Our guest this week is Dr. Diane Ehrensaft. Dr. Ehrensaft is director of mental health, and a founding member of the UCSF Benioff Children's Hospital Child and Adolescent Gender Center Clinic. She is a developmental and clinical psychologist in the San Francisco Bay Area. We're kidding. Sort of. Aaron T recently attended the Annual Trans Health Summit in San Francisco, where he met up with two similarly concerned local Bay Area clinicians - one a pediatrician and the other a clinical psychologist - the latter of whom managed to covertly record every session of the conference she attended. What you are about to hear is that audio in full, along with commentary by the Aarons and the aforementioned clinicians who will remain anonymous for professional reasons. This is the first in a multipart series, where we will bring you into the room of the recent Annual Trans Health Summit. Phil Illy is an autoheterosexual author who, in his words, “aims to mainstream autoheterosexuality and destigmatize it so that more of his kind can jettison shame and live in union with their inner cross-gender spirit.” Prior to writing a soon to be released book about his sexual orientation, Phil received degrees in physics and mechanical engineering before retiring to Portland, Oregon, where he became best known for his skill at handstand twerking and juggling hula hoops. Here is a brief glossary of terms: "Allo" means other, or external. "Auto" means self, or internal. "Gyno" means female. "Andro" means male. "Philia" means love (or, less euphemistically, sexual attraction). "Meta" means self-referential A typical heterosexual male is technically "allogynephilic". The vast majority of people are, in sexologic terms, alloheterosexual. But a male who is attracted to the thought or image of himself as female is "autogynephilic", while a female who is attracted to herself as male is "autoandrophilic". These last two are what our guest has termed "autoheterosexual". The final term above, meta, is used in this context to describe the type of external sexual behaviors of autoheterosexuals, who often experience (and act upon) homosexual desires as a means of validating their self-perception as the opposite sex. For example, an autogynephilic male is not attracted to other males (not alloandrophilic), but sexual behavior with males serves as a validation of the self as female. That is metaandrophilia. What is the common ground for modern psychotherapy? - Professor Stefan G. Hoffman, Professor Jacques P. Barber, Professor Paul Salkovskis, Professor Bruce E. Wampold https://english.swps.pl/eaclipt The field of psychotherapy has developed from different roots, and many clinicians and scientists still consider psychotherapy as a collection of unconnected groups of theories and associated interventions. This approach prevents a cohesive development of the discipline and a holistic treatment of patients. During the webinar, Professor Stefan G. Hofmann from the Philipps-University Marburg, Germany and Boston University, USA will talk about process-based psychotherapy, which takes the view that mental health problems are assumed to exist as systems of inter-connected elements. The keynote lecture will be followed by a panel discussion on finding the common ground for evidence-based psychotherapies. KEYNOTE LECTURE: PROCESS-BASED PSYCHOTHERAPY For decades, evidence-based therapy, such as Cognitive Behavioral Therapy (CBT), has been defined in terms of treatment protocols focused on syndromes as defined by the DSM and ICD. These psychiatric classification systems assume that psychological problems are expressions of latent disease entities. However, there is little evidence to support this restrictive assumption. A process-focused approach (Process-Based Therapy, PBT) is now emerging. This approach does not rely on the assumption that psychological problems are expressions of latent disease entities, but it does not rule it out either. Instead, mental health problems are assumed to exist as systems of inter-connected elements. As is true for many complex networks, such a change can occur abruptly once the network reaches a tipping point. PBT directly links treatment techniques to processes in the individual client, thereby linking classification to treatment based on functional analysis and complex networks. This offers a less restrictive and more externally valid alternative to the latent disease model, while offering exciting new directions for future research in psychiatry and is in line with personalized medicine. KEYNOTE SPEAKER Stefan G. Hofmann, Ph.D. – is the Alexander von Humboldt Professor of Clinical Translational Psychology of the Philipps-University Marburg, Germany, and also Professor of Psychology at the Department of Psychological and Brain Sciences at Boston University. He has been president of ABCT and IACP, and is editor-in-chief of Cognitive Therapy and Research. He has published more than 400 peer-reviewed journal articles and 20 books. Hi everyone, today is a very