Epi 12Welcome back to The Cutting Edge Podcast with host Mel featuring Dr. Omar E. Beidas, MD, FACS (plastic surgeon) and Ashlyn Douglass Barnes, LCSW (therapist and post weight loss patient). In this episode of our Fear Series, we tackle one of the biggest hurdles before body contouring and plastic surgery after major weight loss: the fear of the unknown and the fear of regret. You'll hear what regret actually looks like in real life (hint: it's far less common than the fear of it), how unrealistic expectations can fuel anxiety, why “post op blues” can happen even when everything is going well, and what you can do before surgery to feel grounded, prepared, and confident. We also talk about how people treat you differently in a smaller body, how confidence can impact relationships, and why choosing the right procedure (and the right provider) matters if you want results you won't second guess. If you're researching tummy tuck, skin removal surgery, body contouring after weight loss, 360 lipo, breast lift or breast augmentation, or you're navigating the mental side of transformation, this one is for you.Watch nextIf you're new here, start the Fear Series from the beginning and then work your way through the topics in order for the full picture. What you'll learn in this episode• Why fear of regret is more common than actual regret• The top reasons people feel regret after plastic surgery (and how to avoid them)• How swelling, pain, recovery fatigue, and needing help can trigger “blues”• How to set realistic expectations for healing and final results• Why non invasive “skin tightening” options usually do not solve loose skin after major weight loss• How weight stability affects long term results• How body changes can shift attention, confidence, and relationships• How to choose the right procedure and avoid “internet validation” traps• What board certification and proper training really mean in aestheticsChapters0:18 Intro and Fear Series focus: fear of the unknown1:07 Fear of regret vs actual regret3:07 What plastic surgery means after major weight loss4:19 Post op blues, swelling, recovery expectations6:19 Surgery day realities and “the flip” discussion12:21 Picking the wrong procedure and online trends14:20 Beware unqualified aesthetic providers15:18 Loose skin facts after major weight loss18:04 Fear of social judgment and relationship changes21:46 How people treat you differently after weight loss27:47 Chasing perfection vs unrealistic expectations31:27 The viral “RGB implant” moment and internet misinformation35:26 Implant sizing and why many people wish they went bigger39:53 Weight stability, timing, and how surgeons “read” readiness44:49 Fear of regret and decision making stylesFeatured expertsDr. Omar E. When should you attempt to save the root filled molar that everyone else thinks is doomed? What are the key steps to safely remove, treat, and replant a tooth without causing fractures or resorption? And how do you manage patient expectations and post-op care to maximize success? In this episode, Dr. Samuel Kratchman and Dr. Shivakar join Jaz to explore intentional tooth replantation—a procedure that rarely gets the spotlight but can completely change treatment options for challenging cases. They cover everything from case selection and imaging, to managing crowns and fragile teeth, to simple tools and techniques that make this procedure predictable and accessible. They also dive into patient communication, consent, and how to include this procedure as part of your everyday dental armamentarium, giving you the confidence to consider it when the right case comes along. https://youtu.be/SjJTzbJ_AXs Watch PDP256 on YouTube Key Takeaways: Intentional replantation is a viable alternative to extraction. The success rate of intentional replantation is documented at 88-89%. Patient education is crucial for successful treatment outcomes. The periodontal ligament must be kept moist during the procedure. Imaging is essential for understanding tooth anatomy before replantation. The procedure can be performed atraumatically with proper technique. Replantation can be a last chance for teeth that are difficult to replace with implants. A mindset shift is needed in dentistry to prioritize saving natural teeth. Apical infections are often linked to the root tip and surrounding tissue. A good coronal seal is essential before any restorative work. Common complications include ankylosis and resorption. Inflammation can aid in the extraction process by serving the ligament. Post-operative care is vital for successful