The New York Times needs, one day – one day soon, one day now – to devote the entirety of the front page – to a headline, and an editorial, signed by the publisher Sulzberger and the editor Kahn and the key columnists and correspondents – headlined “TRUMP IMPERILS DEMOCRACY” and sub-headlined “YOUR LIFE AT STAKE.” We need a Walter Cronkite moment from The New York Times and when Trump went to a Philadelphia area seaside resort called Wildwood, drew maybe 10,000 cultists, lied and had the Republican mayor lie and say it was 80,000, complained that immigrant students don't speak English and immediately afterwards said something like “Borden-in-riv-iv,” said something else like “carry doubt-ite-by-rite,”claimed the president between Ford and Reagan was named Jimmy Connors, said the Chinese were preparing to invade Beijing (their own capital), insisted the entire country was grateful that he killed Roe-V-Wade, thanked – by name - the Supreme Court justices who gutted it, suddenly invoked the fictional cannibal character Hannibal Lecter, seemed to praise him, claimed the character was dead, and got the name of the movie wrong, and then insisted all immigrants are Hannibal Lecter – and all of that was after he was introduced by some immigrant who called him “President CHUMP"... the New York Times story, by a sixth-stringer named Michael Gold, mentioned… none of that. This was what Editor Joe Kahn's writer told consumers of the most influential news organization in America, quote: “After a long and often tense week in his criminal trial in Manhattan… Trump… took part in a time-honored ritual enjoyed by countless New Yorkers in need of a break: He went to the shore.” Oh ho ho, how clever. The Times instead lets Maggie Haberman dismiss as “hearsay” Michael Cohen's first-hand recounting of what Trump told him about ‘not being on the market for long' if Melania dumped him and if Haberman doesn't know the legal definition of “hearsay” get rid of her. And the Times made room for an op-ed bashing Joe Biden by Mark Penn, a dishonest right wing pollster who has been posing as a Democrat for at least 20 years. In Court: “Michael Cohen calmly describes Trump's hush money instructions,” reads the headline in The Washington Post. The SUB-headline quotes Trump: “Just do it.” That's what the prosecution needed out of Cohen. And it needs it again out of him today. AND whenever the cross-examination begins. It needs him making more self-abnegating jokes about ‘angry, even for me.' It needs him testifying as he did yesterday: that he was there in Trump Tower, days before Trump was sworn in as president, with Allan Weisselberg, reviewing a handwritten document plan to repay Cohen for the Stormy Daniels hush money and how they would hide it. And that Trump said “smart individuals” had told him, Trump, to pay the $130,000. And that Trump told him he knew if the Daniels story got out it would be a disaster for the CAMPAIGN. And it needs him producing one outstandingly sleazy quote from Trump per day on the stand, like when Cohen asked Trump about the impact on his wife MELANIA if the story got out and Trump said “don't worry. How long do you think I'll be on the market for? Not long.” B-Block (22:54) EVERY DOG HAS ITS DAY: California Assembly Bill 2265 and what it can do to save dogs - and save shelters the horrible cost of killing them. According to numerous reports, Trump pled not guilty to 34 felony counts before State Supreme Court Justice Juan M. Merchan. During the arraignment, Trump's attorneys are expected to request a change of venue—arguing that Trump is unlikely to receive a fair trial in Manhattan, an area that votes overwhelmingly in favor of Democrat party candidates. The New York Times reports: “Mr. Trump's 11-vehicle motorcade arrived just before 1:30 at the district attorney's office, part of the towering Manhattan Criminal Courts Building. Sec. 11404: Congestion Relief Program Creates a grant program, funded at a minimum of $10 million per grant, for projects aimed at reducing highway congestion. Eligible projects include congestion management systems, fees for entering cities, deployment of toll lanes, parking fees, and congestion pricing, operating commuter buses and vans, and carpool encouragement programs. Buses, transit, and paratransit vehicles "shall" be allowed to use toll lanes "at a discount rate or without charge." Sec. 11405: Promoting Resilient Operations for Transformative, Efficient, and Cost-saving Transportation (PROTECT) Program Establishes the "PROTECT program", which provides grants for projects to protect some current infrastructure from extreme weather events and climate related changes. Types of grants include grants for "at-risk coastal infrastructure" which specifies that only "non-rail infrastructure is eligible" (such as highways, roads, pedestrian walkways, bike lanes, etc.) Sec. 11406: Healthy Streets Program Establishes a grant program to install reflective pavement and to expand tree cover in order to mitigate urban heat islands, improve air quality, and reduce stormwater run-off and flood risks. Caps each grant at $15 million TITLE III: RESEARCH, TECHNOLOGY, AND EDUCATION Sec. 13001: Strategic Innovation for Revenue Collection Provides grants for pilot projects to test our acceptance of user-based fee collections and their effects on different income groups and people from urban and rural areas. They will test the use of private companies to collect the data and fees. Sec. 13002: National Motor Vehicle Per-mile User Fee Pilot Creates a pilot program to test a national motor vehicle per-mile user fee. DIVISION B - SURFACE TRANSPORTATION INVESTMENT ACT OF 2021 TITLE I - MULTIMODAL AND FREIGHT TRANSPORTATION Sec. 21201: National Infrastructure Project Assistance Authorizes $2 billion total per year until 2026 on projects that cost at least $100 million that include highway, bridge, freight rail, passenger rail, and public transportation projects. Authorizes $1.5 billion total per year until 2026 (which will expire after 3 years) for grants in amount between $1 million and $25 million for projects that include highway, bridge, public transportation, passenger and freight rail, port infrastructure, surface transportation at airports, and more. TITLE II - RAIL Subtitle A - Authorization of Appropriations Sec. 22101: Grants to Amtrak Authorizes appropriations for Amtrak in the Northeast Corridor at between $1.1 billion and $1.57 billion per year through 2026. Authorizes appropriations for Amtrak in the National Network at between $2.2 billion and $3 billion per year through 2026. Subtitle B - Amtrak Reforms Sec. 22201: Amtrak Findings, Mission, and Goals