptoms of menopause, right? We are not trying to fix a person's hormones. We aren't gaslighting them and saying, oh, well, you know, this is your natural aging consequences, so you're just gonna deal with your menopausal symptoms. No, we're at the forefront advocating for topical estrogens and the use of exogenous hormones to be able to help individuals at the end of their reproductive window. So then why are we telling individuals with chronic diseases like PCOS that we can't or shouldn't use, that we should be fearful of using oral contraceptives in their reproductive window when they do not want to be pregnant? Right, and we know that it is a chronic disease that has no cure, and we make them feel bad for treating the symptoms with these exogenous hormones. So we just need to be so careful in our profession about how we are catching onto these trends. I always talk about the fact that I am scrunchy, not crunchy. I am a huge advocate in holistic care. And I think that holistic care can come alongside Western medicine in an evidence-informed way. All of my research is in health promotion, which means that I am in the science-based crunchy. So we just need to be mindful about not having this knee-jerk reaction and saying that birth control is bad. That is the messaging that I'm seeing. And that is absolutely not true. In the messaging, the logic in the messaging is flawed. When we're thinking about gynecological conditions, many of them are chronic conditions that do not have 100% curative rate. PCOS is a chronic disease with no cure. Endometriosis and fibroids can have excision, but the only thing that's going to guarantee that you are not gonna have another growth is a hysterectomy, which is not obviously a viable option for individuals who wanna get pregnant. And therefore, using oral contraceptives for managing signs and symptoms of those conditions is a evidence-informed utilization or medication that people can do. That does not mean that it is for everybody. That does not mean that people can self-select. It's okay for them to self-select away from it. We just wanna make sure that they're getting the right information about what it is and what it isn't. Birth control does not impact your future fertility. We now have multiple systematic reviews and meta-analyses that pending normal reproductive status, normal fertility rates, that we have no infertility-related conditions that there is no difference in conception rates once getting off birth control. And then we are huge advocates for the use of supplemental hormones through menopausal hormone therapy at the end of a person's reproductive window. All right, that was my rant for the day. I hope you guys found that helpful. I really just wanna get into the nuance of this, right? Like we wanna make sure that we are being mindful of our messaging and we are not, inadvertently shaming people or making them fearful or Gaslighting them and saying you don't need birth control you can use all these natural methods When we don't have the same effectiveness data in some of those health promotion technology or health promotion interventions SUMMARY All right You probably wonder why we're deep diving into this. This is because of level two, right? We have a huge role, right? We are doing level two right now for our pelvic course, and we are trying to do fitness-forward pelvic PT in a variety of different conditions. Fertility, baseline fertility, infertility-related conditions, and our role coming alongside those who are going through assisted reproductive technologies is in our curriculum. So we are in the weeds of that research and talking about the ways that we can be involved in rehab. In the intricate dance of human reproduction, a peptide called Kisspeptin plays an important role in guiding the timing of ovulation in women and the production of sperm in men. Its discovery has sparked a wave of excitement in the field of reproductive medicine, offering new insights and potential treatments for those facing fertility challenges.  