recovery. Implant placement is one thing, but making sure those implants survive years of chewing forces is another. In this episode, Dr. Grant Stucki welcomes back Florida general dentist Dr. Vic Martel to dive deep into occlusion from an implant perspective. In today's conversation, Dr. Martel explains why implant occlusion is often even more critical than occlusion on natural teeth, how uneven load sharing leads to problems like loose screws, chipped porcelain, bone loss, and even loss of integration, and why surgeons are ultimately at the mercy of their restoring dentists' occlusal skills. He discusses practical guidelines for single posterior implants, angled implants in full-arch cases, maxillary second molars, anterior implants, and zirconia hybrids, as well as how to check occlusion correctly in centric, excursions, and with the patient both reclined and upright. Dr. Martel also shares how to manage bruxers, when to splint restorations, why screws need to be replaced after loosening, and ways frequent maintenance and clear patient instructions can prevent many complications before they start. Tune in now!Key Points From This Episode:How Dr. Martel defines occlusion as “load sharing” in the context of implant dentistry. Why occlusion is critical for natural teeth and how uneven load sharing leads to failures.Hear how to set occlusion for an implant using shim stock in normal closure and clench.Explore how to manage axial and non-axial loading on angled implants in full-arch cases.Steps for checking occlusion and why screw-retained restorations have become his standard.Discover common reasons for loose screws and why the screw often needs to be replaced.Steps for navigating microtrauma on implants, patients who brux, and splint restorations.Typical reasons why certain implants fail and guidelines for anterior implant occlusion.Find out how to distribute forces across implant-supported anterior bridges to avoid load.Unpack how the shift from acrylic hybrids to zirconia has changed occlusal schemes. Learn about the complications from occlusion and why it should be checked frequently. What to tell patients about soreness, looseness, night guards, and regular maintenance. Dr. Martel's final takeaways for preventing occlusion-related implant problems.Links Mentioned in Today's Episode:Dr. Victor Martel on LinkedIn — https://www.linkedin.com/in/victor-martel-dmd-fagd-91431922/ Dr. Victor Martel on Instagram — https://www.instagram.com/drvicmartel/ Dr. Victor Martel Email Address — vicmartel@gmail.com Dr. Victor Martel Phone Number — 561 602 7222 Martel Academy — https://martelacademy.com/ Everyday Oral Surgery Website — https://www.everydayoralsurgery.com/ Everyday Oral Surgery on Instagram — https://www.instagram.com/everydayoralsurgery/ Everyday Oral Surgery on Facebook — https://www.facebook.com/EverydayOralSurgery/Dr. Grant Stucki Email — grantstucki@gmail.comDr. Grant Stucki Phone — 720-441-6059

In this episode of Wellness at the Speed of Light, Dr. Stefano Sinicropi speaks with Dr. Galen Buckwalter about the evolving science of brain-computer interfaces and the deeply human stories behind innovation. Dr. Buckwalter shares his experience following a spinal cord injury that led him to become one of the earliest participants in a brain implant research program. The conversation explores how brain-computer interface research seeks to translate neural signals into movement through thought, and why this work represents an important area of inquiry in neuroscience and rehabilitation research. Rather than focusing solely on technology, the episode centers on the emotional realities of living with paralysis and the meaning of participating in research that may benefit future generations. Together, Dr. Sinicropi and Dr. Buckwalter discuss the intersection of science, resilience, and purpose. They reflect on the psychological and emotional dimensions of scientific breakthroughs, the uncertainty inherent in experimental research, and what motivates individuals to contribute to progress even when personal outcomes are unknown. The discussion highlights how purpose can emerge from adversity and why human experience remains central to medical innovation. This episode is relevant for healthcare professionals, researchers, patients, caregivers, and listeners interested in neuroscience, brain health, recovery, and integrative approaches to wellness. It offers an educational, thoughtful look at the future of brain-body research while honoring the personal strength and perspective that drive discovery forward.