Changes the goal of cooperation between Amtrak, governments, & other rail carriers from "to achieve a performance level sufficient to justify expending public money" to "in order to meet the intercity passenger rail needs of the United States" and expands the service areas beyond "urban" locations. Changes the goals of Amtrak to include "improving its contracts with rail carriers over whose tracks Amtrak operates." Sec. 22208: Passenger Experience Enhancement Food and beverage service: Amtrak will establish a working group... Sec. 22212: Enhancing Cross Border Service Amtrak must submit a report... Sec. 22213: Creating Quality Jobs Amtrak will not be allowed to privatize the jobs previously performed by laid off union workers. Sec. 22214: Amtrak Daily Long Distance Study Amtrak would study bringing back long distance rail routes that were discontinued. Subtitle C - Intercity Passenger Rail Policy Sec. 22304: Restoration and Enhancement Grants Extends the amount of time the government will pay the operating costs of Amtrak or "any rail carrier" that provides passenger rail service from 3 years to 6 years, and pays higher percentages of the the costs. Sec. 22305: Railroad Crossing Elimination Program Creates a program to eliminate highway-rail crossings where vehicles are frequently stopped by trains. Authorizes the construction on tunnels and bridges. Sec. 22306: Interstate Rail Compacts Authorizes up to 10 grants per year valued at a maximum of $ million each to plan and promote new Amtrak routes Sec. 22308: Corridor Identification and Development Program The Secretary of Transportation will create a program for public entities to plan for expanded intercity passenger rail corridors, operated by Amtrak or private companies. When developing plans for corridors, the Secretary has to "consult" with "host railroads for the proposed corridor" Subtitle D - Rail Safety Sec. 22404: Blocked Crossing Portal The Administration of the Federal Railroad Administration would establish a "3 year blocked crossing portal" which would collect information about blocked crossing by trains from the public and first responders and provide every person submitting the complaint the contact information of the "relevant railroad" and would "encourage" them to complain to them too. Information collected would NOT be allowed to be used for any regulatory or enforcement purposes. Sec. 22406: Emergency Lighting The Secretary of Transportation will have to issue a rule requiring that all carriers that transport human passengers have an emergency lighting system that turns on when there is a power failure. Sec. 22409: Positive Train Control Study The Comptroller General will conduct a study to determine the annual operation and maintenance costs for positive train control. Sec. 22423: High-Speed Train Noise Emissions Allows, but does not require, the Secretary of Transportation to create regulations governing the noise levels of trains that exceed 160 mph. Sec. 22425: Requirements for Railroad Freight Cars Placed into Service in the United States Effective 3 years after the regulations are complete (maximum 5 years after this becomes law), freight cars will be prohibited from operating within the United States if more than 15% of it is manufactured in "a country of concern" or state-owned facilities. The Secretary of Transportation can assess fines between $100,000 and $250,000 per freight car. A company that has been found in violation 3 times can be kicked out of the United State's transportation system until they are in compliance and have paid all their fines in full. Sec. 22427: Controlled Substances Testing for Mechanical Employees 180 days after this becomes law, all railroad mechanics will be subject to drug testing, which can be conducted at random. DIVISION C - TRANSIT Sec. 30017: Authorizations Authorizes between $13.3 billion and $14.7 billion per year to be appropriated for transit grants. DIVISION D - ENERGY TITLE I - GRID INFRASTRUCTURE AND RESILIENCY Sec. 40101: Preventing Outages and Enhancing The Resilience of the Electric Grid Creates a $5 billion grant distribution program to electric grid operators, electricity storage operations, electricity generators, transmission owners and operators, distribution suppliers, fuels suppliers, and other entities chosen by the Secretary of Energy. The grants need to be used to reduce the risk that power lines will cause wildfires. States have to match 15%. The company receiving the grant has to match it by 100% (small utilities only have to match 1/3 of the grant.) Grant money be used for micro-grids and battery-storage in addition to obvious power line protection measures. Grant money can not be used to construct a new electricity generating facility, a large-scale battery facility that is not used to prevent "disruptive events", or cybersecurity. The companies are allowed to charge customers for parts of their projects that are not paid for with grant money (so they have to match the grant with their customer's money). Sec. 40112: Demonstration of Electric Vehicle Battery Second-Life Applications for Grid Services Creates a demonstration project to show utility companies that electric car batteries can be used to stabilize the grid and reduce peak loads of homes and businesses. The demonstration project must include a facility that "could particularly benefit" such as a multi-family housing building, a senior care facility, or community health center. TITLE II - SUPPLY CHAINS FOR CLEAN ENERGY TECHNOLOGIES Sec. 40201: Earth Mapping Resources Initiative The US Geological Survey will get $320 million and ten years to map "all of the recoverable critical minerals." Sec. 40204: USGS Energy and Minerals Research Facility Authorizes $167 million to construct a new facility for energy and minerals research. The facility can be on land leased to the government for 99 years by "an academic partner." Requires the USGS to retain ownership of the facility. Sec. 40205: Rare Earth Elements Demonstration Facility Authorizes $140 million to build a rare earth element extractions and separation facility and refinery. Does NOT require the government to retain ownership of the facility. TITLE III - FUELS AND TECHNOLOGY INFRASTRUCTURE INVESTMENTS Subtitle A - Carbon Capture, Utilization, Storage, and Transportation Infrastructure Sec. 40304: Carbon Dioxide Transportation Infrastructure Finance and Innovation Authorizes $600 million for 2022 and 2023 and $300 million for each year between 2024 and 2026 for grants and loan guarantees for projects for transporting captured carbon dioxide. Each project has to cost more than $100 million and the government can pay