On this episode, Errin and Jeff talk about being comfortable with being UNcomfortable. Is there a new skill you want to learn or a hobby you want to pick up? Maybe there's a physical habit you want to make routine, but it just hasn't happened yet. Is there something you want but haven't had the nerve to ask for? Uncertainty and discomfort can stifle growth and progress. On the flip side, healthy uncertainty with a solid support system can be invigorating and lead to growth. Yes, it can feel overwhelming to make even small changes or add anything to our plate when it already feels so full. But we rarely experience progress without some level of uncertainty or unease. Errin tells us to embrace these feelings, and to work on gaining perspective and developing a plan to move forward. What steps does Errin suggest we take to embrace being uncomfortable and spark growth? Step 1 – Awareness – Understand if you are running away from something or moving toward something. What is underlying your desired change? Step 2 – Assessment – Find clarity around WHY you feel this way and what has led to this point. Step 3 – Action – Take INCREMENTAL steps to start moving forward. Small steps can power SUBSTANTIAL growth. Most importantly, embrace the uncomfortable! Growth comes from a place of discomfort, starting with incremental steps.

Crispy around the edges. It's how I prefer some of my favorite breakfast foods. Pancakes. Waffles. Sunny-side up eggs. Hash browns. Bacon. All are extra delicious (and in my opinion cooked to perfection) when their centers are soft and their edges crispy. You know what's not best crispy around the edges? You and me. It's a great analogy Doctor Of Osteopathic Medicine Errin Weisman uses to describe people who are headed for burn out. She would know. Errin hit rock bottom early in her career and bounced in and out of burn out several times. During those encounters with burnout, she felt empty, exhausted, and cynical about everything and everyone else around her. Errin had it all and yet, she was the only one not to see it because she was blinded by the crippling fog of burnout. Over time, Errin was able to pull herself up by her own bootstraps by making different choices in her life. She slowed down, prioritized herself, and created a schedule that works for her - one in which she refuses to start work until 10am, even if that means calling in that day. Errin took charge of her mental health and is a new person these days. She's now in the business of coaching others to respect their guardrails so they can prevent burnout, heal, and live fulfilling business and personal lives without overdoing it. In this episode, Errin takes the reins and interviews Jeff about his recent surgical sabbatical. Earlier this summer, Jeff had lumbar spine surgery to address a herniated disc and foraminal stenosis. Since the procedure, he has been doing his best to be a good patient by taking six weeks off from taking calls and performing surgery. It's been a difficult transition, since Jeff has never taken that much time away from surgery in his entire career! As physicians, we often make for terrible patients. According to Jeff: “Despite my knowledge that wellness includes the mental, emotional, and spiritual, I was much more prepared for the physical experience and recovery. The forced experience of not being the one delivering the care as a surgeon gave me a new perspective and tremendous opportunity for growth.” What tips does Jeff have for surgeons who are taking a step back (either via sabbatical, medical leave, or figuratively) so they can move forward? Tip 1 – Reach out to a coach or someone who has the skill set to listen and support you through the experience. listen and support you through the experience. Tip 2 – Start working with that person well before the big challenges come along as this will help prepare you for dealing with such a significant change. Tip 3 – Be prepared for the work of the experience including the unease, discomfort, turmoil, and pain mixed in with the positives. In this episode, Errin and I definitely get sassy and talk about how burnout was the worst year of her life and left her asking: "What is wrong with me?!?!" Reaching the moment where she had everything she'd been striving for and then realizing something was missing she described as "when you reach the mountaintop and it feels like a deep dark canyon, and then you find the basement". It only took her 3 months into her medical career before she was looking for an out. When she reached out she got told she'd get used to it, it was normal, and maybe she should pop an antidepressant. Then, she met a coach. And the moment she finally felt seen, heard and understood - she began to heal. Now, she helps other female physicians and moms heal their own burnouts. In addition to 1:1 coaching, she has an online course called From Burnt Out To Badass, and she started a directory of physician coaches so that everyone can find the person they need. She credits learning to name and claim her emotions outside of 'numb' and 'angry' as crucial to her healing and talks a ton about really being her sassy self and how leaving her personality behind when she became DR. WEISSMAN was a major burnout factor for her. She wants you to know this: You are not broken. You are not the problem. This [burnout] is not a character flaw. And she'd love for you to ask yourself, "What if...... ..... you gave yourself the permission to explore? ..... you allowed your personality to shine through? ..... you could let go of your identity that you gained because of your career or degree? ..... you could be empowered to just be you? Holy shit! We've made it to 400 episodes. I honestly cannot believe it. I started this podcast to have fun, share my story, share the stories of other physicians, and help raise a voice against the failures of the medical industry in