This cost-effective transdermal Skinject patch delivers a chemotherapeutic agent applied in a clinician's office over three sessions to remove the lesion, potentially avoiding the need for Mohs surgery, the current standard of care. Episode 025 | Dr. Teo Soleymani, a Los Angeles native, one of my Stanford residency mates, and founder of California Dermatology and Mohs Surgery Specialists (CDMS), completed his undergraduate studies in Evolutionary Biology at UCLA. He went on to graduate from the UC Irvine School of Medicine, where he received numerous accolades for academic excellence including induction into the prestigious Alpha Omega Alpha honor society. Dr. Soleymani completed his internship at Cedars-Sinai Medical Center, where he was awarded the UCLA David Geffen School of Medicine Excellence in Teaching with Humanism Award. He went on to complete his residency at Stanford University in Palo Alto, California. While at Stanford, Dr. Soleymani worked with world-renowned experts in cutaneous oncology and complex skin diseases. Following residency, he completed a rigorous fellowship in Mohs Micrographic Surgery, Cutaneous Oncology, and Facial Plastic and Reconstructive Surgery at the Zitelli and Brodland Skin Cancer Center under the tutelage of John Zitelli, M.D. Considered by many to be the world's most prestigious and competitive fellowship, the program is directed by both John Zitelli, M.D. and David Brodland, M.D, widely considered to be two of the pre-eminent authorities in Mohs surgery. Notably, Dr. Zitelli, was trained directly by Dr. Frederic E. Mohs, the founder and pioneer of Mohs micrographic surgery. This direct lineage from Dr. Mohs enables Dr. Soleymani to give his patients the highest-quality Mohs surgery experience. Dr. Soleymani's unique fellowship training was the only program in the world that included fellowship training in both Mohs Micrographic Surgery and Facial Plastic and Reconstructive Surgery as well. As a result he is one of only a handful of Mohs Surgeons in the world with dedicated specialized training in Facial Plastic and Reconstructive Surgery as well, allowing him to provide the best possible functional and aesthetic reconstructive results. Prior to founding CDMS, Dr. Soleymani practiced as a full-time Clinical Professor of Dermatology and Dermatologic Surgery at UCLA, where he was also the Director of the Pigmented Lesions and Melanoma Clinic, as well as Co-Director of Mohs Micrographic Surgery and Dermatologic Oncology Fellowship Education and Resident Surgery Education programs. This week, Courtney is joined by Dermatologist, Dr. Michelle Henry to talk about treating skin of color & the many misconceptions of skin cancer. Dr. Jeanine Cook-Garard learns about skin cancer - one of the most common cancers in the United States. It is estimated that one in five Americans will develop skin cancer in their lifetime. Her guest today is Dr. Victoria Sharon, the Director of Dermatologic Surgery and Dermato-Oncology, and the Director of Mohs Surgery at Northwell Health Cancer Institute in Lake Success, New York.

Melanoma is the most serious type of skin cancer. It develops in the cells that produce melanin, the pigment that gives skin its color. Melanoma is one of the most common cancer types in the U. S. Roughly 2% of people will be diagnosed with melanoma of the skin at some point during their lifetime, according to the National Cancer Institute. Treatment for early stage melanomas usually includes surgery to remove the melanoma. Mohs surgery is a precise surgical technique used to treat skin cancer. During Mohs surgery, thin layers of cancer-containing skin are progressively removed and examined until only cancer-free tissue remains. "Mohs surgery is essentially skin cancer removal," explains Dr. Nahid Vidal, a dermatologic surgeon at Mayo Clinic. "It's a surgical removal process that's highly specialized, where we're removing the skin cancer with a goal of not only removing all of it, but also leaving behind as much healthy tissue as possible."Mohs surgery allows surgeons to verify in real time through pathology that all cancer cells have been removed at the time of surgery. There are over 5 million people diagnosed with skin cancer a year, of which 95% get cancer on their face. We don't think about it often, but every day we go about our lives without really using any protection on our skin, we are putting ourselves in danger of getting skin cancer. The face, nose, ear, neck, and eyelids are the areas exposed the most, and it's the one that has the least amount of skin. The possibility of getting a visible scar is the highest. They use a device called a dermatoscope to examine skin spots and are trained in biopsy which means removing a sample of your skin to test, and if required your GP may refer you to a dermatologist.A skin cancer known as a melanoma can look like a mole and can be serious. Talk to your GP if you notice a new spot on your skin or an existing spot that has changed in shape, colour or is feeling uncomfortable.Non--melanoma cancers can cause soreness and can look like a pimple.Dr Edward Upjohn spoke about advancements in treatments over the past 20 years, such as creams and new drugs.Dr Edward Upjohn specialises in MOHs surgery, which is a type of surgery that can give immediate confirmation that all the skin cancer has been removed.Normally it is a day procedure involving a local anaesthetic.During this procedure a tissue sample is removed and then immediately checked in a laboratory so that the surgeon knows where the skin cancer is located and further tissue can then be removed if required, instead of waiting for test results which can take a number of days.Dr Edward Upjohn quoted a well-known phrase - “early diagnosis can save lives” which he says is especially true for skin cancers. As the sun comes out our thoughts can easily turn to how we hope to get some nice coloring. Unfortunately, exposure to the sun and UV rays can have some harmful consequences. Skin cancer is the most common cancer in the United States and worldwide, with more than 9,500 people diagnosed in the US every day. Diagnosis is followed by treatment, which is a specialty of Ogden Clinic dermatologist Dr. Chad Tingey. Dr. Tingey practices the most advanced method of skin cancer removal: Mohs Surgery; this exact and precise method of skin cancer removal was part of Dr. Tingey's unique fellowship-training by the American College of Mohs Surgery (ACMS).  

