Abstract: Human trophoblast organoids (TOs) are a three-dimensional ex vivo culture model that can be used to study various aspects of placental development, physiology, and pathology. Previously, we showed that TOs could be derived and cultured from full-term human placental tissue and used as models of trophoblast innate immune signaling and teratogenic virus infections (Yang et al., 2022). However, a remaining challenge of TOs cultured in domes of Matrigel or other extracellular matrix is their inverted polarity, with proliferative cytotrophoblasts (CTBs) on the outer surface of organoids and the multi-nucleated syncytiotrophoblast (STB) primarily localized within the inner surface, which is in direct contrast to the orientation that occurs in vivo. Here, we developed a method to culture TOs under conditions that recapitulate the cellular orientation of chorionic villi in vivo. We show that standard TOs containing the STB layer inside the organoid (STBin) develop into organoids containing the STB on the outer surface (STBout) when cultured in suspension with gentle agitation. STBout organoids secrete higher levels of hormones and cytokines from the STB, including human chorionic gonadotropin (hCG) and interferon (IFN)-lambda2. Using membrane capacitance measurements, we also show that the outermost surface of STBout organoids contain large syncytia comprised of greater than 60 nuclei compared to STBin organoids that contain small syncytia ( less than 6 nuclei) and mononuclear cells. The growth of TOs under conditions that mimic the cellular orientation of chorionic villi in vivo thus allows for the study of a variety of aspects of placental biology under physiological conditions. Abstract: Background: Neuroimaging and lesion studies suggest that the dorsolateral prefrontal and posterior parietal cortices mediate visual metacognitive awareness. The causal evidence provided by non-invasive brain stimulation, however, is inconsistent. Objective/Hypothesis: Here we revisit major experiments with an improved methodology and a new Kanizsa figure task trying to determine whether bilateral continuous theta-burst transcranial magnetic stimulation (cTBS) over these regions affects perceptual metacognition. Specifically, we tested whether subjective visibility ratings and/or metacognitive efficiency are lower when cTBS is applied to these two regions in comparison to an active control region. Methods: A within-subjects design including three sessions, at one-week intervals, was implemented. In each session, every participant was administered bilateral cTBS to either prefrontal, control or parietal cortices. Two concurrent tasks were performed: real and illusory figure, using a staircase procedure to stabilize objective performance. Results: We found strong null findings for the replicated task, suggesting that cTBS affected neither visibility ratings nor visual metacognition. Nevertheless, we also observed that the implementation of illusory Kanizsa figures disrupted metacognitive ability, specifically when cTBS was applied to prefrontal but not parietal areas. Conclusion(s): This study adds new evidence to the debate regarding the role of the PPN in metacognition: specifically, using rigorous criteria, evidence is adduced to show that the DLPFC cTBS selectively interferes with metacognitive awareness. No two states are alike in how telehealth is defined and regulated. While there are some similarities in language, perhaps indicating states may have utilized existing verbiage from other states, noticeable differences exist. These differences are to be expected, given that each state defines its Medicaid policy parameters, but it also creates a confusing environment for telehealth participants to navigate, particularly when a health system or practitioner provides health care services in multiple states. In most cases, states have moved away from duplicating Medicare's restrictive telehealth policy, with some reimbursing a wide range of practitioners and services, with little to no restrictions. One of the most common trends with live video reimbursement was the addition of eligible services to the list of telehealth eligible services, with applied behavioral analysis being the most common service addition mentioned in Medicaid manuals. Additionally, in the wake of the COVID-19 pandemic, some states do seem to be adopting the Center for Medicare and Medicaid Services (CMS) communication technology-based services (CTBS) codes, including the virtual check-in and remote evaluation of prerecorded information, audio-only service codes and remote physiologic monitoring. All fifty states and the District of Columbia have a definition in law, regulation, or their Medicaid program for telehealth, telemedicine, or both. Additionally, because