driving so many amazing physicians toward burnout. Sometimes you know immediately that a professional relationship simply isn't going to work. That was the case with me and my husband's accountant. This man had been doing accounting work for my husband's family for years and when we first got married, Craig and I decided to stay with him. Needless to say, that's no longer the case! The relationship got off to a bad start from the beginning. This man never treated me the same as he did Craig despite the fact that I was the breadwinner in our household. Add in the fact that he refused to do business through email? Deal breaker! Here's the thing, you deserve to work with people who are doing the full-assed job. This goes for anyone that you've hired no matter their role. When I realized that not only was he disrespecting me and my intelligence but he also operated his practice in a way that completely undermined my entire mission. I decided it was time. I now have an amazing accountant who understands the online business world, understands what coaches do, and isn't afraid to do business through email. “What I tell students and residents that I work with is you need to lean into your weird,” shares Dr. Errin Weisman, self-proclaimed queen of sass and burnout. She changed her career by leaning into her weird and now focuses on helping patients with substance abuse disorders, something that interested her from a young age. Dr. Weisman went from burnt out to thriving because she gave herself permission to try something new. Have you ever dealt with mom guilt? I know that I have. My mom guilt started before I was even officially a mom. I was pregnant and in residency and I couldn't shake the feeling that all of my late nights and crazy schedule was messing up my unborn child. Since mom guilt is so prevalent, I wanted to talk about ways we can deal with it. Because mom guilt never goes away, we just figure out better ways to manage it. First things first, you need to practice self-compassion. We ALL feel it. We ALL think we're messing up our kids. Give yourself grace, know that you're only human, and the most likely outcome is that your kids will be just fine! Next, realize that not all of our guilt is ours. We take on the scripts of other people in our lives, so make sure you're only dealing with your own guilt. Then set boundaries between you and those people who trigger your guilt. Like these tips? As long as you're doing the best that you can do, you're doing great! “Mom guilt is the feeling that either I'm not enough of a parent for my children, that I'm not doing the right things or the decisions I am making now will screw up my children in the future.” Dr. Errin Weisman In this episode: [02:51] Mom Guilt, do you have a dose of that? [03:21] I share my definition of mom guilt. [04:46] I don't believe that “mom guilt” goes away. I think it changes and gets different. [06:16] Now I discuss how we live peacefully with mom guilt. [08:13] Step 2, is when the guilt pops up, ask yourself, “is this mine?” [10:08] Step 3 is to set boundaries with those who trigger the guilt. I have been struggling with the emotion of resentment lately. If you've been following along, you know that I returned to full on clinical medicine recently and it has brought some things up for me. Listen as I share a bit more and then give my tips for approaching your own resentment. This is something I've been working through with my therapist, and it struck me one day when she said, “Man. You really hold onto things.” It's true. I do. I have some deep seeded issues with resentment and I hold onto them tight. So now, I've been working to recognize when I feel resentful, have someone on hand to validate my hurt, spend more time focusing on self compassion, and naming and finding my core values in every situation. More importantly, I'm learning how to stop repressing my resentment. I've been more mindful about figuring out the triggers for my resentment as well. We all have a choice. We can choose to hold on or we can choose to let go. As I practice with little moments, I'm getting closer and closer to being able to let go. How about you? Do you struggle with resentment? I'm wrapping it all up. This whole series came about as a way to dig into the research of work addiction as I began stepping back into an employed role. So much of what I talk about on this podcast has roots grounded in what's happening in my own life and it's been fun digging in. So first, I shared all about definitions and where work addiction starts. Then I moved into the actual research that has been done on work addiction and workaholics. It was part three however that has really stuck with me. The pause. Many of you have reached out and said that you can't take the pause, but if you don't you may find yourself in deeper waters than you can get yourself out of. You NEED to pause. Next, I chatted all about re-entry because that's where I'm at right now. Re-entry into the workforce with new strategies and a clear understanding of myself and my tendencies. This part is very important. Last, we discussed workaholic leadership and how it impacts the work culture as a whole. After you've taken a pause and have realized that you are indeed a workaholic, it's time to start into the R's. Relapse is not a moral failing. It's going to happen. It's a part of human psychology. It's time to remove the guilt and shame and sit in self-compassion. Self-compassion radically changed my life. I've been talking about it a lot on this podcast. Welcome back to part III of the work addiction series. In part I, we talked about the definition of work addiction. Part II was a review of the literature overall. Now in Part III I want to talk about how to navigate work addiction and how to re-enter the work environment. Listen in! If you want to change your work addiction you need to change the way you