up to 80% of the costs. If the project is financed with a loan, the company will have 35 years to pay it back, with fees and interest. Loans can be issued via private banks with guarantees provided by the government. Sec. 40305: Carbon Storage Validation and Testing Creates a new program for funding new or expanded large-scale carbon sequestration projects. Authorizes $2.5 billion through 2026. Sec. 40308: Carbon Removal Creates a new program for grants or contracts for projects to that will form "4 regional direct air capture hubs" that will each be able to capture 1 million metric tons of carbon dioxide per year. Authorizes $3.5 billion per year through 2026. Subtitle B - Hydrogen Research and Development Sec. 40313: Clean Hydrogen Research and Development Program Changes a goal of an existing research and development plan for hydrogen fuels (created by the Energy Policy Act of 2005) from enhancing sources of renewable fuels and biofuels for hydrogen production to enhancing those sources and fossil fuels with carbon capture and nuclear energy. Expands the activities of this program to include using hydrogen for power generation, industrial processes including steelmaking, cement, chemical feestocks, and heat production. They intend to transition natural gas pipelines to hydrogen pipelines. They intend for hydrogen to be used for all kinds of vehicles, rail transport, aviation, and maritime transportation. Sec. 40314: Additional Clean Hydrogen Programs Creates a new program to create "4 regional clean hydrogen hubs" for production, processing, delivery, storage, and end-use of "clean hydrogen." At least one regional hub is required to demonstrate the production of "clean hydrogen from fossil fuels." At least one regional hub is required to demonstrate the production of "clean hydrogen from renewable energy." At least one regional hub is required to demonstrate the production of "clean hydrogen from nuclear energy." The four hubs will each demonstrate a different use: Electric power generation, industrial sector uses, residential and commercial heating, and transportation. Requires the development of a strategy "to facilitate widespread production, processing, storage, and use of clean hydrogen", which will include a focus on production using coal. The hydrogen hubs should "leverage natural gas to the maximum extent practicable." Creates a new program to commercialize the production of hydrogen by splitting water into hydrogen and oxygen. The overall goal is to identify barriers, pathways, and policy needs to "transition to a clean hydrogen economy." Authorizes $9.5 billion through 2026. Sec. 40315: Clean Hydrogen Production Qualifications Develops a standard for the term "clean hydrogen" which has a carbon intensity equal to or less than 2 kilograms of carbon dioxide-equivalent produced at the site of production per kilogram of hydrogen produced." Subtitle C - Nuclear Energy Infrastructure Sec. 40323: Civil Nuclear Credit Program Creates a program, authorized to be funded with $6 billion per year through 2026, that will provide credit from the government to nuclear reactors that are projected to shut down because they are economically failing. Subtitle D - Hydropower Sec. 40331: Hydroelectric Production Incentives Authorizes a one-time appropriation of $125 million for fiscal year 2022. Sec. 40332: Hydroelectric Efficiency Improvement Incentives Authorizes a one-time appropriation of $75 million for fiscal year 2022. Sec. 40333: Maintaining and Enhancing Hydroelectricity Incentives Authorizes a one-time appropriations of $553 million for repairs and improvements to dams constructed before 1920. The government will pay a maximum of 30% of the project costs, capped at $5 million each. Sec. 40334: Pumped Storage Hydropower Wind and Solar Integration and System Reliability Initiative Authorizes $2 million per year through 2026 to pay 50% or less of the costs of a demonstration project to test the ability of a pumped storage hydropower project to facilitate the long duration storage of at least 1,000 megawatts of intermittent renewable electricity. Subtitle E - Miscellaneous Sec. 40342: Clean Energy Demonstration Program on Current and Former Mine Land Creates a new program, authorized to be funded with $500 million through 2026, to demonstrate the technical and economic viability of putting clean energy projects on former mine land. There will be a maximum of 5 projects and 2 of them have to be solar. Defines a "clean energy project" to include "fossil-fueled electricity generation with carbon capture, utilization, and sequestration." TITLE X - AUTHORIZATION OF APPROPRIATIONS FOR ENERGY ACT OF 2020 Sec. 41001: Energy Storage Demonstration Projects Authorizes $505 million through2025 for energy storage demonstration projects. Sec. 41002: Advanced Reactor Demonstration Program Authorizes between $281 million and $824 million per year through 2027 for advanced nuclear reactor demonstration projects. Sec. 41004: Carbon Capture Demonstration and Pilot Programs Authorizes between $700 million and $1.3 billion through2025 for advanced nuclear reactor demonstration projects. Sec. 41007: Renewable Energy Projects Authorizes $84 million through 2025 for geothermal energy projects. Authorizes $100 million through 2025 for wind energy projects. There is a clarification that this is definitely NOT in addition to amounts wind gets from another fund. Authorizes $80 million through 2025 for solar energy projects. SpaceX and Blue Origin may be making the headlines, but they are far from the only companies involved in the emerging space economy. And at the same time, NASA intends to continue manned missions to the Moon with its Artemis program. In a recent event hosted by the American Enterprise Institute, John Roth, Rich Boling, Mike Gold, and Matthew Weinzierl discussed the present and future of space commerce, from tourism to manufacturing and beyond. John Roth serves as vice president of business development for Sierra Space, a subsidiary of the Sierra Nevada Corporation. Richard Boling is vice president of corporate advancement at Techshot. Mike Gold is executive vice president of civil space business development and external affairs at Redwire Space. For this week, we start with author Michael Gold and editorialist Sana Al-Khatib as they discuss the article "Primary Results from the Understanding Outcomes with the S-ICD in Primary Prevention Patients with Low Ejection Fraction (UNTOUCHED) Trial." Then, we switch to an important discussion about children and COVID-19 as author Israel Valverde and Associate Editor Gerald Greil discuss "Acute Cardiovascular Manifestations in 286 