Why does a potentially deadly condition that affects 1 in 7 adults fly under the radar, undetected and undiscussed? Join us on this episode of Quality Matters as we explore Chronic Kidney Disease (CKD), a silent killer that's finally coming into the limelight of quality improvement. Our conversation begins with Dr. Ben Oldfield, a primary care physician, who sheds light on the startling prevalence and hidden complexities of CKD. Why is it so hard to detect, even with simple blood and urine tests? And why does this condition, despite its extensive impact, often get lost in the shuffle of busy health care visits? We'll unpack the challenges of diagnosing and managing CKD, exploring the emotional weight the diagnosis carries for patients and the hurdles clinicians face in communicating about it. Discover why CKD is a “loaded term” and how we can shift the narrative from fear to empowerment. But that's not all. We'll also tackle the big picture: How CKD fits into the broader landscape of cardiovascular, kidney, and metabolic (CKM) disorders. Could a holistic approach to these interconnected conditions revolutionize patient care? The conversation ends with Ben's thoughtful observations on how classic literature, particularly epic poetry, helps doctors make sense of health care's emotional complexities, connect with patients and find empowerment in the face of life and death.Join us as we explore how we can move CKD from the sidelines to the front lines of quality improvement, and how collaborative, longitudinal care can make a real difference.Key Quote:“Thinking about chronic kidney disease is a longitudinal process. It's only half the story to make the diagnosis in a snapshot in time. What's the follow up like? How are we doing with the patient over time? Because oftentimes quality measures can look at things in a snapshot in time. Chronic kidney disease really begs us to think more longitudinally.”“Benjamin Oldfield, MDTime Stamps:(02:54) The Silent Nature of CKD(07:05) Quality Measurement and CKD(11:08) The Link Between CKD and Cardiovascular Kidney Metabolic syndrome(13:06) CKD and CKM as Quality Priorities (15:45) Narrative Medicine and Personal InsightsLinks:NCQA Kidney Health ToolkitConnect with Benjamin Oldfield

HEALTH NEWS · Personalized nutrition more effective than general diet advice, study finds · Vitamin K1 may improve insulin sensitivity and blood sugar levels for pre-diabetics · Frequent salting of food increases the risk of stomach cancer · A 30-year US study links ultra-processed food to higher risk of early death · Mechanism outlined by which inadequate vitamin E can cause brain damage · About 90% of US adults are on the way to heart disease, study suggests Personalized nutrition more effective than general diet advice, study finds King's College London, May 8, 2024 (Medical Xpress) Personalized nutrition approaches can improve several key health metrics, including lower weight, lower cholesterol, better mood, improved gut health, lower heart disease risk, and better metabolic health. Improvements for those following personalized guidance were also greater in some areas than for those following current, generalized government advice. In this study, researchers wanted to see if a personalized diet plan, tailored to an individual's biology, lifestyle, and health history, would have a greater impact than generic nutrition advice such as avoiding red meat. 347 Americans took part in the study, with researchers comparing the effects of following an 18-week personalized program to generic US government-issued nutrition advice. While both groups improved their health overall, participants on the personalized diet plan lost more weight than the control group and lowered their triglyceride levels more—decreasing their risk of heart disease. Participants following the personalized diet plan were also twice as likely to report improved mood, twice as likely to feel less hungry, and more than four times more likely to report better sleep quality and energy levels compared with the control group. Vitamin K1 may improve insulin sensitivity and blood sugar levels for pre-diabetics Ahvaz Jundishapur University (Iran), May 6, 2024 (Nutraingredients) Supplements containing vitamin K1 (phylloquinone) may improve glycemic status and insulin sensitivity for premenopausal and pre-diabetic women, says a new study. “To the best of our knowledge, the present study is the first one that investigated the effects of vitamin K1 supplementation on the glycemic status and insulin sensitivity via different forms of [osteocalcin] in pre-diabetic women,” wrote the researchers in the European Journal of Clinical Nutrition . Osteocalcin (cOC) is a vitamin K-dependent protein. Without adequate vitamin K, the osteocalcin remains inactive (uncarboxylated osteocalcin, ucOC), and thus not effective. The new study used phylloquinone at a daily dose of 1,000 micrograms for four weeks. Eighty-two pre-diabetic and pre-menopausal women participated in the study and were randomly assigned to either the vitamin K1 group or placebo. Results showed that cOC levels increased as a result of K1 supplementation, while ucOC levels decreased, compared with placebo. In addition, results of the two hour post-oral glucose tolerance test showed improvements in both glucose and insulin levels in the K1 group, compared with placebo. Data from the insulin sensitivity index (ISI) showed increases in the K1 group, they added. Frequent salting of food increases the risk of stomach cancer Medical University of Vienna (Austria), May 8, 2024 (Eurekalert) In Asian countries, where high-salt foods are popular, the link between high salt consumption and stomach cancer has already been proven. A long-term study by MedUni Vienna has now shown for the first time