Berkowski, who serves as a vice chair of the AASM's Practice Guidelines Task Force, provided clinical insight on the reasons behind the guidelines, pointing to the vast research advances and literature updates since its last iteration in 2012. Additionally, he touched on the greatest changes to the guidelines, noting things like the shift away from dopaminergic agents, the importance of iron in RLS pathophysiology, and the use of alpha-2-delta ligands and intravenous iron as first-line treatments. Berkowski also shared thoughts on how the guidelines impact care for patients of all ages, sexes, and RLS subtypes, as well as some of the more troubling parts of the guidelines to write and areas that were left unanswered. Furthermore, he gave his thoughts on how these guidelines will transform care going forward, including accelerating conversations on preventing RLS in many cases in the near future. The AASM is introducing a new service-based accreditation model and updated Standards for Accreditation, which will take effect in January 2025. Based on feedback from members and accredited programs, this accreditation model provides greater flexibility to accommodate all practice types, making it easier to manage, add, and remove accredited services. Changes to the Standards for Accreditation were also made to remove barriers and promote administrative and operational efficiencies in sleep practice management. In this episode, AASM Accreditation Committee Chair Dr. Fady Jamous and AASM Director of Accreditation Ray Anthonijsz help explain some upcoming changes to AASM accreditation programs.

According to the Academy of Sleep Medicine (AASM), the Sleep Research Society (SRS), and the National Sleep Foundation (NSF), adults should regularly sleep at least 7 hours a night to promote optimal health. However, over one-quarter of adults do not meet this recommendation. (Oops, me included!) AASM states that about 30% of adults have insomnia symptoms, and about 10% of adults have insomnia that is severe enough to cause daytime repercussions. What is it about sleep that it's so important to have? What is the parameter of good and quality sleep? What are the health risks of not sleeping enough? Hai ngày sau, ngày 18/01, bộ trưởng Quân Lực Pháp Sebastien Lecornu chính thức ra mắt « liên minh pháo binh », nhằm tài trợ và cung cấp 78 khẩu đại bác Caesar cho Ukraina trong năm 2024. Liệu Paris có đủ phương tiện để thực hiện các cam kết vào lúc khả năng sản xuất vũ khí của Pháp thấp hơn rất nhiều so với nhu cầu của Ukraina ?   Những thông báo này được đưa ra vào lúc một báo cáo của Ủy ban Đối ngoại và Quốc phòng Thượng Viện Pháp, được công bố ngày 17/01, kêu gọi nước Pháp tăng tốc càng nhanh càng tốt hậu thuẫn quân sự cho Ukraina. Chủ tịch ủy ban Cedric Perrin cùng với ba thượng nghị sĩ khác, sau chuyến thăm Kiev hồi cuối tháng 12/2023, đã lấy làm tiếc là hậu thuẫn của phương Tây dành cho Ukraina đã suy giảm mạnh.Báo cáo của Thượng Viện Pháp nêu rõ : « Trợ giúp của phương Tây đang hụt hơi và tình hình ở Ukraina ngày càng trở nên phức tạp. Trong giai đoạn tháng 5 đến 12/2023, Nga vẫn chiếm ưu thế. Quân số Nga trên mặt trận dường như đã được tăng thêm 20% và số lượng xe tăng, khẩu đại pháo tăng 60% ».Bốn thượng nghị sĩ Pháp cảnh báo : « Nga rất có thể mở lại chiến dịch trên bộ quy mô lớn trước thềm bầu cử tổng thống Mỹ. Một cuộc tấn công mới của Nga nhắm vào Kiev là không thể loại trừ trong trung hạn nếu nguồn hậu thuẫn của phương Tây bị thất bại ».Pháp : Nhà hảo tâm xếp thứ 15 ?Theo số liệu chính thức, tính từ đầu cuộc xung đột, Paris đã giao cho Kiev nhiều trăm đơn vị, đủ loại vũ khí, từ đại bác Caesar, pháo