Today we bring on a different perspective from Phil Illy, a man who self-identifies as autogynephilic, meaning that he experiences a sexual attraction to the idea of himself as a woman. Originally described by sexologist Ray Blanchard, autogynephilia is a common contributor to gender dysphoria in young men. Yet the sexual component of “gender identity” is rarely addressed in the process of diagnosing and reviewing “treatment options” for gender dysphoria. Why is this?Phil considers autogynephilia, as well as its counterpart in women, autoandrophilia, to belong to a broader category he calls autoheterosexuality. While many in the gender critical community consider these to be sexual fetishes, Phil refers to them instead as sexual orientations. Within that framework, he researched the issue extensively and wrote a book on the subject, Autoheterosexual: Attracted to Being the Other Sex.Phil makes several further assertions in this interview, while I play devil's advocate and push back, all in an attempt to bring understanding to listeners who are concerned for loved ones and expose both sides of the debate over this taboo topic. What does femininity mean to him? Is autoheterosexuality just another form of narcissism? What is its connection with autism?  How influenced is it by fetish pornography and niche online microcultures? Would it occur in a cultural vacuum? Is it heritable? After having seen enough of these laughable denials of autogynephilia, the main question in my mind has become less, "Is the two-type androphilic/autogynephilic taxonomy of MtF transsexuality approximately true?" (answer: yes, obviously) and more, "How dumb do you (proponents of gender-identity theories) think we (the general public) are?" (answer: very, but correctly). In some ways this is not a completely unreasonable critique, but I think it has a lot less bite once one realizes just how dysfunctional the discourse on this subject is, even among the ideology that Zack endorses as a replacement. A microcosm of the dysfunction can be seen in Michael Bailey and Kevin Hsu's study, How Autogynephilic Are Natal Females? (pdf). They are responding to a debate where Charles Moser, Scott Alexander, Julia Serano, and Jaimie Veale argue that cis women are often autogynephilic too, and that this suggests that autogynephilia in trans women is not so significant. In response, Michael Bailey (the effective leader of autogynephilia theory) and Kevin Hsu recruit samples of highly active members of online erotic AGP communities, and find that these samples are on average much more AGP than cis women, and therefore conclude that AGP in cis women is dubious. Substituting a sample of highly active members of online erotic AGP communities for trans women is an insanely biased methodology! And when called out on it (pdf), Michael Bailey essentially goes "well the debate isn't about trans women, it's about AGPs, so it's totally appropriate" (pdf), which again is just an insane response when very obviously the debate is about trans women! All the people Bailey is responding to were clearly talking about trans women in their texts! One would think that people would point out these problems so the study would be appropriately rejected. I have tried to do so, and some trans activists have tried to do so, but people on the Blanchardian side have generally rejected it. Now here's a thought experiment for you: If people cannot converge to the truth on objectively observable facts about who has said what that can be seen by reading the debates, how can they ever be expected to converge to the truth about complex psychological causal and distributional inference questions? Especially when the objectively observable facts concern what psychological causal and distributional inference questions are under dispute? Even if the specific point of AGP in cis women doesn't move you much (I don't think it should), this dysfunctional discourse might make you tempted to infer that Blanchardians do a lot of other shenanigans to make their theories look better than they really are. And I think you would be right to make that inference, because I have a lot of points of critique on my gender blog that go unaddressed. But my critiques aren't the core point I'm raising here, rather I'm pointing out that people have good reasons to be exhausted with autogynephilia theorists. There's been a lot of autogynephilia talk lately. One subject that sometimes comes up, but hasn't been discussed much on LessWrong, is how common autogynephilia is in cis women. Two datasets that are sometimes used for this question are Scott Alexander's and Aella's. Part of how they happened to be made is that I reached out to Scott and Aella, suggesting experimental questions for assessing autogynephilia, in ways that might function in cis women too and not just cis men: Picture a very beautiful woman. How sexually arousing would you find it to imagine being her? Do you find the thought of masturbating alone as a woman to be erotic? This is somewhat different from usual autogynephilia measures, which contain questions such as: Did you ever feel sexually aroused when putting on females' underwear or clothing? Have you ever become sexually aroused while picturing your nude female breasts? The reason I didn't include these latter questions is because it seems likely to me that they will be interpreted differently for males and females (e.g. if males do not have female anatomy, then they cannot be aroused by it literally, so instead they get aroused by imagining some other female anatomy that they don't actually have), and because these usual questions seem very bad if taken literally ("ever" and "while" rather than "how frequently" and "by" seem like there could be a lot of ways to get affirmative answers while not actually being autogynephilic - though the low rate of endorsement among women suggests to me that they are not taking it literally?). Anyway, Michael Bailey (activist researcher for autogynephilia ideology) responds in Aporia Magazine, saying "It's important not to confound y'know being seen as a woman or having a female body from having sex with a partner while having a female body or the prospect of going out on a date while wearing sexy clothes". I don't think this makes any sense. "Do you