The ENG implant has data placing it as the most reversible, hormonal contraceptive agent available with a typical use failure rate of 0.05%. Unfavorable bleeding patterns, such as frequent or prolonged bleeding, affect approximately 40% of ENG implant users within the first 3 months but typically improve over time. Nonetheless, it is the main reason for patient discontinuation. In the past, various medications have shown to have at least some short-term reduction in bothersome breakthrough bleeding (BTB). These include doxycycline, ethinyl estradiol (EE), mefenamic acid, combined oral contraceptives (COCs), short term tamoxifen, norethindrone, and ulipristal acetate. In this episode, we will summarize a new RCT (AJOG, released as epub on Jan 7, 2026) which describes the use of TXA for ENG related BTB. Did it work? Listen in for details.1. What if a haunting didn't involve ghosts — but the lingering smell of carnival food? This episode of The Box of Oddities opens with an unsettling sensory mystery tied to a long-demolished amusement park, then plunges into one of the most stubborn and controversial archaeological puzzles of modern times: the tridactyl mummies of Peru. Discovered near the Nazca region, these small humanoid mummies feature three fingers, three toes, elongated skulls, and internal anatomy that does not appear to be the result of a simple hoax. CT scans and MRIs show articulated skeletons with no apparent signs of assembly. Carbon dating places them roughly 1,700–1,800 years old. DNA testing reveals material consistent with known Earth life — alongside a troubling percentage classified as unknown. Some specimens even appear to contain metallic implants made from rare alloys, positioned as if intentionally placed during life. One reportedly shows signs of a fetus, suggesting reproduction rather than fabrication. Scientists remain cautious. Skeptics remain vocal. And yet, after years of imaging and analysis, these bodies stubbornly resist tidy explanations. They may not be aliens — but they also may not be anything science has fully named yet. Then, in classic Box fashion, the episode pivots from the inexplicable to the unexpectedly hopeful. Meet the real-world heroes you probably didn't expect: trained landmine-detecting rats. These remarkable animals are saving lives across former war zones by sniffing out explosives buried decades ago. One rat in particular, Ronan, has broken world records and helped return deadly land to safe use — proving that sometimes the strangest solutions are also the most effective. From phantom fairground smells to unresolved biological mysteries to rats quietly changing the world, this episode is a reminder that the universe is weird, complicated, and occasionally wonderful — whether we understand it or not. Brain-computer interfaces are moving out of the lab and into real medical use. In this episode of The Michael Shermer Show, Michael Shermer talks with Dr. Matt Angle, founder and CEO of Paradromics, a neurotechnology company developing one of the most advanced high-data-rate brain implants in the world, similar to Neuralink. These devices record activity from individual neurons, making it possible to restore speech in people with paralysis, reconnect the brain to external devices, and potentially treat chronic pain and neurological disorders with far greater precision than existing approaches. Angle explains why progress in neuroscience has been limited not by biology, but by data—how much information we can actually read from the brain, and how fast. He describes how patients who can no longer speak may soon communicate fluently using only brain signals, why invasive implants can sometimes be safer than long-term drug treatments, and what it takes to bring a brain implant through FDA approval and into the clinic. The conversation also touches on the larger questions raised by this technology, including autonomy, consciousness, and what happens when the boundary between brain and machine begins to blur. Matt Angle is the Founder and CEO of Paradromics, a neurotechnology leader developing the world's most advanced and clinically viable brain-computer interface (BCI) platform—bridging human thought and digital capability. Paradromics' BCI platform records brain activity with unmatched precision, capturing data at the level of individual neurons. This advanced technology enables the decoding of vast amounts of brain data, opening the door to next-generation treatments for paralysis, chronic pain, addiction, mental health conditions, and more. With the power of AI, this platform has the potential to radically shift how healthcare providers approach some of the most challenging medical conditions. Angle earned his PhD in Neuroscience from the University of Heidelberg, followed by postdoctoral research at Stanford University. Paradromics engineered its first clinical product, the Connexus® BCI, received two FDA Breakthrough Device Designations, and performed the first-in-human neural recording in May 2025. The company is now preparing to launch a clinical trial in early 2026, pending regulatory approval.