kéo TRF1, hệ thống pháo phản lực đơn nòng (LRU), nhiều loại xe bọc thép, rồi còn có tên lửa hành trình không đối địa Scalp, các loại hệ thống phòng không SAMP/T và Crotal NG, hệ thống radar Ground Master 200, tên lửa Mistral, Milan và Akeron MP, cũng như drone trinh sát, nhiều thiết bị thăm dò phát hiện tấn công mạng hiệu quả…Dù vậy, Pháp chỉ là nhà hảo tâm xếp thứ 15 về trợ giúp quân sự cho Kiev. Báo cáo mang tên Ukraine Support Tracker do Viện Kiel, một trung tâm nghiên cứu của Đức, công bố gần đây, cho thấy đến cuối tháng 10/2023, nguồn hậu thuẫn của Paris được ước tính ở mức 500 triệu euro, thua xa Mỹ (43, 9 tỷ), Đức (17,1 tỷ), Vương Quốc Anh (6,6 tỷ), Na Uy (3,6 tỷ) và Đan Mạch (3,5 tỷ). Pháp thậm chí còn bị Slovakia, Cộng hòa Séc và Litva qua mặt trong bảng tổng sắp.Phương pháp thống kê này của Viện Kiel đã bị Pháp phản đối, cho là chỉ tính đến những cam kết đóng góp mà không xét đến thực tế đã giao. Bộ trưởng Quân Lực Pháp, khi trả lời phỏng vấn đài phát thanh France Inter ngày 18/01, chỉ trích bảng xếp hạng này là không đáng tin cậy, khi « trộn lẫn bắp cải và cà rốt », và « chỉ dựa trên các tuyên bố ».Nhưng Paris cũng chưa bao giờ đưa ra được những con số thay thế khả tín, trang mạng Challenges lưu ý. Một báo cáo do Quốc Hội Pháp công bố hồi tháng 11/2023 đưa ra con số 3,2 tỷ euro viện trợ quân sự dành cho Ukraina, nhưng phương pháp thống kê này cũng có thể gây tranh cãi, nếu không muốn nói là mù mờ.Bất kể ra sao, nguồn hậu thuẫn này của Pháp rõ ràng là chưa đủ. Báo cáo của các nghị sĩ đánh giá, « việc chuyển qua nền kinh tế chiến tranh, như yêu cầu, đã không diễn ra ». Thượng nghị sĩ Cedric Perrin còn cho rằng cụm từ « kinh tế chiến tranh » mà tổng thống Macron sử dụng là không phù hợp. Trên kênh truyền hình Public Senat, chủ tịch Ủy ban Đối ngoại và Quốc phòng Thượng Viện Pháp giải thích :« Trong một nền kinh tế chiến tranh giống như trong nền kinh tế mà chúng ta không may đã trải qua sau Đệ Nhị Thế Chiến, ngân sách dành cho quốc phòng chiếm đến 28% của GDP. Tỷ lệ này là 5% trong những năm 1960 và hiện nay gần như chưa tới 2%. Đó là thời kỳ mà  người ta trưng dụng vật tư, trưng dụng nhân lực và trưng dụng nhà xưởng. Hiện chúng ta chưa rơi vào trong tình trạng đó. Một nền kinh tế chiến tranh nghĩa là phải có phương tiện để sản xuất nhanh hơn, sản xuất nhiều hơn. Nước Pháp hiện không trong tình trạng chiến tranh. Do vậy, theo tôi, khi nói  chúng ta đang trong nền kinh tế chiến tranh, thì tuyên bố này chỉ mang tính chất truyền thông ! »Nòng pháo Caesar : Điểm hạn chế của Pháp ?Liệu Pháp có khả năng tăng thêm mức viện trợ quân sự cho Kiev như thông báo ? Cũng trên đài France Inter, bộ trưởng Quân Lực Sebastien Lecornu nêu rõ chi tiết cách thức chi viện 78 khẩu đại bác Caesar cho Ukraina trong khuôn khổ « liên minh pháo binh »: Sáu khẩu đầu tiên do Ukraina chi trả sẽ được giao trong những tuần sắp tới, 12 khẩu tiếp theo là do Pháp tài trợ với tổng trị giá là 50 triệu euro và 60 chiếc còn lại sẽ do các nước khác đóng góp. Vậy những nước nào sẽ gánh vác phần còn lại đó ? Đây vẫn là một câu hỏi lớn.Giới quan sát cũng tỏ ra hoài nghi về khả năng thực hiện cam kết của Paris. Lãnh đạo bộ quốc phòng Pháp khẳng định, hãng Nexter có thể sản xuất 72 khẩu đại bác đó trong năm nay cho Ukraina. Theo phóng sự của kênh truyền hình quốc tế Pháp France 24, lãnh đạo Nexter cho biết hiện tại hãng này có thể cung cấp 6 khẩu đại bác mỗi tháng. Con số này sẽ tăng lên thành 8 trong năm 2024.Mức tăng đó có được là nhờ vào những khoản đầu tư được dự trù trước khi có xung đột. Ông Stephane Ferandon, phụ trách bộ phận sản xuất nòng pháo của hãng ở vùng Bourges, cho biết, trong tương lai, những cỗ máy mới của Nexter còn có thể sản xuất các loại nòng đại bác dài 12 mét, calip 62 thay vì nòng 155 ly, dài 9 mét, calip 52 như hiện nay.Tuy nhiên, trên làn sóng Sud Radio, Jean-Baptiste Noé tổng biên tập trang mạng Conflit, chuyên về địa chính trị, lưu ý thêm một chi tiết :« Rắc rối của đại bác Caesar là ở nòng pháo. Loại đại bác này phải được sử dụng tương đối ít, nhưng trong cuộc chiến tranh này, chúng được sử dụng quá mức, điều đó có nghĩa là nòng pháo bị hao mòn rất nhanh. Vì vậy, chúng phải được thay thường xuyên, tốn nhiều tiền hơn, kim loại sử dụng để sản xuất cũng hiếm. Thế nên, rủi ro ở đây là dù có đại bác Caesar, nhưng nếu bị sử dụng quá nhiều, chúng sẽ trở nên không sử dụng được. »Pháo 155 ly : Cung không đáp ứng đủ cầuBên cạnh việc cung cấp đại bác, Pháp cũng thông báo chi viện đạn 155 ly cho Ukraina. Trang mạng Challenges ngày 18/01/2024 nhắc lại một báo cáo khác của Thượng Viện công bố hồi tháng 11/2023 do hai thượng nghị sĩ Hugues Saury và Hélène Conway-Mouret đồng ký tên, từng nhận định trong năm 2024, việc cung cấp đạn pháo 155 ly do Pháp sản xuất phải đạt mức 20 ngàn quả.Nhưng sản lượng này chỉ đủ để Ukraina tiêu thụ trong vòng từ 3-4 ngày chiến sự. Trong báo cáo mới nhất của bốn thượng nghị sĩ Pháp, mỗi ngày Ukraina sử dụng từ 5-8 ngàn quả đạn pháo, phía Nga là từ 10-15 ngàn. Ngay cả khi Pháp có tăng sản lượng từ 2.000 quả đạn pháo lên thành 3.000 mỗi ngày như thông báo của bộ trưởng Quân Lực Pháp, thì con số này chỉ tương ứng với hai ngày chiến sự bên phía Nga.  Giải thích với kênh truyền hình France 24, Anthony Cesbron, phó giám đốc nhà máy Forges de Tarbes, xưởng vũ khí duy nhất nằm ở tây nam nước Pháp có thể sản xuất loại pháo đại bác 155 ly, cho biết hãng có kế hoạch tăng gấp ba sản lượng, đi từ 55 ngàn quả pháo trong năm 2024 lên thành 160 ngàn từ đây đến năm 2026. Nhưng điều này đòi hỏi phải gia tăng nhân sự.« Một trong những khó khăn của chúng tôi là tuyển dụng được những người có trình độ. Chúng tôi không tìm được người và hầu hết các vị trí ở hãng chúng tôi, quá trình đào tạo kéo dài từ sáu tháng đến một năm. Điều này có nghĩa là chúng tôi buộc phải có kế hoạch trước. »Xung đột ở Ukraina đã làm lộ rõ những điểm yếu của phương Tây về sản xuất vũ khí đạn dược. Chiến tranh lạnh kết thúc, bức tường Berlin sụp đổ, cỗ máy công nghiệp quốc phòng của Pháp giảm mạnh nhịp độ hoạt động. Giờ mục tiêu đặt ra là nâng cao trở lại nguồn dự trữ và khuyến khích các ngành công nghiệp quốc phòng sản xuất nhiều hơn, nhanh hơn.Nhưng mong mỏi này không thể thực hiện trong một sớm một chiều, theo như giải thích của Renaud Bellais, đồng giám đốc Đài Quan sát Quốc phòng ORION với kênh truyền hình France 24 :« Chúng ta gần với sản xuất thủ công hơn là đại trà. Chúng ta đang sống trong một thời đại mà ở họ có cảm giác rằng tất cả mọi thứ đều có thể là tức thì. Người ta có thể được cung cấp hàng hóa một cách cực kỳ nhanh chóng. Thế giới kỹ thuật số là một thế giới của tốc độ, nhưng họ cũng chợt nhận ra rằng trong thế giới cụ thể, việc chế tạo một cỗ máy đòi hỏi nhiều thời gian. Bất kỳ hoạt động công nghiệp nào đều phải được chuẩn bị, ngay cả khi chúng ta tăng mức sản xuất xe ô tô trong lĩnh vực dân dụng hay nơi khác thì cũng phải lên kế hoạch cho nhiều năm. »Điểm yếu nhưng cũng là thế mạnh của PhápTrước nhu cầu lớn từ phía Ukraina, trước sự cấp thiết tái lập lại kho dự trữ của quân đội Pháp và trước những hạn chế của ngành công nghiệp quốc phòng hiện nay, báo cáo của các thượng nghị sĩ đề nghị Pháp « tăng tốc sản xuất vũ khí tại Ukraina cùng với doanh nghiệp Pháp », như trường hợp nòng đại bác Caesar chẳng hạn.Nhu cầu của Ukraina là rất rõ ràng : Từ đại bác Caesar, cùng với đạn pháo