Without that slightly dodgy pseudo-scientific reasoning, Copernicus and Newton wouldn't have "fixed" and refined it. - Newton invented calculus and his three laws. Do you consider the new Physics Project developing a new tool like calculus, or are you inventing new laws? - Why was gravitoelectromagnetism ignored for so long? - ​I disagree with Wolfram. I think there is no "end" to how infinitesimal the universe gets. There's always more—the discoveries will never end. - I understand a great deal of human history was lost in ancient Alexandria. Modevetaren Susanna Strömquist har djupdykt i historien om den exklusiva syateljén och modesalongen NKs franska damskrädderi, som fanns i närmare 60 år. Hon har grävt i garderober och arkiv och fått låna festblåsor och andra kläder från personer över hela landet. På NKs franska, ett av de mer exklusiva damskrädderierna, köpte kvinnor från societeten, kungligheter och artister sina kläder, men också yrkeskvinnor som behövde kläder av hög kvalitet i yrkeslivet. Hit kom dammode från Paris som sedan påverkade modet i hela Sverige. Det handlar om haute couture och couture, måttsydda, handsydda kläder som bara syddes upp i enstaka exemplar. Sömmerskorna träffade aldrig kunderna Och det handlar också om två skilda världar, en lyxig ateljé med mjuka mattor där kunderna vistades, och ett skrädderi där sömmerskorna sydde. De två världarna möttes aldrig och den enda länken mellan dem var måttagerskan, som träffade både kunder och sömmerskor. Forskning i sista minuten Nu har Susanna Strömquists forskning blivit en tjock bok om när Paris kom till Stockholm, och en utställning som just nu pågår på Nordiska museet. Och det var verkligen forskning i sista minuten, för det finns inte så många kvar i livet, som varit kunder eller anställda. Men en dam född 1927 kom in med sin röda cocktailklänning (se bild) som hon hade låtit sy upp på 1960-talet. We have to believe all experts after all - There are plenty of Other experts – who have differing opinions Dr Joel Kettner- former Chief Public Health Officer for Manitoba province (Canada between Saskatchewan and Ontario) and Medical Director of the International Centre for Infectious Diseases – must be worried right? What he says: I have never seen anything like this, anything anywhere near like this. – Sounds bad right? I’m not talking about the pandemic, because I’ve seen 30 of them, one every year. It is called influenza. And other respiratory illness viruses, we don’t always know what they are. But I’ve never seen this reaction, and I’m trying to understand why. I worry about the message to the public, about the fear of coming into contact with people, being in the same space as people, shaking their hands, having meetings with people. I worry about many, many consequences related to that. In the province of Hubei, where there has been the most cases and deaths by far, the actual number of cases reported is 1 per 1000 people and the actual rate of deaths reported is 1 per 20,000. So maybe that would help to put things into perspective. But it might be very deadly – or infectious – Dr Sucharit Bhakdiis a specialist in microbiology - head of the Institute for Medical Microbiology and Hygiene and one of the most cited research scientists in German history What he says: We are afraid that 1 million infections with the new virus will lead to 30 deaths per day over the next 100 days. But we do not realise that 20, 30, 40 or 100 patients positive for normal coronaviruses are already dying every day. [The government’s anti-COVID19 measures] are grotesque, absurd and very dangerous. The life expectancy of millions is being shortened. The horrifying impact on the world economy threatens the existence of countless people. The consequences on medical care are profound. Already services to patients in need are reduced, operations cancelled, practices empty, hospital personnel dwindling. All this will impact profoundly on our whole society. All these measures are leading to self-destruction and collective suicide based on nothing but a spook. What about Italy? Dr Yoram Lassis an Israeli physician, politician and former Director General of the Health Ministry. He also worked as Associate Dean of the Tel Aviv University Medical School and during the 1980s presented the science-based television show Tatzpit. What he says:Italy is known for its enormous morbidity in respiratory problems, more than three times any other European country. In the US about 40,000 people die in a regular flu season and so far 40-50 people have died of the coronavirus, most of them in a nursing home in Kirkland, Washington. In every country, more people die from regular flu compared with those who die from the coronavirus.…there is a very good example that we all forget: the swine flu in 2009. That was a virus that reached the world from Mexico and until today there is no vaccination against it. But what? At that time there was no Facebook or there maybe was but it was still in its infancy. The coronavirus, in contrast, is a virus with public relations. Whoever thinks that governments end viruses is wrong.   