Merhabalar... Yazıma başlamadan önce birkaç şeyden bahsetmek istiyorum. Acil asistanlığımın yarısından fazlasını geride bıraktım. Acilci.net yazarlığına başladıktan sonra asistanlık hayatım ve tuttuğum nöbetler benim için daha bir anlamlı oldu.  Yazımı hazırlamaya başladığımda ufak bir stres yaşasam dahi bu süreç beni nöbete giderken motive ediyor, ‘'farklı bir şey öğrenebilir miyim'' sorusunu taze tutuyor. Yazılarım umarım birilerine faydalı oluyordur lakin pek tabi yazmak en çok faydayı bana sağlıyor. Konumuza dönecek olursak değerli uzmanlarımız, hocalarımız bazen tecrübe ettikleri nadir vakalardan bize bahsederler. Nörojenik akciğer ödemi de daha önce aramızda bahsi geçen ancak tanık olmadığım bir klinikti.  Blog yazarı gibi bir girişten sonra yine bir kırmızı alan nöbetimde; 35 yaşında kadın hasta acil servise solunum sıkıntısı şikayeti ile 112 ambulansı tarafından getiriliyor. Hasta yakınlarından alınan anamnezde yakınları, hastanın şiddetli bir baş ağrısı olduğunu ifade ettikten yarım saat sonra solunum sıkıntısı olduğunu tarifliyorlar. Hastanın herhangi bir travma öyküsü, ilaç veya madde kullanımının olmadığını ifade ediyorlar. Bilinen hastalığı ve geçirilmiş cerrahi öyküsü olmayan hastayı genel durumunun kötü görünmesi nedeniyle kırmızı alanda güvenlik çemberine alıp değerlendiriyoruz. Geliş vitalleri; tansiyon 160/120 mm Hg, nabız:135 atım/dk, satürasyon: %80, solunum sayısı: 45 solunum/dk olarak ölçülüyor. Hastanın GKS 9 (M3V4S2) olarak değerlendiriliyor. Bilinç konfüze, non oryante, non koopere. Pupiller izokorik, DIR:+/+, IIR: +/+, belirgin lateralizan bulgusu yok. Oskültasyonla bilateral tüm zonlarda raller duyuluyor ve ağzından pembe köpükler geldiği görülüyor. Batın muayenesi rahat defans veya rebaund alınmıyor. Tüm ekstremitelerinde nabızları palpabl, alt ekstremitekerinde çap veya ısı farkı yok, pretibial ödem yok. Hastanın genel durumunun kötüleşmesi ve hava yolunu koruyamayacağı şüphesinin varlığı nedeni ile, hasta ilaç aracılı entübe edildi. Entübasyon sırasında herhangi bir aspirat görülmedi. Ancak entübasyon sırası ve sonrasında hastanın yoğun pembe sekresyonları nedeni ile yoğun aspirasyon gereksinimi duyuldu. Hastanın kliniği ve yapılan fizik muayenesi sonucu hastada akciğer ödemi tanısı düşünüldü. Hastanın Yapılan Görüntülemeleri: Sırasıyla : Toraks BT Kesiti-Kontrastsız Beyin BT Kesiti- Beyin BT Anjio Kesiti Kardiyoloji tarafından kardiyak patoloji ya da akciğer ödemine neden olabilecek ek yapısal anomali tespit edilmeyen hastada anevrizma rüptürü sonucu subaraknoid kanamaya bağlı gelişen nörojenik akciğer ödemi düşünüldü ve hasta yoğun bakım takibine alındı. Nörojenik Akciğer Ödemi Dispne acil servise başvuru nedenlerinin başında yer almaktadır. Dispneye yaklaşımda görülme sıklığı ilk sıralarda yer almasa dahi, kardiyojenik olmayan akciğer ödemi ayırıcı tanılardan biridir. Kardiyojenik olmayan akciğer ödeminin alt başlıklarından birini bu yazımda konu alacağım. Ayırıcı tanı kısmında kardiyojenik olmayan akciğer ödeminin diğer alt başlıklarından bahsedeceğim. Nörojenik akciğer ödemi (NAÖ), genellikle merkezi nörolojik bir hasardan kısa bir süre sonra ortaya çıkan akut pulmoner ödem olarak tanımlanır.1 Etiyolojinin, kardiyopulmoner disfonksiyonla sonuçlanan katekolamin artışı olduğu düşünülmektedir. Omurilik yaralanması, subaraknoid kanama (SAK), travmatik beyin hasarı , kafa içi kanama, status epileptikus, menenjit ve subdural kanama dahil olmak üzere sayısız  santral sinir sistemi olayı bu sendromla ilişkilendirilmiştir. 2 Tarihçe NAÖ sendromu uzun süredir bilinmektedir. 1903'te Harvey Williams Cushing, merkezi sinir sistemi hasarı ile hemodinamik disfonksiyon arasındaki bağlantıyı tanımladı3; ve 1908'de WT Shanahan, epileptik nöbetlerin bir komplikasyonu olarak 11 akut pulmoner ödem vakası bildirdi. 4 Francois Moutier, I. Dünya Savaşı'nda başlarından vurulmuş askerler arasında ani başlayan akciğer ödemini tarif etti.

Herkese merhaba, ülkemizde günlük covid-19 vaka sayılarının 60.000’i geçtiği bu tam kapanma döneminde tartışmaların devam ettiği bir konu ile yeniden beraberiz. Doktorların boyunlarından düşürmediği, tıp öğrencilerinin gözleri gibi baktığı fizik muayenenin enstrümanı niteliğindeki bir cihaz olan stetoskop acaba artık gereksiz bir alet mi? Hatta kendimize ve hastalarımıza yaptığımız bir kötülük mü?  Stetoskop bir dinleme cihazı olsa da aslında "toraksa bakmak" anlamına gelir. Yunanca bir kelime olan stetoskop; stetos (göğüs) ve skopein (bakmak) kelimelerinin birleşmesinden oluşur. Tarihçesine bakacak olursak, 1816 yılında Rene Theophile Hyacinthe Laennec, kağıdı rulo yaparak bir ucunu hastanın kalbine diğer ucunu kulağına dayayıp kalp seslerini dinledi. In this issue of the series Czech Conteporary Art in Berlin we would like to introduce Julia Gryboś (*1988, PL) and Barbora Zentková (*1986, SK). They are an artistic duo, working together since 2010. They met during their studies at Ostrava Faculty of Arts and continued with their studies already as a duo at the faculty of Arts in Brno. They made themselves known to wider audiences mainly in the Czech Republic and in Slovakia, where they were awarded the Oskár Čepan Award for young artists in 2016. In 2019 they completed the Berlin Sessions Residency and a residency at ZK/U, the Center for Art and Urbanistics in Berlin. These were the impulse for them to stay in Berlin. In their work, they examine the impact of the neoliberal chase after non-stop productivity. They are interested in the symptoms of the diseased society. One of the returning motifs in their work is the topic of tiredness and its depiction. They create site specific objects and environments, often out of old metal constructions and some textiles. Trnka s Kočnerom na nahrávke hovoria aj o tom, ako zobrali peniaze za niektoré kauzy, pričom Trnka má tieto peniaze uložené práve u Kočnera. Dobroslav Trnka napriek tomu zostáva prokurátorom. Generálny prokurátor Jaromír Čižnár aspoň zbavil Dobroslava Trnku mlčanlivosti, aby mohol vypovedať. Budete počuť aj politické reakcie na túto kauzu. Šéfka komisie Marcela Kosová však naznačuje, že súdna rada s týmito sudcami neurobí nič – lebo to vyšetruje polícia.