và các loại nòng pháo thay thế, pháo 155 ly, tên lửa hành trình Scalp, vốn đã chứng tỏ tính hiệu quả khi phá hủy tầu ngầm, tầu chiến và tầu đổ bộ của Nga tại bán đảo Crimée, cho đến các loại bom dẫn đường AASM Hammer, hệ thống phòng không SAMP/T và Crotale và tên lửa tầm ngắn Mistral…Tuy nhiên, các báo cáo viên lại không đề cập đến tiêm kích Mirage 2000 của Pháp. Loại chiến đấu cơ, phiên bản Mirage 2000D (dành để yểm trợ tấn công trên bộ), do hãng Dassault sản xuất, đã được lãnh đạo không quân Ukraina Mykola Oleshchuk ngày 14/01 nhắc đến trên mạng Telegram. « Nếu có thể, năng lực oanh tạc cơ Su-24M cũng nên được tăng cường bằng Mirage 2000D và máy bay tấn công Su-25 bằng chiếc A-10 Thunderbolt II (Thần sấm II của Mỹ). »Cuộc đua sản xuất vũ khí đã được khởi động. Mục tiêu là làm sao đáp ứng cùng lúc nhu cầu to lớn của Ukraina và nhu cầu thiết yếu cho quân đội Pháp. Nếu chiến tranh Ukraina đã phơi bày những hạn chế công nghiệp quốc phòng phương Tây nói chung và Pháp nói riêng, thì điều an ủi là, cuộc xung đột này khẳng định rõ tầm nhìn đúng đắn của Pháp từ nhiều thập niên qua : Quyết tâm duy trì thế mạnh công nghiệp bất chấp chiến tranh lạnh kết thúc.Điều này giải thích vì sao Pháp vẫn có thể đáp ứng mức cầu tăng, dù là hạn chế, theo như nhận xét của Renaud Bellais với France 24 : « Tại Pháp, chúng ta vẫn có khái niệm về tự chủ chiến lược. Quả thật, chúng ta cố gắng có toàn bộ mọi lĩnh vực. Vì vậy, chúng ta đã chọn giữ lại hầu hết các năng lực bởi vì chúng ta muốn có thể tự mình phát triển chúng, sản xuất chúng tại Pháp và đôi khi với các đối tác. David E McCarty MD FAASM attended medical school at Duke University, and completed an internship and residency in Internal Medicine in Boston, at Massachusetts General Hospital. He began his career practising primary care, but after a handful of years in practice, he followed his own white rabbit into a whole new world called Sleep Medicine. Dr McCarty learned the art from the great Dr Andrew Chesson (world-class physician, and past president of the AASM). While at LSU, Dr McCarty won some teaching awards, published a few papers, and it's also where he developed the Five Finger Approach, which has remained the cornerstone of his teaching about patient-centred problem-solving, to this day, and is featured as the pinnacle for orienteering around the Isle. Patrick takes Regina through his life as an actor and into his current show about Victor Hugo. Patrick is co-artistic director and founding member of O'Punksky's. Essentially an actor he also designs, directs, produces, teaches, builds sets, [this web site] etc etc...... Patrick met Maeliosa Stafford at the Crossroads Theatre in Sydney's Darlinghurst in late 1989 and accepted his offer to join the cast of Observe the Sons of Ulster Marching Towards the Somme. There was a tiny budget so Patrick also volunteered to design the show. The production was a small triumph and O'Punksky's theatre was born. In 1990 Patrick played Malcolm Scrawdyke in O'Punksky's next production, Little Malcolm and His Struggle Against the Eunuchs (chosen for its long title), also directed by Maeliosa. Patrick again took on the task of designer and a creative partnership was galvanized. Patrick has been acting professionally since 1976, starting his career as AASM at the Theatre Royal in Stratford, East London. After a year he joined the Roundabout Theatre in Education Company in Nottingham and then, after another year at Stratford he emigrated to Australia. Over the years Patrick has worked with many companies in Australia, most notably Thalia, Bell, Griffin, Ensemble, Studio and most State companies. Patrick is