special episode of The Psychology Podcast. A few nights ago, the legendary psychiatrist Dr. Aaron Temkin Beck passed away peacefully in his sleep at the age of 100. Tim, as his friends and family affectionally called him, lived an exemplary, full life well lived. Personally, he was a dear mentor and friend of mine. I used to live in the building next door to him in Philadelphia and we'd have tuna sandwiches together on Sundays at his place and discuss humanistic psychology and how to treat patients as humans first. He was always so encouraging of my work, and I enjoyed our discussions about his life and work immensely. I will miss his bow tie, fist bumps, and a sharp mind, which lasted all the way until the end. In my last in-person meeting with him just before the pandemic hit, I handed him a microphone and asked if he would talk about what research he was most excited about these days and whether he could give any advice to young psychologists. That recording is what you will hear today on the podcast.Remarkably, Tim worked all the way up until his death. To many, he is most known for his work in Cognitive Behavior Therapy (CBT), which is a time-sensitive, structured, present-oriented psychotherapy that has been scientifically tested and found to be effective in more than 2,000 studies for the treatment of many different health and mental health conditions. When implemented correctly, CBT can help individuals get better and stay better.However, not many people know this, but Tim's work was much more than the seminal work he did pioneering CBT. Tim was recently working on a new form of psychotherapy with his colleagues Ellen Inverso and Paul Grant called "Recovery-Oriented Cognitive Therapy”, which deeply humanizes psychiatric patients. Guided by Tim's cognitive model, Recovery-Oriented Cognitive Therapy (CT-R) is an evidence-based practice that provides concrete, actionable steps to promote recovery and resiliency. Originally developed to empower individuals given a diagnosis of schizophrenia, Recovery-Oriented Cognitive Therapy applies broadly to individuals experiencing extensive behavioral, social, and physical health challenges. It is a highly collaborative, person-centered, and strengths-based approach, as it is focused on developing and strengthening positive beliefs of purpose, hope, efficacy, empowerment and belonging. The approach is specially formulated and effective for individuals (i) who have a history of feeling disconnected and distrustful of service providers, (ii) who are not help-seeking, or (iii) who experience challenges that impede action towards aspirations. The reach of Recovery-Oriented Cognitive Therapy extends to mental health professionals across all disciplines, families and loved ones, and peers with lived experience.One other thing I'd like to mention before we get to today's episode is the Beck Institute. In 1994, Tim and his daughter, Dr. Judith S. Beck, founded Beck Institute as a 501(c)3 nonprofit with the mission of improving lives worldwide through excellence and innovation in Cognitive Behavior Therapy training, practice, and research. In 2019, Beck Institute opened the Beck Institute Center for Recovery-Oriented Cognitive Therapy to train professionals and staff who work with individuals given a diagnosis of a serious mental health condition, such as schizophrenia. In this episode, Xavier Bonilla has a dialogue with Paul Grant and Ellen Inverso about Recovery-Oriented Cognitive Therapy (CT-R) . They provide an overview of Aaron Beck’s cognitive model both theoretically and in clinical practice. They discuss the power of Cognitive-Behavioral Therapy (CBT) as an Evidence-Based Treatment (EBT) and why that’s important. They talk about whether one can use CBT techniques without conceptualizing. They discuss what CT-R is and how it has had an emphasis on treating the negative symptoms of schizophrenia spectrum disorders. They explain why they started with schizophrenia spectrum disorders and how the theoretical model integrates many aspects of humanism. They talk about the differences between CT-R and CBT-Psychosis (CBT-P). They also mentioned the empirical evidence and support for CT-R and how they continue to move forward in exploring other aspects of CT-R’s application. Dr. Paul M. Grant is a clinical psychologist and Co-Director of the Center for Recovery-Oriented Cognitive Therapy (CT-R). For the past 20 years, Dr. Grant, along with Aaron T. Beck, MD, originated and advanced CT-R, which is a revolutionary evidence-based approach to treating severe mental health conditions. He has extensively researched and studied the theoretical basis and application of CT-R to individuals given a diagnosis of schizophrenia. Dr. Inverso is a licensed psychologist and subject-matter expert in Recovery-Oriented Cognitive Therapy (CT-R). She provides training and consultation to mental health care providers in CT-R for individuals with serious mental health conditions. Along with her colleagues, Dr. Inverso is involved