find the thought of masturbating alone as a woman to be erotic?" explicitly states that one doesn't have a partner around. "Picture a very beautiful woman. How sexually arousing would you find it to imagine being her?" doesn't suggest a partner any more than "Did you ever feel sexually aroused when putting on females' underwear or clothing?" or "Have you ever become sexually aroused while picturing your nude female breasts?" does. This seems like a completely unfounded critique. I think people who dismiss these questions should admit that they are starting with The Bottom Line that cis women are not autogynephilic, and reasoning backwards about what sorts of measures they do or do not want to endorse, rather than pretending that they are driven by evidence and desire for good measurement. Joseph Burgo is a psychoanalyst and author and a co-director of Genspect, an organization representing thousands of parents of children caught up in the ongoing transgender social contagion. The organizatioin now seeks to supplant the World Professional Association of Transgender Health as an authoritative source of guidance for how to deal with gender dysphoric youth. Burgo gave a talk at the Genspect: the Bigger Picture conference in April, held concurrently and in the same city as the annual gathering of the European branch of WPATH, titled Autogynephilia, and the Sexualization of Shame which I published at this Substack last month. How did we come to know that this was a condition at all and what is it?Joseph BurgoIt's very simple, it's men's sexual arousal at the idea or image of themselves as a woman. They are what we used to think as fetishistic cross-dressers — men who dress up in women's clothes and get off on it sexually. We know this from Ray Blanchard, who came up with the two types of transsexuals, the homosexual transsexual and the autogynephilic transsexual. Mike Bailey wrote about it, and Anne Lawrence did an exhaustive study on autogynephilia. What I find really frustrating is that these histories are really accounts of a fetish. I think of what Anne Lawrence wrote as the account of a fetish. It's all about the fetish — the history of the fetish, when it appeared, how it affects their sex life and their relationships, with no sense that it means something. I mentioned in my paper that Anne Lawrence had collected all this data, and then deleted the lengthy family histories of these guys, as if it were peripheral to her study. I find it enormously frustrating. But I do think that's the way the mental health profession is these days. The idea that symptoms have meaning, that they might have an unconscious significance that can be sorted out; that it's a way of resolving internal conflict, or it represents some defensive compromise. People don't think that way anymore, I do feel kind of like a dinosaur these days.Wesley YangIn liberal enclaves like this, especially in ones that are surrounded by red hinterlands, they just accept the next new thing immediately. They don't question them. “Yeah, being born in the wrong body? That makes perfect sense to us.”Were you already in tension and crisis with peers and with the school administration as a result?Joseph BurgoWe didn't have much to do with the school administration. I was set back, and I didn't really know how to cope with this. I approached the people that I thought would help. I think I speak for parents all over America who are having exactly this experience: the mental health professionals will tell you that you need to affirm your child's identity, and you're a bigot. We consulted with the endocrinologist just to get an idea of what the reality was of these drugs. I said to her, just in passing, that it's kind of frustrating that I can't find anybody who's willing to look at the mental health aspects of this, what else might be going on. She looked at me with utter contempt, and said, “You're not going to find that.” Like, “What an outlandish idea that there's a meaning to these symptoms.” It was really humiliating and awful.Wesley YangIncredible. You're a credentialed medical professional within this field, a colleague, and you have standing within that community. The consensus had coalesced to the point where your reflexive belief that, “of course, no one is born in the wrong body,” had already made you a pariah.Joseph BurgoTotally. This has a chilling effect on the profession. A number of people at the conference talked about this, and I know lots of people who won't go anywhere near gender because it's just too fraught. Or they've jumped on the affirmative bandwagon and have built practices affirming children, which is unconscionable to me, but they do it.Wesley YangThere are many states that have bans, I guess North Carolina doesn't, on what they call conversion therapy. Meaning if you help a child be comfortable in their own body, you are converting them from this fleeting psychological sense that they might be a member of the opposite sex, which we're going to reify and entrench as if it's their true identity and any deviation from it is a crime.Joseph BurgoWhich is an absolutely insane position. It's actually the opposite of what's true. One of the things I'm very upset about is that a lot of these gender nonconforming kids who would grow up to be gay or lesbian are being converted to trans. That's the real conversion therapy. I used to sit on the board of directors and was an officer and friends with everybody at my LGBT center where I live. When I dove into this space, I gave them an article I'd written and I said, “Do you want me to step down?” These are my friends and they said, “Yes, please go away.” They'd all jumped on board with the trans thing. It's the LGBT Center but now everything is trans. This was my big question: what if they're gay and not trans. And I'm treated like, “Go away.”Wesley YangYou're still able to practice. They didn't cancel and destroy you, or they didn't try to, or they didn't succeed at it.Joseph BurgoThe fortunate thing is that I'm at a point in my career where I have enough money. I'm in private practice. They can't really cancel me. What are they going do to me?Joseph BurgoAnd it justifies any kind of violence on your behalf, coming from you. The book I want to write next is really an update of Lasch. He