Double board-certified obstetrician-gynecologist and reproductive endocrinology and infertility specialist Erica Bove discusses her article, "Why do high-quality IVF embryos fail." Erica explores the heartbreaking reality of recurrent implantation failure, challenging the assumption that it is a rare phenomenon. She analyzes the physical factors often missed in standard studies, such as chronic endometritis, silent endometriosis, and adenomyosis, while explaining why the endometrial receptivity analysis is losing favor in the field. The conversation also highlights a critical, often overlooked variable: the role of stress and mental health in fertility outcomes. Erica advocates for a holistic approach that combines rigorous medical guidelines with emotional support to help patients navigate the complex journey of IVF. Note: This episodes contains discussion and language of a sexual nature and may not be appropriate for all audiences. In this episode of Marrow Masters, we sit down with Dr. Christian Nelson, a psychologist at Memorial Sloan Kettering Cancer Center and an expert in psycho-oncology, to address a deeply personal yet often overlooked topic: male sexual dysfunction following cancer treatment. Together, we explore how treatments can impact not only physical function but emotional well-being, identity, and relationships. We start by acknowledging that sexual dysfunction is one of the most common side effects of cancer treatment, second only to fatigue. Yet, it's rarely discussed. Dr. Nelson emphasizes the importance of normalizing the conversation and encouraging patients to raise the issue with their treatment teams. He walks us through the emotional toll that erectile dysfunction can take on men, noting how it strikes at the core of masculinity and can lead to increased depression, frustration, and a general sense of brokenness. He stresses that it's not just about what happens in the bedroom—erectile issues can spill over into all aspects of a man's life, including his relationship with his partner.Dr. Nelson outlines a range of treatment options, from well-known medications like Viagra to lesser-known but effective methods like penile injections and implants. He breaks down the fear around these options, especially injections, and highlights how pain levels are often far lower than anticipated. Beyond physical treatments, we discuss the emotional and relational work that's often required. One key issue is avoidance—men avoiding sexual situations due to performance anxiety, which can snowball into long-term distance and silence between partners. Dr. Nelson makes it clear: the real risk isn't failure, it's not trying. We also dive into how couples can redefine intimacy. Many men associate sex solely with penetration, while their partners often value closeness and emotional connection more. Dr. Nelson advocates for expanding the sexual repertoire and restoring non-sexual forms of affection, which can be just as meaningful. We talk about the impact of testosterone—how its depletion can lower libido and cause men to unknowingly withdraw from their partners—and how testosterone replacement may be a viable option for some, depending on cancer type and treatment history. As roles shift from caregiver back to partner post-treatment, Dr. Nelson stresses the importance of open communication. He urges couples to work toward understanding each other's perspectives, not convincing each other. He shares an "aha" moment involving a couple who waited five years before seeking help, only to reconnect within three sessions after simply opening the lines of communication. The takeaway: don't wait.We close by pointing listeners toward additional resources, including certified sex therapists and specialized urologists, and Dr. Nelson highlights two key professional directories: SSTAR and AASECT. We're reminded that even the simplest questions—like whether it's okay to kiss your partner—deserve answers. It's on all of us, both patients and providers, to make room for these conversations.More:Episode with Dr. Flores: https://marrowmasters.simplecast.com/episodes/mens-sexual-health-gvhdEpisode with Dr. El Jawahri: https://marrowmasters.simplecast.com/episodes/dr-el-jawahriSSTAR (Society for Sex Therapy and Research) – https://sstarnet.orgAASECT (American Association of Sexuality Educators, Counselors and Therapists) – https://www.aasect.orgThanks to our Season 19 sponsors, Incyte and Sanofi.https://incyte.com/https://www.sanofi.com/en00:00 - Introduction to Season 19 and Dr. Christian Nelson  01:16 - Normalizing Conversations on Sexual Dysfunction  04:50 - Emotional Toll of Erectile Dysfunction  07:06 - Treatments: Pills, Injections, and Implants  09:03 - Avoidance and Anxiety in Sexual Relationships  12:17 - Expanding the Definition of Intimacy  16:43 - Role of Testosterone in Sexual Health  20:05 - Shifting from Caregiver to Partner  22:17 - Resources and Where to Get Help  26:29 - A Patient Story: Five Years of Silence  28:07 - Closing Thoughts and Resources   National Bone Marrow Transplant Link - (800) LINK-BMT, or (800) 546-5268.nbmtLINK Website: https://www.nbmtlink.org/nbmtLINK Facebook Page: https://www.facebook.com/nbmtLINKFollow the nbmtLINK on Instagram! https://www.instagram.com/nbmtlink/The nbmtLINK YouTube Page can be found by clicking here.To participate in the GVHD Mosaic, click