that this risk is also reflected in the cancer statistics in Europe. As the analysis recently published in the specialist journal Gastric Cancer shows, people who frequently add salt to their food are around 40 per cent more likely to develop stomach cancer than those who do not use the salt shaker at the table. Data from more than 470,000 adults from the large-scale British cohort study "UK-Biobank" was analysed. This revealed that people who said they always or frequently added salt to their food were 39 per cent more likely to develop stomach cancer over an observation period of around 11 years than those who never or rarely added an extra pinch of salt to their food. In the list of the most common types of cancer worldwide, stomach cancer is in fifth place. The risk of this tumour disease increases with age, but the latest statistics paint a worrying picture of an increase in adults under the age of 50. "Our research shows the connection between the frequency of added salt and stomach cancer in Western countries too," emphasises first author Selma Kronsteiner-Gicevic. "With our study, we want to raise awareness of the negative effects of extremely high salt consumption and provide a basis for measures to prevent stomach cancer," summarises study leader Tilman Kühn. A 30-year US study links ultra-processed food to higher risk of early death Harvard University, May 8, 2024 (Medical Xpress) Higher consumption of most ultra-processed foods is linked to a slightly higher risk of death, with ready-to-eat meat, poultry, and seafood based products, sugary drinks, dairy based desserts, and highly processed breakfast foods showing the strongest associations, finds a 30-year US study in The BMJ today. Mounting evidence links ultra-processed foods to higher risks of obesity, heart disease, diabetes and bowel cancer, but few long term studies have examined links to all cause and cause specific deaths, especially due to cancer. To address this knowledge gap, researchers tracked the long-term health of 74,563 female registered nurses from 11 US states in the Nurses' Health Study and 39,501 male health professionals from all 50 US states in the Health Professionals Follow-up Study with no history of cancer, cardiovascular diseases, or diabetes at study enrollment. Compared with participants in the lowest quarter of ultra-processed food intake (average 3 servings per day), those in the highest quarter (average 7 servings per day) had a 4% higher risk of total deaths and a 9% higher risk of other deaths, including an 8% higher risk of neurodegenerative deaths. The association between ultra-processed food intake and death varied across specific food groups, with meat, poultry, and seafood-based ready-to-eat products showing the strongest and most consistent associations, followed by sugar-sweetened and artificially-sweetened beverages, dairy-based desserts, and ultra-processed breakfast food. Mechanism outlined by which inadequate vitamin E can cause brain damage Oregon State University, May 3, 2024 (Daily Science) Researchers at Oregon State University have discovered how vitamin E deficiency may cause neurological damage by interrupting a supply line of specific nutrients and robbing the brain of the "building blocks" it needs to maintain neuronal health. The research showed that zebrafish fed a diet deficient in vitamin E throughout their life had about 30 percent lower levels of DHA-PC, which is a part of the cellular membrane in every brain cell, or neuron. Other recent studies have also concluded that low levels of DHA-PC in the blood plasma of humans is a biomarker than can predict a higher risk of Alzheimer's disease. Just as important, the new research studied the level of compounds called "lyso PLs," which are nutrients needed for getting DHA into the brain, and serve as building blocks that aid in membrane repair. It showed the lyso PLs are an average of 60 percent lower in fish with a vitamin E deficient diet. The year-old zebrafish used in this study, and the deficient levels of vitamin E they were given, are equivalent to humans eating a low vitamin E diet for a lifetime. In the United States, 96 percent of adult women and 90 percent of men do not receive adequate levels of vitamin E in their diet. "This research showed that vitamin E is needed to prevent a dramatic loss of a critically important molecule in the brain, and helps explain why vitamin E is needed for brain health," said Maret Traber, at OSU and lead author on this research. About 90% of US adults are on the way to heart disease, study suggests Brigham and Women's Hospital & Harvard Medical School, May 8, 2024 (Medical Xpress) Nine of 10 American adults are in the early, middle or late stages of a syndrome that leads to heart disease, a new report finds, and almost 10% have the disease already. "Poor cardiovascular, kidney, and metabolic health is widespread among the U.S. population," concludes a team led by Dr. Muthiah Vaduganathan of Brigham and Women's Hospital and Harvard Medical School in Boston. Researchers looked specifically at rates of what the American Heart Association has dubbed cardiovascular, kidney and metabolic (CKM) syndrome Among adults age 20 or older, only 10.6% did not have some level of CKM syndrome. About 26% fell into the early stage 1 category, meaning they were gaining dangerous levels of body fat. Nearly half (49%) of adults fell into stage 2 CVM syndrome, and 5.4% were stage 3. According to the study, 9.2% of adults were in stage 4, with full-blown heart disease and, in some cases, failed kidneys. The bottom line: "Almost 90% of US adults met criteria for CKM syndrome (stage 1 or higher) and 15% met criteria for advanced stages, neither of which improved between 2011 and 2020," according to the Boston researchers.