So why are scientists referring to it as COVID19 - Dr Wolfgang Wodargis a German physician specialising in Pulmonology, politician and former chairman of the Parliamentary Assembly of the Council of Europe. In 2009 he called for an inquiry into alleged conflicts of interest surrounding the EU response to the Swine Flu pandemic – where some members of their board represented the companies producing the vaccines What he says: Politicians are being courted by scientists…scientists who want to be important to get money for their institutions. Scientists who just swim along in the mainstream and want their part of it […] And what is missing right now is a rational way of looking at things. We should be asking questions like “How did you find out this virus was dangerous?”, “How was it before?”, “Didn’t we have the same thing last year?”, “Is it even something new?” That’s missing. What are we spooked of – Covid19 – how did this get its naming – COronaVIrus Disease 2019. From the director-general of the WHO: “Coronavirus” refers to the family of viruses that the disease belongs to and is named for its crown-like shapes under a microscope - “corona” comes from the latin word for “crown” – then 2019 is the year In naming - “We had to find a name that did not refer to a geographical location, an animal, an individual, or a group of people, and which is also pronounceable and related to the disease,” But what is the name given to it within the medical community: SARS-CoV-2 Coronavirus Study Group of the International Committee on Taxonomy of Viruses - pathogen is “a sister to severe acute respiratory syndrome coronaviruses,” hence the “SARS” in the name But the WHO is already pushing back on using the name SARS-CoV-2 for the virus - “From a risk communications perspective, using the name SARS can have unintended consequences in terms of creating unnecessary fear for some populations. For that reason and others, in public communications WHO will refer by using: ‘the virus responsible for COVID-19’ or ‘the COVID-19 virus,’ but neither of these designations is intended as replacements for the official name of the virus.” - Which has SARS at the front – or as scientists call it SARS – 2 - It is essentially SARS 1 and SARS 2 - Hendrik Streeckis a German HIV researcher, epidemiologist and clinical trialist. He is professor of virology, and the director of the Institute of Virology and HIV Research, at Bonn University. What he says: The new pathogen is not that dangerous, it is even less dangerous than Sars-1. The special thing is that Sars-CoV-2 replicates in the upper throat area and is therefore much more infectious because the virus jumps from throat to throat, so to speak. But that is also an advantage: Because Sars-1 replicates in the deep lungs, it is not so infectious, but it definitely gets on the lungs, which makes it more dangerous. You also have to take into account that the Sars-CoV-2 deaths in Germany were exclusively old people. In Heinsberg, for example, a 78-year-old man with previous illnesses died of heart failure, and that without Sars-2 lung involvement. Since he was infected, he naturally appears in the Covid 19 statistics. But the question is whether he would not have died anyway, even without Sars-2. So it is branding – they don’t want to say it is just another form of SARSs – But even Taking what we are being told at face value- it is a new deadly type of coronavirus – Like SARS, MERS or what was a fictional disease (CAPS) use in a ‘war games’ scenario called Event201 –   I like to read and research events – and found some interesting works by a few groups – based around hypothetical scenarios of a virus outbreak – similar to war games, or model UN projects play out – Occurred in October 2019 – Event 201 simulates an outbreak of a novel zoonotic coronavirus transmitted from bats to pigs to people that eventually becomes efficiently transmissible from person to person, leading to a severe pandemic. The pathogen and the disease it causes are modeled largely on SARS, but it is more transmissible in the community setting by people with mild symptoms. Event 201 – A global pandemic exercise - CAPS Pandemic  - Official Statement - Selected moments from the Event 201 pandemic tabletop exercise hosted by The Johns Hopkins Center for Health Security in partnership with the World Economic Forum and the Bill and Melinda Gates Foundation on October 18, 2019, in New York, NY. The exercise illustrated the pandemic preparedness efforts needed to diminish the large-scale economic and societal consequences of a severe pandemic. Going back further - Operation Lock Step – The Rockefeller Foundation 2010 Report Title - “Scenarios for the Future of Technology and InternationalDevelopment- Covers a scenario called “LOCK STEP: A world of tighter top-down government control and more authoritarian leadership, with limited innovation and growing citizen pushback.” This about most countries in the world – more government top down control – not much happening with the economy – innovation was being stifled – lots of protests and civil unrest – yellow vests – France and EU – protests in Hungary over shutting their borders, Wuhan over environment, HK over instilling their own rulers – lots of unrest – Similar to where the world was just 3 months ago Scenario - “the pandemic that the world had been anticipating for years finally hit. Unlike 2009’s H1N1, this new influenza strain — originating from wild geese — was extremely virulent and deadly. Even the most pandemic-prepared nations were quickly overwhelmed when the virus streaked around the world, infecting nearly 20 percent of the global population and killing 8 million in just seven months…The pandemic also had a deadly effect on economies: international mobility of both people and goods screeched to a halt, debilitating industries like tourism and breaking global supply chains. Even