here: https://amp.livemosaics.com/gvhd Hosted by Simplecast, an AdsWizz company. This week we welcome back the dynamic duo of digital dentistry, Blake Roney and Patrick Dewey from S.I.N. 360 (https://sin360.us/) (Simplicity • Innovation • Nanotechnology) for a massive deep dive into implants, photogrammetry, and the ever-evolving world of full-arch workflows. Since their last visit in 2022, Blake has gone from new kid on the block to full-blown Exocad (https://exocad.com/) educator, and Patrick breaks down how S.I.N. has doubled down on innovation across implants, CAD/CAM, and photogrammetry. The guys walk us through the new Versalis implant line (https://sin360.us/versalis/), why one connection for all indications is a big deal for labs, and how S.I.N. is pushing efficiencies for high-volume, complex full-arch clinicians. Then comes the star of the show: the MicronMapper (https://sin360.us/micronmapper/)—a lighter, faster, more accurate photogrammetry system that doesn't just capture implants but verifies manufacturing accuracy, scans soft tissue, and reduces surgical guesswork. Blake breaks down real-world accuracy numbers, what RMSE actually means, why intraoral scanners aren't cutting it for full-arch, and how FitCheck is saving labs from misfires, wasted zirconia, and bad days. They also reveal the newest frontier: Tissue Mapper, a photogrammetry-based, scanner-free workflow that pulls bite, tissue, and implant data without fiducials or messy post-op scans. It's nerdy, innovative, and ridiculously cool for the high-volume teams ready to level up. If you love accuracy, numbers, full-arch workflows, or just really good dental tech nerding — this is your episode. Happy Holidays from Ivoclar! While Part 1 introduced Esquivel's framework of space, volume, and time, this episode focuses on the next frontier—provisionalization, emergence-profile design, and the E-B-C concept that defines the biologic foundations of aesthetic implant therapy. Dr. Esquivel begins by demystifying the provisionalization workflow for anterior implants—whether immediate or delayed. He walks listeners step by step through decision-making factors such as ridge healing, tissue phenotype, and the importance of minimizing the number of disconnections to protect the fragile mucosal seal. He stresses that efficiency in implant dentistry isn't about finishing fast—it's about achieving predictability in the fewest biologically respectful appointments. The discussion then turns to the team-based dynamics between the restorative dentist and the surgeon. Dr. Esquivel details how he coordinates cases digitally, sending STL files and mock-ups for joint planning before the first incision. His six-phase system—Acquisition, Blueprint, Confirmation, Design, Execution, and Follow-up—illustrates how restorative-driven implant dentistry must begin with facial and prosthetic design, not with a drill guide. From there, the episode dives into the chairside artistry of immediate provisionalization. Using a pre-milled shell and temporary cylinder, Dr. Esquivel describes how to "pick up" a provisional and sculpt the subgingival contours in real time. But as he emphasizes, the true challenge lies not in fabrication—but in understanding how material additions and reductions influence soft-tissue behavior. This insight leads to one of the most clinically valuable segments of the episode: Dr. Esquivel's E-B-C concept, an evolution of the classic critical- and subcritical-contour framework. The E-Zone (Esthetic Zone), roughly the first millimeter below the gingival zenith, governs margin stability and the visible pink–white transition. The B-Zone (Biologic or Boundary Zone) supports the mucosal seal and should remain concave to create space for healthy soft tissue. The C-Zone (Crestal Zone), the most apical 1–2 mm, must be slim and biologically quiet to preserve the crestal bone. Dr. Esquivel explains how even subtle over-contouring in these zones can trigger recession or bone loss, while proper shaping promotes long-term harmony between implant and tissue. Using tactile language, he describes his preference for pear-shaped acrylic burs to sculpt the concavity of the B-zone, and how digital workflows now allow clinicians to model these micro-contours virtually before any material is placed. The conversation culminates in a reflection on philosophy as much as technique. Dr. Esquivel urges clinicians to focus on mastering the basics—aesthetic symmetry, biologic respect, and patience—rather than chasing novelty. "An implant is not a tooth," he reminds us. "Our goal is to protect the bone, guide the tissue, and understand what we can't see." Listeners will leave this episode with a detailed understanding of: How to design, print, or fabricate custom provisionals that shape the soft tissue biologically The dimensional logic behind the E-B-C zones and how each governs tissue stability Why limiting abutment disconnections preserves the mucosal seal How digital and analog workflows can be integrated seamlessly And the mindset shift required to see implants not as prosthetic devices, but as biologic restorations Part 2 is both a masterclass in subgingival design and a meditation on restraint. Dr. Esquivel reminds us that excellence in implant aesthetics doesn't come from doing more—it comes from doing less, but with absolute intention.

