Show Notes Your fee schedule is a revenue ceiling. And for most independent practices doing over $3 million a year, that ceiling is set too low in ways that never generate a denial and never appear on a standard report. EP186 covers the five gaps that are quietly capping your revenue, the exact fix for each one, and three actions to run this week. Gap 1 — Billing Below Your Own Allowables: You negotiate a better payer contract. The billing system does not get updated. The payer pays what you billed, not what you are owed. A practice with 20 high-volume CPT codes averaging a $10 billing gap across 800 monthly claims is losing $8,000 a month, $96,000 a year, from a contract they already won. Gap 2 — Inconsistent Fee Schedules Across Locations: A secondary location runs on its legacy fee schedule from before acquisition. Location A bills $210 for a procedure. Location B bills $165 for the same code. A site doing 400 visits a month with a $35 average billing gap is under-billing $14,000 a month, $168,000 a year. Gap 3 — No Medicare Multiplier Anchor: Fees set by instinct drift downward every year while costs move in the opposite direction. The fix: anchor to 200–300% of the current Medicare allowable and recalculate every November when CMS publishes updated rates. Gap 4 — Suppressing Global Fees for Self-Pay Patients: A practice protecting 15% self-pay volume by keeping fees low inadvertently discounts 100% of encounters. 850 commercial patients billed $40 below the correct rate: $34,000 a month, $408,000 a year. The fix: raise the global fee schedule and implement a separate documented sliding fee scale for uninsured patients. Gap 5 — No Annual Fee Schedule Review: A fee schedule that is right in year one becomes the revenue leak of year five. A $4 million practice drifting 3% below where it should be loses $120,000 a year in collectible revenue. HEALTH NEWS   Ginger Supplementation Reduces Muscle Soreness, Review Finds Artificially sweetened and sugar-sweetened beverage intake and risk of liver cancer Plant-based quinoa burgers reduce post-meal blood sugar spikes Sleep and exercise may curb heart risk from mutant white blood cells Pregnant women may reduce key health risk through less sitting, more light exercise     Ginger Supplementation Reduces Muscle Soreness, Review Finds Old Dominion University, June 4 2026 (Natural News)     A review published in Nutrition Reviews found that consuming 2 grams of ginger daily for 11 consecutive days before exercise reduced delayed-onset muscle soreness (DOMS) by 23% to 25%.   The review examined multiple placebo-controlled studies and found that single doses taken just before exercise did not produce significant pain reduction, but consistent daily intake over the 11-day period yielded measurable results.   DOMS typically occurs 24 to 72 hours after exercise and is a common reason individuals skip subsequent workouts. In two placebo-controlled studies reviewed, participants consumed 2 grams of either raw or heat-treated ginger daily for 11 days before performing eccentric exercise, which lengthens muscles under tension. The effective dose identified in the review was 2 grams per day, roughly equivalent to one teaspoon of fresh grated ginger.   Artificially sweetened and sugar-sweetened beverage intake and risk of liver cancer Yale University, National Cancer Institute, Boston University, June 10 2026 (Eurekalert) Are artificially sweetened beverage (ASB) and sugar-sweetened beverage (SSB) intakes associated with risk of liver cancer overall and by subtype (hepatocellular carcinoma [HCC] and intrahepatic cholangiocarcinoma [ICC])? In this pooled analysis of 11 prospective cohort studies comprising 1,518,411 adults, SSB intake per 1-beverage/day increment was associated with increased risk of HCC and ICC, whereas ASB intake was not associated with liver cancer overall or by subtype. There was no evidence of effect modification by diabetes status.     Plant-based quinoa burgers reduce post-meal blood sugar spikes Federal University of Golas (Brazil), June 10 2026 (News-Medical) A study published in ACS Nutrition Science suggests that a plant-based burger made from baru pulp and red quinoa could reduce post-meal blood glucose responses in healthy adults. Red quinoa (Chenopodium quinoa Willd.) is a pseudocereal that is rich in protein, fiber, and micronutrients. It provide spolyphenols that have been shown to slow gastric emptying and overall digestion, and may reduce enzymatic degradation of carbohydrates in the gut. This would influence the rate at which glucose is absorbed into the bloodstream after a meal, altering the GI. The authors of this pilot study examined GI in a small sample of eight volunteers after consuming two plant-based burgers: one made with baru pulp and red quinoa, and the other with red quinoa alone.  