Children with Multisystem Inflammatory Syndrome Associated with COVID-19 Infection in Europe." TRANSCRIPT BELOW: Dr. Carolyn Lam: Welcome to Circulation on the Run. Your weekly podcast summary and backstage pass to the journal and its editors. We're your co-hosts. I'm Dr. Carolyn Lam, associate editor from the National Heart Center and Duke National University of Singapore. Greg Hundley: And I'm Greg Hundley, director of the Pauley Heart Center at VCU Health in Richmond, Virginia. Well, Carolyn, we have two features in this issue. Dr. Carolyn Lam: I know. Greg Hundley: Yeah. And I've got the first one. It's going to be evaluating the acute cardiovascular manifestations in 286 children with multi-system inflammatory syndrome. Dr. Carolyn Lam: Wow. That is so important in the current pandemic. Well, the other is also so important. It's really, really critical results from the subcutaneous ICD trial called UNTOUCHED. I think that one might change clinical practice. So, do you want to tell us about other papers first? Greg Hundley: Yes. Carolyn, I'm going to grab my cup of coffee and we'll get started and my first paper comes from China. Professor Junbo Ge. So Carolyn PCSK9, mainly secreted by the liver and released into the blood elevates plasma load density lipoprotein cholesterol by degrading LDL receptors. Pleiotropic effects of PCSK9 beyond lipid metabolism have been shown. However, the direct effects of PCSK9 on platelet activation and thrombosis, as well as the underlying mechanisms still remain unclear. Greg Hundley: So this group detected the direct effects of PCSK9 on agonist-induced platelet aggregation, dense granule ATP release, integrin a2b beta-3 activation, alpha granule release, spreading and clot retraction. They also investigated the underlying mechanisms. Using myocardial infarct models, they explored the effects of PCSK9 on microvascular obstruction and infarct expansion, post myocardial infarction. Dr. Carolyn Lam: Oh, nice. And what did they find? Greg Hundley: Well, Carolyn, PCSK9 in plasma directly enhances platelet activation and in vivo thrombosis, as well as myocardial infarct expansion post MI by binding to platelet CD36 and thus activating the downstream signaling pathways. PCSK9 inhibitors, or aspirin, abolished the enhancing effects of PCSK9 supporting the use of aspirin in patients with high plasma PCSK9 levels in addition to PCSK9 inhibitors to prevent thrombotic complications. Dr. Carolyn Lam: Wow. Very interesting. Indeed, the pleiotropic effects. Well guess what? My paper talks about the first non-LDL lowering treatment that has been shown to reduce CABG in a blinded randomized trial. Greg, can you guess what that treatment was? Well, here's a hint, here's a hint. It's the REDUCE-IT trial. Greg Hundley: Well, Carolyn, thanks for the hint. It must be icosapent ethyl. Dr. Carolyn Lam: Indeed, indeed. As a reminder, REDUCE-IT was a multicenter double-blind placebo controlled trial, which randomized statin treated patients with elevated triglycerides, controlled LDL and either established cardiovascular disease or diabetes plus other risk factors to receive icosapent ethyl at four grams daily or placebo. The primary and secondary composite end points were significantly reduced. The current paper examined all coronary revascularizations, recurrent revascularizations and revascularization subtypes. First revascularizations were reduced by icosapent ethyl versus placebo with a hazard ratio of 0.66, which is a number needed to treat of only 24. Similar reductions were observed in total revascularizations and across elective, urgent and emergent revascularizations. Icosapent ethyl significantly reduced PCI and CABG with a hazard ratio of 0.61. So, icosapent ethyl reduce first and total coronary revascularization, including PCI and CABG in patients with elevated triglycerides and high cardiovascular risk despite well controlled LDL. Isn't that cool? Greg Hundley: Very nice Carolyn. Well, my next paper comes from Professor Kari Alitalo, from the University of Helsinki. So Carolyn recent discoveries have indicated that in the developing heart, sinus venosus and endocardium provide major sources of endothelium for coronary vessel growth that supports the expanding myocardium. The author set out to study the origin of the coronary vessels that develop in response to vascular endothelial growth factor B or VEGF-B in the heart and the effect of VEGF-B on recovery from myocardial infarction. Dr. Carolyn Lam: So what were their results? Greg Hundley: Well Carolyn, the myocardial VEGF-B trans-gene promotes the formation of endocardium-derived coronary vessels during development, endothelial proliferation in sub-endocardial myocardium in adult mice and structural and functional rescue of cardiac tissue after myocardial infarction. So VEGF-B could provide a new therapeutic strategy for cardiac neovascularization after coronary occlusion to rescue the most vulnerable myocardial tissue. Well, Carolyn that's all of the main articles. How about we turn to some of the other articles and letters in the issue? Dr. Carolyn Lam: Yeah. Why not? And Greg, let me start by talking about an exchange of letters between Dr. Wei and Dr. Fox on interpreting the net clinical benefit from rivaroxaban plus aspirin versus aspirin for chronic vascular disease. There's also an ECG challenge by Dr. Patel elusively entitled, A Rainy Day. Here's a hint, it's about hypothermia. In cardiology news by Bridget Kuhn. She talks about how the pandemic throws cardiovascular trials off course, there is an On My Mind paper by Dr. Most entitled, The Striking Similarities of Multi-system Inflammatory Syndrome in Children and a Myocarditis-like Syndrome in Adults: The overlapping manifestations of COVID-19. As a couple of Research Letters, one by Dr. Malhotra on Defining the Normal Spectrum of Electrocardiographic and Left Ventricular Adaptations in Mixed Race, Male, Adolescent Soccer Players, as well as by Dr. Qi on adherence to a healthy sleep pattern and incident heart failure, a prospective study of more than 400,000 UK Biobank participants. Dr. Greg Hundley: Very nice Carolyn. Well, I've got a primmer review of Cardiac Involvement in Multi-system Inflammatory Syndrome in Children with the corresponding author being Dr. Kevin Freedman. Well, how about we get off quickly to those next two feature discussions? Dr. Carolyn Lam: Yay! Let's go, Greg. Dr. Greg Hundley: Well, listeners, we are to the first of our double feature for the new year 2021. And with me is Israel Valverde from King's College in London and our own associate editor, Dr. Gerald Greil from UT Southwestern in Dallas. Welcome