In this podcast, we'll talk about how kisspeptin works for fertility issues and much more. What is Kisspeptin? Kisspeptin is a neuropeptide that plays a vital role in reproduction, sexual behavior, and attraction. It enhances brain activity linked with sexual arousal and attraction while also prompting the release of gonadotropin-releasing hormone (GnRH) from the hypothalamus. This, in turn, triggers the pituitary gland to release luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which are crucial for testosterone and estradiol production. The controlled stimulation of LH and FSH by Kisspeptin offers a potential advantage over mainstay fertility treatments. This is because it may reduce the risk of ovarian overstimulation often associated with hormonal injections. Because of this, Kisspeptin is currently being studied as a promising alternative to current fertility treatment protocols. Moreover, Kisspeptin's ability to elevate LH and FSH levels may hold promise for men undergoing testosterone replacement therapy (TRT) programs. In cases where exogenous testosterone has led to decreased LH or FSH levels, Kisspeptin could offer a solution to restore hormonal balance. How Does Kisspeptin Work for Fertility Issues? Kisspeptin exerts its effects on fertility primarily through the hypothalamic-pituitary-gonadal (HPG) axis, a complex network of interactions between the brain and reproductive organs. In women, kisspeptin stimulates the release of gonadotropin-releasing hormone (GnRH) from the hypothalamus, which then triggers the secretion of luteinizing hormone (LH) and follicle-stimulating hormone (FSH) from the pituitary gland. These hormones, in turn, regulate the menstrual cycle and ovulation. In men, kisspeptin plays a similar role in stimulating GnRH release, which then prompts the production of LH and FSH, essential for spermatogenesis and testosterone production. Thus, kisspeptin is crucial for both female and male fertility. Utilizing Kisspeptin in Fertility Treatments: For individuals experiencing fertility issues, especially those related to irregular menstrual cycles or anovulation (lack of ovulation), kisspeptin has emerged as a promising therapeutic option. In clinical settings, synthetic forms of kisspeptin are administered via injection to stimulate the HPG axis and induce ovulation in women or improve sperm production in men. Common Kisspeptin Side Effects: While kisspeptin therapy is generally well-tolerated, like any medication, it can cause side effects. Common side effects may include: Injection site reactions: Pain, swelling, or redness at the injection site. Nausea or vomiting: Some individuals may experience gastrointestinal symptoms following kisspeptin administration. Headaches: Mild to moderate headaches have been reported in some cases. Hot flashes: Temporary episodes of feeling flushed or overheated may occur. PKK/PYD terör örgütünün sözde Ölümüzler Taburu'na bağlı intihar timinin Suriye'den illegal yollarla Ankara'ya gelerek İçişleri Bakanlığı'na iki canlı bomba ile saldırması, daha sonra PKK'nın sözde askeri kanadı HPG'den yapılan açıklamada TBMM üzerinden devleti hedef alan tehditkar ve alçakça açıklamaları YPG/ PKK terör örgütü için bardağı taşıran son damla olmuştu. Çünkü bu terör saldırısını kahraman iki polis cevval ve kendi hayatlarını tehlikeye atarak etkisiz hale getirmeseydi sonuçları çok vahim olabilirdi. Eğer teröristler İçişleri Bakanlığı'na girselerdi içeride katliam yapabilir, rehin alabilirlerdi. Kahraman iki polisin teröristleri kısa sürede etkisiz hale getirmesi aynı zamanda terörist saldırıyı sönükleştirmişti. Hatta Fransa PKK'nın Türkiye'de eylem kabiliyetini kaybettiğini bile gündeme getirmişti. Aslında Irak ve Suriye başta olmak üzere bölgedeki ülkelerin toprak bütünlüğünü parçalayarak siyasi yapılarını da değiştirmeyi amaçlayan, Türkiye'yi de içine alan çok ciddi anlamda tehdit eden bir proje yürütülüyordu. ABD ve bazı kolonyalist Avrupa ülkeleri, Süleymaniye merkezli KYB'yi de işin içine katarak bu iki yapıyı birbirine entegre etmeyi başarabilirlerse bu projeden şu ana kadar uzak duran Barzani'yi de baskı altına alarak bu kaos projesine dahil etmeye çalışıyorlardı. Türkiye ısrarla bu konuyu takip ediyor, etmekte de zorunda anlaşılan. Irak ve Suriye'nin toprak bütünlüğünü parçalayarak siyasi veya ideolojik yapısını değiştirmeyi amaçlayan terör örgütünün başat unsur olduğu bir oluşumu ortaya koyarak Türkiye'yi ulusal güvenliğini hedef alan bir tehditle karşı karşıya bırakmaya çalışan güç odakları ile mücadele şimdilerde yapılıyor. Şu an mesela terörle mücadele bu işin içinde var ama bu bir jeopolitik karşılık aslında. Türkiye bu konuda şunu söylüyor: “Burada Türkiye ye karşı kurulan jeopolitik tuzakları entrikaları gördüm, buna karşılık veriyorum.” Şimdi bu noktada ortaya çıkan irade önemli! Neden bahsediyoruz? Sadece SİHA ve savaş uçaklarının etkisi çok farklı bir süreci tetikledi. Biz buraya önce SİHA'lar ile girdik. Bu ne anlama geliyor? Bizim