All three foods produced their highest blood glucose levels 30 minutes after consumption. The glucose reference food generated the largest peak at 174 mg/dL, while the baru pulp–red quinoa and red quinoa burgers reached substantially lower peaks of 118 mg/dL and 120 mg/dL, respectively. By 120 minutes, blood glucose levels had declined in all groups. Compared with the glucose reference, both plant-based burgers caused only modest increases in blood glucose relative to fasting levels: 15.5% for the red quinoa burger and 18% for the baru pulp–red quinoa burger.   Sleep and exercise may curb heart risk from mutant white blood cells Mount Sinai Hospital, June 10. 2026 (Medical Xpress) Healthy sleep and regular exercise can work to counteract genetic mutations in white blood cells that are associated with cardiovascular disease and are most common among older people, Mount Sinai researchers have found. In a study published in Nature, the team reported for the first time that sufficient sleep and exercise can help reduce the cancer-like cell expansion and atherosclerotic risk linked to mutations that spontaneously occur in white blood cells. These mutations accumulate over our lifetimes and occur most often in hematopoietic stem cells, which are the cells in bone marrow that make blood cells, including macrophages and monocytes, immune cells that help defend the body. When these cells develop mutations, they start to proliferate, multiplying faster than they should, and become more inflammatory, irritating or damaging tissues in the body. This condition, known as clonal hematopoiesis (CH), is detectable in a quarter of people over age 70 and half of people over 80, though it is infrequent in young, healthy people. Healthy sleep and exercise was found to selectively influence immune cells with clonal hematopoiesis mutations, repressing their proliferative programming and expansion, as well as their ability to promote the formation of harmful plaque in the arteries of the heart. The findings reveal that CH mutant cells are malleable and selectively responsive to lifestyle behavior in a way that can mitigate atherosclerotic risk. Mount Sinai researchers discovered that moderate-to-vigorous physical activity was associated with a reduced incidence of gene-specific CH and fewer mutant cells in the blood. Sufficient sleep and exercise "turned off" the detrimental effects of rogue Jak2 and Tet2 mutant CH hematopoietic stem cells in the bone marrow, decreasing their ability to proliferate and grow, a precancerous process known as "clonal expansion.   Pregnant women may reduce key health risk through less sitting, more light exercise University of Iowa, June 10 2026 (Eurekalert)   Women who engage in light physical activity and lessen their sedentary time may significantly reduce the risk of key health problems during pregnancy, according to a new University of Iowa-led study. Researchers examined the daily behaviors of 470 pregnant women across all stages of pregnancy. Each participant wore a monitor that measured physical activity in 24-hour cycles and another monitor that recorded the time they spent asleep. Based on observational data collected from the study's participants, the researchers propose a “Goldilocks Day”-like guide for pregnant women that could reduce by nearly 30% the risk of developing hypertensive disorders of pregnancy (HDP), commonly occurring complications of pregnancy that include chronic hypertension, gestational hypertension, and preeclampsia. Those recommendations are: • Reduce sedentary time to fewer than eight hours each day. • Engage in light physical activity for at least seven hours each day. • Engage in approximately 22 minutes of moderate to vigorous physical activity, such as a brisk walk, each day. • Get nearly nine hours of sleep each night.

Global heating continues, despite the increased use of renewable energy sources and international policies attempting otherwise. Even as emissions reduction efforts continue, our world faces more extreme weather, sea level rise, and human health impacts, all of which are projected to accelerate in the coming decades. This raises an important but controversial question: at what point might more drastic interventions, like geoengineering, become necessary in order to cool the planet? In this episode, Nate interviews Professor Ted Parson about solar geoengineering (specifically stratospheric aerosol injection) as a potential response to severe climate risks. They explore why humanity may need to consider deliberately cooling Earth by spraying reflective particles in the upper atmosphere, how the technology would work, as well as the risks and enormous governance challenges involved. Ted emphasizes the importance of having these difficult conversations now, so that we're prepared for the wide range of climate possibilities in the future. Wendy remarks to Kirsh that she wants to save her brother from death. 