of the allowance in most states to utilize telephone as a form of telehealth during COVID-19, some states are taking steps to broaden its permanent definitions of telehealth or telemedicine by removing the explicit exclusion of telephone or including audio-only services within the definition itself. One of the states with the most significant changes to their telehealth policy was Massachusetts which passed a comprehensive telehealth law to require reimbursement for both Medicaid and private payers if the services are covered in-person and it is appropriately delivered through telehealth. The law contained some unique elements including specifying that the rate of payment for telehealth services provided via interactive audio-video technology and audio-only telephone may be greater than the rate of payment for the same services delivered by other telehealth modalities. It also provided payment parity for in-network providers of behavioral health services delivered via interactive audio-video technology or audio-only telephone only. Abstract: Recent evidence demonstrates that activation-dependent neuroplasticity on a structural level can occur in a short time (2 hour or less) in humans. However, the exact time scale of structural plasticity in adult human brain remains unclear. Using voxel-based morphometry (VBM), we investigated changes in gray matter (GM) after one session of continuous theta-burst stimulation (cTBS) delivered to the anterior temporal lobe (ATL). Twenty-five participants were received cTBS over the left ATL or occipital pole as a control site outside of the scanner and had structural and functional imaging. During functional imaging, participants performed a semantic association task. VBM result revealed decreased GM in the left ATL and right cerebellum after ATL stimulation compared to the control stimulation. In addition, cTBS over the left ATL induced slower reaction time in sematic task performance, reduced regional activity at the target site, and altered functional connectivity between the left and right ATL during semantic processing. Furthermore, the ATL GM changes were associated with the functional connectivity changes in the ATL-connectivity during semantic processing. These structural alterations are mirrored by functional changes in cortical excitability attributed to the GM changes and demonstrate the rapid dynamics of cortical plasticity. Our findings support fast adjusting neuronal systems, such as postsynaptic morphology changes and neuronal turnover. Abstract: Background: The beneficial effects of both single-session bouts of aerobic exercise and therapeutic exercise interventions on the cortical regions associated with executive functions (i.e., prefrontal cortex (PFC)) have been well documented. However, it remains unclear whether aerobic exercise can be used to offset temporary fluctuations in cortical activity. Objective: The current study sought to determine whether a single session of moderate intensity aerobic exercise can offset the attenuating effects of continuous theta burst stimulation (cTBS) targeting the dorsolateral prefrontal cortex (dlPFC). Methods: Twenty-two right-handed participants between 18-30 years completed a 20 minute session of light intensity (10% heart rate reserve (HRR)) and moderate intensity (50% HRR) exercise in a counterbalanced order. Following each exercise session, participants received active cTBS to the left dlPFC. Changes in executive functions were quantified using a flanker paradigm employed at baseline, post-exercise and post-cTBS time points. Additionally, EEG methodologies were used to measure changes in inhibitory control specific event-related potential components (i.e., P3 and N2) in response to the flanker task. Results: Behavioural results from the flanker task revealed a significant improvement in task performance following an acute bout of moderate intensity exercise. Furthermore, the effect of cTBS in both light and moderate intensity conditions were non significant. Similarly, EEG data from P3 and N2 ERP components revealed no changes to amplitude across time and condition. P3 latency data revealed a significant effect of time in the light intensity condition, such that latency was faster following cTBS. Similarly, latency data within the N2 ERP component revealed a significant effect of time on congruent trials in the light intensity condition; N2 latency was faster following cTBS. Conclusion: The current study revealed that light and moderate intensity exercise may provide a buffer to cTBS- induced attenuation of the dlPFC. Abstract: Neuromodulation of the primary visual cortex using anodal transcranial direct current stimulation (a-tDCS) can alter visual perception and enhance neuroplasticity. However, the