In today's podcast we were delighted to be joined by the presenters of the top scientific abstracts for the Annual Assembly of the American Academy of Hospice and Palliative Medicine (AAHPM) and the Hospice and Palliative Medicine Nurses Association (HPNA). Eric and I interviewed these presenters at the meeting on Thursday (before the pub crawl, thankfully). On Saturday, they formally presented their abstracts during the plenary session, followed by a wonderful question and answer session with Hillary Lum doing a terrific job in the role of podcast host moderator. Our three guests were Marie Bakitas, who conducted a trial of tele/video palliative care for Black and White inpatients with serious illness hospitalized in the rural south; Yael Shenker, for a trial of patient-directed Prepare-for-your-care vs. facilitated Respecting Choices style advance care planning interventions; and, Na Ouyang, who studied the relationship between prognostic communication and prolonged grief among the parents of children who died from cancer. From just the abstracts we had so many questions. We covered some of our questions on the podcast, others you can ponder on your own or in your journal clubs, including: Marie's tele/video palliative care intervention was tailored/refined with the help of a community advisory board. Does every institution need to get a community advisory board to tailor their rural tele-palliative care initiative (or geriatrics intervention) to the local communities served? Who would/should be on that board? How to be sensitive to the risks of stereotyping based on recommendations from the few members of the board to the many heterogeneous patients served? Advance care planning has taken a beating. For the purposes of a thought exercise, no matter what you believe, let's assume that there are clear important benefits. Based on the results of Yael's study, should resources be allocated to resource intensive nurse facilitated sessions (Respecting Choices), which had significantly better engagement, or to low resource intensive patient-facing materials (Prepare), which had significantly less engagement but still plenty of engagement (e.g. 75% vs 61% advance directive completion)? One interpretation of Na's study is that clinicians can lean on the high levels of trust and high ratings of communication to engage with parents of children with cancer about prognosis. Another interpretation is that clinicians avoided telling the parents prognosis in order to bolster their ratings of trust and communication quality. Which is it? Bonus: Simon says he composed the song Sounds of Silence in a dark echoing bathroom about his concerns that people had stopped listening to each other in the 1960s (still resonates, right?). Robin McAuley is releasing a new solo album on February 28, 2025 entitled Soulbound.Robin stops by to talk about the new record, gives us an update on the next Black Swan record, and an update on Michael Schenker. Join us as we talk the Rock, and stick around to hear two killer songs from Soulbound! In case you don't know a lot about Robin's history, here's a little information about this amazing singer! Robin McAuley is widely recognized as one of the great singers of a golden period in hard rock music. His voice can be heard on albums from Grand Prix and Far Corporation, and of course, on the chart-topping MSG records 'Perfect Timing,' 'SaveYourself,' and 'MSG.' Through these releases, he was introduced to hard rock fans around the globe, who no doubt recognized the inherent talent and skill in his vocal abilities. Once he departed MSG, McAuley released one solo record in 1999, then joined AORlegends Survivor. He then re-grouped with Schenker in Michael Schenker Fest on their recent records 'Resurrection' and 'Revelation,' And of course, in what was an absolute revelation to hard rock fans around the world, in 2020, he came out swingin' as the frontman of the supergroup Black Swan, where he joined forces with Reb Beach (Whitesnake, Winger), Jeff Pilson (Foreigner, ex-Dokken), and Matt Starr (Ace Frehley, Mr Big). This episode has three main ingredients: Shifty, Schenker, and shredding. What more do you need? Schenker was just 11 when he played his first gig with The Scorpions, and recorded on their debut LP, Lonesome Crow, when he was 16. He's been playing a Gibson Flying V since those early days, so it's only natural that both he and Shifty bust out the Vs for this occasion. While gigging with Scorpions in Germany, Schenker met and was poached by British rockers UFO, with whom he recorded five studio records and one live release. (Schenker's new record, released on September 20, celebrates this pivotal era with reworkings of the material from these albums with a cavalcade of high-profile guests like Axl Rose, Slash, Dee Snider, Adrian Vandenberg, and more.) On 1978's Obsession, his last studio full-length with the band, Schenker cut the solo on “Only You Can Rock Me,” which Shifty thinks carries some of the greatest rock guitar tone of all time. Schenker details his approach to his other solos, but note-for-note recall isn't always in the cards—he plays from a place of deep expression, which he says makes it difficult to replicate his leads. Josh Schenker may well be the only CFO we've interviewed who claims to have dropped out of high school—which he actually did, in order to accelerate his academic ascendance by passing the General Educational Development (GED) test, which in turn allowed him to enroll at college ahead of schedule. “I never received a diploma from my high school, so, technically, I am a dropout,” explains Schenker, who logged many hours during his high school years at his father's wealth management company. It was there, he tells us, that he first developed a keen understanding of finance. Schenker relates that he would immerse himself in learning the intricacies of portfolio allocation and market optimization, which ultimately laid the foundation for his future career. With a strong interest in finance and business, Schenker pursued further studies in these fields, eventually earning a master's in finance from MIT. This academic background, combined with his early hands-on experience, equipped him with the skills needed to navigate the complex world of finance. Schenker began his career in strategy consulting at PwC, where he further developed his analytical skills by working across multiple industries to identify key business metrics and optimize performance. His transition to AOL's Corporate Development Group marked a significant step in his career. There, he participated in numerous M&A deals, including the high-profile acquisition of Yahoo by Verizon. Eager to gain deeper involvement in business operations, Schenker joined Yahoo Sports as head of business operations, a position that enabled him to hone his skills in budgeting and investment strategies. Driven by a desire for hands-on experience in running a business, Schenker then moved into the start-up world, assuming roles at companies like clean.io and Cluster. In these CFO positions, he leveraged his M&A experience and strategic insights to drive business success, which would lead to his current role as CFO at Aditude, where he continues to apply his diverse expertise in finance and business management.