in efforts to infuse recovery into large systems of care and has developed innovative and transformative strategies for implementing CT-R on inpatient psychiatric units, in community residences and forensic facilities, and on assertive community treatment (ACT) teams. Joy, fear, panic, sadness, anxiety, compassion, jealousy, shame and anger. Emotions are an inherent element of life and they play a very important role. Unfortunately, many people experience difficulties in dealing with various emotions, which leads to emotional suffering. Depression, anxiety or insufficient anger management are some of the most common psychological disorders. During his lecture, Professor Stefan G. Hofmann, a preeminent expert in Cognitive Behavioral Therapy from Boston University, will present the latest research on emotions, their management and forms of therapy. The lecture was coupled with a launch of the Polish edition of prof. Hofmann’s book "Process-based CBT: The science and core clinical competencies of cognitive behavioral therapy”, published by Gdańskie Wydawnictwo Psychologiczne (GWP). Emotions are critical aspects of mental health. A staple theme in clinical psychology, emotion regulation, or the ability to manage one’s emotions, is directly linked with personal wellbeing and the ability to effectively navigate the social world. Although conventional treatments for emotional disorders are generally effective interventions, many people still suffer from residual emotional distress, such as anxiety, depression, and anger problems. Moreover, contemporary treatments that have focused primarily on negative affect rarely leads to lasting improvements in positive affect, quality of life, and happiness. Recent scientific evidence and theoretical models of emotions can enrich the therapeutic strategies when treating emotional disorders. Until recently, this concept has been limited to a focus on intrapersonal processes, or the process of regulating one’s own emotions. Less emphasis has been placed on developmental, social, and cultural aspects of emotion regulation. However, as social beings, our engagement in emotion regulation may often occur interpersonally, with trusted others helping us to regulate our emotions. I will highlight recent research on interpersonal emotion regulation processes. Although social factors are of critical importance in the development and maintenance of emotional disorders, the contemporary view of emotion regulation has been primarily limited to intrapersonal processes. Based on diverse perspectives pointing to the communicative function of emotions, the social processes in self-regulation, and the role of social support, I will present an interpersonal model of emotion regulation of mood and anxiety disorders. This model provides a theoretical framework to understand and explain how mood and anxiety disorders are regulated and maintained through others. The literature, which provides support for the model, is reviewed and the clinical implications are discussed. These strategies range from adaptive and flexible intrapersonal and interpersonal emotion regulation strategies to various mindfulness-based practices. These insights can inform the treatment for emotional disorders. Stefan G. Hoffman – is a clinical psychologist, a preeminent expert in Cognitive Behavioral Therapy and professor in the clinical program at Boston University, where he directs the Psychotherapy and Emotion Research Laboratory at the Center for Anxiety and Related Disorders. His research focuses on the mechanism of treatment change, translating discoveries from neuroscience into clinical applications, emotions, and cultural expressions of psychopathology. He is former president of the Association for Behavioral and Cognitive Therapies, and the International Association for Cognitive Psychotherapy. He is also editor in chief of Cognitive Therapy and Research and is Associate Editor of Clinical Psychological Science. He was an advisor to the DSM-5 Development Process and a member of the DSM-5 Anxiety Disorder Sub-Work Group. Recipient of many prestigious professional awards, including the Aaron T. Beck Award for Significant and Enduring contributions to the Field of Cognitive Therapy.

Dr. Brinen is an assistant professor in the Drexel College of Medicine Department of Psychiatry, where he directs a center for the dissemination, development study and practice of recovery oriented cognitive therapy for psychosis. Additionally, he trains psychiatry residents in CT-R. Dr. Brinen is a licensed clinical psychologist specializing in cognitive behavioral therapy for individuals with schizophrenia, PTSD, and other disorders. In addition to training, research and treatment development, Dr. Brinen has a small practice where he sees individuals. Dr. Brinen is also a trainer for the Academy of Cognitive Therapy and was a faculty member at the Beck Institute, where he trains nationally and internationally on a variety of topics, including CBT for schizophrenia and PTSD.