mechanisms that underpin these effects are currently unknown. When applied to the motor cortex, a-tDCS reduces the concentration of the inhibitory neurotransmitter gamma aminobutyric acid (GABA), an effect that has been linked to increased neuroplasticity. The aim of this study was to assess whether a-tDCS also reduces GABA-mediated inhibition when applied to the human visual cortex. Changes in visual cortex inhibition were measured using the mixed percept duration in binocular rivalry. Binocular rivalry mixed percept duration has recently been advocated as a direct and sensitive measure of visual cortex inhibition whereby GABA agonists decrease mixed percept durations and agonists of the excitatory neurotransmitter acetylcholine increase them. Our hypothesis was that visual cortex a-tDCS would increase mixed percept duration by reducing GABA-mediated inhibition and increasing cortical excitation. In addition, we measured the effect of continuous theta-burst transcranial magnetic stimulation (cTBS) of the visual cortex on binocular rivalry dynamics. When applied to the motor or visual cortex, cTBS increases GABA concentration and we therefore hypothesized that visual cortex cTBS would decrease the mixed percept duration. Binocular rivalry dynamics were recorded before and after active and sham a-tDCS (N=15) or cTBS (N=15). Contrary to our hypotheses, a-tDCS had no effect, whereas cTBS significantly increased mixed percepts during rivalry. Abstract: Repetitive Transcranial Magnetic Stimulation (rTMS) is an established neuromodulation technique, using electromagnetic pulses that, depending on the precise parameters, are assumed to lead to lasting neural excitability changes. rTMS has widespread applications in both research and therapy, where it has been FDA approved and is considered a first-line treatment for depression, according to recent North American and European guidelines. However, these assumed excitability effects are often difficult to replicate, and highly unreliable on the single subject/patient level. Given the increasing application of rTMS, especially in clinical practice, the absence of a method to unequivocally determine effects of rTMS on human neuronal excitability is problematic. We have taken a first step in addressing this bottleneck, by administering excitatory and inhibitory rTMS protocols, iTBS and cTBS, to a human in vitro neuron model; differentiated SH-SY5Y cells. We use live calcium imaging to assess changes in neural activity following stimulation, through quantifying fluorescence response to chemical depolarization. We found that iTBS and cTBS have opposite effects on fluorescence response; with iTBS increasing and cTBS decreasing response to chemical depolarization. Our results are promising, as they provide a clear demonstration of rTMS after-effects in a living human neuron model. We here present an in-vitro live calcium imaging setup that can be further applied to more complex human neuron models, for developing and evaluating subject/patient-specific brain stimulation protocols. Abstract: Background: It is well established that proprioception (position sense) is important for motor control, yet its role in motor learning and associated plasticity is not well understood. We previously demonstrated that motor skill learning is associated with enhanced proprioception and changes in sensorimotor neurophysiology. However, the neural substrates mediating these effects are unclear. Objective: To determine whether suppressing activity in the cerebellum and somatosensory cortex (S1) affects proprioceptive changes associated with motor skill learning. Methods: 54 healthy young adults practiced a skill involving visually-guided 2D reaching movements through an irregular-shaped track using a robotic manipulandum with their right hand. Proprioception was measured using a passive two-alternative choice task before and after motor practice. Continuous theta burst stimulation (cTBS), was delivered over S1 or the cerebellum (CB) at the end of training for two consecutive days. We compared group differences (S1, CB, Sham) in proprioception and motor skill, quantified by a speed-accuracy function, measured on a third consecutive day (retention). Results: As shown previously, the Sham group demonstrated enhanced proprioceptive sensitivity after training and at retention. The S1 group had impaired proprioceptive function at retention through online changes during practice, whereas the CB group demonstrated offline decrements in proprioceptive function. All groups demonstrated motor skill learning, however, the magnitude of learning differed between the CB and Sham groups, consistent with a role for the cerebellum in motor learning. Conclusion: Overall, these findings suggest that the cerebellum and S1 