He taught himself to fish, starting with lakes and gradually moving to streams. Originally from the auto repair industry, he eventually transitioned to fly fishing. 7:43 - We talk about the Missouri River. Craig explains that the river originates from the confluence of the Gallatin, Jefferson, and Madison rivers at Three Forks, located south of their area. He also notes that they fish on the stretch below the Holter Dam down to the town of Cascade, a 40-mile stretch known for its diversity and clear waters due to the reservoirs storing cold water. 10:56 - Craig discusses their lodge, a small, family-run lodge located between Great Falls and Helena, Montana, providing convenient access to the Missouri River. They offer a personalized experience with Craig as a guide and his wife and son assisting in operations. The lodge provides easy access to fishing spots, with launch points within a 10 to 30-minute drive. 13:57 - Their fishing trips span from April to October. Craig explains that although the season has ended, the favorable weather could have allowed fishing into November. April begins with variable weather, offering both sunny and cold, rainy days, but it features excellent fishing with minimal crowds. The fishing season wraps up with blue wing olives and October caddis. Craig highlights the abundance of dry fly fishing opportunities throughout the season, expressing gratitude for the rich fishing environment they enjoy. 17:16 - Craig discusses fly fishing techniques on the Missouri River, emphasizing the excitement of tricos fishing, where timing and the right drift are crucial as fish rise to eat small flies like size 18s or 20s. He notes that while the river is known for technical fishing with small dry flies, there are also opportunities to use larger flies like hoppers in sizes 6 and 8 for a few months. Craig prefers using a single dry fly for a better drift but acknowledges that a dry dropper setup can be effective. 19:14 - We get into the use of dry flies during a PMD (Pale Morning Dun) hatch. Craig explains that during such hatches, fish move to shallow gravel bars, sometimes in waters as shallow as a foot deep. Anglers often anchor on these bars, either wading or fishing from the boat. He notes that it's common to find a spot rich with fish and spend several hours there without needing to move much. 21:03 - We talk about fishing for caddis flies, which are prevalent during the June to July summer period and continue into the fall with species like the tan caddis and big October caddis. Trico hatches are challenging due to the number of rising fish, but the PMD hatch is considered easier, as early-season fish are less picky. Craig shares that both of his sons started fishing at a young age, initially using spinning rods before naturally progressing to fly fishing. Craig explains that while the river does experience pressure, especially during busy times like June and July, its large size allows anglers to spread out, minimizing the impact of crowded conditions. He advises anglers to fish downstream to avoid spooking fish and ensure the fly drifts naturally, as drag can deter fish from biting. Comparison of FDG-PET/CT and CT for Treatment Evaluation of Patients With Unresectable Mesothelioma

BUFFALO, NY- July 15, 2024 – A new #research paper was #published in Oncotarget's Volume 15 on June 20, 2024, entitled, “Comparison of FDG-PET/CT and CT for evaluation of tumor response to nivolumab plus ipilimumab combination therapy and prognosis prediction in patients with unresectable malignant pleural mesothelioma.” Malignant pleural mesothelioma (MPM) is an aggressive neoplasm and affected patients have low survival rates. In this new retrospective study, researchers Kazuhiro Kitajima, Kozo Kuribayashi, Toshiyuki Minami, Hiroyuki Yokoyama, Akifumi Nakamura, Masaki Hashimoto, Takashi Kijima, Seiki Hasegawa, Hayato Kaida, and Koichiro Yamakado from Hyogo Medical University and Kindai University Faculty of Medicine examined the effectiveness of fluorodeoxyglucose positron emission tomography (FDG-PET) criteria, i.e., immunotherapy-modified PET response criteria in solid tumors (imPERCIST), with morphological computed tomography (CT) criteria, i.e., modified response evaluation criteria in solid tumors (mRECIST), to evaluate patients with unresectable MPM undergoing nivolumab plus ipilimumab combination therapy as first-line treatment regarding response and prognosis prediction. “Results for malignant pleural mesothelioma (MPM) patients following first-line treatment with nivolumab plus ipilimumab obtained with immunotherapy-modified PERCIST (imPERCIST), shown by [18F] (FDG-PET/CT), and modified RECIST (mRECIST), shown by CT, were compared for response evaluation and prognosis prediction.” Twenty-six patients (23 males, 3 females; median 73.5 years) with histologically proven MPM