locally, normally bustling shops and office buildings sat empty for months, devoid of both employees and customers.” This sounds eerily familiar. Then the scenario gets very interesting: “During the pandemic, national leaders around the world flexed their authority and imposed airtight rules and restrictions, from the mandatory wearing of face masks to body-temperature checks at the entries to communal spaces like train stations and supermarkets. Even after the pandemic faded, this more authoritarian control and oversight of citizens and their activities stuck and even intensified. In order to protect themselves from the spread of increasingly global problems — from pandemics and transnational terrorism to environmental crises and rising poverty — leaders around the world took a firmer grip on power.” What was this scenario – one of panic and fear – if enough panic and fear can be forced onto populations – the more they will allow their own freedoms to be reduced for safety Safety over freedom - Fear is never a good guide to sound reason. Many people would gladly surrender some of their liberties, in exchange for peace and stability. This is what the game-plan is all about. How much liberty will people give up? Too much hype and limited facts; the conclusion is that there is 99% fear, and 1% facts. The main stream media are scant with information. Experts opinion on the subject - Dr Peter Goetzscheis Professor of Clinical Research Design and Analysis at the University of Copenhagen and founder of the Cochrane Medical Collaboration. He has written several books on corruption in the field of medicine and the influence of big pharmaceutical companies. What he says: Our main problem is that no one will ever get in trouble for measures that are too draconian. They will only get in trouble if they do too little. So, our politicians and those working with public health do much more than they should do. No such draconian measures were applied during the 2009 influenza pandemic, and they obviously cannot be applied every winter, which is all year round, as it is always winter somewhere. We cannot close down the whole world permanently. Should it turn out that the epidemic wanes before long, there will be a queue of people wanting to take credit for this. And we can be damned sure draconian measures will be applied again next time. But remember the joke about tigers. “Why do you blow the horn?” “To keep the tigers away.” “But there are no tigers here.” “There you see!” – Simpsons did this with a rock - I find these Hypothetical scenarios very accurate to how events have played out – if nobody had tuned into the news last few weeks – and hadn’t talked to anyone – wouldn’t know anything is going on beyond Government announcement to policy changes – living off grid that isn’t reliant on the system Thought of something very interesting – None of us have a say – Government does the measures – nobody asked if we would like to keep our jobs – they are deemed a hazard and cancelled – The Gov officials still get to keep their jobs – in QLD we are still being forced to go vote this Sat – using social distancing which will be enforced though – for our protection of course – same pencil   The question remains – why would any government destroy the freedoms of the people it is meant to protect – but instead we are treated like children with no say – but in this case the kids are paying the adults – similar to child celebrity parents – didn’t end out so well for calkin – Psychological effects of this – everyone becomes afraid of each other – told to socially distance – other people are the carries and the threat – don’t be close to communities you can rely on – isolate – sperate – fear other people and trust us to protect you Social snitching – where we are turned against one another – dobbing in those having people over = house parties or going outside – similar to Stalin, Mao’s and NKs populations – use social shame as a way for people to self police Based around the normalisation of these measures – what stops this occurring any flu season or the outbreak of a virus – flu season basically lasts all year due to the seasons – if the experts say we need to – people may be likely to follow Thank you for listening to today's episode. Vårdskandaler, vårdsatsningar och vårdpartier har dominerat den senaste veckans politiska samtal. Och hur länge kommer FI:s nya partiledare hålla ut? Valrörelsen är igång och diskussionen om NKS satte tonen för vårddebatten som satte fart förra veckan. Nu tävlar partierna om vilket som vill satsa mest på världens bästa vård.  På onsdagen slog företagshälsovården Helsa fast att talman Urban Ahlin (S) från inte utsatt medarbetare för trakasserier eller kränkningar, något han anklagats för. Innebär det att historien avklarad eller finns det en fortsättning?  