Host: Javed Butler, MD, MBA, MPH Guest: Chiadi Ndumele, M.D., Ph.D., M.H.S. For the first time, the American Heart Association (AHA) has identified cardiovascular-kidney-metabolic (CKM) syndrome, which reflects a strong overlap between heart disease, kidney disease, and obesity. The major clinical consequence of CKM syndrome is multi-organ dysfunction with a particularly high incidence and burden of cardiovascular disease (CVD), and the result of this is premature morbidity and mortality. So as a result, recognizing CKM syndrome is the first step in earlier diagnosis and treatment. Dive into this episode with Dr. Javed Butler who's joined by Dr. Chiadi Ndumele, Associate Professor of Medicine and the Director of Obesity and Cardiometabolic Research at Johns Hopkins University.

Abstract: When faced with a DNA double strand break, cells activate an elaborate signaling cascade called the DNA damage response to protect genomic integrity. To identify novel factors that modulate the DNA damage response to DNA double strand breaks, we performed an epistatic miniarray profile analysis of Mec1 and Rad53, two essential kinases that coordinate the DNA damage response in budding yeast. Through this analysis, we discovered a genetic interaction between the kinase module (CKM) of the Mediator of transcription and Rad53. We find that all four subunits of the CKM, as well as CKM's kinase activity are critical for cell cycle re-entry following a DNA break, whereas the core Mediator subunits are dispensable. Notably, CKM mutants do not impair DNA repair by homologous recombination or confer sensitivity to DNA damaging reagents, suggesting that CKM specifically impinges on DNA damage signaling. In support of this, we find that Rad53 and CKM physically interact in response to DNA damage. Following the induction of a DNA break, CKM is a critical regulator of global transcription inhibition. In addition to this global effect, we illustrate that CKM functions locally at DNA breaks together with the core Mediator. In the absence of catalytically active CKM, the CKM-Mediator complexes at DNA breaks are replaced by RNAPII. Taken together, our results reveal a previously uncharacterized role for CKM in the DNA damage response. Copy rights belong to original authors. Ale my dziś, rzekomo, żyjemy w czasach transparencji. Czyli: DECORO prowadzi do SPREZZATURY a SPREZZATURA rodzi GRAZIE. DECORO bez SPREZZATURY to wyrywanie sobie zębów. SPREZZATURA bez DECORO to pewna klęska.
Hosť podcastu Roman „Flebo“ Juhás, hudobný dramaturg a dídžej Rozprávame sa s hudobným dramaturgom a bývalým dídžejom Romanom Flebom Juhásom, ktorému vyšla kniha Dídžej – historky z kultových bratislavských barov. Vraciame sa do 80. a 90.rokov, kedy Flebo počúval na Ö3 hitparádu a zapisoval si pikošky o interpretoch. Sníval o práci moderátora...a napokon sa mu to splnilo. Ako sa kedysi robievali diskotékarské skúšky? Ako sa robili moderované diskotéky? Kde zháňali dídžeji informácie za socializmu? Vtedy neboli bežne dostupné pesničky – ako ich dídžeji zháňali. Prichystali som hosťovi malý test: podľa názvov troch skladieb musí identifikovať interpreta a pridať o ňom pikošku.