irademizi test ettiler. Sonra SİHA'mızı düşürdüler. ABD'nin SİHA'mızı düşürmekle aslında bize verdiği mesaj netti: “YPG'yi ben burada koruyorum, himaye ediyorum lojistik destek ve eğitim veriyorum, yönetiyorum ve yönlendiriyorum. Bu nedenle sen YPG'yi vuramazsın.” Bu tehdide karşı 5 Ekim gecesi saat 23.00'ten itibaren 50 Km derinlikte Suriye'yi savaş uçaklarıyla vurmaya devam etmesi öncelikle Türkiye'nin etkisini arttıran önemli bir nedendi. Cumhurbaşkanımız Erdoğan'ın “Bir gece ansızın gelebiliriz” düsturu ile kollar halinde jetlerimiz ve akıncı taaruzi İHA'nın da aralarında bulunduğu insansız hava araçlarımız, Tel Rıfat, Cizre ve Derik'teki terör hedefleri bir yıl içinde ikinci kez vurmuş oldu. Cezire Kampı olarak bilinen alanda Haseki'nin üstüne kadar inerek Abdülaziz ve Karaçok Dağları da vuruldu. Bu kez terör örgütünün kullandığı petrol kuyuları depolama tesisleri de hedef alındı. Suriye sınırının 40-50 kilometre derinliğindeki hedefleri F 16'larımız kullanılamaz hale getirdi. Operasyonlarda yerli milli mühimmat kullanıldı. Şu ana kadar 65 hedef imha edildi. Amerika'ya cevap böyle verildi, verilmeye de devam ediliyor.

İçişleri ve Emniyet Genel Müdürlüğü binası önünde nöbet tutan kahraman polislerimiz canlarını yok sayarak BTÖ PKK'lı iki canlı bombacıyı etkisiz hale getirmişlerdi. PKK'lı canlı bombalardan biri polislerimizin müdahalesini görerek kendisini patlatmış diğer canlı bomba da teröristlerle çatışmaya giren polislerimizce öldürülmüştü. Bu terör eylemini planlayan PKK'nın sözde askeri kanadı HPG'den teröristlerin başarısızlığı ve korkaklığını örtmek amacıyla yapılan tuhaf açıklamalar imalar ve tehditler çok enteresan doğrusu. Şöyle ki “Meclis'in açılış gününde, Meclis'e çok yakın bir yerde” saldırının yapıldığının altı çizilirken şöyle dendi: “Ölümsüzler Taburu'nun üyeleri eğer isteseydi eylem saatinde küçük bir değişiklik yaparak çok daha farklı sonuçların oluşmasına yol açabilirdi. Ancak bilinçli bir biçimde böyle bir tercih yapılmayıp, esas amaç olan; ilgili yerlere gereken mesajın ulaştırılması ve ciddi anlamda uyarılması hedefine bağlı kalınmıştır.” Bu yalana kimse inanmaz çünkü birkaç kahraman polis karşısında korkarak kendisini patlatan, çatışmada öldürülen sözde Ölümsüzler Taburu teröristleri şüphesiz terör eylemini gerçekleştirmek için TBMM önünden geçtiğinde Özel Harekat başta olmak üzere onlarca kahraman polisi görerek korkup Emniyet Genel Müdürlüğü önüne giderek korku içinde öldüklerini sanıyorum. İÇİŞLERİ BAKANI ALİ YERLİKAYA TERÖR SALDIRISINDAN KISA BİR SÜRE SONRA ŞU AÇIKLAMAYI YAPTI İçişleri Bakanı Yerlikaya, olaydan birkaç saat sonra İçişleri Bakanlığı önünde kameraların karşısına geçerek son duruma ilişkin bilgi verdi. “Şükürler olsun kahraman polisimizin cevvaliyeti, kararlılığı, sezgisiyle teröristler araçtan iner inmez onlara silahla mukavemette bulunmuş, bu esnada teröristlerden biri zaten canlı bomba düzeneği var, kendini patlatmış, diğeri kendini patlatmaya fırsat bulamadan başından vurulmuş, etkisiz hale getirilmiştir. Olayın ardından polisler tarafından etkisiz hale getirilen teröristin üzerinde de canlı bomba yeleği tespit edildi. Yoğun güvenlik tedbirlerinin alındığı bölgeye sevk edilen bomba imha ekiplerince teröristin üzerindeki bomba kontrollü şekilde çıkarıldı ve fünye ile patlatılarak imha edildi. Ankaralılara geçmiş olsun” dileklerini ileten Yerlikaya, saldırıya karşılık veren polisleri kutladı. İÇİŞLERİ BAKANLIĞI'NI HEDEF ALAN ÇİFTE CANLI BOMBA SALDIRISINI PKK'NIN SÖZDE ASKERİ KANADI HPG ÜSTLENDİ Ankara'da İçişleri Bakanlığı'na yönelik düzenlenen terör saldırısını PKK'nin sözde askeri kanadı HPG üstlendi. Saldırının doğrudan PKK lideri Murat Karayılan'a bağlı sözde “Ölümsüzler Taburu” adlı grup tarafından düzenlediği kaydedildi. Açıklamada saldırının “Rojhat Zilan” ve “Erdal Şahin” kod adlı üyeler tarafından gerçekleştirildiği bilgisine yer verildi. People often use terms such as "slow" or "fast" metabolism, or even "broken" metabolism, to describe their own metabolic status without really understanding what that means. It's natural to throw those terms around when other variables impact your body weight, exercise, or non-exercise activity. The metabolism is malleable and adaptive based on the environment, diet, exercise, health history, and other criteria. So it all depends on your choices and daily actions. Menopause and metabolic changes Hormonal fluctuations of testosterone, progesterone, estrogen, and cortisol occur during perimenopause and menopause, leading to an increase in body