Och efter helgens FI-kongress blev det också klart att Gita Nabavi blir den nya partiledarhalvan för partiet. Vem är hon, vad vill FI med sin politik och vad klargörs egentligen med partiets nya valfilm? Sällan har det väl bråkats så mycket om ett sjukhusbygge som Nya Karolinska i Solna. Det är också den hittills största konstsatsningen för offentlig miljö i Sverige med en budget på 118 miljoner. Lyssna på tidigare delar i reportageserien om offentlig konst - länkar under artikeln Projektet innefattar ett 60-tal fasta utsmyckningar och ett tusental lösa konstverk. Det mesta är förstås förbehållet patienter och personal, men några av dom mest påkostade verken kommer att finnas för allmän beskådan i fasader och entréhallar, när sjukhuset väl öppnar någon gång i år. I november 2015 lotsas en liten skara journalister för första gången runt i det blivande Nya Karolinska, alla är vi klädda i hjälmar och skyddskläder, precis som dom intensivt sysselsatta byggjobbare vi kryssar emellan. Glaskonstnären Fredrika Linder visar sina avgjutningar av grenar som hänger i ett 40 meter djupt ljusschakt mitt i byggnaden, grenarna är självlysande och skiftar färg över dygnet: Naturen återkommer på många håll i dom verk som animerar dom djupa ljusschakten. Även danska Astrid Krogh har i sitt schakt hängt stora runda ljustavlor med himlen som motiv, en himmel som förstås också skiftar färg efter solens rörelse. Ingen kan vara riktigt säker på himlens utseende, särskilt inte i här uppe i Norden, menar Astrid Krogh, vars ljustavlor sakta skiftar från djupblått till solnedgångsrött och gult som gjort för att sitta och meditera vid, om man är patient och kanske vill ha något annat att tänka på. Den mest framträdande platsen är tveklöst den höga vägg mot ett hisschakt i den 60 meter långa entréhallens fond, en vägg dit alla besökares blickar kommer att dras, och där Andreas Erikssons jättemålning Stenbrottet hänger, 11 meter hög och 6,5 meter bred, alltså en drygt 70 kvadratmeter stor duk att bemåla i en industrihall utanför Lidköping, och det fick ta tid. Tre år fick han på sig. Att man påverkas av den speciella sjukhusmiljön visar konstnären Anna Åstrand, som satt upp avgjutningar av vardagliga föremål som madrasser, kartonger och rep på väggar och pelare ovanför entréhallen. Briljant ingenjörskonst eller vettlöst krångel? Snart behandlas första patienten på jättesatsningen Nya Karolinska. Men hur många vet att sjukhuset också genomför en omfattande förändring på insidan? Om två veckor ska första patienten behandlas på Nya Karolinska sjukhuset - ett projekt som har planerats i 15 år och är ett av de dyraste sjukhusbyggena i världen. Om det har det skrivits åtskilligt.Men hur många vet att Karolinska samtidigt genomför en väldigt stor omorganisation?Kaliber idag om ett pressat sjukhus.Klinikerna, som i över hundra år har varit navet i sjukhusorganisationer, ska upphöra. I stället ska vården organiseras efter så kallade flöden, specifika patientgrupper. En organisation som är helt ny och för många svår att förstå.Förhoppningen är att den nya verksamhetsmodellen också ska hjälpa till att lösa de ökande problemen sjukhuset har med stängda vårdplatser och växande köer på flera områden.Nyheter i SVT:I nio månader har Linnea Nilsson väntat på att hennes mamma ska få en tid på Karolinska Solna. Operation har planerats fyra gånger men aldrig blivit av. Men som det är nu så måste hon vara extremt försiktig, eftersom det går ett hål rakt in i hjärnan. Så då får man gå med hjälm på.Inslaget i SVT är från i våras. För den som är uppmärksam framgår att Karolinska har något viktigt på gång. Vi hör divisionschef på neurologmottagningen Göran Günther. Göran Günther i SVT: Vi hoppas att en ny verksamhetsmodell som Karolinska nu går in i inom kort, en helt ny organisation, kommer att förbättra vården för patienterna och även arbetsmiljön.Värdebaserad vårdDen 5 juli är sjukhusledningen i Almedalen och presenterar sin nya organisation. Inspelningen kommer från Dagens Medicin Agenda och Moderator är Christina Kennedy:-Det är tufft i vården just nu. Ni har svårt att rekrytera personal. Kommer det här nya sättet jobba, hur kommer det att hjälpa till att lösa de problemen? - Absolut, annars vi aldrig gjort det här. Produktionsdirektör Andreas Ringman Uggla från den här inspelningen:- Det som är väldigt tydligt när man gör ett sånt här omvälvande förändringsarbete är att det finns väldigt många stenar att lyfta på.Och sjukhusdirektören Melvin Samsom är där och visar en presentationsfilm.To illustrate what we are doing I would like to show you a short film. Ur presentationsfilmen från Karolinska:Vi på Karolinska universitetssjukhuset vill öka värdet för våra patienter genom att skapa en ny modell. Huvudstrategin som vi bygger vår nya verksamhetsmodell på är därför värdebaserad vård.Det hela bygger på en amerikansk managementteori. En teori som, förenklat, handlar om hur man kan förbättra kvaliteten i vården och samtidigt spara pengar. Vi ska återkomma till det.I Almedalen finns även representanter från Sahlgrenska I Göteborg och Akademiska i Uppsala som berättar om sina erfarenheter av värdebaserad vård. De har infört konceptet i liten skala, och de har kvar sin vanliga organisation.På Karolinska går man nu betydligt längre. Sjukhusledningen har kommit fram till att det bästa sättet att införa värdebaserad vård, är en total omstöpning. Karolinska ska därför avskaffa sina kliniker och organisera vården på ett helt annat sätt. Produktionsdirektör Andreas Ringman Uggla förklarar i seminariet från Dagens Medicin Agenda:Det vill säga, slutar att kämpa emot en organisationsstruktur som inte är gjord för att utföra det arbete vi vill utföra, och vrider om den så att vi i stället kan ligga och harmonisera.Ledningen vill skapa en organisation där man kan följa patienternas väg i vårdkedjan och lättare mäta och förbättra kvaliteten. Vinsten för patienterna ska vara att de får bättre vård, och samtidigt ska sjukhuset komma tillrätta med de problem man brottas med.Från Karolinskas presentationsfilm:Vi vill också möta våra utmaningar med en fragmenterad organisation, ojämn vårdkvalitet och återkommande problem med ekonomin.Samma dag som den nya verksamhetsmodellen