On today's episode of Jimmy Makes Science Simple on the LLVLC Show, Jimmy peels back the layers on another stack of cutting edge keto research. “The side effects of the drugs they give people with Alzheimer's can make quality of life difficult.” Jimmy Moore In this first clip Jimmy shares from a February 23, 2021 study published in the journal Alzheimer's Research & Therapy entitled “Randomized crossover trial of a modified ketogenic diet in Alzheimer's disease”: https://alzres.biomedcentral.com/articles/10.1186/s13195-021-00783-x New Zealand researchers at Waikato Hospital in Hamilton, New Zealand used a modified ketogenic diet over a 12-week period on patients with a diagnosed case of Alzheimer's disease. Primary outcomes included basic cognitive tests and quality-of-life improvements. Secondary outcomes included cardiovascular risk marker changes. The ketogenic diet beat the low-fat one across the board! Next up Jimmy shares from an April 9, 2021 study published in the Journal of Diabetes Science and Technology entitled “Feasibility of Continuous Ketone Monitoring in Subcutaneous Tissue using a Ketone Sensor”: https://journals.sagepub.com/doi/full/10.1177/19322968211008185 At long last, a continuous ketone monitor (CKM) very similar to how a continuous glucose monitor (CGM) works is currently undergoing research and testing before coming to the market. Just like the CGM, the new CKM will not be measuring blood but rather interstitial fluid which is generally excepted as a proxy for blood readings. In the first-ever human trial of continuous ketone monitoring, they tested 12 healthy individuals eating a low-carb diet and wearing 3 CKM‘s over a 14-day period and comparing those readings to a finger prick blood ketone test (using the Precision Xtra device from Abbott). Watch this video to find out just how accurate the CKM is and a couple of questions that Jimmy has about this new technology. To wrap up this episode, Jimmy shares from a March 12, 2021 study published in the journal Medicine & Science in Sports & Exercise entitled “The Effects of Exercise on Beta-Hydroxybutyrate Concentrations over a 36-h Fast: A Randomized Crossover Study”: https://pubmed.ncbi.nlm.nih.gov/33731648

A lot of companies shy away from hiring veterans because they don’t understand the value that they may bring to the table. Amazon, Google, Microsoft, Cisco & Walgreens have programs in full force but the majority of startups have yet to really get on board. The diversity in thought that comes from hiring a vet may be the competitive edge that your startup needs to propel your business ahead of your competition. This show is proudly sponsored by Vidoori Today’s Quote: "We hire military veterans because they make great employees. They bring proven technical and leadership skills. They understand teamwork, and they're adaptable. Bottom line, hiring veterans is good for business." Randall L. Stephenson - CEO of AT&T Guest Bio: Brian Erickson serves as the Vice President, Strategy and Solutions at Vidoori, Inc. On July 4th, Sharie and Clayton had the privilege of interviewing their good friend, Conner Moon, about what it is like to live out his faith as a medic in the military. The Kings met Conner when he was a college student. He felt called to ministry and has always loved adventure. He served CKM as a medic on the Crossroads Summer Camp staff and felt called to ministry. Years later, God fulfilled this desire when he joined the US Military. Had a chat (a few weeks ago) with Cliff Mander, CEO of CKM Sports Management. CKM is one of the leading hockey specific player agencies on the planet, using their technology and data analytics to empower their clients, while also using holistic player improvement - meaning that they focus on improving players’ psychological strength, physical ability and education. So naturally we discussed the state of player development and where that is trending - but we also talked shop and what it was like starting an agency in a competitive market filled with long established brands and businesses. Thanks for the talk Cliff, learned a lot - keep up the good work! Why do we exist? CP violation is an integral part of this question as its understanding is crucial to explain the matter-antimatter asymmetry observed in our universe. Several experiments were designed and carried out to precisely measure CP violation, especially in the B meson system where large asymmetries where predicted and found. With Belle II and LHCb, two new experiments are going to improve the existing measurements. Belle II will be based on the very successful Belle experiment at the KEKB collider, currently holding the world record on luminosity with 2.11×10³⁴ cm⁻²2s⁻¹. The B meson system has a very rich decay topology and many of theses decay modes and their CP asymmetry parameters have already been measured at Belle. The most famous decay channel, B0⟶J/ψK0s, poses very tight constraints on sin 2φ₁ but leaves a twofold ambiguity on the actual value of the angle φ₁ in the CKM triangle. The decay mode B0⟶D*-D*+K0s, while experimentally much more challenging, offers the unique possibility to also extract cos2φ₁ and thus resolve this ambiguity. In the first chapters of this thesis we present the principle of this measurement and the results for the branching fraction and the time-dependent CP violation parameters of B0⟶D*-D*+K0s decays. These results are obtained from the final data sample of the Belle experiment containing 772 million BBbar pairs collected at the Υ(4S) resonance with the Belle detector at the KEKB asymmetric-energy e+e- collider. We obtain the branching fraction BR(B0⟶D*-D*+K0s) = (5.35+0.35−0.34(stat) ± 0.57(syst))×10⁻³, which is in agreement with the current world average. In a 3 parameter fit sensitive to cos2φ₁, we extract the currently most precise values for the CP parameters Jc/J0 = 0.37 ± 0.10(stat) ± 0.02(syst), (2Js1/J0) sin(2φ₁) = 0.30 ± 0.16(stat) ± 0.03(syst), (2Js2/J0) cos(2φ₁) = 0.16 ± 0.16(stat) ± 0.03(syst). This allows us to exclude a negative value for cos2φ₁ at a 85% confidence under the assumption that that (2Js2/J0) is positive. Finally, we describe the implementation of the vertex detector geometry for the upcoming Belle II experiment. The upgrade to Belle aims to increase the integrated luminosity by a factor of 50 and will receive, among other upgrades, a completely new vertex detector. To produce simulated events, a precise description of the sensor geometry and material budget is needed.