are important for distinct aspects of proprioceptive changes during skill learning. Copy rights belong to original authors. Abstract: In skilled object lifting, fingertip forces need to be carefully scaled to object weight, which can be inferred from object properties, such as size or material. This anticipatory force scaling ensures smooth and efficient lifting movements. However, even with accurate motor plans, weight perception can still be biased. In the size-weight illusion, objects of different size but equal weight are perceived to differ in heaviness, with the small object perceived to be heavier than the large object. The neural underpinnings of the size-weight illusion and anticipatory force scaling to object size are largely unknown. In this study, we hypothesized a possible role of the anterior intraparietal cortex (aIPS) in predictive force scaling and the size-weight illusion, which we investigated by applying continuous theta burst stimulation (cTBS) prior to participants lifting objects designed to induce the size-weight illusion. Participants received cTBS over aIPS, the primary motor cortex (control area), or sham stimulation. We found no evidence that aIPS stimulation affected the size-weight illusion. Small effects were, however, found on anticipatory force scaling, where grip force was less tuned to object size during initial lifts. These findings suggest that, while aIPS might be peripherally involved in sensorimotor prediction, other brain areas underpin the processes that mediate the size-weight illusion. Abstract: Decisions are typically guided by what we have experienced in the past. However, when direct experience is unavailable, animals and humans can imagine or infer the future to make choices. Outcome expectations that are based on direct experience and inference may compete for guiding behavior [1, 2], and they may recruit distinct but overlapping brain circuits [3-5]. In rodents, the orbitofrontal cortex (OFC) contains neural signatures of inferred outcomes and is necessary for behavior that requires inference, but it is not necessary when responding can be based on direct experience [6-10]. In humans, OFC activity is also correlated with inferred outcome expectations [11, 12], but it is unclear whether the human OFC is selectively required for inference-based behavior. To test this, here we used non-invasive targeted continuous theta burst stimulation (cTBS) [13] to inactivate the human OFC in a sensory preconditioning task designed to isolate inference-based behavior from responding that can be based on direct experience [6, 12, 14]. We show that OFC-targeted cTBS disrupts reward-related behavior only in conditions in which outcome expectations have to be mentally inferred, but that it does not impair behavior that can be based on stimulus-outcome associations that were directly experienced. These findings suggest that OFC is necessary for decision making when outcomes have to be mentally simulated, providing converging cross-species evidence for a critical role of OFC in model-based but not model-free behavior. Die Wirkung der repetitiven transkraniellen Magnetstimulation (rTMS) auf die Stimmung gesunder Probanden wurde bereits vielfach in Studien untersucht. Ziel solcher Untersuchungen ist einerseits, die neurophysiologischen Grundlagen der Emotionsregulation zu erforschen und andererseits einen Beitrag zur Entwicklung von Hirnstimulationsverfahren zu therapeutischen Interventionen bei affektiven Erkrankungen zu leisten. Die Ergebnisse der Studien zum Effekt der rTMS auf die Stimmung gesunder Probanden sind in der Gesamtschau allerdings weitgehend inkonsistent. Grund dafür könnten methodische Unterschiede, eine interindividuelle Variabilität der präfrontalen Konnektivität oder andere Faktoren, die das Paradigma rTMS-induzierter Stimmungsänderung beeinflussen, wie etwa interindividuelle Unterschiede der Persönlichkeit der Probanden, sein. Die vorliegende Dissertation befasst sich mit der Untersuchung dieses möglichen Einflussfaktors der Persönlichkeit auf die Art und Ausprägung der Stimmungsänderung nach rTMS. In zwei Studien wurden insgesamt 41 gesunde Probanden eingeschlossen. Die Ergebnisse wurden im Hinblick auf die Annahme, dass die Persönlichkeit einen Einfluss auf die Stimmungsänderung nach rTMS hat, ausgewertet. In der Studie A erhielten 17 Probanden eine 1 Hz rTMS (600 Stimuli, Intensität: 120% der rMT) im Crossover-Design an drei Loci auf der Großhirnrinde (medialer präfrontaler Kortex - mPFC, linker dorsolateraler präfrontaler Kortex - LDLPFC und auditorischer Kortex als Plazebo-Bedingung mit der Spule um 45° abgewinkelt). In der Studie B wurden 12 Probanden ausschließlich am mPFC, weitere 12 Probanden nur am LDLPFC stimuliert. Beide Gruppen erhielten jeweils im