In this episode we talk to Mark Holmes and Mike Ward about the Blue Light approach, which aims to challenge the belief that nothing can be done with drinkers who may be deemed as 'not wanting to change'. The Blue Light approach aims to show there are a range of positive strategies that can be used to manage risk, reduce harm and promote change. We discuss the ideas and myths behind 'change resistant drinkers', and explore what approaches and strategies are most effective when working with people with severe alcohol problems who are not currently engaging in alcohol treatment or other support. Mike Ward is from a Social Work background but has worked most of his career in the alcohol and drug field.  He is the co-author of the Blue Light Manual on Working with Change Resistant Drinkers but also other practical guides including Safeguarding Vulnerable Dependent Drinkers.  He is the author of over thirty serious case reviews about individuals with alcohol and/or drug use disorders.Mark Holmes. BSc, RMN, SPMH has been a mental health nurse for over 30 years.  Mark was being twice awarded Nursing Times Awards; The Mental Health Nurse of the Year in 2012 and LTC Team of the year 2016. He also works as an associate for Alcohol Change UK co-authoring the Blue Light Project and has had over 20 publications. He currently sits on UK alcohol clinical guidelines expert group, and Alcohol Care Teams Steering group (ACTION). Details of the Blue Light approach can be found here. In this episode of The Health Coach Show, we discuss how rewards can be useful to help clients change their behaviours. The big question is 'Are rewards helpful and do they help with long-term behaviour change?' How rewards can support behaviour and habit change: Rewards provide positive reinforcement Rewards can give us that little motivation boost on days when we don't feel like it - especially useful during the early stages of change when motivation is low.  Rewards bring benefits into the present: providing immediate feedback that the behaviour is worth doing - particularly useful when changes may not be noticed for a few weeks. Examples of effective rewards in health coaching: Celebrating small milestones /consistency - reward yourself with a new book, water bottle or workout mat Replacing alcohol with zero alcohol drinks Buy the expensive chocolate (and therefore eat less) Non-food rewards - bath,  listening to a podcast, going to a movie, reading a book/magazine for pleasure, permission to do nothing When rewards can be unhelpful: Extrinsic vs. intrinsic motivation: when we rely on the external rewards - ie the only reason we engage in a behaviour is to get the reward, we're extrinsically motivated - once the reward is removed, the motivation to do the behaviour stops. Risks of relying on rewards: Short-term compliance without long-term behaviour change. Creating dependency on rewards rather than fostering internal motivation.  Reward chasing: Constantly using rewards can reduce their effectiveness. We can lose interest in routine rewards and look for additional rewards to stay motivated. Using unhealthy rewards: e.g. going out for a big breakfast after a gym session, or rewarding a long day with a glass of wine - can un-do the good work of the behaviour itself.  In coaching, we can use both intrinsic and extrinsic rewards - the extrinsic can be really valuable in the early stages of change- while connecting to intrinsic ones over time. This might look like: Encouraging self-chosen rewards aligned with values. Social recognition (sharing progress with friends/family). Celebrating achievement with meaningful experiences (e.g., self care/connecting - a client rewarding themselves with a massage after sticking to a new routine for a week) Self-reward through positive self-talk and reflection on progress Connecting to how it makes you feel immediately or later that day - eg  more energy, better sleep. Mini rewards to help stick to a behaviour  Alternative treats Tracking progress can serve as a motivating reward: Visual progress (habit trackers, fitness apps) provides immediate feedback. Self-monitoring increases self-awareness and accountability. Associate Professor Genevieve Healy and Dr. Ana Goode provide us with the latest research findings from BeUpstanding program of research: a program aiming to support workplaces to stand up, sit less, and move more for their health and well-being. Associate Professor Genevieve Healy is a NHMRC Career Development Fellow at the Cancer Prevention Research Centre in the School of Public Health at the University of Queensland, and an honorary research fellow at the Baker IDI Heart and Diabetes Institute, and Curtin University. Her PhD research reported some of the first evidence regarding the importance of regularly interrupting sedentary time for heart health. Her current research builds on this work to examine population-level variations in prolonged sedentary time as well as the feasibility and acceptability of reducing this behaviour in key settings, such as the workplace.  Dr. Ana Goode currently works at the Cancer Prevention Research Centre, University of Queensland. Her program of research brings together training and research experience in health psychology and health behavior change interventions, including their adaptation and translation to practice to build the evidence base for the dissemination of physical activity, dietary change, and weight loss interventions in applied community settings. Ella: @thefitnesspsych_  Me: @transformxruby Fearful that they are never going to change Communication; with ourselves & others - People think they SHOULD know how to do things External goals Both sides of motivation Do we need to listen to ourselves and actually rest Or do we need to act to build momentum I know what to do but im not doing it Tradeoffs Saying yes to something = no to other things as well Everything serves you in some way Flexibility VS restrained/rugged flexibility i never had it to start with but thats where social media c Motivation Helping clients tap into intrinsic motivation Why the groundwork is so important .. motivation When we don't enjoy something (which we need to do things we don't enjoy to get to where we want to be in the sense that its so different from where you were, of course you wont like it at first - but thats the identity piece and then realising that it can be something you do enjoy (like i hated exercise to start with and then that all changed as i realised the benefits and saw it changing me physically and etcetc I feel this is where alot of people slip up because they failed in the past before bc they are the ones doing it to themselves Or even with a coach they feel like this is how it SHOULD be So they don't open up that they need to slow down All or nothing thinking Being self critical in the process How this holds you back How to navigate it and move forward from it Mindset barriers Feeling like they should have their shit together and it should be easy i want to talk about ways to build up our confidence bucket and finding proof & our track record of the past times we did well and how to approach it then if we havent got one in the way that we would like to proceed forward - when there really is alot of change into the new identity and also how at times its a fear but not knowing that its a fear because we cant recognise the emotion for example a client comes to mind: she started a deficit and she already preempts falling off and sabotaging and the whole idea of being in a deficit she feels hungry just from that! You arent going to love it all the time You will always come across resistance hard doesn Not being so focused on our body means letting ourself go CHANGE in itself process of Change Physical Mental Communication comes in there again There is no normal compartmentalizing VS catastrophizing and blending it all together - duality of thinking 2 things can exist Its not EITHER/OR It is AND ------ that was perfect --- WHY was it? --- i didnt p Change not being linear and different for everyone if its something we want to last because theres usually alot of work to do and how our past experiences shape our process - ie belief in ourselves. Shahroo defines imposter syndrome as not being able to internalise your accomplishments, so that even the most successful people never feel as if they are enough. This is all linked to low self-worth, shame and guilt, coming from not being able to give yourself permission to accept that what others made find easy you find difficult. As you will need to change habits your whole life, Shahroo believes that you need to change the conversations you have with yourself, feeling smart, calm and proud of yourself so that you can reframe challenges to opportunities for showing your capacity.
