True data sovereignty now encompasses control, visibility, and accountability over data wherever it resides, moves, or is processed. In an era shaped by AI adoption and increasingly fragmented cloud environments, sovereignty has become a core driver of enterprise resilience and operational autonomy rather than a regulatory checkbox. In this episode of The Security Strategist, Tim Pfaelzer, Senior Vice President and General Manager, EMEA at Veeam, explains how the meaning of data sovereignty has fundamentally changed.From Compliance Concept to Strategic PriorityA decade ago, data lived in well-defined corporate environments managed by internal IT teams. Today, it is distributed across public cloud platforms, SaaS ecosystems, edge devices, and third-party suppliers. A prolonged outage or ransomware incident can cause significant damage, but the difference between minutes and days of downtime often comes down to recovery architecture and how rigorously it has been tested under real-world conditions. In this context, sovereignty and resilience are directly linked. Without control over data, there is no predictable recovery.AI Has Raised the StakesArtificial intelligence has introduced a new layer of data risk that many organisations are still underestimating. As AI systems increasingly automate decision-making and customer interactions, the quality and integrity of training and operational data become critical. The development came after a Standard investigation uncovered cases of vulnerable young girls from across London who reported allegations of rape from multiple men but saw inadequate police action. Thousands of cases of rape and sexual assault are being reviewed as part of the National Crime Agency's Operation Beaconport, which has been designed to rectify police failures to tackle grooming gangs. A worker trips. A forklift crashes into a rack. The quick conclusion? “Someone messed up.” But stopping there doesn't fix the issue. It just points fingers. Week 4 of our Incident Reporting & Root Cause Analysis focus is all about looking deeper. Not just what went wrong, but why it went wrong—and how to stop it from happening again. That's where the difference between surface cause and root cause matters. Surface causes are usually what's visible right away. Root causes are often buried in procedures, training gaps, or system failures. If we want long-term fixes, we need to go past the obvious. Here are a few ways to shift your focus from surface cause to actual root cause: 1. Don't accept “human error” as the final answer. It's rarely that simple. Human error is usually a symptom, not the disease. What caused the mistake? Was there a lack of training? Confusing instructions? An unrealistic production deadline? 2. Ask “Why?” more than once. One “why” barely scratches the surface. Ask it five times if needed. Each answer should bring you closer to what really caused the issue. Example: “Why did they fall?” leads to “Why wasn't the area clear?” leads to “Why wasn't housekeeping done?” and so on. 3. Review systems, not just people. Blaming a person doesn't change a system. Look at processes. Were checklists skipped? Were shortcuts taken because of time pressure? Is the layout making safe work harder? 4. Don't rush to patch it—solve it. Putting cones around a spill after a fall is fine—for now. But why did the spill happen in the first place? Surface fixes are temporary. Root cause fixes are lasting. 5. Track repeated incidents. If you keep seeing the same near-misses or injuries, the issue isn't random. Look for patterns. That's where root causes tend to hide. As always, these are potential tips. Please be sure to follow the rules and regulations of your specific facility. Getting to the root cause isn't about assigning blame. It's about building a safer facility from the inside out. A strong Safety Culture doesn't just react—it investigates, adapts, and improves. When we fix the system, we protect the people. And remember—if you ever feel like something “just isn't right,” trust your instincts. Speak up. Report it. Safety isn't about silence. It's about action. But between insurance coverage, hospital billing departments, and ongoing treatments, out-of-pocket costs can spiral — even when you're fully insured. In 2021, I was diagnosed with cancer. I underwent 12 rounds of chemotherapy and 8 weeks of daily radiation. While my insurance was a blessing, I quickly discovered that being covered doesn't mean you're not vulnerable to costly billing mistakes — ones that could cost you thousands if you're not paying attention. Why You Must Compare Your EOB With Hospital Bills — Line by Line One of the most important lessons I learned: Always compare your Explanation of Benefits (EOB) from your insurance provider to the bill from your hospital or healthcare provider. Here's what happened to me: Even after I had hit my out-of-pocket deductible, I noticed monthly hospital charges averaging around $900 in my online