co-producer/writer of a series of training films in which he performs and sometimes directs. See O'Punksky Pedagogic page on this site. He has taught acting courses and directed students at UNSW and ACTT. TV Credits include,: The Timeless Land, Carson's Law, Home and Away, Rake and Seachange. Film credits include: A Bridge Too Far, The Fluteman, Panacea, Flotsam Jetsam and Rogue Nation. Patrick has extensive voice credits which include many commercial voice overs, prose and poetry readings for ABC Radio Arts, and book readings which include Treasure Island, The Marx Sisters, The Drowner and A Tale of Two Cities. Artificial intelligence is the vanguard of a new technological revolution that will affect all aspects of human society. Our work, our health, the cars we drive, even the media we consume - all will be radically transformed in the coming years and decades by 'intelligent machines'. Essentially, these are computers that have learnt how to do many of the tasks we thought only humans were capable of. But the difference is, AI can do things quicker, more accurately, and of course, without needing human intervention. Sleep medicine will not be an exception to this march of progress. So much so, that earlier this year, the American Academy of Sleep Medicine (AASM) issued a position statement, outlining a roadmap for sleep professionals to start engaging now with the multifaceted ways that AI will inevitably impact on patient care, diagnostics, staff allocation, treatment protocols and much more. We talk to Dr Cathy Goldstein, neurologist, and chair of the AASM's committee on AI about how artificial intelligence will shape the field of sleep medicine in coming years, and also how Dr Goldstein's own research into longitudinal sleep tracking with consumer wearables may also play a role in expanding our vision for how we manage sleep health outside of the clinic. This episode's guest: Cathy Goldstein, MD, is an Associate Professor of Neurology at the University of Michigan and faculty in the Sleep Disorders Center. Her current research focus, through work with mathematics, is the improvement of sleep tracking via algorithm development and validation using consumer sensors. Desde o início da pandemia, o município seguiu orientações do Governo Federal, do Estado e da Organização Mundial da Saúde, e adotou o isolamento social, determinando que apenas aqueles essenciais seguissem funcionando. Após várias manifestações, inclusive com carreatas de empresários, o então Prefeito Antônio Silva, decidiu flexibilizar o funcionamento das lojas e prestadores de serviço, abrindo entre 10h e 17h. Foi muito criticado e voltou atrás: revogou o decreto ontem à noite. Hoje cedo, Antônio Silva renunciou ao cargo. O vice assume amanhã. Diante do alerta da Organização Internacional do Trabalho (OIT) para os governos redobrarem esforços pela manutenção dos empregos, diante do grande impacto do coronavírus sobre a economia mundial, e após as recentes medidas das empresas e do governo brasileiro, o Bate-Papo Van desta quinta-feira 19 tratou de home office, no Jornal de Vanguarda. O entrevistado foi o vice-presidente da Associação da Advocacia do Sul de Minas (AASM), Adriano Santos Oliveira. A OIT estima em 25 milhões de desempregados a mais no mundo por conta da crise. Uma das alternativas para prevenir a proliferação do novo coronavírus é o home office. Adriano Oliveira explicou o que diz a lei, se há alguma licença especial ou concessão legal em períodos de crise como esse, se a empresa pode impor o home office ou o colaborador tem o direito de ir trabalhar na companhia, se o tempo de teletrabalho pode ser