In today's episode, we offer invaluable insights on how to effectively partner with leadership to make a compelling and effective case for physician well-being and organizational transformation.   Dr. Brooke Buckley is an expert in navigating organizational systems and advocating for wellness and culture change within healthcare settings. She is currently the Chief Medical Officer of Henry Ford Wyandotte Hospital. She is responsible for the leadership of the hospital's physicians. Dr Buckley is a general surgeon by training.  She also serves on the Committee of Clinical Leadership for the American Hospital Association and on the American Medical Political Action Committee Board for the American Medical Association. Dr. Jessie Mahoney is an experienced physician wellness leader and organizational leader. She led local and regional wellness initiatives at The Permanente Medical Group from 2003 to 2020. She was also a Site Chief of Pediatrics. She is well-versed in addressing the challenges of balancing operational pressures with the needs of physicians. Dr. Buckley emphasizes the importance of understanding healthcare systems, their rules, and structures, and how to work within them to advocate for change. She also outlines strategies for speaking the language of executive leadership to bring wellness to the forefront of the conversation—whether that's improving retention rates, reducing turnover, or addressing the impact of burnout on patient care. Translating wellness needs into the impact on finances, turnover, productivity, and quality of care can help you make a compelling case for change. Two tips we share in this episode: Be Strategic and “Not a Victim.” Advocacy for wellness requires a shift in mindset. Instead of focusing on fairness or what's right or wrong, it's crucial to be strategic, understand what leadership “needs”.  Speak to what they care about. Bring the impact of physician unwellness on hospital finances, quality of care, cost of care, physician availability, and patient access to the forefront of discussions. Grieve and Move Forward. Burnout is not an excuse to disengage. It can be an invitation to assume leadership and advocate for change.  Physicians have the most profound understanding of the issues that need fixing and should be the ones to lead the charge.  Before jumping in, it's important to acknowledge and allow yourself and others to grieve the loss of what they thought medicine would be like. As parents, children, physicians, friends, and members of society. We were also trained in professional schools to have unreasonable expectations of ourselves (think residency!)   The expectations of our patients and our employers are also often unreasonable and/or unrealistic. We don't have clarity or confidence to stand tall in what our job actually is. We default to it must be our job and we twist ourselves in knots trying to make it all work, please others, be enough, and get external validation. When we can't do it all. we fall into blame, shame and guilt.  We feel like an imposter.   Others shouldn't have unreasonable expectations of us. During this episode with Dr. Jessie Mahoney, we talk about how we can change the stories we tell ourselves, retrain our nervous system and create more mindful practices that can help us improve our lives. In addition, we address the following topics: Choosing to change yourself can create shifts in your relationship. Understanding the griefor depression each of you may feel and understanding. Getting clear about things out of your control that can't be changed. Retraining our own nervous system first. Parasympathetic nervous system is focused on rest and digest. Sympathetic nervous system is running from the tiger. We can feel bewildered and respond or react at different speeds. In adrenal depletion we just react and are not pausing. Fight or flight response releases cortisol. Self compassion releases dopamine and oxytocin which feels good. Learn different breathing practices to see what works for you.  This can help you be less reactive. Restorative yoga can be very helpful. We have held onto stories that may be misinterpretations. How can we create positive stories and remember the things you fell in love with and focus on how you're the same. Understand your neurodiverse relationship is going to look different than other relationships. Create a positive story about the way your partner shows you love.  Reframing stories can be so helpful. Tell the story that we're both doing the best we can.  Judge the effort, not just the outcome! Change the way you think and then determine if you need to make a change. Determine what your non-negotiables are and after doing that you may realize that you are no longer compatible. When you're in a state of constant frustration you can't be fully invested. Focus on both of your individual strengths. What is your partner good at that you don't like and vice versa? Generosity of thinking and assuming good intent can help you get out of a tug of war with your partner. Create a toolbox for moving through the world more smoothly. Mindfulness is acceptance (even if you don't like what is happening). You can have acceptance and still be angry or have other feelings and this can empowering. What you practice grows. Be patient with yourself and wour partners.  Our partners may have a different nervous system pace. Allow yourself and your partner to be angry. Accepting and not liking and still listening is so important. 69% of our problems in our relationships are perpetual and 31% are solvable. Ask yourself-“What would love do?” You don't always have to understand. Self compassion is a life raft. Mindfulness can be a learned way of being that creates spaciousness in a healer's life.   In this episode we share the concept of building a scaffold of mindfulness as taught in Dr. Liang's Mindful Physician course.  From her journey through her own burnout and cancer experience, she could not wait for the system to change.  Her suffering became a catalyst for practicing and teaching mindfulness as a way to build her own “mindful scaffold of support.”   Reflection Questions offered in this episode: What could the practice of medicine feel like if you had a scaffold of mindfulness supporting you? What if you embodied mindfulness, and in practicing it noticed spaciousness in your life? How could you be the change that you want to see in healthcare? The Mindful Healthcare Collective is now a vibrant community of over 2100 healthcare professionals from across the country and the world who are coming together to heal themselves, learn, grow and support one another through the challenges of working in healthcare at this moment. Dr Jessie Mahoney is a coach for physicians and parents. She is also a Pediatrician and a yoga and mindfulness instructor. She was a pediatrician at Kaiser Permanente for over 20 years and has been a leader in the Physician Wellness movement her entire career. She is graduate of Dartmouth College and UCSF Medical School. Jessie leads wellness, coaching and yoga retreats and has her own coaching practice, Pause and Presence Coaching. She primarily coaches those who are successful on the surface yet struggle underneath. She helps her clients to struggle less and guides them to find more calm and ease, and create a more intentional life.
