To celebrate the release on Netflix of the new film about IVF 'Joy' this is an old episode of The Fertility Podcast, which Natalie produced in celebration of Louise Brown's 40th Birthday and the fact that at that point in time, over 300,000 babies have been born through IVF treatment with more than 1 million cycles having taken place. TheHFEA asked Natalie to make a podcast with them, so Natalie is in conversation with its chair Sally Cheshire and one of its Inspectors, a lady called Janet Kirkland MacHattie, who worked as a nurse at Bourne Hall in 1982, under Patrick Steptoe, Robert Edwards and Jean Purdy. In fact, it was during this conversation with Janet, that Natalie first learnt about Jean, and found herself feeling somewhat ashamed that she didn't know about her sooner! What was discussed: Sally Cheshire talked about how at the time the treatment was quite experimental and Louise Brown's parents were quite brave in putting themselves forward She explains how the Government and Parliament decided to investigate how treatment should be regulated and asked Mary Warnock to form a committee in 1984 to look at how infertility should be regulated. The law came into being in 1990 and the HFEA came to be in 1991. Sally talked about her role at the HFEA and what has changed over the last 40 years and the importance of the focus of its work being on the 70% of people who aren't successful with their treatment. The HFEA has a vital role, thinking about how emotional support can be improved in clinics because, as she explains 'We do know that even if you are pregnant after IVF there's a level of anxiety you carry with you". The importance of being medically, emotionally and financially equipped to try again when going through fertility treatment. Sally explained how 'We still feel a lot of media stories about fertility treatment are positive… very often, there's a lot of around celebrities with the example of Bridget Neilson who gave birth at 54. Are they being truthful about using donor eggs? Some of those stories give you a false impression." Janet explained how she went for the job at Bourn Hall, to join the nursing team - not really knowing what was involved and when she asked Patrick Steptoe his advice on a book she could read to learn more, his reply was 'My dear, they've not been written yetWhat the treatment process was like initially, with Ladies staying at Bourn Hall for ten day periods and how as they didn't have blood tests, the Ladies urine was tested every three hours to find out if they were ovulating.Janet talked about an interview Patrick Steptoe gave where he spoke about his vision for the future being "centres set up where people have been properly trained and this technique will be available to women all over the world" To compare clinics like for like, or rate your clinic visit the HFEA website Follow @YourFertilityNurse on InstagramFollow @TheFertilityPodcast on Instagram

July 25, 1978 was a great day, and not just because July 25 is Ruby's birthday. This particular day in 1978, the first IVF baby, Louise Brown, was born! IVF is a lot different now than it was then — a lot was learned on the way to this first successful human pregnancy, and a lot has been learned since. In this snack, Ruby and Anne are talking about the research and experimentation that got us where we are now. One of the biggest changes (and dare we say, improvements) from the experience of Lesley and Peter Brown is how much less invasive (physically and frankly, of the Brown family's privacy) the process is now than it was then.  Early hurdles included getting past people's notions of whether or not IVF was ethical. And IVF cycles were so much more involved, including blood draws every four hours. Egg retrieval was a surgical process back then, it involved a single egg, and it was much more often unsuccessful. Transfers were done in the middle of the night until one didn't and it worked out fine, and that was (thankfully) abandoned. The fact that Louise Brown was later able to conceive without medical intervention helped allay fears that IVF would create a generation of people unable to reproduce on their own.  So Happy Birthday, Louise Brown, and thank you, Lesley and Peter Brown and Elizabeth Carr (first IVF baby in the US) for going through so much to get us where we are today.  Learn more about All Things Fertility at our website: thewholepineapple.com!

Are we entering a ‘newgenic' age - where cutting-edge technologies and the power of personal choice could achieve the kind of genetic perfection that 20th century eugenicists were after? In 2018, a Chinese scientist illegally attempted to precision edit the genome of two embryos. It didn't work as intended. Twin sisters - Lulu and Nana - were later born, but their identity, and the status of their health, is shrouded in secrecy. They were the first designer babies. Other technological developments are also coming together in ways that could change reproduction: IVF can produce multiple viable embryos, and polygenic screening could be used to select between them. Increased understanding and control of our genetics is seen as a threat by some - an inevitable force for division. But instead of allowing genetics to separate and rank people, perhaps there's a way it can be used - actively - to promote equality. Professor Paige Harden shares her suggestion of an anti-eugenic politics which makes use of genetic information. Welcome to Episode 2 of the International IVF Podcast and this is the first in our upcoming profile episodes where we speak to the greats of IVF, those amongst us who have been working in the field since the early days. Jacques Cohen is the CEO of the International IVF Initiative and has an extremely impressive CV. His postdoctoral studies (1982-1985) were performed at Cambridge University (UK) and Bourn Hall Clinic working under Prof Robert Edwards which is what we wanted to talk with him about in this episode. What we discussed: When Jacques first met Bob in the late 70's at a two conference in the Netherlands and the impact that Bob's closing lecture had on him, as it was unlike anything Jaques had ever heard before. He had been looking forward to it as he was 12 months into a Masters in ART. Jacques went and spoke to Bob then and described how despite his fame, he was still happy to talk to people. Jacques talked about meeting Bob some years later who remembered his time in the Netherlands fondly as he had returned back in the UK and received a call from Gene Purdy about Leslie Brown, the mother of Louise Brown the UK's first IVF baby, who had a positive pregnancy test. In 1982, Jacques applied for an embryology job, Simon Fishel had already been hired and Jacques talked about Jean Purdy and her role as a technician and her role was to make sure that the ‘Mad Science' was in an environment like an operating theatre. Bob came from an animal science background and wasn't used to sterile and organised laboratories. Jean Purdy was the person who helped to organise Bob and his practice. Jacques described what Lab life was like eg.how egg collection couldn't be timed like it is today. How they looked at LH's collecting samples several times a day. Bob was exhausted as the only embryologist, only being assisted by Simon Fishel and then after advertising in Nature magazine, Jacques and Carol Faheley were employed out of 200 applicants. Jacques also describes what the patient's experience was like in the early days - being inpatients, which added financial pressures to Bourn Hall. Husbands were also having to be nearby either in a hotel or a room in the village. All of the studies were way before Ultrasound became a science - they had to rely on estradiol and LH. Jacques talks about maternal serum - which was used in the procedure for Louise Brown's birth, also using donor serem. The consent procedure was mainly oral. How Porta Cabins were used for the treatment rooms and parts of the clinic, meant to be used for 6 months and were used for 5 years. Jacques described working with Bob and Jean when they were making culture media and what the process was like to witness, with all the checks and how much he learnt from it. He talks about how the different labs were organised. The daily meetings the team would have. Bob's work running journals as Editor and then his decision to leave and then setting up RBM online and how he went on to make a career out of editing, sharing knowledge and publishing and how ahead of his time with the whole concept of ‘online' information. Not only was he a pioneer in IVF, but he was also a visionary in his editing and publishing of major journals for 25 years. How Bob was consumed 365 days a year about publishing and sharing knowledge and was a big fan of the modern ways of sharing communication. Media production and how they have changed and its impact on success rates. SOCIALS: Twitter