and no curative surgery received nivolumab plus ipilimumab combination therapy. FDG-PET/CT and diagnostic CT scanning at the baseline, and after 2–4 cycles (2 in three, 3 in 17, 4 in six patients) were performed. Therapeutic response findings evaluated using imPERCIST and mRECIST were compared. PFS and OS analyses were done using log-rank and Cox methods. Results: imPERCIST indicated nine progressive metabolic disease (PMD), eight stable metabolic disease (SMD), four partial metabolic response (PMR), and five complete metabolic response (CMR) cases. mRECIST showed nine with progressive disease (PD), nine stable disease (SD), seven partial response (PR), and one complete response (CR). Although high concordance was noted (κ = 0.827), imPERCIST correctly judged a greater percentage with CMR (15.4%). Following a median 10.0 months, 15 patients showed progression and eight died from MPM. For Jacob Trossman's whole life, his mother Marcy White has had to fight for his medical care. Jacob has an ultra-rare degenerative disorder called PMD, and at 12 years old, he became a patient in the Complex Care Program at SickKids in Toronto. But now, at 21, Jacob has aged out. SickKids says their services can be adequately replaced by specialists in the adult care system. But Marcy White is fighting harder than ever to keep Jacob with his pediatric team.

The New York Times' Glenn Thrush writes: “In 2021, federal prosecutors began examining whether Mr. Gaetz…broke federal sex-trafficking laws, focusing on his relationships with women recruited online for sex, and whether he had a sexual relationship with a 17-year-old girl.” But hasn't the damage to Rep. Gaetz's reputation already been done? 3:15pm- On Wednesday, House Judiciary Committee Chairman Rep. Jim Jordan (R-OH) announced that he has subpoenaed the CEOs of Alphabet, Amazon, Apple, Meta, and Microsoft. He explained that the House Judiciary Committee reached out to the aforementioned companies late last year, but they did not comply with requests. In his letter, Rep. Jordan writes, “Twitter recently set a benchmark for how transparent Big Tech companies can be about interactions with government over censorship. The Twitter Files have exposed how Big Tech and the federal government have worked hand in hand in ways that undermine First Amendment principles.” 3:25pm- On Tuesday night, U.S. Senators took part in a classified briefing with Biden Administration officials regarding the mysterious flying objects that have been shot down by U.S. fighter jets over the last week. Sen. Marco Rubio (R-FL) told reporters that “95% of what was discussed could be made public.” 3:50pm- Senator Josh Hawley (R-MO) has introduced legislation that would create a social media minimum age requirement of 16-years-old for users—explaining that “big tech” companies are neglecting to protect children. 4:00pm- Do you support federal legislation to create a social media minimum age requirement, similar to the bills proposed by Senator Josh Hawley (R-MO)? Rich hears from callers. 4:35pm- Rich continues to take calls. The federal government's involvement in most issues typically ends up being problematic—is there any reason for conservatives to deviate from that belief when it comes to social media? 4:55pm- Caller Mark wonders: If Rep. CDC lists oil of lemon eucalyptus as comparable to DEET for mosquitoes CDC, June 9, 2022 Even the CDC recommends this botanical ingredient as comparable to DEET for repelling disease-carrying insects. While DEET is the gold standard of insect repellents, it is also a strong synthetic chemical with a tarnished reputation. Known to the chemistry set as N,N-Diethyl-meta-toluamide, DEET was potentially linked to cases of brain damage in the 1980s and 90s, inciting a flood of fear amongst consumers that has yet to recede. BUT, there's an alternative. Behold oil of lemon eucalyptus (OLE), the plant-based active ingredient derived from eucalyptus leaves and approved for efficacy by the CDC. “Oil of lemon eucalyptus (OLE) or PMD (chemical name: para-menthane-3,8-diol), the synthesized version of OLE. Products containing OLE and PMD include, but are not limited to, Repel and Off! Botanicals. This recommendation refers to EPA-registered products containing the active ingredient OLE (or PMD). “Pure” oil of lemon eucalyptus (essential oil not formulated as a repellent) is not recommended; it has not undergone similar, validated testing for safety and efficacy and is not registered with EPA as an insect repellent.” Just 5 Minutes Of Breath Training Every Day Makes Exercise Easier For Adults University of Colorado, June 11, 2022 A handheld device which builds breathing muscles could help older people get in shape. Doing just five minutes of the breath training exercises every day for more than six weeks improved people's performance on the treadmill by 12 