gentlemen. And as we get started Israel, could you tell us a little bit about the background that framed this study and the hypothesis that you wanted to address? Dr. Israel Valverde: Thank you. I mean, the main problem we had in Europe around April 2020, was that both the Center for Disease Control and Prevention in the U.S. simulated a clinical ALIT about a newly described immune disease and inflammatory syndrome in children associated with a COVID infection. And there were similarities with all the well-known syndromes, such as viral myocarditis, Kawasaki disease, Kawasaki shock syndrome, and toxic shock syndrome, that we were a bit of confused, because of the overlap in clinical presentation, because it was a true diagnostics challenge. So the hypothesis in this study is we wanted to describe the cardiovascular implications in this newly described sinus syndrome. Dr. Greg Hundley: Very nice. So tell us a little bit about your study design and what study population did you assemble to address this question? Dr. Israel Valverde: I think that was the most difficult thing in the study, because thanks to the Association For European Pediatric Cardiology, the APC, we coordinated a multicenter study involving all they would appear on centers that we were 55 centers from 17 countries all over Europe. And we were able to recruit 286 children with this new newly described syndrome and cardiovascular manifestations. Dr. Greg Hundley: And tell us, what did you find? Dr. Israel Valverde: I think we can summarize that in three main findings. First is that cardiac involvement is very common in children with multi-system inflammatory syndrome associated with coronavirus disease, 2019 infection. Second, that inflammatory markers were significantly raised in most children, particularly their C-reactive protein, ferritin, pro-calcitonin, N-terminal pro BNP natriuretic peptide, interleukin 6, and D-dimer level. And finally, that 65% of patient with MIS-C had evidence of previous infection with severe acute respiratory syndrome coronavirus too, either by PCR, immunoglobulin M or immunoglobulin G. Dr. Greg Hundley: Were there any particular aspects of your results that may have segregated to the boys versus the girls? Dr. Israel Valverde: Not really. We couldn't find any differences between boys and girls, because our study population was quite similar, but we couldn't find any differences between them. What we found is that there is a huge difference between children and adults. Dr. Greg Hundley: And what was the age range of these children? Dr. Israel Valverde: From a couple of weeks, until 18 years old. Dr. Greg Hundley: And no differences in the scope of the syndrome that they experienced in the younger children versus the- Dr. Israel Valverde: Initially we found two peaks. Similarly to the peaks of viral myocarditises, which is affecting more of the children below two years, and also adolescence. Dr. Greg Hundley: Well Gerald, multi-system inflammatory syndrome in children. How to results from this study compare with perhaps other inflammatory disease processes that have been observed in children, such as Kawasaki disease, et cetera. Dr. Gerald Greil: So we and Circulation were very lucky to get a bunch of submissions regarding a MIS-C and also have a radio article regarding this and I would like to point our readers towards that. I think the key issue is it's a new disease entity. The community is pretty clear about it, that other diseases like Kawasaki disease have similarities, but it's not the same thing. And we wanted to be very clear about this. Obviously, more things need to be investigated, but the key findings where Dr. Valverde pointed out in other study groups within the U.S. and within Europe, all point to the same direction, that is a new entity, we need to further investigate. Dr. Greg Hundley: How about the cardiovascular aspects, what differences in cardiovascular disease with this syndrome, again, relative to other perhaps inflammatory diseases that might affect the heart in children? Dr. Gerald Greil: As far as we know. And once again, when we're talking about a very preliminary data, is that the outcome in children having MIS-C usually good. Dr. Valverde pointed out in his study series of more of 286 children, there was only one death and recovery was pretty high rate. We have similar findings from U.S. groups, other findings from a group in Paris and Europe. So, I would like to point our readers to other summaries where it's pretty clear that it's a new disease entity. Overall, it's actually rare in children. So we want to make a pretty clear point that it's a rare disease, which at this point in time seems to have a good outcome. Dr. Greg Hundley: Well Israel, I'm going to come back to you. What do you see is the next study that should be performed perhaps in this general area of cardiovascular disease for children? Dr. Israel Valverde: So I think what we have learned is that the way to move forward is the collaboration between centers. So now that we have a large study multicenter group, our idea is continue describing the long-term outcomes of the study population, because most of them recovered, but a few of them keep, for example, coronary artery dilatation. So do they recovering when you have time until you have time, do they do the ejection fraction, the function of the heart go back to normality soon? Or do we have to wait? So I think that's the next step forward to probably the long-term outcome of the study population. Dr. Greg Hundley: Longitudinal follow-up. And Gerald, do you have anything to add to that? Dr. Gerald Greil: I would just like to reiterate what Israel said, that we need to focus on multicenter studies. I hope Circulation can be a platform to reunite different groups around the world. Because as I mentioned before, I think we have a new disease entity in front of us. We don't know the long-term outcomes. And in the interest of our children confronted with new disease, we need to be very, very careful to learn how we need to follow up these children and how we potentially need to treat them in the long-term. Dr. Greg Hundley: Well, listeners, this has been a fantastic discussion with Dr. Israel Valverde from King's College in London and our own Dr. Gerald Greil from UT Southwestern, revealing some aspects of this multi-system inflammatory syndrome that's associated with COVID-19 in children. And so, on behalf of Carolyn and myself, well, we've got to get to the next feature. So, I'm not going to let you go just yet. Well, listeners, we are in the double feature year 2021. In our second feature discussion today, we have Dr. Michael Gold