Berkowski, who serves as a vice chair of the AASM's Practice Guidelines Task Force, provided clinical insight on the reasons behind the guidelines, pointing to the vast research advances and literature updates since its last iteration in 2012. Additionally, he touched on the greatest changes to the guidelines, noting things like the shift away from dopaminergic agents, the importance of iron in RLS pathophysiology, and the use of alpha-2-delta ligands and intravenous iron as first-line treatments. Berkowski also shared thoughts on how the guidelines impact care for patients of all ages, sexes, and RLS subtypes, as well as some of the more troubling parts of the guidelines to write and areas that were left unanswered. Furthermore, he gave his thoughts on how these guidelines will transform care going forward, including accelerating conversations on preventing RLS in many cases in the near future. Pramipexole (Mirapex) is a dopamine agonist that can be used in Parkinson's disease and Restless Legs Syndrome. Dopamine agonists like pramipexole can worsen psychiatric symptoms in some patients by causing hallucinations, and obsessive/compulsive symptoms. Nausea, vomiting, hypotension, and fatigue are the most common adverse effects of pramipexole. Pramipexole is primarily eliminated through the urine. I discuss how this matters clinically in greater detail in this podcast episode.

Dopamine agonists like ropinirole can worsen psychiatric symptoms in some patients by causing hallucinations, and obsessive/compulsive symptoms. Nausea, vomiting, hypotension, and fatigue are the most common adverse effects of ropinirole. Ropinirole has drug interactions with antipsychotics and CYP1A2 inhibitors. He has a clinical teaching appointment as an Associate Professor of Clinical Medicine and Surgery at the Washington University School of Medicine. Dr. Weinstock’s lectures and research have been presented at national and international conferences on diseases of the esophagus, stomach, small intestine, and colon.  He is actively researching the connection of the gut and small intestinal bacterial overgrowth (SIBO) with several medical problems including restless legs syndrome and chronic pelvic pain syndromes. He presented several lectures at the first SIBO symposium and in France at the international rosacea study group. Dr. Weinstock’s clinical practice emphasizes colon cancer screening, therapeutic videoendoscopy, motility disorders, management for irritable bowel syndrome, and comprehensive management of inflammatory bowel disease. His experience in treating IBS patients who have SIBO dates back to 2000. The journal Neurology published a paper called “Prospective Study of Restless Legs Syndrome and Total and Cardiovascular Mortality Among Women.” This study proves to me that your leg cramps are telling you that your magnesium deficiency is serious and needs to be remedied.  Why? Because I’ve seen countless people eliminate their symptoms of leg cramps / Restless Leg Syndrome when they take ReMag and when people have magnesium deficiency they are at risk for CV disease. The highest amounts of magnesium in the body are in the heart so if there is magnesium deficiency, the heart is at risk. The association between restless legs and CV disease places the blame on magnesium deficiency as the shared cause. RLS is said to be a sleep and a neurological sensory disorder, which tells me nothing about its cause. In fact, medicine has no idea what causes RLS or how to treat it. This latest study just makes people with RLS more anxious because they have been told there is no cure for RLS and now they are told it increases their risk of heart disease. RLS symptoms occur at rest, lying, or sitting and include creeping, crawling, tingling, pulling, twitching, tearing, aching, throbbing, prickling, or grabbing sensations in the calves. The result is an uncontrollable urge to relieve the uncomfortable sensation by moving the legs. Some research shows that people with RLS may suffer from iron deficiency. However, Professor Nordlander who came up with this theory admits that only about 15% of the RLS clinical population appears to have peripheral iron deficiency (serum ferritin < 50 mcg/l). Yet, 20-50mcg/l is said to be the optimal level for ferritin and indicates you don’t have iron overload and taking iron could increase iron toxicity in the body. Professor Nordlander won’t drop his theory and says there can exist an iron deficiency in the tissues in spite of normal serum iron. At his insistence, researchers are investigating low iron levels in the brain! Unfortunately, you can cause more problems by taking unnecessary iron. People do have to be careful of self-medicating with iron. Investigators are also looking at dopamine because drugs that stimulate dopamine production may help the symptoms of RLS. And, of course, they are investigating the role genes play in RLS. Nowhere in mainstream literature do I see magnesium being studied in the diagnosis and treatment of RLS. Yet, every day we hear stories of people who take ReMag and say their RLS symptoms disappear – which means they were suffering from magnesium deficiency. It just makes sense to investigate magnesium deficiency as a cause of RLS before taking iron or dopamine drugs or just giving up and blaming your genes.

He earned his Board Certification as an Orthopedic Clinical Specialist through the American Physical Therapy Association. He is a Credentialed Instructor from the North American Mulligan Concept Teachers Association Eric has an extensive sports medicine background, has worked closely with physicians from the Steadman-Hawkins Clinic in Denver, and has treated athletes from youth, to Olympic, and professional levels. He is a published author and was a key-note speaker at the International Mulligan Conference in 2009. Other clinical interests include treatments for Restless Legs Syndrome and utilizing the Motion Guidance system to improve visual feedback in rehabilitation. He is a  Physical Therapist at Select Physical Therapy in Castle Rock, Colorado. Restless legs syndrome (RLS) is a sensorimotor disorder with an age-dependent prevalence of up to 10% in the general population above 65 years of age. Affected individuals suffer from uncomfortable sensations and an urge to move in the lower limbs that occurs mainly in resting situations during the evening or at night. Moving the legs or walking leads to an improvement of symptoms. Concomitantly, patients report sleep disturbances with consequences such as reduced daytime functioning. We conducted a genome-wide association study (GWA) for RLS in 922 cases and 1,526 controls (using 301,406 SNPs) followed by a replication of 76 candidate SNPs in 3,935 cases and 5,754 controls, all of European ancestry. Herein, we identified six RLS susceptibility loci of genome-wide significance, two of them novel: an intergenic region on chromosome 2p14 (rs6747972, P = 9.03 x 10(-11), OR = 1.23) and a locus on 16q12.1 (rs3104767, P = 9.4 x 10(-19), OR = 1.35) in a linkage disequilibrium block of 140 kb containing the 59-end of TOX3 and the adjacent non-coding RNA BC034767.

Background: Restless legs syndrome (RLS) is associated with common variants in three intronic and intergenic regions in MEIS1, BTBD9, and MAP2K5/LBXCOR1 on chromosomes 2p, 6p and 15q.Methods: Our study investigated these variants in 649 RLS patients and 1230 controls from the Czech Republic (290 cases and 450 controls), Austria (269 cases and 611 controls) and Finland (90 cases and 169 controls). Ten single nucleotide polymorphisms (SNPs) within the three genomic regions were selected according to the results of previous genome-wide scans. Samples were genotyped using Sequenom platforms.Results: We replicated associations for all loci in the combined samples set (rs2300478 in MEIS1, p = 1.26×10-5, odds ratio (OR) = 1.47, rs3923809 in BTBD9, p = 4.11×10-5, OR = 1.58 and rs6494696 in MAP2K5/LBXCOR1, p = 0.04764, OR = 1.27). Analysing only familial cases against all controls, all three loci were significantly associated. Using sporadic cases only, we could confirm the association only with BTBD9.Conclusion: Our study shows that variants in these three loci confer consistent disease risks in patients of European descent. Among the known loci, BTBD9 seems to be the most consistent in its effect on RLS across populations and is also most independent of familial clustering.
