They discuss how community can help with regaining perspective in difficult times. Dr Ashlea White is an early career addiction and adult psychiatrist with a varied career in public and private psychiatry, spanning tertiary and regional healthcare. In a previous life, she was a radiographer and now she is committed to building a flexible career in psychiatry that aligns with her many interests while also spending time with her growing family. Dr White continues to be passionate about gender equity and improving access to quality mental health treatment for all. She also sits on the RANZCP Member Wellbeing Subcommittee. Dr Roderick McKay is a psychiatrist whose current work focuses on working with older people and postgraduate education with psychiatry trainees and multidisciplinary courses. He is an Adjunct Associate Professor at the University of Notre Dame, Adjunct Senior Lecturer at the University of NSW. At RANZCP, he was the former bi-national chairs of both the Faculty of Psychiatry of Old Age and the Community Collaborative Committee. Our understanding of infectious disease and pathology stems from the 19th century with Robert Koch pioneering research. Dr Koch help to shaped modern microbiology and medicine when he identified the causative agents for anthrax and tuberculosis. Since that time, the number of infectious microorganisms that we know of that cause illness and diseases has increased exponentially. Thanks to modern day techniques, laboratories can test for these agents directly or indirectly and get an answer often within hours. As such, microbiology is on the verge of a new era. Instead of the principle of one test: one micro-organism, we are moving into the realm where one test can detect hundreds of potential pathogens and antimicrobial resistance genes in a single stool sample. The new test is called Metapanel. This includes 175 gene targets using metagenomics (shotgun gene sequencing) and is able to detect bacteria, viruses, helminths, protozoa, fungi, microsporidia. This information is collated and reported by a pathologist and advice given as to what is the implications of the result. The test is ideally suited for patients with chronic diarrhoea, inflammatory bowel disease and/or inflammatory bowel syndrome. Many conversations assume that LGBTQ people and religious people are two separate identity groups. There is a heteronormative religious culture that falsely claims that queer people can't be religious or spiritual. Some queer people absolutely need to walk away from religion in order to heal and become whole, but others are seeking a spiritual path that is different from what they grew up with. Dr. Nick Literski joins Jen on today's episode of In the Den to talk about how to rediscover a personal spirituality that is unique, expansive, and LGBTQ inclusive.Special Guest: Nicholas S. Literski, JD, PhD, is an Adjunct Senior Lecturer in Psychology at the California Institute of Integral Studies, and a professional spiritual guide. Welcome back to a NEW season of PT Elevated where we are broadening our topics to include more researchers but still focusing on topics that you can use in your clinic every day. This season we will have some of our speakers as guests who will be live in-person at the EIM Align Conference this August 26-28 in Dallas, Texas.  On our third episode of season 3, guest Mark Jones, BSc (Psyc), PT, M App Sc, who is an Adjunct Senior Lecturer in the University of South Australia with 35 years' experience teaching undergraduate and postgraduate physiotherapy joins us. Mark graduated from the University of Florida with a B.S. in Psychology and then completed his Physical Therapy studies at the University of Iowa. Having developed an interest in manual therapy Mark travelled to Australia to study Manipulative Physiotherapy and after completing his Graduate Diploma in 1985 under Geoff Maitland he completed his master's degree by Research in 1989. The title of his thesis was “Facilitating Students' Clinical Reasoning in Physiotherapy Education”. Mark has a special interest in biopsychosocial health care and the teaching and assessment of clinical reasoning in physiotherapy. He has conducted and supervised research in the areas of clinical reasoning and musculoskeletal physiotherapy with over 90 publications including three editions of the text “Clinical Reasoning in the Health Professions” and the text “Clinical Reasoning for Manual Therapists”. His latest text “Clinical Reasoning in Musculoskeletal Practice” was published in 2019. Mark has presented 30 Keynote Conference presentations and has taught professional development courses in clinical reasoning and musculoskeletal physiotherapy across 26 countries. They focus their discussion on the conversation of clinical decision making and reasoning that is the focus of Mark's Align Conference discussion. Mark has dove deep into the technique of clinical reasoning to grow and develop it. Here are some of the highlights: Mark gave a little insight on how he got interested in clinical reasoning - In 1985 when Mark said he was studying clinical reasoning it was not considered as much as it is now in physical therapy programs. He studied with Robert S. Maitland in Australia and was very intrigued by the thought process he taught without it being overt. Robert didn't have students think about their thinking, he gave them principles to follow. Mark came across the work Arthur S. Elstein, who wrote what probably still is considered the seminal study and paper in clinical reasoning in medicine, he immediately saw that applicably to physiotherapy. Everything that Arthur said about that process that people go through and how important your knowledge is to the success of that process, the concept of patterns, Mark connected that to what physical therapist did. He did his master's degree surrounded around that. Looking at educational strategies to strategize reasoning. I began to theorize what this would look like in physiotherapy - “I found out that good clinicians have always reasoned. It would be arrogant to suggest that it was not always here. The beginning of Physios, there was reasoning always inherent