from Medical University of South Carolina and our own associate editor, Dr. Sana Al-Khatib, who has written an editorial on this paper. And she is from Duke University. Welcome to you both. Michael, we'll start with you. Could you describe for us a little bit about the background related to this paper and what hypothesis did you want to address? Dr. Michael Gold: Well, thank you. And thank you for this opportunity. The implantable defibrillator is a fundamental aspect of the treatment and prevention of sudden cardiac death. It's been around for almost 40 years now and is commonly used in the system has evolved from being a surgical procedure, requiring a thoracotomy to a transvenous procedure in which leads could be placed into the heart. While it's very effective, the major limitation of this in many people's minds were, major limitations were both of complications associated with the transvenous lead. Those include infections and lead failures, as well as unnecessary or inappropriate shock to patients. And based on that, the subcutaneous ICD was developed as the latest iteration of this technology in which a lead is placed under the skin and tunneled up along the sternum, so that one could sense and shock the heart when necessary without being subjected to an intravascular lead. Dr. Michael Gold: And the device has been around for 10 years or so. It's proven to be effective, but primarily used in niche populations of younger patients, patients with poor vascular access and those considered at relatively low risk with few comorbidities, such as patients with Brugada syndrome or long QT syndrome or possibly hypertrophic cardiomyopathy. We felt it was important both to establish the role of the subcutaneous ICD in a more typical group of defibrillator patients, as well as with the multiple evolutions now of programming, as well as technology within the device of seeing if the more modern contemporary devices were as effective in these sicker populations, and also could reduce some of the issues seen earlier in terms of higher rates of inappropriate shock therapy. Dr. Greg Hundley: So Michael we're working with subcutaneous ICDs. What was your study population and how did you design this study to really test the efficacy of the subcutaneous ICDs relative to the more conventional transvenous lead systems in these high-risk patients? Dr. Michael Gold: So what we did was to first identify a population and the most common population price of the implantation in the United States have for sure, in many of the countries is primary prevention patients and low ejection fraction. So we restricted the study to patients with an ejection fraction less than 35% and primary prevention, meaning have never had an episode of sustained ventricular tachycardia or cardiac arrest. So starting with that population, this was a prospective large registry. The bandwidth was out there to do a very large randomized trial, which was just done, although not as in a sicker patient population that study called PRAETORIAN was going on simultaneously. And what we decided was to be aggressive if you will. And we defined our endpoints for the study by looking at identified studies previously with transvenous ICDs that have the most contemporary programming and the lowest risk of inappropriate shock. Dr. Michael Gold: So we used the MADEIT-RIT study, which most people probably would consider it to be the ultimate, if not one of the two contemporary studies for that. And we wanted to essentially compare head-to-head results with that, obviously without those specific patients, but using that as in some ways a historical control or benchmark. So, we set what we wanted to use as our performance goal based on the MADEIT-RNT study for inappropriate shocks, as well as looked at complication rates, using performance goals that have been well-established by the FDA. And we use an 18-month time window, because that's a time when most inappropriate shocks as well as most complications will show up with this device. Dr. Greg Hundley: So what did you find? Dr. Michael Gold: So, what we found was first that we accomplished the goal of having patients who were had much more comorbidities or sicker, if you will. A majority of patients of the over 1,000 patients in the study had ischemic heart disease. I mean, ejection fraction, it was down 26%. In perspective, the early studies of the S-ICD, I mean, ejection fractions are 40% higher, almost 90% of patients have heart failure rather than a third of patients in previous studies and the incidents of diabetes, kidney disease, hypertension and other things with two to three times higher than what would have been done or shown in any early registries of the S-ICD. So we're very pleased about that. Again, there were over 1,100 patients in this study. And what we showed was that at 18 months, the inappropriate shock rate for the whole population was only 4%, hopefully 2.7% annually, which is quite low compared to any other S-ICD study and was very favorably compared with the MADEIT-RIT study easily met the performance goal for that. Dr. Michael Gold: And if we looked at that subgroup of patients who had generation three of the most modern of the S-ICD devices that have a newer discrimination algorithm, that number even became lower than that. So again, very, very reassuring to see what we found there and for the generation three device, it was only 2.9% of patients in 18 months had any inappropriate shock for that. And if we then looked at the other aspects of it, the all shock rate for appropriate and inappropriate shock was only 10% of 18 months, which meant the performance goal for that as well. And what's important about the all shock rate is that it's nice to show that the device can defibrillate a patient in the lab when you're testing them. And this was successful in 98, 99% of the time, which is typical for transvenous or subcutaneous devices. But when we looked at spontaneous arrhythmic events, it was highly effective, 100% of VT storms, and all but one case of a VTVF episode was converted with defibrillator, that one case actually spontaneously converted, but was officially listed as a failure, because it didn't convert right away. Dr. Michael Gold: But no patients had a cardiac due to ventricular fibrillation that could not be successfully treated or ended up requiring external defibrillation. And finally, despite a much sicker population, again, if you will, we showed that at 18 months, the overall complication rate from the device was only about 7% in total, which again is very low compared to what we'd expect to see with other devices. And no patient developed a