Before the CMSC discussion, Marini runs through a roundup of recent neurology news, including approval of at-home initiation dosing for lecanemab-irmb (Leqembi; Eisai, Biogen), fast track designation for remlifanserin (Acadia Pharmaceuticals) in Alzheimer disease psychosis, approval of the oral PCSK9 inhibitor enlicitide (Lipfendra; Merck), and phase 3 data on tavapadon (AbbVie) for Parkinson disease showing a lower rate of impulse control disorders than traditional dopamine agonists.The pharmacists then turn to CMSC sessions on off-label and evolving areas of MS care. Es ist heiß, der Sommer ist da. Deshalb gehe ich mit „Marinics Freiheit“ in die Sommerpause. Doch bevor es soweit ist, will ich unbedingt noch über Sachsen-Anhalt und die Landtagswahlen sprechen, die da Anfang September stattfinden. Noch sieben Wochen also. Und später im September wählen dann auch noch Mecklenburg-Vorpommern und Berlin. Damit könnte Ostdeutschland in weiten Teilen Regierungsland der AfD werden. Viele Medien glauben, dass sich das nicht mehr verhindern lässt und malen für September schwarz. Das stört mich. Denn ist das die richtige Herangehensweise? Bestärken Berichte, wie schlimm alles wird und ist, diesen Trend nicht erst? Wenn Medien auf den Osten gucken, ist häufig von Jammer-Ossis die Rede, von abgehängten Regionen und Nazis, die die Demokratie nie gelernt haben. Stereotype, die die AfD für sich zu nutzen weiß. Denn die Partei ist dann besonders stark, wenn es viel zu klagen gibt. Seit Jahren fordern Politologen und Ost-Experten: Es braucht eine positive Erzählung vom Osten, um die Geschichte vom kaputten Osten zu durchbrechen. Das sehe ich auch so. Spotlight deshalb auf die, die die Zivilgesellschaft in Ostdeutschland weltoffen gestalten wollen! Erzählen wir doch lieber mal, wo es in Sachsen-Anhalt oder Mecklenburg-Vorpommern so richtig gut läuft mit der Demokratie-Bewegung. Kein Augen-Verschließen vor offenkundigen Problemen, sondern positive Verstärkung für die gute Sache. Als Gast hab ich mir den bisher jüngsten Gast in „Marinics Freiheit“ eingeladen: Max Schneller. 2025 ist der heute 17-Jährige mit einem Video viral gegangen, in dem er Querdenkern in seiner Heimatstadt Bad Dürrenberg die Meinung gesagt hat. Ich will, dass Ihr seine Geschichte hört und Euch ein besseres Bild machen könnt von den Menschen, die im Moment engagiert sind, statt einfach nur Umfragen abzunicken. Er erzählt mir, weshalb die etablierten Parteien anders auftreten sollten, welchen Preis er für die Haltung bezahlt und was er sich für die nächsten Wochen wünscht von den Journalisten, die über die Wahlen berichten. Also Leute, es ist so weit: In dieser Folge nehme ich mir die Freiheit, mal allein und solo über ein Thema und mit Euch zu sprechen. Ich mache etwas, was ich eigentlich ungern mache: Über Menschen reden, die mich empören. Lieber ignoriere ich sie. In diesem Fall geht es um eine Empörung, die viral gibt und zu einer Empörungsdebatte um einen „Witz“ von Dieter Nuhr wurde, den er auf Kosten von getöteten Frauen reißt, die Opfer von Femiziden wurden. Nuhr sieht das natürlich ganz anders und habe es so auch nicht gemeint, wie es jetzt viele verstehen. Dennoch, die Lacher im Saal waren ihm sicher. Die Kritik kam später, weil viele Menschen diese Geschmacklosigkeit nicht amüsiert, sondern total aufgeregt hat: Frauen aber auch Männer. Und das hat dann wieder die empört, die eigentlich nur lachen wollten. Ich will in dieser Folge mit Euch darüber reden, wieso diese Debatte so krasse und heftige Reaktionen und Emotionen auslöst, wer sich hier und warum überhaupt angegriffen fühlt und was das alles mit dem Zustand unserer Gesellschaft und dem Feminismus in Deutschland zu tun hat. Diese Woche gab es viel Talk um Superreiche. Zum einen war da der Tech-Milliardär Elon Musk, der mit dem Börsengang seines Raumfahrt- und Technologie-Unternehmens SpaceX so viel Geld eingesammelt hat, dass er jetzt der erste Dollar-Billionär der Welt ist. Viele sagen, dass er damit der Allmacht noch ein Stück nähergekommen ist. Zumindest der Allmacht auf Erden; die Raketen, die er ins All schickt, stürzen regelmäßig ab. Doch nicht nur in den USA nimmt Musk immer häufiger Einfluss auf die ganz reale Politik. Und das kann er, weil er superreich ist. Es entsteht ein Kult um die Schamlosigkeit der Superreichen. Ich frage mich, ob demokratische Gesellschaften dem noch etwas entgegenzusetzen haben, oder ob am Ende alle sein wollen wie die Superreichen. Wie sie sind, das zeigte eine andere superreiche Familie. Das Ehepaar Ivanka Trump und Jared Kushner will die albanische Insel Sazan zu einem schicken Hotel-Ressort ausbauen. Das wollen ziemlich viele Albaner aber nicht und gehen deswegen Tag um Tag auf die Straße in der Landeshauptstadt Tirana. Weil in der Lagune vor der Insel Flamingos rasten, sind die Proteste als „Flamingo-Proteste“ weltweit in die Schlagzeilen geraten. Die Proteste werden bewundert, doch können die Demonstranten noch etwas bewegen gegen die Macht der Ultrareichen? Die albanische Regierung unter Rama schwärmt weiter über das Projekt und wie es Albanien voranbringen könnte - eine unheilige Allianz zwischen Oligarchen und Regierungen? Zum gemeinsamen Nachdenken habe ich Klimaaktivistin Luisa Neubauer eingeladen, die zuletzt gesagt hat, dass es "eine andere Art des Aktivismus" braucht, um zu gewinnen. Mich interessiert: Welche könnte das sein? Und kann man gegen die Macht der Superreichen überhaupt noch etwas ausrichten? Dextromethorphan/bupropion offers a novel NMDA-based mechanism for treating agitation in Alzheimer dementia, but its clinical effect size is modest, and it carries meaningful safety considerations—particularly around the bupropion component in elderly patients. Like all pharmacologic options in this space, it remains a later-line choice after nonpharmacologic interventions have been exhausted, and medication reconciliation is critical given its interaction potential with memantine and CYP2D6 inhibitors.2. Anti-amyloid therapies are imperfect but not ready to be written off. The April 2026 Cochrane review drew significant attention with its conclusion that anti-amyloid monoclonal antibodies produce only trivial cognitive benefits, but its findings are limited by the inclusion of older, withdrawn agents, heterogeneous trial populations, and an 18-month time horizon that may be too short to capture the full trajectory of benefit.3. The pharmacist's role in anti-amyloid therapy goes well beyond dispensing. As illustrated by Sobhanian's practice at the University of Maryland, clinical pharmacists embedded in neurology clinics play a critical role in patient selection, expectation-setting, ARIA counseling, and informed decision-making for patients considering anti-amyloid therapy—a complex, high-stakes treatment decision that these patients and their caregivers should never be navigating alone.