billing portal. After comparing the EOB to the hospital bills, I saw that some treatments weren't even being submitted to my insurance company. Instead, I was being billed directly — as if I had no coverage at all. I called my insurance company, who confirmed they hadn't received claims for those charges. Then I contacted the hospital's billing department and was told each time that it was a “mistake,” and they would resubmit. Had I not caught this, I could have paid over $10,000 in wrongful charges — with little chance of a refund later. Pro Tip: Don't Automatically Pay Medical Bills Without Verifying It's tempting to just pay the bill and be done with it — especially when you're overwhelmed and just trying to get through treatments. But trust your gut. Instead: Wait to receive your EOB before paying anything. Compare dates, services, and costs between the EOB and your hospital bill. Call your insurance company if anything doesn't line up. Then, contact your healthcare provider's billing department and request that charges be re-submitted if needed. Always Ask: "Has This Been Submitted to My Insurance?" Make it your mantra when speaking to any healthcare billing department: “Has this claim been submitted to my insurance company?” If they say no or “it was a mistake,” ask for proof of submission and a timeline. It can take weeks or even months for a corrected bill to show up — so document every call and keep all emails or confirmations. Human Error and AI Mistakes Are More Common Than You Think In today's digital world, hospitals and insurers often rely on automated systems for billing. But that doesn't mean the process is error-free. In fact, it may be worse. You might encounter: Incorrect coding of procedures Claims not submitted at all Duplicate billing Coverage miscalculations That's why your personal oversight is essential — especially when you're dealing with ongoing care. How to Protect Yourself from Costly Healthcare Billing Mistakes Here's a quick checklist to help you save money and protect your finances during a medical journey: Know your deductible and out-of-pocket maximum Track every treatment and procedure in a personal log Check your EOBs monthly and match them with bills Dispute errors right away — don't wait Keep records of every call and conversation Ask for itemized bills from providers Request written confirmation of any billing correction Final Thoughts: Be Your Own Advocate — It Could Save You Thousands Between AI systems, human error, and overloaded billing departments, even those with full insurance coverage can fall into financial traps. The stress of calling weekly to fix these issues took a toll, but in the end, it saved me thousands of dollars. The bottom line? Be proactive. Be persistent. And never assume a bill is correct just because it looks official. If you're navigating medical bills or complex treatments, you're not alone. If I hadn't spoken up, I would have unknowingly overpaid by thousands. Don't be afraid to double-check, question, and challenge what doesn't look right. Your health matters — and so does your wallet. 14-Year-Old Girl Makes Pennsylvania Hunting History with Record Bull Elk We've got a record-breaking hunting story that's as heartwarming as it is impressive. According to Outdoor Life, fourteen-year-old Samantha Bartlett just became the youngest hunter ever to tag a bull elk in Pennsylvania — and she did it with a single, well-placed shot from her crossbow! It all happened in the early morning hours of September 16, deep in the mountains of north-central PA. Samantha and her dad, Noel, had already spent three long days hiking and hunting, holding out for the right bull. On the fourth day, it finally came together. After hearing two bulls fighting at dawn, Samantha and her guide crept through tall grass and waited. With a few cow calls, a massive bull came trotting in. At 40 yards, Samantha took the shot — and nailed it. The 6x7 bull elk went down within 100 yards, and the rest is history. The animal was estimated at 800 pounds, and the rack green-scored at 310 inches. Getting the bull out? That was a whole different story. On Monday, Sept. 22, the man became separated from his group near the Three Sisters Trailhead in the Rawah Wilderness. The last anyone heard from him was a text saying he was lost, just before 3:30 p.m. His phone later went dark, and with winter weather rolling in, it was too dangerous for search teams to head out that night. By the next day, over 20 agencies — including Search and Rescue teams, deputies, and wildlife officials — launched a full-scale search in snowy, cold, and windy conditions. Air support was grounded, and rescuers battled through the backcountry on foot. Despite the brutal storm and tough terrain, the search effort continued into a second day. Then, on the afternoon of Wednesday, Sept. 24, hope came from above: the hunter was able to place several 911 calls, helping rescuers pinpoint his location. At around 1 p.m., air support spotted him — alive and in good health. So how did he make it? It's a deep cut, but it's another NOFX song title for the episode.Probably don't go to work wasted, but if you do, make sure you talk about it in the pre-job brief.  