presenteras i Almedalen domineras nyheterna därhemma av att Karolinska tvingas ställa in alla icke-akuta operationer.Det finns en växande kritik mot att sjukhusledningen sätter en sådan tilltro till ingenjörskonst och teoretiska modeller Ja, man kan ju jämföra det med att många just nu upplever att de drunknar, en av kritikerna, läkaren Astrid Seeberger.och till en organisation som ingen vet hur den kommer att fungera: Det är klart att det går att göra annorlunda, men att göra allt detta på en gång, samtidigt som man flyttat in i nya lokaler, ser jag som ett väldigt, väldigt stort riskprojekt, säger förre sjukhusdirektören Birgir Jacobsson.Det här är det första av två program om den stora omvandlingen på Karolinska.Vi är på njurmedicinska kliniken i Huddinge. Den ska upphöra och tillsammans med endokrinkliniken bilda temat infektion och inflammation. Exakt vad det innebär är svårt att sammanfatta. Målet är att kompetensen ska följa patienterna i högre utsträckning än i dag.Om kliniken som finns i dag samlar en rad olika diagnoser och symptom som är kopplade till njurarna, blir den nya organisationen i flöden mer specifik. Vissa läkare och specialistsköterskor kanske enbart kommer arbeta med kronisk njursvikt, andra kanske enbart jobba på en dialysavdelning. Vi säger kanske, eftersom alla formella beslut ännu inte är tagna.Kritiken är alltså utbredd här. Astrid Seeberger, överläkare i njurmedicin, säger att det finns en stor frustration på hennes arbetsplats just nu. Det saknas vårdplatser. Det saknas sköterskor, det saknas läkare. Varje vecka slutar folk, för de orkar helt enkelt inte längre. I ett sånt, läge att då införa en ny modell, som kräver väldigt många möten, en enorm administrativa belastningen, det skapar en stress för personalen som redan är så stressad.Många vi pratar med beskriver hur omställningsarbetet har prioriterats före allt annat senaste året. Hur chefer och personal måste lägga ned mycket tid varje vecka på möten och utbildning. Hundratals workshops har hållits med upp till 15 medarbetare på varje. Dessutom pågår ett ständigt rekryteringsarbete. Nu när hela strukturen görs om i grunden, ska totalt 700 chefstjänster tillsättas, för att ersätta dem i den gamla organisationen.Peter Bárány är just nu verksamhetschef för Njurmedicin. Han säger att alla möten som hålls sätter en stor press på arbetet. Det finns en stor oro för att vi inte ska hinna göra det här på ett bra sätt. Det här läggs ju ovanpå det övriga arbetet, och det är ett stort problem.Hur många timmar sitter du? Jag sitter minst 25 timmar i veckan. Men för en läkare att hitta 3-4 timmar för ett möte i veckan, den tiden är svår att hitta.Och fortfarande återstår en del att förstå när det gäller hur det kliniska arbetet med patienterna ska fogas in i den nya verksamhetsmodellen.Peter Bárány verksamhetschef njurmedicin Huddinge: Det som blir komplicerat är ju att vi har många patienter med flera olika sjukdomar. Man har ju i modellen tänkt att man bara ska vara med i ett flöde, men vi har ju patienter som har två eller tre allvarliga sjukdomar och vilket flöde ska man vara i? Ska man vara med i alla tre eller hur det ska fungera för dem? Det är för mig oklart idag.Vi har försökt att få en uppskattning av kostnaderna för det här omvandlingsarbetet. Det har inte varit lätt. Mycket av kostnaderna tas från Karolinskas ordinarie budget, som är 16 miljarder. Det finns också en omställningsbudget som i år är på 600 miljoner. De pengarna går till programledning, att införa nya arbetssätt och till driftsättning av det nya sjukhuset.En konkret siffra vi får från presstjänsten på Karolinska, är att man hittills i år betalat 26 miljoner till det konsultbolag som hjälper till med den nya verksamhetsmodellen. När vi talar med flera chefer och experter, både tidigare och nuvarande anställda, anser de att den totala kostnaden för värdebaserad vård och ny verksamhetsmodell, måste röra sig om hundratals miljoner, i direkta och indirekta kostnader. De beskriver hur Karolinska, med sina 16 000 anställda, förutom att bli klart i tid till flytten, fokuserar helt och hållet på den nya verksamhetsmodellen.Som vi berättade i början av programmet så har Karolinska sedan i våras haft växande problem med att hålla vårdplatser öppna och hålla uppe vårdproduktionen på planerad nivå. Det går knappt en månad utan att media rapporterar om allvarliga händelser.  Från P4 Stockholm:Allting flöt på som det brukar göra, och Hugo sövdes. Och vi får alltså reda på att operationen är inställd klockan 14, när Hugo redan liggerMagnus Zetterström berättar för P4 Stockholm om när sonen skulle opereras på Astrid Lindgren.Jag förstod inte vad det hela berodde på. De pratade om akutplatser, vårdplatser. Jag ställde frågan till läkaren, så du kan vi åka hem? Alldeles riktigt, Ni kan packa väskan och åka hem.Melvin Samsom från Holland är sjukhusdirektör för Karolinska sedan två år. Vi frågar honom i vilken utsträckning de ökande problemen med platsbrist och köer är kopplade till det väldiga omställningsarbetet. It has to do with specific shortage of professions that we need to have. For instance nurses for the evenings and in the nights. Sometimes also operating nurses.Enligt honom finns inget sånt samband. Dippen i vårdproduktion handlar om andra saker; brist på specifik kompetens, exempelvis sjuksköterskor på kvällar och nätter, och för operation. The other part is moving out care, so called poliklinisering, so that we do it in day care, so to answer your question, no it is not related. Och det handlar