Dr. Steve James is a consultant in Critical Care Medicine and Exercise Testing at King's College Hospital in London, UK. During a visit by the Health Minister Sajid Javid to the COVID critical care unit where he was the Consultant in Charge in early January 2022 he challenged the minister on the requirement for mandatory vaccination saying that the "science wasn't good enough". Overnight the video went viral and was probably the first video of a doctor challenging the status quo to be aired on mainstream media in the UK. Dr James was said by many to have spoken up on behalf of the 100,000 healthcare workers that were due to lose their job and quickly gathered a large social media following. He carried on delivering his message through numerous television, radio, magazine and social media outlets and joined others in a campaign against mandates which the government have now revoked. He continues to speak out on civil liberties issues and on the message he has been working with in his Exercise Intolerance Clinic, that with the right changes in nutrition, movement, circadian rhythm and nervous system inputs, chronic disease ca be reversed, what he calls "Health Medicine." This piece provides an update on the Perioperative Exercise, Testing and Training Society (POETTS); "established back in 2017, in the context, in the United Kingdom, of cardiopulmonary exercise testing becoming much more common in the preoperative period". Now, more than half of acute hospital trusts in the United Kingdom have a cardiopulmonary exercise testing service to evaluate their patient's functional capacity prior to surgery. Furthermore exercise interventions to optimise a patient's fitness before surgery have seen a sudden and growing interest. With just over 400 members POETTS exists to help standardize education and training in the field, develop consensus clinical guidelines, look after patients around the time of surgery and help club together acute hospital trusts across the UK, so that they can do collaborative research.

For Pilates instructors, few topics provoke as much anxiety as working with prenatal clients. As a consequence many instructors compulsively seek continuing education on prenatal and postnatal Pilates. Sadly, most educators don't understand current prenatal exercise guidelines. So despite all the ongoing education they do on the topic, the majority of Pilates instructors still don't know what to do with pregnant clients. In this double-length episode we delve into the mythology of prenatal Pilates, and talk through many of the Pilates Elephants surrounding pregnancy.
