Her research includes the use of innovative qualitative and community-centered approaches to understand and address drivers of disparities in different reproductive health outcomes such as contraceptive uptake, breastfeeding, maternal morbidity and mortality, maternal mental health, postpartum depression, access to health information (and misinformation), and access to abortion. Prior to RAND, she has worked in primary health care delivery, the health nonprofit sector, state government, and as a consultant for the Pan-American Health Organization.She obtained her medical degree from the University of Costa Rica and has a masters in public health and anthropology from the London School of Hygiene and Tropical Medicine and Georgia State University. In the push for competency-based education where faculty verify the skills of what a student can do, rather than their knowledge, our series focuses on the application of didactic content with a practical approach so that you can learn nuances of clinical skills before you reach the bedside.    This episode guides the novice pediatric provider on creations of an acute care differential diagnosis. It starts with a story about a Southerner in a snowstorm and the unfortunate car problem that resulted from an unexpected guest in the engine. A clear parallel ties the mechanic's diagnosis with a few amateur onomatopoeias (“clunk, clunk, clunk”) with the skills needed to form illness scripts and develop differentials. A brief case study on an adolescent with acidosis introduces the idea of broad differential formation and the importance of a complete evaluation before diagnoses are eliminated. Medical decision-making is difficult, and a systematic approach to differential diagnosis formation is essential. The episode uses simple examples to help listeners apply the concepts and form a differential in real time. The discussion covers the importance of careful accrual of information, initial differential creation using a systematic approach, how to narrow your differential based on key findings of the assessment, and how to approach an open-ended differential honestly with families while avoiding cognitive bias. With the understanding that, “disease exists on a continuum that evolves and we see the patient at a snapshot in time,” the episode offers a step by step guide on how to build a differential. Why is affordable housing in San Antonio everyone's problem? This week's guest is Mark Carmona, the city of San Antonio's first Chief Housing Officer, a position created by City Council in 2018 when council members first approved Mayor Ron Nirenberg's housing initiative. Carmona overees the city's Strategic Housing Implementation Plan (SHIP), and previously served as CEO of Haven for Hope and Deputy CEO for the Center for Health Care Services. Listen as Carmona tackles some of the most challenging issues we face: poverty, homelessness, lack of affordable housing, and how San Antonio's projected growth makes these issues relevant to all current and future San Antonians.

As recently as the early 80s, about three of every four doctors in the U.S. worked for themselves, owning small clinics. Today, some 75 percent of physicians are employees of hospital systems or large corporate entities. Some worry the trend is leading to diminished quality of care and is one reason doctors at a large Midwestern health provider decided to unionize. Fred de Sam Lazaro reports.