descontado no salário e se a situação de agora exige algum contrato especial entre empregadores e empregados colocados em home office. Falou, ainda, de adoecimento do colaborador no período de trabalho remoto e esclareceu sobre o direito do empregador de usar alguma ferramenta para confirmar se realmente o colaborador está trabalhando em casa. Porém, o assunto é muito mais sério do que se pensa. Essa foi a palavra mais utilizada pela Presidente da Associação da Advocacia do Sul de Minas (AASM), Debora Anne Pereira da Silva, uma das fundadoras da entidade, em entrevista ao Jornal de Vanguarda desta sexta-feira 8, sobre a atuação fraudulenta de maus advogados por meio de ações predatórias. A prática estigmatizou particularmente os advogados de Varginha, embora seja comum em todo o Brasil. A AASM lidera um movimento em favor do resgate da credibilidade dos advogados. O programa teve, também, a participação do Presidente da Subseção Varginha da Ordem dos Advogados do Brasil, Alexandre Prado. Segundo a Associação dos Magistrados Mineiros (AMAGIS), ações predatórias são usadas para golpes em comarcas no interior do Estado. Em publicação no site Jusbrasil, a AMAGIS informa que uma verdadeira fábrica de fraudes tomou conta de algumas varas cíveis do interior de Minas e de São Paulo -- inclusive com casos em Varginha. São ações predatórias: advogados em busca de ganhos ilegais. Esses maus advogados usavam informações privilegiadas. Depois de flagrados em Varginha, alguns mudaram para outras cidades do estado e para Campinas e Ribeirão Preto (SP). Em Varginha houve cerca de 20 mil ações em 2018, prejudicando o trabalho da comarca. Hoje, depois de vários arquivamentos, restariam cerca de 8 mil ações, muitas delas de uma única pessoa. Ready For Takeoff - Turn Your Aviation Passion Into A Career

What is Obstructive Sleep Apnea? OSA affects a person’s upper airway in the area of the larynx (voice box) and the back of the throat. This area is normally held open to allow normal breathing by the surrounding muscles. When an individual is asleep, these muscles become slack, and the open area becomes smaller. In some individuals, this area becomes so small that breathing and resulting normal oxygenation of the blood is impaired. The person may actually choke. This causes some degree of arousal from normal sleep levels which the individual may or may not be aware of. These people do not get restorative sleep, and wake feeling tired. OSA has significant safety implications because it can cause excessive daytime sleepiness, personality disturbances, cardiac dysrthythmias, myocardial infarction, stroke, sudden cardiac death, and hypertension, and cognitive impairment such as decreased memory, attention, planning, problem-solving and multi-tasking. What is the background on the FAA’s actions on OSA? The FAA’s has always used the special issuance medical certification process to certificate pilots with OSA. In November 2013, the FAA proposed guidance that would have required treatment for pilots with a body mass index (BMI) of 40 or more. It would have grounded those pilots until they successfully completed treatment, if required, and they obtained a Special Issuance medical certificate from the FAA. Key aviation industry stakeholders, as well as members of Congress, expressed concern about this enhanced screening. The FAA has now revised the guidance to address those concerns. What is the new guidance?  An AME will not use BMI alone to assess whether the pilot applicant has OSA or as a basis for deferring the medical certificates (except in cases where the OSA risk is extreme). AME’s will screen for the risk for OSA using an integrated assessment of history, symptoms, and physical/clinical findings.  