relate to yourself, your environment, and your work. So many people come to me in the midst of burnout convinced that they need to leave their jobs. But here's the deal, no matter where you work, you will always take yourself. Work addiction is not inherent in the environment that you work in necessarily. More often than not, this is an internal conflict that stems from conditioning, perfectionism, and other internal stimuli. Essentially, there are two main ways to classify physicians - independent or employed. Today's health care in the US is becoming increasingly consolidated as health systems and corporate entities (venture capital groups, etc.) are purchasing physician practices. With the advent of the pandemic of COVID-19 and the partial and complete shutdowns in medicine over 2020, the trend of selling practices accelerated. More and more physicians are now employed by someone - either a hospital system or corporation - eclipsing 70% of all physicians. The era of independent physicians may be coming to a close if these trends don't turn around. The question we should be asking is why is this happening and should we care? What are the Trends? The Physician Advocacy Institute surveyed physicians over 2019-2020 and found that there was an acceleration of physicians selling their practices to hospitals and corporate entities. This was not unique to any specific region of the country. You can see their paper here and it is clear that this is a trend that is not going away any time soon. Over the last two years, 48,400 doctors left independent practice and moved to employed status. Nearly half of all physician practices are now hospital or corporate based leaving an ever smaller base of people who can be nimble and innovative in the health care space. It is important to point out that many I have spoken to over the past two years have left big systems and struck out on their own to be disruptors in the space like Dr. Randy Lovell, Dr. Richard Rosenfield, Dr. Diana Bitner, and others. Reasons Docs Become Employed Cash out for retirement Avoid overhead/business frustrations like meeting payroll, etc. Market swings Paying off student debt (usually ~$250,000 from med school and undergrad) Perceived stable/reliable income Increasing regulations and reporting for things like value based care. MediCare reporting and data mining. Electronic medical records Audit processes Why Docs Leaving Independent Practice is a Problem for Us All Increased health care costs Less competition Less innovation in how to deliver care Prescription drug plan restrictions (and increased costs) Worse autonomy Increased utilization of midlevel providers Stagnant pay because no risk taking Doctors are increasingly becoming casualties of bigness in medicine. After training near me at SUNY Downstate in Brooklyn in neurology and neurophysiology, Dr. Scott Abramson moved cross country and joined Northern California Kaiser Permanente in July 1979 and retired from the Neurology department in San Leandro in 2020. For over 25 years, he had been involved in the communication mission at Kaiser Permanente, which all started at Toastmasters. He has been a physician coach and communications instructor. His two favorite courses were: “The Secret of Happiness” and “All the Wisdom We Need To Be Joyous Physicians Can Be Found in Country Music.” For over 20 years, he wrote a monthly column in our Kaiser newsletter on communication issues such as, “The Three Most Intelligent words a Physician Can Utter,” “Why I failed to cure my wife's headache” and The Most Important Words One Can Say to the Bereaved.” Several of these stories have been published in medical journals. When the child's condition failed to improve, the mother investigated. Get ready since we have an amazing guest and world renowned expert who is a leading expert on the subject of Covid-19 and has the most citations in the National Library of Medicine. Not only is he humble and gracious but very wise and passionate about helping and treating our PATIENTS first! Aside from being an expert, he TREATS Covid patients and is also a Covid survivor. Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology. He also has a Master of Public Health from University of Michigan School of Public Health. He cares for advanced patients with common medical problems including heart and kidney disease, lipid disorders, and diabetes. Dr. McCullough has broadly published on a range of topics in medicine with > 1000 publications and > 600 citations in the National Library of Medicine. His works have appeared in the New England Journal of Medicine, Journal of the American Medical Association, Lancet, British Medical Journal and other top-tier journals worldwide. He is the editor-in-chief of Cardiorenal Medicine, Reviews in Cardiovascular Medicine, and senior associate editor of the American Journal of Cardiology. On November 19, 2020, Dr. McCullough testified in the US Senate Committee on Homeland Security and Governmental Affairs concerning early ambulatory treatment of high-risk patients with COVID-19. He has appeared on news outlets and is passionate about spreading the word about treatment. Today we discuss evidence based treatments of Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. He believes it is immoral, unethical and bordering on illegal to advise NO TREATMENT of outpatients who have Covid-19. The focus has been on PPE, lockdown and isolation as well as treating only hospitalized patients. Forgotten are the patients caught in the middle. We can't just be passive doing nothing to prevent hospitalizations! It is therapeutic nihilism! While the FDA is a regulatory body, NIH is a research organization and the CDC is a public health organization- none of these organizations give guidelines and put the onus on physicians to figure out how to treat their patients. Remember physicians, we work for the patient!