bloodborne infection or a true lead failure, which is one of the major complications that, and dreaded complications of transvenous devices. Dr. Greg Hundley: Well, Sana as our editorialist, what stood out to you as being really important findings in this study? Dr. Sana Al-Khatib: First of all, I want to start by congratulating Michael and his team on the completion of this important study. And I agree that this has been a really important addition to the armamentarium of studies related to subQ-ICDs. We have had registries in the past. In fact, Michael alluded to the publication of the PRAETORIAN trial, which was really the first trial to compare the S-ICD with the transvenous ICD in a randomized control trial design. However, despite the important contributions of the PRAETORIAN trial, there were several things that remain unaddressed or unanswered in terms of the rate of the end points, especially in appropriate shocks with the newer S-ICD models. So where we had an abundance of those in the UNTOUCHED study, and then really the important question of do the results of that trial, which was largely not done in the United States, apply to the average patient, seen in the United States. Dr. Sana Al-Khatib: And then could those results be extrapolated to sicker patients, because as Michael very nicely highlighted that those patients were not as sick as patients that we see in routine clinical practice. So from my perspective, those are the findings of the UNTOUCHED study are really important, because we now have confirmation that the S-ICD is actually effective and safe in a patient population representative of the average patient seen in clinical practice in the U.S. with a primary prevention indication for the ICD, no really sicker patients, good representation of women, black patients. So overall, I think this trial really gives me a reassurance that the subcutaneous ICD is safe and effective, that the rate of inappropriate shocks with the newer generation of ICDs is really low. That said, I just wanted to highlight a couple of points that I'm hoping to see more data on as we go forward. Dr. Sana Al-Khatib: The study was only 18 months long. It would be good really to have a longer studies. Again, the average age of patients enrolled in the trial was on the younger side in the 50s, and I would love really to see more data on the S-ICD in older patients. And also as Michael said that these results were obtained in patients who had a primary prevention indication for the ICD on the basis of systolic heart failure. Well, we have other patient populations. And especially with, for example, patients with hypertrophic cardiomyopathy, where the rate of inappropriate shocks might be higher, and it would be great to see even more data as we go forward on that patient population and other subgroups of patients that were not included or not well-represented in the trial, but overall a really important trial. Dr. Greg Hundley: Very nice Sana. And Michael, just swinging back to you, what do you think are some of the next studies that might be performed in this space? Dr. Michael Gold: No, I think that Sana made some nice points. I should point out that, in its, I think appropriate vision, the FDA require as long-term evaluation of devices and studies. So the S-ICD post-approval study, which I'm fortunate enough to be involved with as well, is a five-year followup study. We've already published a little bit on two and three-year data, but we will have five-year data on this device in a patient that is closer to the S-ICD than the early registries. It was not restricted to just low EF patients, but it was a U.S.-only study of over 1,500 patients. So, a larger study that has the typical distribution of patients that we see in a U.S. practice who would be eligible for the S-ICD, those without pacing indications and both primary and secondary prevention patients. So I think that will be very important. Dr. Michael Gold: Where the technology is going is largely, there will still be iterations of longer battery life, smaller devices and those things, but there's been a real push to be able to also provide pacing therapy without the need for intravenous leads. So there are several models and systems approaches being used, including adding a lead-less pacemaker that will communicate with a subcutaneous ICD or placing a lead under the sternum, still extra vascular with the capabilities of pacing. So I think the next sort of group of trials will likely be seeing on this device other than simply looking at other populations in a little more depth will be further expansion of the technology to allow for a greater use of a device for those patients who may require a pacing. Dr. Greg Hundley: Well listeners, we want to thank Dr. Michael Gold from Medical University of South Carolina and Dr. Sana Al-Khatib from Duke University, our editorialists for describing this study. And in regards to those, or in regards to subcutaneous ICDs, providing confirmation that these devices are both effective and safe, particularly in a U.S. population for primary prevention of sudden death and really a high-risk patient population with low ejection fraction. Host Elliot Michael gold interviews John Barbour, born on April 24, 1933 in Toronto, Ontario, Canada is an actor, comedian, television host known as host of the NBC reality television series Real People, for which he was also a creator and co-produce and is the only performer in TV to win Emmys for both entertainment and news shows. Barbour moved to the United States in the early 1960s. His comedy act, particularly his 1965 album It's Tough to Be White, dealt in part with civil rights and black-white relations. In 1971, he hosted a local daytime talk show on CBS-owned KNXT (now KCBS-TV) in Los Angeles. He also hosted the pilot for The Gong Show in the mid-1970s (walking away from the show in objection to its farcical premise and forcing producer Chuck Barris to step in front of the cameras as host), and was a regular panelist on the 1988 Canadian (US syndicated) version of Liar's Club. Barbour portrayed game show host Harry Monte in a 1975 episode of Sanford and Son. Barbour was the executive producer of the game show That **** Quiz Show (That Awful! Quiz Show) hosted by Greg & John Rice in 1982. Barbour wrote and narrated Keith Burns' documentary Ernie Kovacs: Television's Original Genius. He also directed and wrote the 1992 documentary The JFK Assassination: The Jim Garrison Tapes. This film covers the investigation of District Attorney Jim Garrison, who, after the 1963 assassination of John F. Kennedy, decided to further investigate the official report