OSA screening will only be done by the AME at the time of the physical examination using the American Academy of Sleep Medicine (AASM) guidance provided in the AME Guide. Pilots who are at risk for OSA will be issued a medical certificate and will then, shortly thereafter, receive a letter from the FAA’s Federal Air Surgeon requesting that an OSA evaluation be completed within 90 days. The evaluation may be done by any physician (including the AME), not just a sleep medicine specialist, following AASM guidelines. If the evaluating physician determines, using the AASM guidelines, that a laboratory sleep study or home study is warranted, it should be done at that time. The pilot may continue flying during the evaluation periodand initiation of treatment, if indicated. The airman will have 90 days (or longer under special circumstances) to accomplish this, as outlined in the Federal Air Surgeon’s letter. The FAA may consider an extension in some cases. Pilots diagnosed with OSA and undergoing treatment may send documentation of effective treatment to the FAA in order to have the FAA consider them for a special issuance medical certificate. How is OSA treated? Though several types of treatment are available depending on the severity of OSA, the most effective treatment involves the use of a continuous positive airway pressure (CPAP) or Automatic Positive Airway Pressure device that is worn while sleeping. In fact, there are currently 4,917 FAA-certificated pilots who are being treated for sleep apnea and are flying with a special issuance medical certificate. When will the new guidance take affect? The FAA plans to publish the new guidance in the FAA Guide for Aviation Medical Examiners on March 2, 2015. What are the FAA’s current rules on OSA? Untreated OSA always has been and will continue to be a generally disqualifying medical condition requiring a special issuance medical certificate. AMEs are advised by the FAA to be alert for OSA and other sleep-related disorders such as insomnia, restless legsyndrome,and neuromuscular or connective tissue disorders, because they could be signs of problems that could interfere with restorative sleep, which are needed for pilots to safely perform their duties. Is the FAA changing the rules on OSA? The FAA is not changing its medical standards related to OSA. The agency is revising the screening approach to help AMEs find undiagnosed and untreated OSA. Have there been any accidents or incidents associated with OSA? The National Transportation Safety Board (NTSB) determined that OSA was a contributing factor in the February13, 2008 Mesa Airlines (operated as go!) Flight 1002 incident, in which both the captain and first officer fell asleep during the flight. They flew 26 miles past their island destination into open ocean, and did not respond to air traffic controllers for more than 18 minutes. After normal communication was resumed, all three crewmembers and 40 passengers onboard arrived safely at their destination.  The captain was found to have undiagnosed severe OSA. The NTSB has investigated accidents in all modes of passenger transportation involving operators with sleep disorders and believes OSA to be a significant safety risk. The NTSB database lists 34 accidents – 32 of which were fatal – where sleep apnea was mentioned in the pilot’s medical history, although sleep apnea was not listed as “causal” or “contributory” in those accidents. The database includes an additional 294 incidents where some type of sleep disorder was mentioned in the history.