2 "" Noah Hawley August 12, 2025 0.380 Boy Kavalier tells Dame Sylvia, that he formed the Hybrid project to allow humanity to compete with , and has granted Wendy additional abilities as he wishes to create a person smarter than him. Kavalier declines Weyland-Yutani's request to secure the Maginot's proprietary contents, warning any incursion on Prodigy's territory will be considered a hostile act. Joe is chased by a Xenomorph and separated from his colleagues. The Xenomorph tracks Joe to the higher floors of the tower, killing another soldier and massacring an apartment of wealthy residents who did not evacuate. Joe is saved by Morrow, who  Joe and the Xenomorph, but the Xenomorph regains consciousness and escapes after killing numerous other soldiers, sparing Morrow. Arriving in New Siam, Hybrids Tootles, Smee, Nibs, and Curly encounter other extraterrestrial specimens. Wendy locates Joe along with Slightly but Joe does not recognize Wendy. Slightly reveals Wendy's true identity to a shocked Joe who believed Wendy/Marcy had died. The three encounter several Xenomorph eggs and are ordered by Kirsh to contain them until a  team arrives. Joe is dragged away by the Xenomorph, and Wendy chases after it. 3 "" Dana Gonzales Noah Hawley and  August 19, 2025 0.441 Nibs questions Curly about why the hybrids are all named after  characters and why Marcy gets to be Wendy. Kavalier orders the specimens be brought to Neverland Island for study, despite objections from Kirsh and Dame Sylvia. Morrow finds Smee and Slightly guarding the Xenomorph eggs and interrogates them, suspicious of their childlike behavior. Morrow downloads all the data from the Maginot and erases the data from the source. Kirsh confronts Morrow and Morrow plants a device on Slightly's neck before escaping. Meanwhile, Wendy and Joe fight the Xenomorph with a , and Wendy kills the Xenomorph, although both sustain serious injuries. Returning to the island, Joe undergoes surgery and the Arthur and Dame tend to Wendy. Having escaped into New Siam, Morrow calls Ms. Yutani, and insists on retrieving the specimens despite being ordered to return home. Curly confronts Kavalier about his favoritism towards Wendy, asserting herself to be the best hybrid. Morrow contacts Slightly via the embedded device, convincing Slightly to be Morrow's friend. Kirsh, Tootles, and Curly dissect a facehugger and introduce its larva to Joe's damaged lung, removed during surgery. Wendy awakens, seemingly intercepting signals from the Xenomorph eggs which cause pain. Kirsh and Tootles test Trypanohyncha Ocellus (an octopus-like creature) on a sheep, and it implants itself in the sheep's brain via its eye socket. Tootles names himself as Issac. Kumi Morrow pressures Slightly into revealing his real name, Aarush Singh, and he is from India, as a sign of trust, and asks Slightly to steal a Xenomorph egg. Nibs, claiming to be pregnant, becomes violent after Dame Sylvia tries to interrogate her about the rescue mission, and is unknowingly put under house arrest. Joe, jaded after seeing how Wendy is treated as an experiment, attempts to quit and leave. However, Atom Eins tells Joe that if he does, Joe will be sent a large bill for his new lung and never see Wendy again. Slightly contacts Morrow to tell him that he cannot steal an egg, but the latter reveals he used Slightly's real name to track down his family, and uses them as blackmail. Morrow instructs Slightly to take a human near the eggs so they can be infected by a facehugger. It is revealed that Kirsh had been eavesdropping on Morrow and Slightly's conversations. Later, Wendy observes the Xenomorph-infected lung as the newborn Xenomorph bursts out, and Wendy placates it by communicating. 5 "" Noah Hawley Noah Hawley September 2, 2025 0.361 A flashback shows the events that occurred on the Maginot before it reached Earth. Junior security officer Clem wakes Morrow from cryo-sleep and tells him that a fire has led to two facehuggers escaping containment, which latched onto Captain Dinsdale and Science Officer Bronski. Dinsdale dies from the facehugger's acid blood when medical officer Rahim attempts to cut its tail. After being told by engineer Shmuel that the ship's navigational systems were also damaged in the fire, Morrow concludes that someone has sabotaged the ship. Executive officer Zaveri assumes command of the ship, but Morrow threatens to relieve her if she does not prioritize the creatures above the crew. Bronski is placed in cryo-sleep to prevent the gestation of the Xenomorph, but this fails. In science officer Chibuzo's lab, one leech specimen manages to open its test tube from the inside, and lays its larvae in her water bottle. Witnessing this, the Ocellus also escapes afterwards. Apprentice engineer Malachite drinks the water, and the larvae kill him internally; Chibuzo and Rahim are killed by their defensive toxic gas when trying to extract them. Navigator Teng is killed by the now full-grown Xenomorph. Chief engineer Petrovich is revealed to be behind the sabotage, having been promised a hybrid body by Kavalier if the ship crashes in Prodigy territory. Petrovich kills Clem and is in turn killed by Morrow, who retreats to the control room and seals himself in. Zaveri is killed by the Xenomorph outside the door. With everyone dead, Morrow locks himself in the panic room and awaits the crash. Back in the present, Morrow meets with Yutani, offering to retrieve the specimens by force and kill Kavalier. 