fat and a decrease in muscle mass. As a result, women undergoing these changes must pay close attention to their body's transformation and modify their training, nutrition, and lifestyle accordingly. High-intensity training without adequate nutrition during perimenopause or menopause can result in metabolic disruption. Engaging in resistance training, walking, and reaching step goals are the most effective ways to maintain optimal metabolic balance during these stages. Calories in versus calories out - myth or reality? Weight loss or gain is not only about calories in or out. Hormones also play a significant role. When we consume food, the number, type, and spacing of the calories and the quantity of energy coming into the system will impact the system from a hormonal perspective. If you're following a standard American diet, which is calorically dense but not micronutrient dense, that will influence your hormones. But hormones also impact everything from hunger and cravings and blood sugar regulation to when you want that next meal, both the hedonic and homeostatic eating systems in terms of the food decisions you're making daily. So find a balance between calorie control and hormone optimization to reach your goals. Is dieting good for you? Often, individuals seeking to reduce their body fat levels resort to drastically lowering their calorie intake. While this approach initially appears logical as it aligns with conventional weight loss education, it often lacks a sufficient runway and an exit strategy. Consequently, weight regain occurs, and upon resuming dieting, metabolic adaptation sets in, leading to elevated cortisol, downregulation in the thyroid, and the HPG axis. Balancing the stimulus and making gradual changes to nutrition is key to successful dieting without overdoing it. Moreover, a well-defined exit strategy is essential to avoid compromising one's metabolic function over the long term. Last week, roughly 5,000 liver community stakeholders gathered in London for the 2022 International Liver Congress (#ILC2022,) the first major hepatology Congress to be held in person since the start of the pandemic (smaller, but very valuable, meetings like NASH-TAG, LiverCONNECT and Paris NASH have taken place with an in-person component, but the International Liver Congress and The Liver Meeting have not). On the last full day of the program, several vitally important drug development studies were presented during the late-breaker and dedicated sessions. The conversations in this episode will review some of the most important findings. This particular conversation discusses two presentations on resmetirom, the late-breaker safety results (and some preliminary, secondary efficacy markets) from the Phase 3 MAESTRO NAFLD-1 trial, and a separate study looking at 180 cirrhotic patients in the open-label cohort of MAESTRO.MAESTRO NAFLD-1 was a double-blind, randomized, placebo-controlled trial of 80 or 100 milligrams of resmetirom versus placebo, with close to 1,200 patients in these three cohorts. There was a fourth 100mg open-label cohort as well, with 180 cirrhotic patients among the trial population. For MAESTRO NAFLD-1, the primary endpoint was safety. In addition, there were several key secondary endpoints, including pathogenic lipids, and certain MRI characteristics, along with FibroScan (both KAP and KPA). To primary author Harrison, the "take-home point" from this study was "there was no bad news from MAESTRO NAFLD-1, and it did meet its primary endpoint with all the key secondary endpoints under hierarchical control having statistical significance as well. One more interesting point from this presentation. Because the study was fielded during the COVID-19 pandemic, patients in the three closed-label arms missed an average of two months of doses due to pandemic-related logistical challenges. Despite this, the secondary endpoints demonstrated efficacy, although some demonstrated reduced efficacy compared to what we would expect from a full course of therapy.Stephen then shares significant data from the first 105 patients in the open-label cirrhosis group. Without going into detail, the "take-home point" here is that "in addition to the safety and tolerability... we were moving noninvasive tests in a way that made us feel like something positive was happening relative to maybe some early portal hypotensive changes." Stephen notes the caveat that the researchers performed neither endoscopy nor HPG measurements in this population.