Listen in as Dr. Sally E Riggs introduces you to Cognitive Behavioral Therapy for psychosis (CBTp). Dr. Riggs shares a brief overview of CBTp; what it is, who may help, evidence of its success, where to find it and how to talk about it with your patients. CBTp is a person-centered, time limited, evidence-based therapy for psychosis. It posits that adverse life events do not cause negative emotions, but instead our own thoughts on the event do. This is a goal and recovery oriented treatment, uniquely tailored to the individual. It helps them make sense of their experience to lower distress and achieve life goals. This process has been easily misconstrued in the United States as skill based, or challenging people’s thoughts. However, this is not case as it is individualized and collaborative, requiring no expert therapist to lead. Dr. Riggs speaks on the evidence collected through randomized control trials over the last 40 years that show the numerous benefits of CBTp. Sally E. Riggs, DClinPsy, ACT, is a licensed psychologist and expert in Cognitive Behavioral Therapy for psychosis with 18 years of experience working in this field in both London, UK and the US. She moved to NYC in 2011 and has been working in public and private healthcare settings, practicing and teaching and training mental health professionals in CBTp since this time. She has been an invited member of the Annual International CBT for Psychosis Research Meeting since 2009, and is a Founding Member and current President of the North American CBT For Psychosis Network (CBTpN). His teaching and research areas of focus are corrections and public policy. He is a consultant that has worked nationally on criminal justice reform, including contracts with the U.S. Department of Justice and has worked with thousands of incarcerated and formerly incarcerated individuals for over 20 years. Kinzel's passion for these issues arose from his own incarceration as a youth in which he spent several months in juvenile detention and then nearly 10 years in adult correctional facilities and finally 6 years on parole in the State of Michigan. In the Cricket licious Café, ChefPV teaches us how to make Mealworm Mac n Cheese Souffles, and on The Night Talker, it’s “Night of the Frogs”… … Mealworm Mac n Cheese Souffle INGREDIENTS 1 ½ TBSP BUTTER 1 TBSP ALL-PURPOSE FLOUR ¾ cup MILK SALT (to taste) PEPPER (to taste) 1 cup SMOKED GOUDA CHEESE (shredded) 1 EGG YOLK 2 EGG WHITES 1 cup Bugeater Foods MEALWORM MACARONI PASTA (cooked) ¼ cup MEALWORMS (thawed) DIRECTIONS Preheat oven to 350°F. In a saucepan melt the BUTTER, add the FLOUR and stir on medium heat for a minute. Add the MILK gradually while stirring to avoid lumps. Season to taste with SALT and PEPPER and stir the sauce until sauce thickens. Remove saucepan from heat and mix in the CHEESE until melted. In a small mixing bowl, whisk the EGG YOLK and mix into cheese sauce. In a large mixing bowl, whisk the EGG WHITES until stiff peaks are formed. Gently fold the cheese sauce into the egg whites until well combined. Butter the ramekins then fill a little more than a third of the way with MEALWORM MACARONI PASTA. Fill the ramekins to almost full with cheese sauce then top with MEALWORMS. Place ramekins on a baking sheet and bake souffles on the middle rack for 15 - 20 minutes until the surface becomes golden. Serve immediately

Today's Cricketlicious Cafe recipe: Pineapple Crick Sticks 1 large Pineapple 6-inch Skewers or Lollipop Sticks 1 TBSP Olive Oil 1 cup Sustainable Boost Crickets 2 TBSP Honey Sustainable Boost Turmeric Sustainable Boost Ginger 1 cup Semi-Sweet Dark Chocolate Chips 1 cup Pistachios (chopped) On a sturdy cutting board, cut the top and bottom off the Pineapple then trim off all the rind from the sides. Cut the Pineapple in half lengthwise, remove the core, then cut the halves into wedges. Arrange the wedges on paper towels to absorb excess moisture. Insert Skewer/Lollipop Stick into each pineapple wedge and set aside. Heat Olive Oil over medium heat and add Crickets to pan and quick-crisp for about 1 minute. Remove from heat and add Honey, Sustainable Boost Turmeric and Sustainable Boost Ginger. Mix until all ingredients are evenly incorporated. Transfer to a bowl then set aside. Place a small saucepan over medium-low heat, add Dark Chocolate Chips and stir constantly until completely melted. Remove from heat. Line a baking sheet or large plate with wax or parchment paper. Dip each of the Pineapple wedges into the Chocolate and place on the wax paper. Lightly press the Crickets into the bottom half of the chocolate on the Pineapple wedges. Lightly Press the Chopped Pistachios on the top portion of the chocolate on the Pineapple wedges. Place the sheet/plate of Pineapple wedges into fridge for 15 minutes to set or until ready to serve. Thanksgiving Chapulines Spoon Bread con Spiced Cricket Ice Cream Spiced Cricket Ice Cream 1 cup Candied Yams Puree (can or fresh) 2 cups Heavy Cream 1 cup Half & Half ⅔ cup Honey 1 tsp Ginger (minced) 1 tsp Ginger (ground) 1 cup 2 TBSP Mealworm or Cricket TIP (from C-fu FOODS) 1 tsp Nutmeg (ground) 1 TBSP Vanilla Extract If you are using Candied Yams from a can, go