om vård som flyttas ut från sjukhuset, och en ökad satsning på dagkirurgi, säger han. That we are doing many things at the same time, that is true, of course.Att vi gör många saker på en gång, det stämmer, säger han. Men NKS lokaler är designade för att främja en tematisering, en sammanslagning av klinikerna. NKS is built to facilitate a thematical organisation.Om vi skulle flytta in med den gamla klinikstrukturen, så skulle vi sedan behöva lägga ett par år på omorganisation, så därför har vi beslutat att det är bäst att göra det nu. So we have decided within the leadership group that this is the best time to change it. Men att de nya lokalerna är designade för en radikal omorganisation där man river ner klinikstrukturen, håller Birgir Jakobsson inte med om. Den förre sjukhusdirektören, som 2012 inledde Karolinskas förberedelser för NKS, gjorde då en helt annan bedömning. Jag var väldigt tydlig med det. Det talades om en tematisering och det bejakade jag. Det fanns ju redan en viss tematisering på sjukhuset. Jag sa: vi behåller den. Vi behåller klinikerna. Vi tänker inte göra en stor omorganisation. Vi ser ju det, att folk blir förvirrade, verksamheten tappar tempo, folk letar efter sina roller och efter nya samarbetspartners. Det sa jag redan från början, att vi gör inte det riskprojektet!Om jag skulle säga att du är bakåtsträvare, hur skulle du försvara dig då? Absolut, du kan ha den åsikten. Men jag har jobbat inom sjukvården i 35 år och jag brinner för att förbättra och förändra sjukvården så att kvaliteten och patientsäkerheten ökar. Och jag har jobbat tillräckligt länge för att förstå att omorganisationer; att dra boxar och streck annorlunda, är en väldigt överreklamerad förbättringsmetod.En modell utan förankring i verkligheten?2014, året då Birgir Jakobsson går i pension och Melvin Samsom tillträder, tar arbetet fart på Karolinska med värdebaserad vård, en managementteori, framtagen av bland andra Michael Porter en amerikansk ekonomiprofessor, här på en konferens i Stockholm för ett par år sedan, utlagd på Youtube.The fundamental purpose of healthcare system is value for the patientEnligt honom är sjukvårdens mål att skapa värde för patienten.Och värdet får man enligt Porter genom att ta behandlingens effekter delat med kostnaderna. Och för att kunna göra det, måste man bli bättre på att mäta vårdresultaten.Det kan låta enkelt, men i själva verket är Porters program invecklat och har gett upphov till många missförstånd. Det säger Carl Savage, forskare på Karolinska Institutet, som i en kartläggning av en stor mängd vetenskapliga artiklar funnit en begreppsförvirring kring vad konceptet egentligen innebär. Han är positiv till mycket i Porters teori, men anser att förvirringen är ett bekymmer.Vi träffar honom på en konferens om värdebaserad vård, där han och forskarkollegan menar att flera av de föregående talarna, missförstått begreppet värde.Hur har vi pratat om värde idag? Det Porter menar med värde, är den här ekvationen..!Var finns då det sjukhus som gått i mål med Porters modell?Carl Savage, doktorand KI: Det finns ingen som har gått i mål, det finns ingen i världen som bedriver värdebaserad vård.Varför inte? Det är nytt. Folk har inte insett konsekvenserna.Hur vet man att det funkar, då? Det vet man inte. Det är precis det man inte vet. Om du går in på Youtube och tittar på Michael Porters tal, så kommer han aldrig att de dig exempel på ett ställe som gör alla sex delarna i hans program, utan han väljer ut ett ställe till och var och en. Till exempel lyfter han fram Jönköping som han skriver om 2006, hur långt de ha kommit. Men frågar de som arbetar i Jönköping så säger de att de helt enkelt arbetar med kvalitetsförbättring. Så han tar upp det som ett exempel på värdebaserad vård. Men det är bara ett exempel på en del av värdebaserad vård.Vi ska göra ett kort besök på ortopeden vid Sahlgrenska Mölndal, där de sedan 2013 arbetat värdebaserat med höftproteskirurgi. Genom att korta vårdtiden på sjukhuet har de minskat antalet infektioner och kortat köerna. På frågan om det verkligen är värdebaserad vård, svarar de att de använder de delar som passar deras arbete.Inne på avdelning 233 får jag en rundvisning av läkaren Maziar Mohaddes som leder förändringsarbetet här på ortopeden och av vårdenhetschef Marta Olsson.Marta Olsson, vårdenhetschef: Tidigare kom väldigt många, ungefär 80 procent och blev inlagda kvällen innan. Det händer nästan aldrig numer, de kommer sex eller halvåtta samma morgon som operation. Så vi har kortat vårdtiden på sjukhus.Maziar Mohaddes, ortoped: Och det ser vi efter den förändring vi har haft de senaste två åren här, att komplikationsfrekvensen minskar, andelen patienten som behöver ny operation, på grund av att de till exempel har fått en infektion eller andra problem minskar.Så den kortare vårdtiden handlar enbart om att man ska minska risken för infektioner, det handlar inte om att spara pengar? Vi har haft den utgångspunkten när vi har förändrat arbetssättet, att i första hand som gör man förändringen för att förbättra patientutfallen, vilket har bidragit till de resultat man ser: förbättrad nöjdhetsgrad ett år efter operation, och en sänkning av komplikationerna med 20 procent."Vi kan inte riktigt avgöra kvalitén"Åter till Karolinska. En viktig del som man tagit fasta på är att låta patient- och anhörigrepresentanter vara med och diskutera fram vilka mått som är viktiga. I en presentationsfilm om Karolinskas nya verksamhetsmodell, får vi träffa kirurgiprofessor Torbjörn Holm.Ur