Injection Molding PDF Email me dr.melissaseibert@gmail.com  Join Elevated GP: www.theelevatedgp.com Follow @dental_digest_podcast Instagram Follow @dr.melissa_seibert on Instagram In this episode of Dental Digest, Dr. Melissa Seibert sits down with prosthodontist and educator Dr. Jonathan Esquivel for part one of an enlightening two-part series that explores the biologic and aesthetic complexities of implant restoration. Known for his meticulous approach and his evidence-based framework of space, volume, and time, Dr. Esquivel brings clarity to one of dentistry's most challenging frontiers: the anterior aesthetic zone. They begin by examining why restoring a single anterior implant is often considered the ultimate test of restorative mastery. Dr. Esquivel explains that implants behave fundamentally differently from natural roots—they lack the periodontal ligament and its vascular support—making soft tissue far less forgiving. Even slight changes in tissue contour or bone volume can compromise long-term success. He introduces his four cornerstones for implant predictability: luck (biologic variability), three-dimensional positioning, emergence-profile design, and patient maintenance, emphasizing that precision and patient education are inseparable. The conversation then turns to three-dimensional implant positioning and the role of reverse-engineered planning. Dr. Esquivel insists that every implant must begin with the end in mind—by first visualizing where the tooth should be. He details how ideal placement, roughly 4 mm apical to the planned incisal edge and aligned toward the cingulum, forms the foundation for natural emergence and long-term stability. But true success, he notes, depends equally on interdisciplinary collaboration—sometimes requiring orthodontic repositioning or periodontal modification before an implant is ever placed. Dr. Esquivel and Dr. Seibert next tackle the aesthetic challenge of adjacent implants and the pursuit of symmetry. Perfect papillae between centrals are notoriously difficult to maintain, and Dr. Esquivel discusses techniques—from soft-tissue grafting to orthodontic extrusion—to preserve harmony between the pink and the white. He stresses that treatment planning is as much about patient selection and expectation management as it is about surgical technique: "The hardest part isn't the implant—it's finding a patient willing to go through the process." The discussion deepens into the critical role of soft tissue in achieving lasting aesthetics. Dr. Esquivel explains his distinction between margin-preservation therapies (maintaining existing tissue contours) and margin-re-establishment therapies (rebuilding lost dimensions). He makes a compelling case that most anterior implants benefit from connective-tissue grafting, since thicker tissue phenotypes promote margin stability, mask restorative materials, and protect against recession. From there, the episode explores ridge dimensional changes after extraction—why bone and soft-tissue collapse are inevitable without intervention, and how provisional restorations can slow this process. Dr. Esquivel cautions against relying on removable flippers, which may accelerate resorption, and instead advocates for properly designed Essix retainers or fixed provisionals that maintain space without transmitting occlusal pressure. His guiding principle: space, volume, and time—allowing tissue to heal in an environment that supports both biological integrity and aesthetic form. The conversation culminates with an in-depth reflection on the biological and prosthetic purpose of provisionalization. Drawing on insights from Dr. Todd Schoenbaum, Dr. Esquivel reframes the provisional phase not as optional, but as biologically mandatory. The provisional guides tissue healing, allows for gradual adaptation, and provides essential information for the ceramist—ensuring that the final crown replicates the ideal emergence contours established chairside. As Dr. Esquivel puts it, "Dentists should take credit not for the white, but for the pink—the transition zone we've designed and stabilized." By the end of this episode, listeners will gain a detailed understanding of: How to apply Dr. Esquivel's space–volume–time model to aesthetic implant planning The biologic reasoning behind connective-tissue grafting and margin-preservation therapies Why provisionalization is central to both soft-tissue stability and prosthetic accuracy How to communicate healing timelines and realistic expectations to patients Part one of this series is both technically rigorous and refreshingly human—an exploration of how aesthetic implant success lies not in speed or convenience, but in respecting biology, sequencing, and time.