Crossover-Design drei Stimulationseinheiten (iTBS, cTBS, imTBS als Plazebo-Bedingung). Bei jeder Einheit wurden 600 Stimuli mit einer Intensität von 80% der motorischen Schwelle bei den beiden aktiven Stimulationsbedingungen und mit 10% der Geräteleistung bei der Plazebobedingung, appliziert. Der Stimulationsort wurde in beiden Studien mit dem internationalen 10/20 EEG-System bestimmt (mPFC: Fz; LDLPFC: F3, auditorischer Kortex: T3). Die Stimulationen wurden jeweils binnen eines Tages und in randomisierter Reihenfolge durchgeführt. Zwischen den Stimulationen war eine Pause von 50 min. Vorher wurden Persönlichkeitstests (NEO-FFI, Sensation Seeking Skale, Version 5 = SSS-V), sowie eine Stimmungsmessung mit der Eigenschaftswörterliste (EWL) von Janke und Debus gemacht, die nach jeder Stimulation wiederholt wurde. Es wurde zunächst ein möglicher Effekt der rTMS auf die Stimmung, sowie ein zeitlicher Effekt ohne die Berücksichtigung des Faktors der Persönlichkeit untersucht. Dabei ergaben sich in beiden Studien keine signifikanten rTMS-spezifischen Effekte auf die Stimmung. Wie in der Mehrzahl der Vorstudien, konnten weder für die unterschiedlichen Modi, noch für die unterschiedlichen Stimulationsorte signifikante Unterschiede hinsichtlich der Stimmungsänderung gezeigt werden. Auch für die Stimmungsänderungen zu den jeweiligen Messzeitpunkten zeigte sich bis auf einen signifikanten Unterschied, kein durchgängiger Effekt. Als Hauptergebnis zeigte sich, dass der Effekt der rTMS auf die Stimmung maßgeblich von der Persönlichkeit der Probanden abhing. Korrelationsanalysen ergaben in der 1 Hz Studie einen signifikanten Zusammenhang zwischen der Ausprägung des Merkmals „Sensation Seeking“ (SS) und einer Stimmungsverschlechterung nach allen Stimulationen (auch nach der Plazebo-Bedingung). Im Extremgruppenvergleich konnte das Ergebnis durch signifikante und hochsignifikante Unterschiede zwischen den Gruppen „hohes SS“ und „niedriges SS“ bezüglich der Stimmungsänderung nach den Stimulationen bestätigt werden. In der TBS-Studie konnten gleichermaßen Zusammenhänge festgestellt werden. Es ergaben sich Korrelationen zwischen einer Stimmungsverbesserung nach cTBS und iTBS über dem mPFC und höheren Punktwerten in der SSS-V, sowie ein Zusammenhang zwischen einer Stimmungsverschlechterung nach imTBS über dem LDLPFC und höheren Punktwerten in der SSS-V. Korrelative Zusammenhänge zwischen der Ausprägung einzelner Persönlichkeitsfaktoren des NEO-FFI und der Stimmungsänderung konnten nur für die TBS-Studie gezeigt werden. Dabei ergaben sich signifikante Korrelationen für die Faktoren „Neurotizismus“, „Extraversion“, „Verträglichkeit“ und „Gewissenhaftigkeit“ mit der Stimmungsänderung nach rTMS. In der 1 Hz Studie wurde außerdem ein Zusammenhang zwischen dem zeitlichen Faktor der Stimmungsänderung und SS Score festgestellt. Die Ergebnisse weisen auf eine besondere Bedeutung der Persönlichkeit als Einflussfaktor für Art und Ausprägung des Effektes einer rTMS-Intervention auf die Stimmung gesunder Probanden hin. Weiterführende Studien sind notwendig, um Persönlichkeitsvariablen als festen Einflussfaktor auf die Wirkung von Stimulationsverfahren auf die Emotionsregulation zu charakterisieren. In Studien könnte durch ein Screening nach Persönlichkeitsvariablen homogenere Gruppen gebildet und so konsistentere Ergebnisse erzielt werden.

Human cytomegalovirus (HCMV) is the major viral cause of congenital infection and birth defects. Primary maternal infection often results in virus transmission, and symptomatic babies can have permanent neurological deficiencies and deafness. Congenital infection can also lead to intrauterine growth restriction, a defect in placental transport. HCMV replicates in primary cytotrophoblasts (CTBs), the specialized cells of the placenta, and inhibits differentiation/invasion. Human trophoblast progenitor cells (TBPCs) give rise to the mature cell types of the chorionic villi, CTBs and multi-nucleated syncytiotrophoblasts (STBs). Here we report that TBPCs are fully permissive for pathogenic and attenuated HCMV strains. Studies with a mutant virus lacking a functional pentamer complex (gH/gL/pUL128-131A) showed that virion entry into TBPCs is independent of the pentamer. In addition, infection is blocked by a potent human neutralizing monoclonal antibody (mAb), TRL345, reactive with glycoprotein B (gB), but not mAbs to the pentamer proteins pUL130/pUL131A. Functional studies revealed that neutralization of infection preserved the capacity of TBPCs to differentiate and assemble into trophospheres composed of CTBs and STBs in vitro. Our results indicate that mAbs to gB protect trophoblast progenitors of the placenta and could be included in antibody treatments developed to suppress congenital infection and prevent disease.