Jason Rizzo is the director of Mohs Surgery at Western New York Dermatology. Mohs micrographic surgery (MMS) entails many controversies – from specialty certification to the types of tumors treated. Three dermatology residents – Dr. Julie Croley, Dr. Elisabeth Tracey, and Dr. Daniel Mazori – discuss MMS for melanoma and other tumors as well as reimbursement for and the cost-effectiveness of the procedure. They also highlight controversies surrounding the Mohs Appropriate Use Criteria (AUC). “It appears further investigation is needed to elucidate and optimize solutions to many of the current controversies associated with Mohs micrographic surgery,” Dr. Croley says.  * * * Help us make this podcast better! In Oregon, ‘war on melanoma’ takes flight A research project hopes to shrink melanoma mortality by emphasizing education, screening. *   *   * Things you will learn in this episode: The 5-year survival in metastatic rates for melanomas treated with Mohs micrographic surgery (MMS) with frozen sections were the same or better when compared with historical controls treated with conventional wide local excision. Immunostaining in melanoma may improve accuracy but includes many challenges: It is time-consuming, reagents can be costly and could expire, some cases are equivocal, clinicians’ exposure to immunostaining education is limited, and training is required to interpret margins. “So there are a lot of barriers to using immunostaining,” Dr. Croley says, “but I think it has a lot of potential in the future.” Utilization of MMS has increased in recent years, possibly due to superior efficacy for appropriately chosen cases and it is being expanded to treat other tumors such a melanoma and Merkel cell carcinoma. There is wide variation in mean number of Mohs stages among dermatologic surgeons. Mailing out individual reports of practice patterns to high-outlier physicians resulted in a reduction in mean stages per tumor as well as an associated cost savings when compared with outlier physicians who did not receive these reports. Codes for MMS are frequently reviewed by a Relative Value Scale Update Committee, and the procedure is listed as a potentially misvalued service according to the Centers for Medicare & Medicaid Services. “Reimbursements for Mohs surgery and reconstructive surgery have gone down by more than 20% in the last 15 years – at least, in part, as a result of this scrutiny,” Dr. Tracey states. Mohs AUC has been criticized for classifying most primary superficial basal cell carcinomas as appropriate for MMS and for not considering variables such as operating on multiple tumors on the same day and operating on a tumor that is incorporating into an adjacent wound. Dr. Hsu is currently an Assistant Professor of Medicine (Dermatology) at Dartmouth Medical School where he gives a Cosmetic and Laser Surgery lecture series to dermatology residents. Dr. Hsu is a member of the American Society for Dermatologic Surgery, American Society for Lasers in Medicine and Surgery, American College of Phlebology, American Academy of Dermatology, International Society of Hair Restoration Surgery, American Society of Cosmetic Dermatology and Aesthetic Surgery and American Society for Mohs Surgery. Deborah Mac Farlane, M.D., Director of MD Anderson Cancer Center’s Mohs Unit in the Department of Dermatology, explains what makes Mohs micrographic surgery unique and why it is important to have this procedure done at a multidisciplinary center, where departments like Radiation Oncology, Diagnostic Imaging, Dermatology, Plastic Surgery, Head and Neck Surgery and Melanoma Medical Oncology collaborate to care for skin cancer patients.

He's also the director of fellowship training in Mohs Micrographic Surgery and Cutaneous Oncology. He's currently Clinical Associate Professor of Dermatology, University of Pennsylvania School of Medicine. Dr. Benedetto was named 'Dermatologist of the Year' (2009). He's authored and co-authored 65 articles in peer review journals and textbook chapters.

Melanoma is the most serious type of skin cancer. It develops in the cells that produce melanin, the pigment that gives skin its color. Melanoma is one of the most common cancer types in the U. S. Roughly 2% of people will be diagnosed with melanoma of the skin at some point during their lifetime, according to the National Cancer Institute. Treatment for early stage melanomas usually includes surgery to remove the melanoma. Mohs surgery is a precise surgical technique used to treat skin cancer. During Mohs surgery, thin layers of cancer-containing skin are progressively removed and examined until only cancer-free tissue remains. "Mohs surgery is essentially skin cancer removal," explains Dr. Nahid Vidal, a dermatologic surgeon at Mayo Clinic. "It's a surgical removal process that's highly specialized, where we're removing the skin cancer with a goal of not only removing all of it, but also leaving behind as much healthy tissue as possible."Mohs surgery allows surgeons to verify in real time through pathology that all cancer cells have been removed at the time of surgery.