While we may think of the German band the Scorpions as an 80's metal band, they started much earlier - way back in 1965. Things didn't really come together for the group until the 1978-1992 personnel lineup of Klaus Meine on lead vocals, Rudolph Schenker on rhythm guitar, Mattias Jabs on lead guitar, Francis Buchholz on bass, and Herman Rarebell on percussion. By the time that the live album entitled World Wide Live came out in 1985, the group had established itself as an international commercial success.Recorded during their 1984 world tour, "World Wide Live" showcases the band at their best, and is a testament to their skill as live performers. The album was produced by Dieter Dierks, and was initially released as a 2-LP vinyl set, a cassette, and a VHS of tour footage showing off the band's stage presence. Folks, you got your money's worth when you originally purchased this album!From the high intensity of rockers like "Big City Nights" and "Rock You Like a Hurricane," to the raw emotion of ballads like "Still Loving You," the Scorpions deliver a performance that connects with their audience. Hearing the live versions gives you a sense of the band in a way that is hard to capture from the studio arrangements.Lynch leads this journey back to 1985, when the Scorpions were rocking the world like a...well...they were rocking the world. Big City NightsThis song from "Love at First Sting" leads off side two of the first LP. Schenker and Meine composed this one, as they did with many Scorpions songs. This homage to night life in the city was released as the third single from their studio album, but failed to chart.No One Like YouOriginally appearing on their 1982 album, "Blackout," this track deals with infatuation and desire. Despite its only reaching number 65 on the Billboard Hot 100, it is one of the songs that established the Scorpions as premiere hard rockers of the 80's.Still Loving YouLong before the hair metal ballad became a trope, the Scorpions demonstrated their ability to handle a slower song while still maintaining intensity and raw emotion. The lyrics are of a guy struggling to convince the girl he has lost to give their love a second chance.Rock You Like a HurricaneThe Scorpions' signature song first appeared on "Love at First Sting" the year before this live album was released. It reached number 25 on the Billboard Hot 100, a rare Scorpions song to break the top 40. The track leads off side three of the album. The risqué video would be a prime exhibit of the PMRC in their quest for more parental control over child access to music videos. STAFF PICKS:Whole of the Moon by The WaterboysWayne gets the staff picks started this week with the best-selling song from an overlooked British-Irish rock-folk band. Cerebral and artsy, with lyrics like "I saw the crescent, you saw the whole of the moon," the song is about individuals with rare depth. The Waterboys reformed in 2000, and can still be seen touring, primarily in Europe.Lay Your Hands On Me by The Thompson TwinsRob brings us a song from the British group that has neither members named Thompson nor twins. It went to number 6 on the Billboard Hot 100. Thompson Twins member Tom Bailey has said that the song takes religious ritual as a metaphor for the connection between artist, performer, and artist.It's Only Love by Bryan Adams and Tina TurnerBruce's staff pick is the sixth and final single from Adam's amazingly popular album "Reckless," and also appears on Turner's "Tina Live in Europe" released in 1988. Loaded with melancholy and anger from a breakup, the song encourages the jilted lover to remember - it's only love. This song went to number 15 on the Billboard Hot 100.Money for Nothing by Dire StraitsLynch features a signature song from Mark Knopfler and Dire Straits, with a strong helping of Sting to boot. This Grammy Award-winning track was a staple of MTV, and spent three weeks atop the US pop charts. The lyrics are a conversation between two working-class men as they watch music videos. There is a thread of musical theory called Schenkerian analysis, based on the work of Heinrich Schenker. Schenker believed that musical works could be boiled down to their fundamental structures and harmonies. Entire works could be described with single chords. If Schenker had applied his analysis to Mahler's 5th symphony, he might have played just two chords for you: a C# minor chord, and then a D Major chord. The reason why? Over the course of 70 minutes, Mahler takes the listener on a wild journey, starting in C# minor with a lonely military trumpet, and then ending in a glorious D Major coda that might be the most unambiguously sunny thing Mahler ever wrote: But of course, how we get there is the most fascinating part of this monumental symphony. Today, on Part I, I'm going to take you through Part I of the symphony, which encompasses the first two movements. Next week, we'll take a look at Parts 2 and 3 together, which take up the final three movements of the piece. Part I of the piece represents both a shift in Mahler's music, and a nostalgic remembrance. As always with Mahler, there are multiple meanings to every phrase. The opening of the symphony, which sounds so unusual, is itself based on a seemingly random moment of the 4th symphony. The funeral march that dominates the first movement is based at least partly on a piece he was writing at the same time, the Kindertotenlieder, or Songs on the Death of Children. And the second movement, one of the most unusual and complicated movements Mahler had ever written up to this point, quotes a motive from Schubert's Death and the Maiden string quartet. Clearly, death, a specter that always haunted Mahler, is alive and well in Part 1 of the symphony. The first two movements of the symphony might be a perfect distillation of Mahler; they are passionate, wild, intense, but also tightly scored, precisely structured, and full of that constant push and pull between the past, the present, and the modern, that makes Mahler's music both a product of its time, but also music that is always relevant to us.