Die präfrontale repetitive transkranielle Magnestimulation (rTMS) wird seit den 90er Jahren angewendet, um einerseits die Bedeutung des präfrontalen Kortex für verschiedene kognitive und affektive Prozesse zu erforschen und andererseits die Pathophysiologie psychiatrischer Erkrankungen zu untersuchen und therapeutisch zu modulieren. Von neuen Stimulationsprotokollen, wie der Theta Burst Stimulation (TBS), die analog zum Tiermodell zur Induktion von Langzeitpotenzierung beim Menschen entwickelt wurde, werden stärkere und länger anhaltende therapeutische Effekte erhofft. Im Gegensatz zur Stimulation des Motorkortex wurde die präfrontale rTMS bislang kaum neurophysiologisch untersucht. In dieser Arbeit werden daher zwei Experimente beschrieben, in denen eine niederfrequente 1 Hz-rTMS und TBS bezüglich ihrer Effekte auf ereigniskorrelierte Potentiale (EKP) in GoNogo-Aufgaben charakterisiert wurden. Sie verfolgten die Fragestellungen, ob EKP analog zu motorisch evozierten Potenzialen (MEP) geeignet sind die Wirkungsweise einer präfrontalen rTMS einzuschätzen und ob sich die TBS qualitativ oder quantitativ von herkömmlichen rTMS-Protokollen unterscheidet (Experiment 1 und 3). In einem Vorexperiment zu Experiment 3 (Experiment 2) wurde erstmals die Sicherheit verschiedener präfrontaler TBS-Formen mittels EEG und kognitiver Tests untersucht. In Experiment 1 wurden 18 gesunde Probanden mit einer als inhibitorisch geltenden 1 Hz rTMS über dem linken dorsolateralen präfrontalen Kortex (DLPFC), dem medialen präfrontalen Kortex (mPFC) und einer Kontrollregion stimuliert. Bei der nachfolgenden Bearbeitung einer GoNogo-Aufgabe, zeigte sich eine Vergrößerung der P3-Amplitude nach Stimulation des mPFC bei zeitgleich größerer parietaler Aktivität. Hypothesenkonform konnte eine Reduktion der N2-Amplitude bei Stimulation des linken DLPFC gefunden werden. Während der rTMS-Effekt auf die P3-Amplitude am besten durch die Initiierung von Aufmerksamkeitsprozessen erklärt werden kann, spricht der Einfluss der 1 Hz-rTMS über dem linken DLPFC für einen inhibitorischen Effekt auf kortikaler Ebene (Experiment 1). Im Hinblick auf die vorbeschriebenen nachhaltigeren Effekte von TBS-Protokollen am Motorkortex wurde in Experiment 2 die TBS am präfrontalen Kortex bei 24 gesunden Probanden plazebo-kontrolliert bezüglich ihrer Sicherheit untersucht, um diese als innovative Stimulationsform für weitere Experimente einsetzbar zu machen. Die Ergebnisse dieses Experimentes zeigten, dass eine präfrontale, als inhibitorisch geltende TBS (continuous TBS - cTBS) und eine als exzitatorisch geltende TBS (intermittent TBS - iTBS) keine epilepsietypischen Potenziale im EEG oder epileptische Anfälle triggerten. Es kamen jedoch bei drei von 25 Probanden vagale Reaktionen vor, deren Auftreten beachtet und deren Ursache in weiteren Studien erforscht werden sollte. In den neuropsychologischen Untersuchungen wurde eine verminderte Leistung im Arbeitsgedächtnis und in einer frontalen Testbatterie (Trend) nach iTBS des linken DLPFC und in der Anzahl der ‚false alarms’ einer GoNogo-Aufgabe nach cTBS des mPFC festgestellt. Diese Veränderungen spiegelten sich auch in neurophysiologischen Parametern wider. Eine Analyse der EEG-Daten mittels standardized low resolution brain electromagnetic tomography (sLORETA) ergab eine Zunahme der Aktivität im Alpha 2-Band links präfrontal nach iTBS des linken DLPFC, die bis zu einer Stunde nachweisbar blieb und einen Zusammenhang mit den Leistungen im Arbeitsgedächtnis und der frontalen Testbatterie zeigte. In Experiment 3 wurden dann 1 Hz rTMS, cTBS und eine Plazebostimulation bei 9 gesunden Probanden miteinander verglichen. Alle Stimulationen erfolgten neuronavigiert bezogen auf eine Aktivierung im individuellen funktionellen Magnetresonanztomographie (fMRT) Bild, die während der Entscheidungskomponente (Volition) einer modifizierten GoNogo-Aufgabe gefunden wurde. Hierbei konnte die in Experiment 1 beobachtete inhibitorische Wirkung der 1 Hz rTMS auf eine relevante EKP-Komponente (N2P2-peak-to-peak-Amplitude) analog repliziert werden (Trend). Bei einer zeitlichen Betrachtung der Stromdichten mittels sLORETA ließ sich deskriptiv eine Verminderung nach 1 Hz rTMS beobachten, während sich der Verlauf der Stromdichten in der cTBS-Bedingung abhängig vom Aktivierungszustand des Kortex zu ändern schien. In einer für ‚conflict monitoring’ relevanten region of interest (ROI) konnte in dem für die N2P2-Amplitude relevanten Zeitfenster eine Verminderung (Trend) der Stromdichte in der 1 Hz-Bedingung gefunden werden, die mit der N2P2-Amplitude korrelierte. Desweiteren waren in Experiment 1 und 3 keine Effekte auf Verhaltensdaten und EKP-Latenzen nachweisbar. Die Ergebnisse dieser Arbeit sprechen dafür, dass die gemeinsame Betrachtung von Verhaltensdaten, EKP- und Stromdichteanalysen eine neurophysiologische Interpretation der rTMS erlaubt. Die alleinige Verwendung von EKP zur Beurteilung der Wirkungsweise einer präfrontalen rTMS hingegen ist methodisch und inhaltlich begrenzt. Am präfrontalen Kortex zeigten cTBS und iTBS andere Effekte als für den Motorkortex vorbeschrieben. Insgesamt betrachtet sprechen die Ergebnisse dafür, dass sich die TBS nicht nur quantitativ sondern auch qualitativ von einer 1 Hz rTMS unterscheidet. Nach diesen Pilotexperimenten stellt die Untersuchung rTMS-vermittelter Effekte auf präfrontal generierte EKP einen vielversprechenden Untersuchungsansatz dar, um die Bedeutung präfrontaler Regionen als Generatoren spezifischer EKP-Komponenten zu erforschen, die Wirkung verschiedener rTMS Protokolle neurophysiologisch zu untersuchen und diese Protokolle für experimentelle oder therapeutische Anwendungen weiter zu entwickeln. So könnte die Wirkung spezifischer TBS-Protokolle durch eine pathologisch veränderte Grundaktivität bei Patienten mit psychiatrischen Erkrankungen im Gegensatz zu gesunden Probanden verändert sein. Dies könnte in einem nächsten Schritt mit dem oben beschriebenen Untersuchungsansatz näher erforscht werden.

Abstract: Although it is well established that fronto-parietal regions are active during action observation, whether they play a causal role in the ability to infer intentions of others from visual kinematics remains undetermined. In experiments reported here, we combined offline continuous theta-burst stimulation (cTBS) with computational modeling to reveal single-trial computations in the inferior parietal lobule (IPL) and inferior frontal gyrus (IFG). Participants received cTBS over the left anterior IPL and the left IFG pars orbitalis, in separate sessions, before completing an intention discrimination task (discriminate intention of observed reach-to-grasp acts) or a kinematic discrimination task (discriminate peak wrist height of the same acts) unrelated to intention. We targeted intentions-sensitive regions whose fMRI-measured activity accurately discriminated intention from the same action stimuli. We found that transient disruption of activity of the left IPL, but not the IFG, impaired the ability of the observers to judge intention from movement kinematics. Kinematic discrimination unrelated to intention, in contrast, was largely unaffected. Computational analyses revealed that IPL cTBS did not impair the ability to see changes in movement kinematics, nor did it alter the weight given to informative versus non-informative kinematic features. Rather, it selectively impaired the ability to link variations in informative features to the correct intention. Abstract: Pneumococcal meningitis (PM) causes damage to the hippocampus, a brain structure critically involved in learning and memory. Hippocampal injury - which compromises neurofunctional outcome - occurs as apoptosis of progenitor cells and immature neurons of the hippocampal dentate granule cell layer thereby impairing the regenerative capacity of the hippocampal stem cell niche. Repetitive transcranial magnetic stimulation (rTMS) harbours the potential to modulate the proliferative activity of this neuronal stem cell niche. In this study, specific rTMS protocols - namely continuous and intermittent theta burst stimulation (cTBS and iTBS) - were applied on infant rats microbiologically cured from PM by five days of antibiotic treatment. Following two days of exposure to TBS, differential gene expression was analysed by whole transcriptome analysis using RNAseq. cTBS provoked a prominent effect in inducing differential gene expression in the cortex and the hippocampus, whereas iTBS only affect gene expression in the cortex. TBS induced polarisation of microglia and astrocytes towards an inflammatory phenotype, while reducing neurogenesis, neuroplasticity and regeneration. cTBS was further found to induce the release of pro-inflammatory cytokines in vitro .