in what they did. We didn't think about our reasoning back then, it wasn't explicit. Maybe that was what I with others contributed to the process. I started theorizing and writing about it suggesting things as to what this would look like in physiotherapy. In our program we started thinking about what it should be because every program that teaches then must start making decisions on the process you want to facilitate and judgements you think are important.” Mark's Align Session Preview: “Physios are noisy.” Mark shares that he will share a podcast and a book at the conference in his lecturer that expands on this quote above. Mark's Clinical Pearl – “My Favorite definition of an expert is somebody who has a lot of experience, they know a lot, they do what they do well, and they can communicate their reasoning well. But they also know what they don't know. They do not have to have a supervisor look at them anymore, they can be on their own and recognize if they didn't understand information or know what a medicine was. They have that mediative congeniality and that is why they have become an expert; they have become aware of their limitations, and they act on it. Ongoing learning cliché is very true, to all of us and our students but it is also being honest. Today we will be speaking with EWP adjunct faculty Sophia Reinders, about her academic and personal journey from phenomenology, to Jungian depth psychology, expressive arts and eco-arts therapy to ecopsychology and evolutionary cosmology. She shares with us the life changing experience of discovering her earth soul at the depth and as the depth of the psyche, embedded in earth. This led her to a deep and passionate exploration of the intricate intertwinement of the human community with the more than human earth and all its life forms. The felt-sense of this encompassing perspective now finds expression in her teaching and her pedagogy as a scholar-practitioner. Drawing on the ecological imagination and guiding students in multimodal practices of creative embodiment, she encourages a shift away from a human-centered towards an earth-cherishing consciousness within which to consider and experience the realm of the human, be this within or beyond academic pursuits. Sophia asks us to think about our senses as a “palette of embodied languages”. Multi-modal sensory awareness, she says, deepens our experience of aliveness and discloses perception as participation with the earth. She sees participatory perception as a form of holistic, embodied understanding in which feeling, intuiting, imagining, thinking and visioning come together and enrich both academic and non-academic pursuits. His latest text “Clinical Reasoning in Musculoskeletal Practice” was published in 2019.And he has been on one of the major contributors to the development of clinical reasoning theory within MSK healthcare in the last 30 years, and we discuss some of his key work, including the seminal work with the late Louis Gifford and Ian Edwards (see Ian's work on clinical reasoning here here and here).So on this episode we talk about:  The development of clinical reasoning theory beyond mere diagnostic reasoning. Different aspects or strategies of clinical reasoning such as procedural, predictive and collaborative reasoning. The pursuit of a structural or pathological diagnosis. Mark's own clinical reasoning framework involving hypothesis categories. Clinical reasoning within a biopsychosocial framework. Diagnostic reasoning approaches such as hypothetico-deductive reasoning and pattern recognition. Clinical reasoning in novice compared to expert clinicians. And finally we discuss the role of metacognition to mitigating errors in reasoning. So it was truly and honour speaking with Mark. The label ‘pioneer' is probably over used, but in Mark's case it captures his status perfectly. In this episode of Psych Matters,  Members of the RANZCP Military, Veterans and Emergency Services Personnel Mental Health Network engage in a lively discussion of how to improve assessment of current serving members and ex-serving veterans of the Australian Defence Force. This includes conducting a tailored history-taking process that includes specific aspects of service and transition to civilian life. Practical guidance for clinicians on specific details to consider and address during assessment is covered. Comorbid conditions, chronic pain and the needs of both women and older veterans are also discussed.Speakers: Dr Wallace was appointed to his current position as psychiatrist at the Australian Defence Force Centre for Mental Health, at HMAS Penguin, Sydney, in 2010. In March 2018, he was appointed as the inaugural chairman of the RANZCP Military and Veterans' Mental Health Network and is now a member of the RANZCP Military, Veterans and Emergency Services Personnel Network Committee. He has been a Visiting Medical Officer at St John of God Hospital, North Richmond since 2015 and is an Adjunct Senior Lecturer in Psychiatry at the University of NSW.Lieutenant Colonel Diana McKay is a fulltime psychiatrist in the Australian Army.  She has worked in public and private civilian practice as an adult psychiatrist, with special interest in trauma and anxiety disorders, perinatal psychiatry and clinical education.  She is a member of the RANZCP Military, Veterans' and Emergency Services Personnel Mental Health Network Committee.  She is Honorary Associate Professor in the Faculty of Medicine, Health and Human Sciences at Macquarie University. Dr Tom Turvey is a senior advanced trainee in the Royal Australian and New Zealand College of Psychiatry. In 2004, Josephine began writing about her experiences with this condition, later earning her PhD in writing from Edith Cowan University in 2011. She is now an Associate Editor at Westerly (the University of Western Australia), and an Adjunct Senior Lecturer in Writing at ECU. This year, her debut novel, "Eye of a Rook" was published by Fremantle Press. It's a book that explores chronic illness in a heartbreaking, but insightful and intelligent manner. I was delighted to have the opportunity to chat with Josephine about her book earlier this year.