As part of the celebration of the new release of Get Your Period Back Playbook available as an anytime purchase, I wanted to record today's episode on a component of hypothalamic amenorrhea (HA) that we have yet to discuss, and that's how excessive exercise contributes to HA.   The hypothalamic-pituitary-gonadal axis or HPG axis refers to the hypothalamus, pituitary gland and gonadal glands almost as if they are a single entity. This axis plays a critical role in the development and regulation of several different body systems like reproduction and immunity. The HPG axis is what controls the cascade of your hormones releasing to create your monthly menstrual cycle, and also what can stop it from happening altogether, too. Remember, HA is when the brain (specifically the hypothalamus) turns “off” the ovulation switch.   The HPG axis is extremely sensitive to the environment. And today we are going to address this from a lens of excessive exercise.   Exercise has benefits no doubt. In a very basic explanation, exercise teaches our bodies to adapt to stress. It's actually a form of stress to the body, typically viewed as a positive stressor, teaching our bodies how to handle stress more effectively.   But when the allostatic load is too much, this positive stressor becomes a negative stressor. And when we have numerous stressors in our lives, from exercise but also our daily lives, work, family, our health habits, and mental stress...it becomes too much stress on our bodies.  The ASX 200 fell 123 points to 6838 (-1.8%) as the correction becomes official. After a promising start the market got whacked from the twin forces of rising 10-year yields above 2% and the US futures turning down on the Fed press conference fall out. The taper tantrum is alive and well. From a 60 rally to a 160 fall today, it was once again vicious as sellers ambushed the market with the Big Bank Basket falling modestly to $168.26 (-0.9%) and other financials also under pressure. MQG off 3.1% and NWL down 4.2%. Healthcare was put into ICU today as CSL tumbled 3.5%. The SPP closes 7th February and has a discount to VWAP option which is now looking interesting. SHL fell 2.1% and RMD off 3.9%. Industrials across the board felt the brunt of the selling, WES down 3.7%, TCL down 4.0% as bond yields rose above 2%, TLS down 1.3% and REITs in a hole, GMG off 3.7%. Even boring consumer staples struggled, COL down 3.1% and WOW falling 2.1%. Tech under serious pressure as WTC fell 9.9% and SQ2 continued to fall down another 5.4% with the Index down 5.1%. In resources, iron ore majors held up, BHP up 1.4% and RIO up 2.1% but gold miners not so lucky getting smashed with EVN down 11.3% and NST off 8.2%. Looks a little overdone considering the bullion move. It is sell first ask questions later. Quarterlies didn't help matters. Lithium stocks remain depressed, PLS down 2.8% and MIN off 5.6%. Energy stocks a rare patch of green with STO up 3.6% and WPL better by 2.5% despite a Myanmar withdrawal and a $300m write off. In corporate news, PMV updated the market on its success in ‘Smiggling' the world  up 2.1% and KGN revealed it was having a tough time making money. Again down 12.3%. GMA got a boost from renewing its CBA contract for mortgage insurance, rising 13.8%. That might come in handy if rates rise. HPG became LoPages after a quarterly update sent the shares down 16.0% and PLT lived up to its name on a positive quarterly up 8.3%. On the economic front, NZ inflation growing at fastest rates in 31 years. Asian markets down heavily. 10 year yields up to 2.02% Why not sign up for a free trial?