6 "" Ugla Hauksdóttir Noah Hawley and Lisa Long September 9, 2025 0.478 As the Xenomorph grows, Wendy works on communicating with it. Nibs undergoes testing, prompting Atom Eins to demand her memory be reset to before the Maginot's crash. Dame reluctantly accepts, but Arthur refuses, and is fired under threat of execution if he is not gone by the end of the day. Wendy, disturbed by Nibs' behavior afterwards, learns she was reprogrammed. Meanwhile, Kavalier meets with Yutani over the ship's return and outwits her, securing 20 billion in damages while keeping the specimens for 6 weeks due to quarantine. Kirsh asks Tootles to feed and water the specimens while he is away. However, the Ocellus surprises him into accidentally locking himself in the same cage as two fly-like creatures, who kill him with acid and feed on his body. Slightly tries to lure Joe to the alien eggs, but he declines due to reassignment. Joe later visits a packing Arthur, who covertly deactivates Wendy's tracking device and gives Joe a boat code to allow them to escape. Noticing Tootles is disconnected, he goes to the lab accompanied by Slightly, who opens the cage of the Xenomorph eggs and locks him in, allowing a facehugger to latch onto Arthur. Kirsh watches this through security cameras, but does not tell Kavalier. Slightly hides along with Arthur's body in an air vent as the flies leave their cell. 7 "" Dana Gonzales Noah Hawley and Maria Melnik September 16, 2025 0.385 Smee discovers Slightly hiding an incapacitated Arthur, and the latter convinces him to help deliver Arthur to Morrow on the beach. Security re-secures the lab specimens. Wendy is disgusted by Kavalier's attitude to Tootles' death, and convinces Nibs to join her and Joe in escaping the island. On her way out, Wendy hacks the lab system, releasing the grown , which rampages and escapes into the forest. Kirsh finds Slightly and Smee carrying Arthur's body, but helps them take a faster route to the beach. Outside, Arthur awakes after the  dies. Shortly after, a newborn Xenomorph bursts from his chest and escapes. The hybrids take the corpse to the beach, where Morrow's Yutani team meets them. Having failed to bring him the newborn, Morrow takes them captive. After Kirsh shows him the Ocellus caused the lab accident, Kavalier becomes fascinated with the creature's intelligence, and wants to place it into a human host. Outside, Wendy, Joe, and Nibs are held at gunpoint by Yutani forces, but Wendy calls the Xenomorph which kills them. She comforts it, sparing them. Morrow's team enter the facility, but they are taken captive by Kirsh, who has also captured the newborn Xenomorph. Wendy, Joe, and Nibs reach the boat, but they are intercepted by Neverland security. Nibs, frustrated, brutally kills a soldier, prompting Joe to incapacitate her. A shocked Wendy scolds Joe, as the Xenomorph watches in the distance. 8 "" Dana Gonzales Noah Hawley &  September 23, 2025 0.469 The hybrids have been imprisoned in one cell, with Joe and Morrow in another. Wendy uses her abilities to block the facility's cameras and communications. She struggles with Joe's decision to shoot Nibs, believing he chose to ally himself with humans instead of her, but ultimately unlocks his cell. Kavalier approaches the hybrids and reveals that at six years old, he built his own synthetic which he used to kill his abusive father. Wendy unlocks their cell, Nibs kills Kavalier's bodyguard, and Kavalier flees. Morrow attacks Kirsh in the lab; Kirsh is badly damaged, but ultimately chokes Morrow unconscious. Smee and Slightly arrive in the lab and they take both Kirsh and Morrow, while Nibs captures Dame. Meanwhile, Atom lures Joe to Kavalier's room and locks him in with the Ocellus. Wendy arrives to save him and fights Atom, who reveals himself as a synthetic, allowing Wendy to control his motor functions. The Ocellus escapes to the beach, where it possesses Arthur's corpse. https://youtu.be/8welI8CK50M Here, in a very simple summary, is what the Austrian theory says: 1) Interest rates can come down in two ways: a) the public saves more; or b) the central bank artificially forces them down. 2) Businessmen respond to the lower interest rates by starting new projects. The projects tend to be those that are the most interest-rate sensitive-in particular, they occur in the so-called higher-order stages of production: mining, raw materials, construction, capital equipment, etc. Production processes farthest removed in time from finished consumer goods, in other words. 