This episode is a departure from recent conversations about COVID-19 material to cover some very important topics on the human side of what we do in the ICU. My hope is you will glean valuable insights from an intensivist I admire greatly as both an expert practitioner and an esteemed educator of high-level intensive care communication - amongst other things, of course. Dr Steve Philpot is an Intensive Care Specialist at Cabrini Hospital in Melbourne with a special interest in end of life care, organ and tissue donation, communication skills training and empathy in the workplace. He is the National Lead Trainer for the DonateLife Family Donation Conversation Workshops, the Convenor of the College of Intensive Care Medicine communication training program, convenor of the Cabrini Health “Shared Decision Making” and “Advance Care Planning Conversations” workshops and chair of the Cabrini Health End of Life Care Committee. Steve is an Adjunct Senior Lecturer at Monash University and is involved in undergraduate communication training. He is currently completing a Masters of Health and Medical Law at Melbourne University. His other interests include playing guitar, singing, walking his dogs, the Fremantle Football Club (in the Australian Football League), and writing. Steve was once an advanced trainee where I worked as a consultant, and always displayed wisdom beyond his experience, something that comes through in this conversation, even if he is highly experienced now. We recorded this conversation before SARS-COV2 had been described, so there is no discussion of the COVID-19 pandemic, but I guess that makes it a favourable choice to listen to for many of you right now. Abstract: We often associate sloth with laziness, but is it possible for a culture to be productive and still be slothful? In this talk, Dr Matthew Tan explores how Sloth is arguably the most pervasive of the deadly sins first by understanding it by its original name, the vice of Acedia. Taking its cue from the writings of the desert father, Evagrius of Pontus, the talk would take a deep dive into what the vices do to the life of monastic virtue, and how acedia acts as a spiritual opiate whose affects are more than just spiritual. It will end by looking at how monastic practice can give us clues into providing new responses to this ancient vice. Bio: Dr Matthew Tan is the private secretary of Bishop Tony Randazzo in the Archdiocese of Sydney, and an Adjunct Senior Lecturer in a Theology at the University of Notre Dame Australia. He received his doctorate in Theology from the Australian Catholic University and his License in Sacred Theology at the Pontifical University of St Thomas Aquinas in Rome. He is the author of two books, the most recent of which is "Redeeming Flesh: The Way of the Cross with Zombie Jesus". Low back pain is among the most common chronic pain conditions in Canada. Four out of five adults will experience at least one episode of back pain at some time in their lives. The cause of back pain, however, is not always apparent; in fact, in approximately 85% to 90% of individuals with back pain, no specific cause can be identified.  To help us better understand this common condition and learn how to manage low back pain, we speak with Dr Owen Williamson. Dr Williamson is an orthopaedic spine surgeon and specialist pain medicine physician. He is also an Adjunct Senior Lecturer in the Department of Epidemiology and Preventive Medicine, Monash University, Melbourne, Australia.  In 2007, Dr Williamson became the first orthopaedic surgeon in Australia to become a Fellow of the Faculty of Pain Medicine, Australian and New Zealand College of Anaesthetists in 2007. He is on the Education and Training Committee and an examiner for the Faculty of Pain Medicine and is involved in the development of pain education programs in both Australia and Canada.