ahead and strain syrup from the yams, process them in a blender or food processor until smooth, then set the yams aside in a bowl. Add Cream, Half & Half and Honey to a medium saucepan and place over medium heat until honey is completely dissolved. Remove from heat and whisk in Yam Puree, minced Ginger and ground Ginger. Put TIP, Nutmeg and Vanilla in a food processor or blender with a small amount of the ice cream mix and blend. Slowly add more until all ingredients are mixed together and smooth. You may need to blend in batches depending on the size of your processing container. Pour mix into a freezer-safe container and place in the freezer to set overnight. Remove from freezer when ready to serve with Spoon Bread. Candied Yams Spoon Bread con Chapulines 1 cup Candied Yams Puree (can or fresh) 2½ cups Nut Milk ½ cup (1 stick) Unsalted Butter 2 tsp Smoked Salt 1 cup Stone-Ground Yellow Cornmeal 3 large Eggs (separated) 1 TBSP Chives (chopped) ½ tsp Thyme (stemmed) ½ tsp ground Cinnamon ¼ tsp Cayenne Pepper 1 tsp Ginger (minced) ¼ tsp ground Nutmeg 1 cup Merci Mercado Chapulines (any flavor) If you are using Candied Yams from a can, go ahead and strain syrup from the yams, process them in a blender or food processor until smooth, then set the yams aside in a bowl. Preheat oven to 350°F. Add Nut Milk, Butter and Salt to a large saucepan over medium heat and bring to a simmer. Gradually whisk in Cornmeal and stir about 2 minutes until thickened. Remove from heat then whisk in Yam Puree. Whisk in Egg Yolks, Chives, Thyme, Cinnamon, Cayenne, Ginger and Nutmeg. In a medium mixing bowl, beat Egg Whites until soft peaks form. Add whipped egg whites to cornmeal mixture and whisk together until evenly combined. Fold-in Chapulines to cornmeal mixture. Pour cornmeal batter into a pie pan or loaf pan. Take a small handful of chapulines and sprinkle over top of batter then place in oven for 30 - 40 minutes until spoon bread is set. Remove from oven, plate a serving of Spoon Bread and add a scoop of Spiced Cricket Ice Cream. Serve. Chirpin’ Tostadas 1 stick Butter or ½ cup Your Favorite Frying/Sauteing Oil (for frying Tortillas and sauteing Crickets) 4 Soft Corn Tortillas 1 head Romaine Lettuce or 1 bunch Cilantro (or Both) 1 Tomato ¼ cup Red Onion 1 Avocado ¼ cup Plain Yogurt (Any Style) ⅛ cup Lemon Juice 1 cup Fresh Crickets or Merci Mercado Chapulines Desired Seasonings Melt ½ stick Butter/heat ¼ cup Oil in a skillet over medium-high heat When you Butter/Oil has reached temperature, place Tortilla(s) in pan for 3 to 5 minutes, until crisp on both sides. Ready some paper towels/kitchen towels on a plate, remove Tortilla(s), place on towel to cool and absorb excess oil. Clean then chop Romaine Lettuce and set aside in a bowl. Medium-dice Tomato then set aside in a bowl. Small-dice Red Onion and set aside in a bowl. Slice open Avocado lengthwise, remove pit and scoop Avocado into bowl. Whisk together with Yogurt and Lemon Juice. Plate your Tortillas for serving and add Lettuce/Cilantro, Tomato, Onion and Avocado Crema. Set aside. Place skillet over medium-heat with remaining Butter/Oil and saute Crickets until crispy, about 10 minutes. Merci Mercado Chapulines are already dry-roasted so only need about 5 minutes to add heat and crispiness. Remove Crickets/Chapulines from heat and put on top of your tostadas to complete the dish. DERUBEIS, PhD BIOGRAPHY Dr. DeRubeis has been on the Penn faculty since his appointment as assistant professor in 1983 after receiving his doctorate in clinical psychology from the University of Minnesota. He has served as associate dean for the Social Sciences in the School of Arts and Sciences, and director of Clinical Training in the Psychology Department’s doctoral training program in Clinical Psychology. He is currently chair of the Department of Psychology. He has authored or co-authored more than 100 articles and book chapters on topics that center on the treatment of depression. He received the Academy of Cognitive Therapy’s Aaron T. Beck Award in 2004 for his contributions to research on cognitive therapy. His empirical research comparing the benefits of cognitive therapy and medications for severe depression, published in the American Journal of Psychiatry and the Archives of General Psychiatry, has been the subject of media reports in The Economist, The Wall Street Journal, and USA Today. In 2010 he presented a briefing to the Congressional Biomedical Research Caucus on Capitol Hill in Washington, D.C. Research Interests and Current Projects Dr. DeRubeis’s research focuses on the processes that cause and maintain disorders of mood, as well as the treatment processes that reduce and prevent the return of mood symptoms. The contexts for this work are randomized clinical trials in which the effects of antidepressant medications are compared with cognitive therapy in people with major depressive disorder. Along with his students and collaborators, he examines the data obtained in these trials to further an understanding of the mechanisms through which these treatments exert their effects. He also develops and refines the methods that are required for testing hypotheses with longitudinal data.