presentationsfilmen:Torbjörn: Jag arbetar tillsammans i en projektgrupp eller arbetsgrupp., där alla professioner och discipliner som tar hand om patienter med kolorektal cancer ingår. Men det viktigaste av allt är att vi har en patientrepresentant, Johan, som har haft den här sjukdomen, och nu hjälper oss i det värdebaserade arbetet.Johan: Jag tror det är helt avgörande att man låter professionen ytterst ta besluten, men att de besluten tas i samråd med patienten som får vara med och påverka.Torbjörn: Han har givit oss idéer och uppslag till förbättringsarbete, till exempel.Den här filmen är från i våras. Vi ska komma tillbaka till professor Torbjörn Holm om en stund och höra hur han ser på läget idag, men först ska vi träffa Läkarföreningen, och ordföranden på Karolinska, Yvonne Dellmark. Det är jättebra att man försöker ta reda på vad patienterna vill och vad de tycker om vården, och använder det i förbättringsarbetet. Det är väldigt positivt. Däremot behöver man inte organisera om vården så här mycket för att åstadkomma det.Hon säger att man fortfarande saknar en referens, till något sjukhus som lyckats genomföra något liknande. Vi har inte hittat något exempel där man har genomfört alla de här förändringarna samtidigt, så vi ser det som ett stort experiment.Förra sommaren, när sjukhusledningen presenterade sitt förslag på ny organisation, för de fackliga representanterna, tog man upp ett tiotal pilotprojekt på värdebaserad vård. Men själva underlaget, med fakta och siffror, fick Läkarföreningen inte ta del av, säger de.När vi begär ut de här piloterna, vill sjukhusledningen få det klassat som arbetsmaterial. Först efter lite krångel får vi ut dem.Yvonne Dellmark för se underlaget: Jag kan inte se några nya uppgifter som man inte hade kunnat åstadkomma med vilket förbättringsarbete som helst.Yvonne Dellmark är inte imponerad. Dessutom säger att hon att vissa av datumen inte verkar stämma. Det är ju några av de här flödena där det inte verkar finnas någon dokumentation, som du har fått ta del av i alla fall, från den tidpunkt då de skulle utvärderas.Hur ska man tolka det? Att det sa att det fanns ett underlag fast det inte fanns? Det är så jag tolkar det. Att det faktiskt inte fanns ett bättre underlag.Vi undrar varför sjukhusledningen tog upp piloterna som ett underlag för den nya verksamhetsmodellen, ifall de inte var relevanta? They were relevant in terms of patient flows and to see wether value based healthcare could work in a university hospital environment as we have.Det var de, säger Melvin Samsom, de inleddes 2014 för att se hur vi kan bedriva värdebaserad vård på ett universitetssjukhus och för att se hur vi ska organisera oss. Men han hävdar att de aldrig varit avsedda att utgöra något stöd för den nya modellen. We cant really grab the quality, we have this constant cost issues.Men vad han bygger sina kalkyler på, att det nya organisationen ska lyckas, får vi inget svar på.  Vi tar det här stegvis, ni får komma tillbaka om ett par år, så får ni se resultaten, säger han: The new organisational structure if focusing on solving this step by step. Lets come back to this in one or two years and we can show you the results.Det finns alltså en kritik mot att den nya organisationen är dåligt underbyggd, samt att införandet sätter press på en redan pressad verksamhet.  Något som ledningen bemöter med att nuvarande organisation har kommit till vägs ände och att nu är det bästa tillfället att göra det här.Men kritiken är bredare än så, den handlar om toppstyrning, alltså sättet man genomför det här på.Kaliber har talat med runt 15 höga chefer, inom olika verksamheter. Hälften av dem har valt att lämna Karolinska på grund av det som sker.En av dem är verksamhetschef Peter Baranys företrädare på njurmedicin Huddinge, överläkare Maarit Korkeila. Hon vill inte bli intervjuad, men skriver i ett mail att:Det kändes meningslöst att sitta i ändlösa möten och workshops. Ledningen försöker skapa en ny modell utifrån Excel-ark och diagnosnummer. Men det finns många och ofta livsviktiga samband som inte syns i siffror. Toppstyrningen gör att man även tappar den starkaste inbyggda drivkraft som finns inom sjukvård, nämligen att göra något bra för patienterna och dela resurserna på bästa sätt, på golvet varje dag.Briljant ingenjörskonst eller ett vettlöst krångel?Ytterligare en kritiker är Annika Bergquist, som här tackas av från sin tjänst som verksamhetschef för Gastrocentrum i Solna. Det har med den nya omorganisationen att göra. Jag är för en temaindelad vård. Men jag är väldigt emot att bryta sönder befintliga strukturer och genom åratal uppbyggda goda relationer, vilket det finns en väldigt stor risk att man gör nu. Problemet som jag ser det är att man inte möter verksamheterna där de är, utan man skapar ett system och drar alla över en kam.Har du kommunicerat dina farhågor till sjukhusledningen? Absolut. Jag har vid ett flertal tillfällen och på olika nivåer kommunicerat det här. Jag får till svar att det är framväxande förändring där det finns möjlighet att vara med och påverka. Min erfarenhet nu är att den möjligheten att påverkar de viktigaste dragen för den verksamhet jag har lett och för de patienter som ingår där, det har inte varit möjligt.Melvin Samsom anser att kritiken om toppstyrning är ogrundad. We are asking many people within the organisation to design the organisational structure in the best way that fits their needs and their patients needs.Han hävdar att även om ledningen tagit beslutet om ny organisation, så har designen och