3a) If the interest rate is lower because of natural causes-e.g., increased saving-then the market works smoothly. People's deferred consumption provides the material wherewithal for businesses' new investment projects to be seen through to completion. 3b) If the interest rate is lower because of artificial causes-e.g., the manipulation by a central bank-then these projects cannot all be completed. The necessary resources to complete them have not been saved by the public. Investors have been misled into production lines that cannot be sustained. 4) Imagine a home builder who believes he has 20 percent more bricks than he actually has. He will build a different kind of house than he would if he had an accurate count of his brick supply. (Assume he can't buy any more.) The dimensions will be different. The style may even be different. And the longer he goes without realizing his error, the worse the eventual reckoning will be. If he finds out his error only at the very end, he'll have to tear down the whole (incomplete) house, and all those resources and labor time will have been squandered. Society will be that much the poorer. 5) The economy is like the home builder. Forcing interest rates lower than the free market would have set them makes economic actors act as if more saved resources exist than actually do. Some portion of their new investment is malinvestment-investment in lines that would have made sense if the saved resources existed to sustain and complete them, but which do not make sense in light of current resource availability. 6) The housing boom is a classic example of this theory in action. Artificially low interest rates misdirected enormous resources into home construction. We now know that was unsustainable. There were only so many $900,000 homes that the public, which had been saving very little, was in a position to buy. 7) The sooner the monetary manipulation comes to an end, the sooner the malinvestment can be shaken out and misallocated resources redirected into sustainable lines. The longer we try to prop things up, the worse the inevitable bust will be. The home builder in our example would have been much better off if he had discovered his error sooner, because far fewer resources would have been irrevocably squandered. PATIENCE AND PACINGPatience is not just a virtue; it's a strategic imperative. When faced with a client in distress, it's tempting to rush into action. The prudent approach, however, is to take a step back, gather information systematically, pose pertinent questions, and attentively listen to the opposing viewpoint. This method lays the groundwork for a robust negotiation strategy. Equally crucial is pacing. Each negotiation possesses its unique rhythm, and it's essential to respect this cadence. Understanding the pace in a negotiation is key to maintaining momentum and avoiding potential issues. Artificially pushing or slowing the pace can put you at a disadvantage and reduce the chances of a good deal. As a negotiator, experience helps in recognizing the conversation flow and using it to your advantage. Questions are a powerful tool in negotiations, but they are often underutilized. THE ART OF ASKING QUESTIONSTo achieve successful negotiations, one must understand the importance of asking questions, listening to answers, and being patient. This approach helps in understanding the other party's concerns, allows for useful information exchange, and enables fulfilling their needs. Imagine this: you're in the midst of a negotiation, and tensions are high. This is where asking questions becomes your superpower. It's not just about gathering information; it's about truly understanding where the other party is coming from. By asking the right questions and really listening to their responses, even when you disagree, you're showing that you value their perspective and are genuinely interested in finding common ground. BUILDING AND MAINTAINING RELATIONSHIPSBuilding and maintaining relationships means establishing trust and rapport. It's about respecting the other party's viewpoint, actively listening to what they have to say, and showing a genuine willingness to collaborate towards a win-win outcome. Why does this matter? Well, for starters, it keeps the conversation flowing, even when things get tough. By fostering a respectful and professional relationship, you create an environment where disagreements can be addressed head-on and solutions can be found.