In the state of the art the Standard Model is the best gauge theory describing interactions among elementary particles. It comprises all of the fundamental interactions in nature except gravitation. Its predictions have been experimentally tested to a high level of accuracy. However, it is not considered to be the fundamental theory of gauge interactions. It contains a lot of arbitrary parameters. It can not predict the fermion masses and fails to explain the smallness of neutrino masses which have been observed by recent experiments. It contains no gauge bosons that can mediate nucleon decays via baryon and lepton number violating process, which are needed to explain the baryon asymmetry in our universe. Furthermore, CP violation has to be introduced into the CKM and MNS matrices by hand. The shortcomings of the Standard Model can be solved in the framework of grand unified gauge theories (GUTs) which have greater degrees of freedom. GUT's which have truly one coupling constant are based on gauge groups that contain the Standard Model as a subgroup. There are a limited number of such gauge groups. SO(10) is a fully symmetric gauge group that has two outstanding features: It unifies all the known gauge interactions under a single coupling strength and classifies all the known fermions of a family under a single spinor. In this work, we will study SO(10) grand unification in its full extent by using different explicit matrix representations which exhibit the structure of SO(10) in a very transparent way. Our approach consists mainly of two stages: We will derive the explicit expressions of the mass-eigenvalues and mass-eigenstates of the physical gauge bosons from a mass squared-matrix that contains all the information about the mixing parameters among the gauge fields and the phases which are sources for CP violation. In the light of this analysis, we will derive the explicit expressions for the interaction Lagrangians of the charged currents, the neutral currents and the charged and colored currents in SO(10). We will present explicit expressions of the vector and axial-vector couplings of the two neutral currents in SO(10). We will show how the baryon, lepton and baryon minus lepton number violating processes and their explicit CP violating phases are accommodated in the SO(10) theory. The Higgs potential that we use to implement in the Higgs mechanism will be constructed in a most general fashion through a careful study of the Higgs fields of SO(10), where we give special emphasis on illustrating the explicit matrix representation of these Higgs fields. The potential part of the Higgs Lagrangian will give us the properties of the minimum of the vacuum, and the kinetic part will give us the mass-squared matrix of the gauge bosons via spontaneous symmetry breakdown. The same Higgs multiplets will be coupled to fermions through a democratic Yukawa matrix. Thereby, we will derive explicit expressions for the fermion masses of the third family including Majorana and Dirac masses for neutrinos. We will introduce a flavor-eigenbasis for neutrinos and find the mass-eigenstates and mass-eigenvalues of the neutrinos. Explicit expressions for CP violation in the neutrino sector will be obtained. In the second stage of our work, we will evaluate all the above mentioned quantities. We will compare our results with those of the Standard Model like the W and Z masses and the vector and axial-vector coupling of the NC current and the fermion masses of the third family. In addition, we will present the values of the physical quantities that are not present in the Standard Model like the masses of new gauge bosons, the vector and axial-vector couplings of a new NC current, the masses of a light left-handed and a heavier right-neutrino, the values of various mixing parameters and CP phases etc. The input values required for these evaluations will be acquired mainly from two sources: First, we will determine the vacuum expectation values and the coupling strengths of gauge interactions given by the SO(10) theory in so far as possible through studying the mass scales in SO(10) in the framework of coupling unification. Complementarily, we will determine the vacuum expectation values and their phases by adjusting them to the masses of the known gauge bosons and fermions below the Fermi scale which are accurately measured and known. We will be able to predict more than 67 parameters with an input of 7 vacuum expectation values, 5 angles, 1 gauge coupling and 1 Yukawa coupling.