strukturen fått växa fram i dialog med verksamheterna. Men sedan behöver inte alla var eniga med det vi till sist bestämmer, säger han. That doesnt entail the fact of course that everyone will agree with the final decisions.Förutom toppstyrning, klagar många vi pratar med, på konsultstyrning; att mycket av förändringsarbetet leds av personer, som saknar vårdbakgrund, kanske aldrig träffat en patient. De konsulter som varit inne som projektledare och presenterat olika scenarios för oss har ju en väldigt fyrkantig bild av verkligheten, sägerYvonne Dellmark, ordförande Läkarföreningen Karolinska, tycker det märks på mötena: Med olika fyrkanter som vi kan välja att gå in i. Om vi föreslår en cirkel som passar verksamheten mycket bättre, så passar inte den i modellen och försvinner snabbt ut ur diskussionen.Om detta vittnar många vi pratar med. Till och med reklampelaren för den nya organisationen. Vi hörde ju kirurgiprofessorn Torbjörn Holm tidigare i programmet, i den presntationsfilm för den nya verksamhetsmodellen, som Karolinska låtit göra.Ur presentationsfilmen:Vad ska vi göra för att förändra våra arbetssätt, så att vi faktiskt uppnår de mål vi sätter upp?I slutet av filmen får han en fråga av patienten.Ur filmen:Kommer ni att klara den här omställningen?Jag tror faktiskt att vi kommer att lyckas. Dels för att personalen har en inställning att arbeta för att öka värdet för patienten, men också för attFilmen är publicerades på Youtube i mars i år. I början av oktober söker vi upp Torbjörn Holm på hans kontor. Nu låter han inte lika positiv.Torbjörn Holm, professor kirurgi: Det är ju fullkomligt obegripligt! Alltså det här, ingen förstår nånting!Vi sitter framför datorn och Torbjörn klickar sig igenom en till synes ändlös radda av Power Points, fyllda med staplar och diagram, cirklar och trianglar i olika färger.Torbjörn Holm, professor kirurgi: Du ser hur många! Det är ju helt sjukt!Har du blivit lite besviken över det som du var så entusiastisk till? Ja, i viss mån. Jag tycker att man har gjort det mycket mer komplext och svårt än vad de kanske borde ha behövt vara. Den grundläggande principen är god, men vägen dit har blivit alldeles för krånglig.Vi frågar sjukhusdirektören, varför de ger managementkonsulterna ett sådant inflytande och vad det är för unik kompetens de tillför. Och det ökar ännu mer med alla konsulter, eftersom deras språk är oftast väldigt obegripligt för oss som är i verksamheten. Och vårt språk i verksamheten är också obegripligt för konsulterna, för de kan inte de medicinska termerna, de kan inte verksamheten, de vet inte vad vi egentligen håller på med.Det tycks ha blivit några konsultledda möten för mycket för kirurgiprofessor Torbjörn Holm. Det här är ju ingen som orkar läsa!Bortom alla svårbegripliga Powerpoints, tycker han sig skönja en organisation som påminner rätt mycket om den gamla, och där problemen inte är löstaTorbjörn Holm, professor kirurgi: Det är nya namn. Vi kallar kliniker för ett flöde i stället, vår klinik delas upp i tre olika flöden, men det vi gör med våra patienter kommer ju vara i stort sett detsamma.Briljant ingenjörskonst eller ett vettlöst krångel? Det får framtiden utvisa.Säkert är att det redan kostat åtskilliga skattemiljoner och otaliga mötestimmar, för stressad personal. Background: The capacity of patient's Natural Killer cells (NKs) to be activated for cytolysis is an important prerequisite for the success of antibody-derived agents such as single-chain triplebodies (triplebodies) in cancer therapy. NKs recovered from AML patients at diagnosis are often found to be reduced in peripheral blood titers and cytolytic activity. Here, we had the unique opportunity to compare blood titers and cytolytic function of NKs from an AML patient with those of a healthy monozygotic twin. The sibling's NKs were compared with the patient's drawn either at diagnosis or in remission after chemotherapy. The cytolytic activities of NKs from these different sources for the patient's autologous AML blasts and other leukemic target cells in conjunction with triplebody SPM-2, targeting the surface antigens CD33 and CD123 on the AML cells, were compared. Methods: Patient NKs drawn at diagnosis were compared to NKs drawn in remission after chemotherapy and a sibling's NKs, all prepared from PBMCs by immunomagnetic beads (MACS). Redirected lysis (RDL) assays using SPM-2 and antibody-dependent cellular cytotoxicity (ADCC) assays using the therapeutic antibody Rituximab (TM) were performed with the enriched NKs. In addition, MACS-sorted NKs were analyzed for NK cell activating receptors (NCRs) by flow cytometry, and the release of TNF-alpha and IFN-gamma from blood samples of both siblings after the addition of the triplebody were measured in ELISA-assays. Results: Patient NKs isolated from peripheral blood drawn in remission produced comparable lysis as NKs from the healthy twin against the patient's autologous bone marrow (BM) blasts, mediated by SPM-2. The NCR receptor expression profiles on NKs from patient and twin were similar, but NK cell titers in peripheral blood were lower for samples drawn at diagnosis than in remission. Conclusions: Peripheral blood NK titers and ex vivo cytolytic activities mediated by triplebody SPM-2 were comparable for cells drawn from an AML patient in remission and a healthy twin. If these results can be generalized, then NKs from AML patients in remission are sufficient in numbers and cytolytic activity to make triplebodies promising new agents for the treatment of AML.