given by the Warren Commission. The documentary hypothesizes connections between the assassination and the FBI, the CIA, the Mafia, the Cuban Missile Crisis, the Vietnam War, and other organizations and foreign affairs issues. The film won an award in 1993 at the San Sebastian Film Festival in Spain. He directed and wrote another documentary, “The American Media And The Second Assassination of President John F. Kennedy,” which released to specific audiences in 2017, but saw a broader audience in 2019 when it was showcased in iTunes, Vimeo and Amazon. This follow-up documentary is an extension of his critically acclaimed "The Garrison Tapes," and has received significant praise for its detail and exposition on the subjects of the JFK murder and the evolution of contemporary Fake News. IMdB has scored the documentary 8.6 out of a possible 10. Ambassador Andrew Jackson Young Jr. born March 12, 1932 in New Orleans, Louisiana. Ambassador Young is a politician, diplomat, and activist. Beginning his career as a pastor, Young was an early leader in the civil rights movement, serving as executive director of the Southern Christian Leadership Conference (SCLC) and a close confidant to Martin Luther King Jr. Young later became active in politics, serving first as a U.S. Congressman from Georgia, then United States Ambassador to the United Nations, and finally Mayor of Atlanta. Today, when unions are again subject to severe legal disadvantage, it is timely to recall the first Trades Union Congress (TUC) and unions’ achievements, and to look forward on union prospects for the future. 28 | Michael Omori Kirchner | Creative Biz: Coaching für Kreative Michael Omori Kirchner ist professioneller Business-Fotograf und Business-Coach für Kreative. Außerdem hat er auch einen eigenen Podcast. Was er genau tut, erfahren wir in dieser HebelZeit-Folge. Michaels Werdegang Michael Omori Kirchner hat zwei Standbeine: Die Fotografie und das Business-Coaching. Bis vor einigen Jahren hat Michael als angestellter Berater und Coach in der Industrie gearbeitet. Dann jedoch hat er sich als Fotograf selbstständig gemacht. Dabei hat er gemerkt, dass das Fotografieren von Menschen auch viel mit Coaching zu tun hat: Man leitet Leute an, etwas gut hin zu bekommen. Die beiden Tätigkeiten liegen also nah beieinander. Sein Marketing-Wissen, was ihm in seiner Selbstständigkeit als Fotograf natürlich viel genutzt hat, hat er damals in seinem kostenlosen e-book Marketing für Fotografen nieder geschrieben. Das hilft übrigens nicht nur Fotografen weiter, sondern allen Kreativen. Aufgrund dieses Buches kamen dann die ersten Anfragen, ob er nicht auch Seminare zu diesem Thema halten könnte. So kam es zu seinem zweiten Standbein, dem Business-Coaching. Wie Michael sein Buch als Hebel benutzt Zu Beginn hat Michael Omori Kirchner sein Buch einfach kostenlos als Download angeboten. Irgendwann bekam er von einem Kollegen den wertvollen Tipp, das Buch gegen die E-Mail Adresse der Leute heraus zu geben. Dieser Tipp war für Michael Gold wert: Nun hat er nämlich eine Liste an E-Mail Adressen von Leuten, die sich für seine Inhalte interessieren. Einmal pro Woche schickt er diesen eine inhaltlich wertvolle E-Mail heraus mit seinen Tipps. Ab und an mischt er dezent auch ein bisschen Werbung für seine Seminare und Angebote darunter, ohne die Leute zu nerven. Dadurch baut er über die Zeit viel Vertrauen auf. Michael nutzt weitere Hebel Abgesehen von der E-Mail Kommunikation nutzte Michael schon so einige Kanäle, z.B auch Snapchat. Allerdings merkte er schnell, dass das nicht seins ist. Ganz im Gegenteil zum Podcast: Diesen entdeckte er erst als Zuhörer. Ihm fiel aber schnell auf, wie einfach ein Podcast zu produzieren ist und hatte Interesse, selbst einen zu veröffentlichen. Seither veröffentlicht er seinen eigenen Podcast. Außerdem bietet er Videotrainings an: Da er immer wieder die Frage nach der Preisgestaltung von Fotografen gestellt bekam, hat er dazu ein Video produziert. Spezialfragen erreichen ihn zwar trotzdem noch, aber viele Menschen stellt er mit diesem Video zufrieden. Ein anderes Videotraining zum Thema Kundengewinnung ist ein Mitschnitt eines Seminars. Das ist Michael wichtig: Individuelle Beratung Michael berät trotz der geringen Hebelwirkung immer noch sehr gerne individuell. Das möchte er auch nicht aufgeben. Er schätzt dabei vor allem das Feedback, das er bekommt. Außerdem kann er persönliche Herausforderungen seiner Kunden nicht über Video, E-Mail oder Podcast lösen. Wenn es um Sachen wie Motivation geht, dann braucht er das persönliche Gespräch im Seminar oder im Einzel-Coaching um zu helfen. Warum kommen die Leute zu Michael? Michael braucht ja keine tausend Kunden; er braucht ein paar Kunden, die ihm vertrauen und die ihn wertschätzten. Außerdem sind seine Kunden Leute, die ihn kennen und die wissen, wann Michael ihnen weiter helfen kann und was sie von ihm zu erwarten haben. Dieses Vertrauen zu seinen Kunden baut er nicht von heute auf morgen auf: Viele seiner Kunden überzeugen sich über Monate oder Jahre zum Beispiel über seinen Newsletter davon, dass seine Arbeit Hand und Fuß hat. Es steckt also viel Arbeit hinter Michaels Kundenbindung, die über Vertrauen funktioniert. Er steckt gerne viel Zeit in seinen Podcast und seinen Newsletter um hochwertige Inhalte zu erzeugen, und um seine Kunden langfristig von ihm zu überzeugen. Typische Anfänger-Fehler im Business Kreative Menschen machen viele Anfänger-Fehler, wenn es ums Business geht. Zwei Themen sind die Preisgestaltung und die Positionierung auf dem Markt. Anfänger sollten sich auf jeden Fall erst einmal Gedanken darüber machen, wie sie beim Kunden ankommen wollen, was beim Kunden im Gedächtnis bleiben soll. Außerdem ist es wichtig, dass das Angebot vom Kunden verstanden wird und nicht zu kompliziert ist. Michaels Hebel-Tipp Such dir eine Aufgabe, die du im Moment machst, die du aber nicht gerne machst und nicht besonders gut kannst. Gib diese Aufgabe dann jemand anderem. Michael wollte zum Beispiel gerne ein Transkript von einem Seminar haben. Also hat er sich jemanden gesucht, der diese Dienstleistung anbietet, und es natürlich viel schneller und besser kann, als er selbst. Zweiter Tipp: Wenn man gerne Podcasts hört, dann kann man mal versuchen, die Abspiel-Geschwindigkeit hoch zu drehen. Das mache ich selbst übrigens auch. Dann hört man einen Podcast statt in einer halben Stunde vielleicht in 20 Minuten. Und schwups, wieder 10 Minuten über :) Noch ein Tipp von mir zum Schluss: Ich persönlich schneide meine Videos sogar in doppelter Geschwindigkeit, zumindest im Rohschnitt. Wenn man das kann und erträgt, dann spart man sich da eine Menge Arbeitszeit.
