Motor vehicle collisions or road traffic collisions are a massive problem worldwide. Data from the World Health Organisation reports that there are around 1.2 million deaths every year and this is the leading cause of death internationally for children and young adults aged 5-29 years.  In the UK there are around 1,500 deaths annually and also around 60,000 patients with significant and life changing injuries, which is 7 patients every hour!! So anything we can do to improve patient care following an MVC is definitely a worthwhile venture. We've looked at Extrication here on the podcast before but we're back on it again because today the Faculty of Pre Hospital Care have released their Consensus Statement on Extrication Following a Motor Vehicle Collision.  The statement builds on the work from the EXIT project and the research that has helped inform our understanding of multiple factors of extrication. The statement will inform a change of practice for both clinicians and non-medical responders and in this episode we run through the statement with two of it's authors and discuss the practical applications. Make sure you take a look at the new Consensus Statement itself and the background evidence which is all linked to on the website. Scott and Ash talk about Swift Water and Rope Rescue and Todd runs us through Auto Extrication training. WHO WE ARE As a group of Rural Volunteer Firefighters, our Fire Department has built our firefighter training from both old and new tactics in the fire service and adapted them to fit the realities of equipment, manpower, and costs around what our community requires. WHAT WE DO We create video, audio and written word content to share these training and tactics methods with our Brothers and Sisters in our extended fire families, and to showcase to those that may be interested in becoming firefighters the fun you can have and the amazing skills and abilities you get to learn, whilst keeping the lives of the people around you safe! WHY WE SHARE Firefighters are problem solvers whose role requires them to be on their best day whilst others are having their worst day, and while we do not claim to be perfect we are willing to put our lives on the line to try. It's during the trying that we continue to develop the mindset, tactics, and training that allow us to be effective, safe and successful where it's needed the most, and by sharing our experiences we hope that it may help other Fire Departments in creating what works for them to help bring everyone home safe. Despite all the improvements that we have seen in trauma care over the past 20 or more years RTCs are still, sadly, a really common cause of both death and disability, with the number of deaths annually in the UK sitting somewhere between 1500-1900 per annum. Survivors, who have serious injuries and are left with ongoing disabilities, total 22,000 people per year. So anything we can do to improve care to these patients is definitely worth looking at and learning about! Extrication is the process of injured (or potentially injured) patients being removed from vehicles involved in road traffic collisions. The fundamentals behind extrication have been based upon protecting the spine and not worsening an injury of it, but at the potential cost of other time critical injuries and with limited to no sound evidence base. The EXIT project brings evidence to the practice of extrication and in this podcast we discuss the findings and implications for practice with the lead author Tim Nutbeam, Clare Bosanko (an EM & PHEM consultant) along with the three of us. We also get the opportunity to hear from Freddie, a patient extricated from a high energy RTC and hear his perspective on Extrication. Enjoy! Simon, Rob & James

Talked about some recent FR calls , multiple MVIs and a Structure Fire. As a group of Rural Volunteer Firefighters, our Fire Department has built our firefighter training from both old and new tactics in the fire service and adapted them to fit the realities of equipment, manpower, and costs around what our community requires. WHAT WE DO We create video, audio and written word content to share these training and tactics methods with our Brothers and Sisters in our extended fire families, and to showcase to those that may be interested in becoming firefighters the fun you can have and the amazing skills and abilities you get to learn, whilst keeping the lives of the people around you safe! WHY WE SHARE Firefighters are problem solvers whose role requires them to be on their best day whilst others are having their worst day, and while we do not claim to be perfect we are willing to put our lives on the line to try. A few weeks ago I posted this scenario on FB . 60 yr male who was in an MVC and is pinned in the driver seat with an obvious closed femur fracture. Firefighters estimate extrication will take 20 minutes. Patient is confused and screaming in pain and asks you to please give him something. He keeps trying to self extricate and is getting in the way of the firefighters tools.  Vitals  BP- 86/52 HR- 118 SPO2 96% on RA  RR 26 You are unable to obtain and IV and firefighters ask you if you are able to give the patient something IM to calm him down through the extrication process.  Do your guidelines discuss extrication sedation? What drug and dose? The comments were very interesting and I wanted to get someone well respected in the HEMS and prehospital environment on the show to give their thoughts. Dr. Cliff Reid is a seasoned retrieval physician who works for Sydney HEMS in Australia. This is his second time on FOAMfrat and we always have a great discussion. I think you will enjoy!