diagnosed America famously as suffering from a narcissistic disorder. I think what we've got now is a culture that suffers from borderline personality disorder. My most recent essay, Living In An As If World, looks at the way this kind of detachment from reality, this “as if” existence that we're living where you can just make it up as you go along, is leading to detachment from reality, but also, increasingly, borderline behavior where borderline rage, vicious assaults, attempts to destroy people, which is kind of what borderlines are like, is widespread. It dominates Twitter, for sure. Twitter is like nonstop borderline rage all the time. I do think there's a cultural pathology that's gotten much, much worse. Even though I say the tide has turned, I think, “Can we really pull out? Can we pull back when the society is this ill?” I don't know.Wesley YangWhat is borderline personality disorder and how do you see it manifesting in the culture?Joseph BurgoThe personality disorders, particularly borderline personality disorder, are characterized by an unstable sense of self that vacillates back and forth between feeling like you're superior and feeling like you're trash. It's unstable emotional states, frequent outbursts of rage, when you are challenged or when you aren't validated. These people are prone to various kinds of addictions. They have a hard time forming realistic and lasting relationships. They tend to blow up their friendships and family relationships. These features sound, to me, a lot like the way trans rights activists behave. They really are the embodiment of a cluster B personality disorder, particularly borderline personality disorder. If you take it just on the way people respond to one another in space, the way people will be enraged about nothing and take other people down. They will feel slighted and go off in an explosive way. I think social media really is characterized by borderline kinds of communication. This is a public episode. Dr. Blanchard is widely known for his typology of transexuals and as the originator of the term Autogynephilia, a controversial paraphilia which describes a heterosexual male's propensity to be sexually aroused by the idea of themselves as women. Dr. Blanchard was the psychologist in the Adult Gender Identity Clinic at the Clarke Institute of Psychiatry—now part of the CAMH (Centre for Addiction and Mental Health)—from 1980–1995. Much of his research in those 15 years concerned transsexualism and milder forms of gender identity disorders. In 1995, he was appointed Head of the newly created Clinical Sexology Services at the CAMH. This unit comprised the Adult Gender Identity Clinic. Since his retirement, he has continued to be active in research on human sexuality.

Stella asks why obsession is not being brought into this rather than just focusing on it as a gender identity topic. Sasha shares her thoughts about the dismissive counseling she has noticed in general. They both talk about their understanding of the depth of Anne's suffering through her experience. Sasha evaluates Anne's actions and responses as an obsession to a rejection of a body part, which is common in most Aspy people. Stella thinks there is still so much to be done on autistic diagnosis brainwork and she is looking forward to the next 15‒20 years. Everyone has a very high regard for the Dutch model, implying that they are being very careful but they both disagree. Sasha recalls how Steensma and de Vries may have perceived them as activists in their line of questioning. Stella and Sasha share more feedback about the Dutch study if they truly wanted it to be neutral and inclusive. What if the whole premise of the gender dysphoria scale is flawed? Sasha asks in retrospect. We shouldn't really change something unless we have a really clear understanding of what the current state of affairs is. Stella reflects on this against puberty blockers. They share how off-putting it was for them to talk about the human body in a flippant manner. What if Jazz Jennings was put on this gender dysphoria scale? Stella and Sasha talk about their disappointment further. Stella mentions Az Hakim and how she admires his approach vs. the rigid coldness of the so-called experts. Psychotherapy vs. Bodily Autonomy. Sasha compares the difference between their approach and their guests in this series. Ray Blanchard was the psychologist in the Adult Gender Identity Clinic at the Clarke Institute of Psychiatry 1980–1995. Much of his research in those 15 years concerned transsexualism and milder forms of gender identity disorders. In 1995, he was appointed Head of the newly created Clinical Sexology Services at the CAMH. This unit comprised the Adult Gender Identity Clinic and the Kurt Freund Laboratory, the latter being the oldest laboratory in North America for the psychophysiological assessment of erotic interests in sexual offenders and other men with problematic sexual behavior. Blanchard retired from the CAMH in 2010. Since his retirement, he has continued to be active in research on human sexuality, often in collaboration with former students, colleagues, and their students.   Today, Ray tells us how he got into the field, as he was primarily interested in conducting sex research. And then he found himself in a gender identity clinic. He got interested in boiling down the cumbersome and sprawling categorizations of gender dysphoria and deemphasizing women's clothing. He wanted to uncover the wider framework that explains a variety of behaviors for dysphoric males. The term autogynephilia was born. Her writing has appeared in The New York Times, TIME, The Guardian, The Daily Beast, Salon, and elsewhere.Dr. Serano is a patient debunker of anti-trans talking points, and has written a number of articles patiently taking apart common misconceptions. Her "transgender glossary of sorts" is also an essential resource for those who find gender and sexuality terms confusing or imprecise. Dr. Serano joined to go through some of the most common arguments made about trans people in the popular press and show why they are pernicious, factually incorrect nonsense. Having previously spent two decades as a professional biologist, she corrects the laughably ignorant claims that transgender people misunderstand or ignore "biology."