percent, according to scientists. Adults are recommended to log 150 minutes of moderate exercise a week. Now scientists have come up with a simple way of making exercising on the treadmill easier for those struggling to get running. “Developing novel forms of physical training that increase adherence and improve physical function are key to reducing the risk of chronic diseases with aging,” says lead researcher Kaitlin Freeberg, of the University of Colorado, in a statement. “High-resistance IMST (high-resistance inspiratory muscle strength training) may be one such strategy to promote adherence and improve multiple components of health in midlife and older adults.” IMST involves inhaling through a handheld device which adds resistance to the breath. A total of 35 adults were divided into two groups, a high resistance training group and a low resistance control group. Participants used a manual breathing trainer for 30 breaths a day, or around five minutes, over a period of six weeks. Those who were part of the high-resistance group improved their treadmill time by 12 percent, the researchers report. Participants in the high-resistance group also showed a relationship with changes in 18 metabolites tested in the study, “predominantly ones that play key roles in energy production and fatty acid metabolism.” Pregnant women produce super antibodies to protect newborns, now scientists know how Cincinnati Children's Hospital Medical Center, June 8, 2022 Scientists discovered years ago that newborn infants depend upon immune components transferred from their mothers to survive the onslaught of pathogens that begin invading their bodies as soon as they are born.  Now, a far-reaching study published in Nature, provides a surprising explanation of how those early days of mother-provided immunity actually work–and what that information could mean for preventing death and disability from a wide range of infectious diseases.  “For many years, scientists believed that antibodies cannot get inside cells. They don't have the necessary machinery. And so, infections caused by pathogens that live exclusively inside cells were thought to be invisible to antibody-based therapies,” says Sing Sing Way, MD, Ph.D., Division of Infectious Diseases at Cincinnati Children's. “Our findings show that pregnancy changes the structure of certain sugars attached to the antibodies, which allows them to protect babies from infection by a much wider range of pathogens.”  “The maternal-infant dyad is so special. It's the intimate connection between a mother and her baby,” says John Erickson, MD, Ph.D., Division of Neonatology, and first-author of the study. Erickson continues, “This special connection starts when babies are in the womb and continues after birth. I love seeing the closeness between mothers and their babies in our newborn care units. This discovery paves the way for pioneering new therapies that can specifically target infections in pregnant mothers and newborns babies. I believe these findings also will have far-reaching implications for antibody-based therapies in other fields.” Fasting boosts stem cells' regenerative capacity Massachusetts Institute of Technology, June 3, 2022  As people age, their intestinal stem cells begin to lose their ability to regenerate. These stem cells are the source for all new intestinal cells, so this decline can make it more difficult to recover from gastrointestinal infections or other conditions that affect the intestine. This age-related loss of stem cell function can be reversed by a 24-hour fast, according to a new study from MIT biologists. The researchers found that fasting dramatically improves stem cells' ability to regenerate, in both aged and young mice. In fasting mice, cells begin breaking down fatty acids instead of glucose, a change that stimulates the stem cells to become more regenerative. The researchers found that they could also boost regeneration with a molecule that activates the same metabolic switch. Such an intervention could potentially help older people recovering from GI infections or cancer patients undergoing chemotherapy, the researchers say. “This study provided evidence that fasting induces a metabolic switch in the intestinal stem cells, from utilizing carbohydrates to burning fat,” Sabatini says. “Interestingly, switching these cells to fatty acid oxidation enhanced their function significantly. Pharmacological targeting of this pathway may provide a therapeutic opportunity to improve tissue homeostasis in age-associated pathologies.” After mice fasted for 24 hours, the researchers removed intestinal stem cells and grew them in a culture dish, allowing them to determine whether the cells can give rise to “mini-intestines” known as organoids. The researchers found that stem cells from the fasting mice doubled their regenerative capacity.