Date of Recording: 19/10/18 Description: Why and how do military governments surrender power to elected civilian governments? The nineteen years since Nigeria emerged from military rule and transitioned to democracy in May 1999 is the longest era of civilian rule in its history. After the military governed Nigeria for 29 of the previous 33 years, 1999 ended a long-standing pattern of failed attempts by military governments to cede power to civilians. However, the transition to civilian rule was not unconditional. Military governments often extract a “price” or concessions in exchange for departing from government. They may acquire economic, political, and other interests that they are reluctant to relinquish when military rule ends. The military ostensibly withdrew from government but maintained influence over its successors by confining them within militarily imposed boundaries. Former military rulers have governed Nigeria for 11 of the 19 years since 1999 (including the current president). Many prior studies in this area focused on external macro factors that cause military withdrawal from governance (such as pressure from external actors like the EU, USA, and UN, and the ‘snowballing’ effects of democratisation in other countries). A distinguishing feature of Nigeria’s transition from military to civilian rule was that the military itself initiated the transition and prepared the way for its own replacement. Why did it do so? Prolonged military rule corroded military professionalism and created intra-military cleavages, injected ethno-regional and political controversies into the military, increased the risk of military coups, and caused premature attrition from the officer corps. Thus pressure for an end to military rule ironically emerged from within the military. Extrication from governance was a decision of military self-interest to give the military space to restore professional norms, while simultaneously preserving influence over its successors, and insulating the military from transformational reforms. Bio: John Ubani Jr is a PhD student in the War Studies department. He is a "lawyer by day and a PhD student at night"! He is researching Nigeria’s 1999 transition from military rule to democracy, and the factors that influence military governments to surrender power to elected civilian governments. John is a corporate lawyer. He has an LLB law degree from University College London, a postgraduate degree in law from the College of Law, London, and a Masters degree in African Studies (with distinction) from SOAS.

We were delighted to be invited to cover the joint Faculty of Prehospital Care and BASICS conference held at the Royal College of Surgeons of Edinburgh. This two-day prehospital extravaganza covered a broad range of topics and the content was delivered by some excellent speakers. As such, we were absolutely spoilt for choice when it came to content for the podcasts but we managed to catch up with: Dr Abi Hoyle – a paediatric emergency medicine consultant with a background in military and retrieval services. She gave us some key tips when dealing with paediatric patients. Ian Dunbar – a technical and medical rescue consultant with years of experience in the UK Fire and Rescue Service and ongoing involvement with British Touring Car Championship and the FIA. He did some myth busting around extrication from vehicles. Professor Mike Tipton – a leading figure in extreme physiology who is the Associate Head of Research at the Extreme Environments Laboratory in Portsmouth, is trustee/director of Surf Life Saving GB, sits on the medical committee for the RNLI and was awarded an MBE for services to physiological research in extreme environments. Mike spoke on the topic of drowning. Massive thanks to Dr Caroline Leech who put together this brilliant programme and extended the invitation to us. She is an educator in mass gathering medicine and the MIMMS system and was part of the instructor team for a multiagency tabletop simulation of a multi-site terrorist attack in Melbourne.    Lionel Lamhaut – An anesthetist with critical care and emergency training and an associate professor of the SAMU de Paris (excuse my French - Service d'Aide Médicale Urgente), Dr. Lamhaut is not only a disaster response physician but an academician of the highest caliber with recent publications on the prospective deployment of ECPR for refractory cardiac arrest in the Paris area with a multicenter study well underway.  He responded directly to the deadly terror attack of Charlie Hebdo and was instrumental in the after action analysis in the coordinated multi-site terror attack that struck Paris on Friday the 13th of 2015.   Raed Arafat – Put a warm round of applause together for the Secretary of State and Minister of Internal Affairs of Romania! Tasked with development of emergency medical care in the country, he is the founder of SMURD – the Mobile Emergency Service for Resuscitation and Extrication. Daniel Kornhall's is an introduction to snow avalanche physiology and the realities of mountain rescue. Dying in an avalanche is an extremely rare cause of death but for us who live in mountain regions and who enjoy winter mountain sports it is a thing that needs to be dealt with. The overall mortality in avalanche incidents is roughly 20% but this increases to 50% in the buried victims, which is why my talk, and most avalanche medicine, focuses on the buried victims. Asphyxia causes the vast majority of deaths, accounting for roughly 80% with trauma in second place at 20%. Hypothermia as the primary cause of death in avalanche victims is extremely uncommon. Asphyxiation causes a dramatic plunge in survival from 80% down to 30% within the first half our of avalanche burial. This is why timely location and extrication of the victim is of vital importance. Kornhall explains why organised rescue service rarely, if ever, manage to get to the victim within this critical asphyxia phase. Survival rather depends on immediate bystander or companion rescue. Extrication times can be reduced by being properly equipped with avalanche transceivers, quality snow shovels and avalanche probes. Kornhall briefly discusses the avalanche airbag, a fairly recent innovation that may reduce the likelihood of being buried if you get avalanched. In the last part of my talk i describe modern extrication techniques and how implementing these into rescue training dramatically improves extrication times.
