Welcome to the Sustainable Clinical Medicine Podcast! In this episode, Dr. Sarah Smith is joined by Sue Peters—a healthcare improvement leader with a background in audiology and extensive experience in quality improvement for clinical teams. Sue and Sarah dive into the day-to-day challenges clinicians face, from the dreaded in-basket workload to the complexities of team communication and patient care coordination. Together, they explore practical strategies to streamline processes, clarify roles within clinical teams, and leverage everyone's unique skills to create more efficient and sustainable practice environments. Sue shares actionable tips on offloading non-physician work, optimizing EMRs, and effectively onboarding patients—all while ensuring that both providers and patients benefit from these improvements. Plus, they chat about breaking old habits, embracing true teamwork, and even finding ways to take things out of everyone's overloaded "backpack." Here are 3 key takeaways from this episode: Embrace Team-Based Care: Don't try to shoulder all the work alone. By clearly defining roles and empowering every member of the care team—from RNs and LPNs to MOAs—practices can improve patient access, reduce wait times, and allow clinicians to focus on what matters most. Optimize Your EMR Processes: Standardizing how we populate and use EMR data not only improves patient care, but also makes it much easier to delegate and manage population health. Consistency in documentation enables actionable reporting—and lets the right team member handle the right task. Involve Everyone in Change: Successful quality improvement requires all voices at the table, not just physicians. When team members participate in designing solutions, they're more engaged, invested, and open to new ways of working. Sue Peters Bio: Sue Peters is a Healthcare Improvement Leader working with Primary Care and Specialty Teams to achieve true team-based care through Quality Improvement. Imagine a team where every member has a critical role in patient care and is working to top of scope, allowing you to focus on the work for which you are highly trained - It is possible. -------------- Would you like to view a transcript of this episode? Health Innovation Hub Ireland (HIHI), the HSE and the Irish College of GPs, announced the winners of HIHI GreenTech in Healthcare. The Health Innovation Hub Ireland (HIHI) is a joint government initiative of the Department of Enterprise Trade and Employment and the Department of Health. Six innovative products and services have been identified, that promote environmental sustainability in both primary and secondary healthcare. The call was the first step in tackling the research and innovation gap in sustainable product pathways in the Irish health sector. Now, the winning products and services in green healthcare, will be trialled. This allows Irish health providers to pilot, refine and adapt sustainable solutions effectively, before wider implementation. GreenTech in Healthcare The HSE, Enterprise Ireland and Health Innovation Hub Ireland (HIHI), will lead collaborations across Irish healthcare sites to deliver trialling of the winners on the ground. Welcoming the announcement Minister for Enterprise, Tourism and Employment Peter Burke said: "The Health Innovation Hub Ireland GreenTech initiative is a clear demonstration of how innovation can support our national climate goals while delivering practical benefits to the health service. Through Health Innovation Hub Ireland - a flagship collaboration between my Department and the Department of Health - we are driving forward sustainable innovation, supporting enterprise, and enabling real-world testing of green solutions in our health system." This initiative sets a strong precedent for future collaborations in green healthcare innovation." Dr Philip Crowley National Director HSE Climate and Global Health said: "We are delighted to be involved in this recent Greentech in healthcare call and initiative with Health Innovation Hub Ireland and the Irish College of GPs. In keeping with the HSE Climate Action Strategy and our work to create greener models of healthcare, it is a great opportunity to encourage and support innovative services and products that are environmentally sustainable for healthcare environments." Dr Andrée Rochfort, Director of Quality Improvement at the Irish College of GPs said: "Protecting the finite resources of healthcare and minimizing the environmental impact of healthcare is important. The innovations identified by the GreenTech Initiative have potential to improve healthcare sustainability in a practical way. In our final episode we discuss severe ME. One in four people with Myalgic Encephalomyelitis (ME) are severely or very severely affected. Our guests for this episode are Dr Robin Kerr, a GP with over 10 years' experience in general practice; and patient advocate Helen Brownlie, who campaigns on severe ME through the UK charity which specifically supports people with severe ME, the 25% ME Group. To represent the voice of people living with severe ME, who are too unwell to speak for themselves, we have readings from international ME and Long Covid writing group, the Pillow Writers. These extracts are read by Laura Anne Collier, an #MEAction Scotland volunteer and Avril McLean, Senior Practitioner at Action for ME.ME is a complex multi system illness. Its impact can devastate a person's life. It is a biological condition not psychological as has been wrongly believed by GPs in the past. A useful analogy is that living with ME is like living with a smart phone that only charges to 20% overnight. So, if people use 5% to make breakfast and eat, 5% to wash, 5% to dress, 5% to commute they have used all their energy by 9am and have none left. For someone with severe ME meeting with a GP can use up all their energy for that day, week or month and the impact will show afterwards. Some of the key points from the discussion are listed below: What do GPs need to know about severe ME?Approach needs to be flexible so may need adaptation, eg. allow patient to submit questions to GP in advance, have an appointment by phone and/or a home visits.Risk assessment may be needed first to see benefits outweigh impact on person's health.Sensory considerations: low/no light, soft voice, slow speech, no chemicals.Harm can occur through over-exertion or pushing beyond person's limits.Cognitive behavioural therapy is not curative for ME, and Graded Exercise Therapy has been debunked as it can cause harm.Nutrition – people may be so unwell they are unable to digest food and may need specialist support to have adequate nutrition and hydration.Check new symptoms are not new comorbidities. Common examples are: Postural orthostatic tachycardia syndrome (PoTS) and Mast cell activation syndrome (MCAS).A range of comorbidities can result due to people being immobile.Coding of condition – Quality Improvement work for practicesData in practice needs to be accurate so that practice has a standardised approach to coding so people with ME can be identified and given the support and planning they need. Bladder cancer remains a prevalent disease with significant disparities in care, particularly in underserved populations. To address this, ACCC launched a national quality improvement initiative in three phases: 1. assessment and preparation, 2. action plan implementation, and 3. monitoring for continuous improvement. Dr. Samuel Washington, from the University of California, San Francisco, discussed the initiative's early findings, highlighting site-specific approaches. Sustainability was a key consideration, ensuring a balance between short-term wins and long-term impact. Action plans need to be both ambitious and practical to drive meaningful change. Continuing outcome monitoring will refine strategies, and the initiative could serve as a replication model in other clinical settings and cancer types. An important opportunity exists to expand multidisciplinary collaboration to include patient advocates and ensure the long-term integration of these improvements. Despite varied site objectives, all participants were committed to improving cancer care. This initiative highlights the power of structured, collaborative efforts to address disparities and enhance patient outcomes. "Now we have an opportunity to bring in patients and advocates… and expand this type of work in a structured way to other cancers and other institutions, other sites in an already national program." – Dr. Samuel Washington “The feasibility of the program overall was important, the fact that we could engage leadership champions at the clinic level and then multi-disciplinary team buy-in at each institution with this goal across oncology, urology, practices, radiation oncology, nursing, to all come together for each of these visits to help develop an action plan, it is something that I have not seen much of in recent years." – Dr. Samuel Washington Samuel L. So if you're, for example, part of an actuarial team, you may actually be getting PDFs, you may be getting spreadsheets, you may be getting access directly to a CSV file. You may get all these things on a one-off basis, and you have to make sense of all this data. The burden of cleaning and making use of this data falls on you." When asked "What matters to you?" many therapists responded that knowing they were delivering a quality service, improved their sense of fulfilment and well-being. Jo describes how she has learned to include everyone (service users and those delivering services) in discussions around 'how we got here, what the problems and potential solutions might be' - rather than feeling she has to go it alone and fix everything herself.