I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Afaf: Greetings, Could you kindly answer an inquiry for my dad who was diagnosed with a prostate cancer? He is 78 years old, 5'10 and weighs 150 lbs. The conclusion of the biopsy shows: - Right prostatic biopsy: adenocarcinoma prostatic grade 4 of ISUP, Score of Gleason at 8 (4+4), infiltrating 80% of the examined sample - Left prostatic biopsy: foyer of 2mm of an adenocarcinoma prostatic grade 4 of ISUP, Score of Gleason at 8 (4+4) The test results from the CT scan showed no other organ is damaged along with his bones. His doctor advised to start radio therapy and hormone-therapy. My inquiry is related to the Prostate Support supplementation as I would like to know if it is compatible with the treatment he is about to start. Can he take the supplements while he is ongoing treatment? Otherwise, should these supplements be taken prior to the start of treatment or after? When and what is the best timeframe to take these? I use to take the generic brand of ambien and that didn't make my sleeping issue any better. Every night when I put my head on my pillow the back of my head and neck hurts to the point where I can't sleep. My head feels restless, if that even makes sense. I saw a neurologist and she said I have occipital neuralgia and she poked me with a needle to block the nerve on the back of my head, It didn't work for me. I also went to a pain management doctor and he told me it was stress and prescribed noretriptilyne to relax me at night, that didn't work either. I stopped taking every medication given to me for this problem and I just bare the pain. I've also purchased every pillow imaginable for the head and neck and nothing seems to work. I've changed my lifestyle by eating healthy for the past two years. I am gluten free and eat clean. I've eliminated processed foods and sugar from my diet. I was thinking of going to a chiropractor. My husband has had consistently low HDL levels (often in the low 30 mg/dl) despite being of normal weight and without other known risk factors. His father (deceased from a heart attack) and his sister have also both had consistently low HDL (although both were/are overweight). The doctor would advise him to exercise more, which is ironic since my husband runs quite a bit. Having said that my questions are : In an otherwise healthy individual is low HDL something to be concerned about? If it is something of concern, other than increasing exercise might there be some natural ways of raising HDL levels despite a possible genetic connection? Thank you for all you do.  Summer: Hi doc! Question regarding EBV. I'm 32 & had mono around age 15. My health tanked after that until about 5 years ago. My white blood cell count, absolute neutrophils, & absolute monocytes are all low. EBV early antigen D AB (IGG) is 29.4 u/mL, EBV viral capsid AG (VCA) AB (IGG) is 297 U/mL, & EBV nuclear AG (EBNA) AB (IGG) is 191 U/mL. I'm told all the EBV levels are high & that these labs point to chronic active EBV. I've been working diligently for at least 5 years on my health. I've overcome PCOS, PMD, endometriosis, H Pylori, depression, arthritis, migraines, exercise intolerance, regained energy & drive, but still having gut & thyroid issues. I've been overdosed on Bioidentical T3 for a number of years & have recently weaned off & my thyroid has improved dramatically. By the time you answer this I'll be about half done with the CBO protocol. I guess I'm wondering how do I know that the EBV is causing my residual health issues? Should I have addressed the EBV first instead of the gut? I've already been doing most of the things listed in ep. 760 so curious why my levels would still show a problem with EBV. I've worked so hard & feel like I just can't quite get all the way there. I'm struggling with the right order to attack these remaining problems. Thanks so much!