Pre-job, or pre-task, or pre-work briefs - or whatever you want to call them - are sort of a contentious topic these days. On one hand, they're often connected to JSA/JHA paperwork, and that's not always helpful. There are exceptions, but there are a whole pile of bad ones out there. Shit like, "It's cold out there, folks, so make sure you watch your footing."  Cool. That helps.A real pre-job brief isn't about paperwork, though. It's about alignment of understanding and expectations (or a shared mental model if that makes you happy). Think of it like using small words and speaking slowly to make sure the bass player knows what's going on.  Good pre-job conversations help identify a lot more issues and deviations from normal work than filling out paper for the purpose of satisfying a (sometimes imaginary) rule. Bad ones are a way to exercise control. And reinforce that people can't take care of themselves.  Ron's got a pretty easy on-ramp to talk about joint activity for this one, so make sure to pay attention to that part. As much shit as we give him about it, he does know a thing or two about coordination of work across boundaries, like what you see between contractors and subs, or even between trades or specialties.We can't give people a process and no capability, as Dave says, and that's what overstructuring a pre-job brief through form or process can do. So how can we facilitate good pre-work discussions? Maybe think about some not-lazy perspective where the purpose isn't the paper, it's to plan the work, identify challenges to it, and build in a bit of buffer between the shit that might kill us. Another listener question. How good, right?  The question? "I am still learning how to apply HOP principles. Can you talk about how they can be used to help us see and respond to risks that never show up in incident data?"It's sort of a version of asking how we know what's happening when nothing is happening, and how the five principles of HOP, if you're into that sort of thing, support that. What five principles, you ask? The ones from Todd Conklin are: 1) Error is normal; 2) Blame fixes nothing; 3) Context drives behavior; 4) Learning and improving are vital; 5) Leadership response matters.Ron starts by answering the question he wishes we were asked, and inadvertently points to some really cool info from the ICAO Human Performance Manual, Document 10151. It's not part of the actual answer, but you should still check it out.Back to the matter at hand, trying to find problems isn't the same as trying to apply principles. Incident data can't tell you everything, but maybe leaning on the intent of the HOP principles can help uncover weak signals hiding in plain sight. No principles will do the work for us, though, and it's not about finding broken procedures, either. It's about finding brittle systems, understanding everyday trade-offs, and asking way better questions.Dave's recipe for success? Give up the 80% of "face-to-screen" time a lot of safety folks waste and get face-to-work (or face-to-face, or whatever else you want to put your face on). There's more to it, but the boys suggest we all stop looking through the rearview mirror and start paying attention to what's just up ahead. A lot of the time, when we decide someone did something dumb, we say things like "dude, how'd you fuck that up, it should just be common sense." It's a good way to distance ourselves, blame someone, and be fucking lazy all at the same time. But what is "common sense" anyway?Sometimes it seems like what we're trying to do is take credit for good luck and call it common sense. Taking credit for something you didn't create sounds like some big record label behavior, doesn't it? And maybe that's it. Maybe common sense is just a label used to feel powerful.The boys kick around a few ideas, trying to decide whether common sense is something innate, taught, or based on experience - or all three. That sort of opens the door to wondering about where expertise comes from. If common sense is just a catalog of knowledge, you'd never cover it all, so maybe the intention is about knowing how to think or reason through uncertainty. Like when the Ceschi from the Codefendants' amp quit working at Punk in Drublic. They still put on one of their best shows by figuring it out as they went along. That kind of response seems to be what we mean a lot of times.After the normal rambling discussion, the conversation takes a surprising turn toward relevance, with some ideas about acceleration of expertise and some of the generational gaps that seem to make discussions about common sense a little more rowdy.You should probably just listen to the episode and see if you can figure out the answer.
