Welcome back to the tasty morsels of critical care podcast. Today we’re going to look at the red stuff – blood, and when to give it. This will cover some of Oh’s Manual chapter 97 covering blood transfusion. But we’ll have a focus on transfusion targets. There’s a nice narrative of evidence here over the past 20 years that has given us a relatively robust evidence base for practice in this area, something quite novel in critical care. Blood is expensive and unlike fossil fuels currently remains a renewable resource in the healthy population, it is obviously quite limited and nations frequently experience shortage of various blood groups and products that can have significant impacts on health care delivery. The red cells we give undergo a number of changes in the donation process with “storage lesions” becoming more prevalent over the duration of storage. A list of potential problems with stored red cells might run as follows: red cells change in shape biconvave to spherocytes (echinocytes) losing flexibility change in red cell membrane leading to sticking to the endothelium (esp in activated states like sepsis) 2,3 DPG depletion (which means Hb holds onto Oxy) reduced NO progressive increase in K+ acidosis The ABO reactions of transfusion should be dealt with by good governance of your transfusion service but fevers and other reactions are still an issue. The wonderfully named TRALI and TACO are also well described and space precludes a detailed discussion of these in this post. Now that we know giving red cells is not an entirely benign intervention we are left with the question that all competitive limbo dancers are faced with on a daily basis – how low can you go. What would be an appropriate Hb target for a critically ill patient. So let me tell you a little story… back in the late 90s when i was binging on OK Computer some Canadians led by Paul Hebert produced a large observational cohort of ICU patients called the TRICC trial suggesting that those with lower Hb did poorly and those who got more transfusions did better. But they were good empiricists and acknowledged that this could all be confounded by unmeasured factors. The only way to deal with that is randomisation and so 2 years later, Paul Hebert was at it again producing the TRICC 2 trial. This time an 800 pt multicentre randomised trial looking at Hb of 7 v 10. The headline result here was that the restrictive group did at least as well and probably better than the liberal transfusion group. This was a major trial and I’m pretty sure triggered a major change in practice. The caveats to this were as expected – those with ischaemic heart disease should probably have a higher target. Things went quiet for a few years but in 2010 we saw the TRACS trial from Brazil looking at one of the sacred cows of transfusion targets – cardiac surgery. Can we lower the Hb target in those with dodgy coronaries? They looked at Hb 9 vs 10.5 and found no difference. Villaneueva in 2013 took on upper GI bleeds. They smartly excluded the unstable active bleeders but in 500 patients randomised to 7 v 9, the lower target won out. The trials started to come thick and fast now with TRISS trial in 2014 taking on sepsis. The problem in sepsis is oxygen delivery so surely more Hb is good. But yet again, in 1000 pts with sepsis there was no benefit in targeting 9 vs 7 2015 brought the TRIGGER trial (hopefully you’re starting to see the unofficial naming convention here…) looking again at UGIB and again finding no benefit to the higher target 2017 brought the TRICS 3 trial, looking at 5000 patients undergoing cardiac surgery. Again randomised, this time 7.5 v 9.5, again no advantage to the higher target in 2021 they took on ACS patients in the REALITY trial, the most obviously ischaemic group and randomised 8 v 11 and no benefit to the higher target Most recently in 2025 the TOP RCT looked at vasculopaths having vascular surgery and in 3000 pts there was no benefit to the higher target. Phew… that’s a lot of trials but I think you’re starting to get the point that in general the answer to the question “what is your Hb target” is going to be 7-8 There are of course caveats to throw in at this stage. Firstly, it’s important to note that none of these trials looked at the exsanguinating patient where you should be targeting physiology like HR and BP and perfusion rather than Hb. Restrictive Hb targets are in general questions for the daily ward round rather than the massive transfusion protocol. Finally, in the past couple of years we’ve seen 2 RCTs looking at critically ill patients with sick brains. One looking at TBI and the other looking at SAH. Both suggest that if you have a sick brain you probably should be targeting a higher Hb of 9 or so. When you look at their outcomes the differences do not reach statistical difference in either trial but the trends are clearly to my eye towards more blood leading to better outcomes. De politieke week overlopen we deze keer met Prof. Herman Matthijs. Die week begon met de naweeën van de DPG-peiling. Die zetten alle partijen van de coalitie, behalve de N-VA onder druk. Zet dat de N-VA in een zetel, of maakt dat akkoorden sluiten net moeilijker? Herman Matthijs ziet deze regering de rit niet uitdoen.  Want op de tafel van de regering liggen nog wat moeilijke dossiers. Zo zal de begroting blijven voor problemen zorgen. Die kunnen niet opgelost worden zonder een grondige hervorming, maar niemand durft. Er is een kans op staatshervorming als Brussel opnieuw geld nodig heeft. En Brussel heeft altijd geld nodig. Een andere mogelijkheid is dat er een samenwerkingsakkoord komt tussen de deelstaten. Maar kan de N-VA het maken om in Vlaanderen overschotten te boeken om de Belgische begroting te stutten?  Herman Matthijs stelt zich alvast kandidaat. Journalist Derk Sauer is dinsdag op 72-jarige leeftijd overleden aan de gevolgen van een zeilongeluk. Jan Kees Emmer (voormalig adjunct-hoofdredacteur Telegraaf) zat pasgeleden nog met Sauer in een talkshow. 'Een fascinerende vent', omschrijft Emmer hem. Dr. Elbe authored the 18th and 19th editions of the handbooks before production ceased in 2019. Seeing the need for reliable information, Dr. Elbe launched Drug Nutrition Interactions (DNI), an extensive database with over 1,100 peer-reviewed drug entries. Senior dietitian Dr. Mary Litchford provided the dietary recommendations and curated dietary guidelines. Le Premier ministre sénégalais Ousmane Sonko prononcera ce vendredi 27 décembre 2024 devant les députés son discours de politique générale (DPG), qui fixera le cap pour les mois, les années à venir. « Une DPG sans risque pour Ousmane Sonko », titre Le Soleil, puisqu'il n'est pas tenu de demander ensuite la confiance des députés. Le Soleil détaille une première annonce, faite lors de ce discours à venir : le Premier ministre « va proposer un vote sans débat » pour faire adopter le LFI, « le projet de loi de finances initiale ». Sans débat, pour permettre une adoption rapide. Le vote aura lieu samedi 28 décembre 2024. « En parallèle » de ce vote, rappelle Senego, « les députés installeront officiellement la Haute Cour de justice, une juridiction d'exception destinée à juger les plus hautes autorités de l'État », dont le président, le Premier ministre et les ministres. « Les huit juges titulaires et huit suppléants, élus par l'Assemblée, prêteront serment avant de prendre leurs fonctions », détaille Senego.La polémique enfle toujours sur les tirailleurs de ThiaroyeLe conseiller ministre Cheikh Oumar Diagne est sous le feu des critiques après avoir traité les tirailleurs de Thiaroye de « traîtres, qui se sont battus contre leurs frères, dans leur pays, pour de l'argent ». Ces affirmations sont « une offense à la mémoire de ces tirailleurs », écrit Seneplus dans un édito « mais aussi à toutes les générations d'Africains qui se sont engagés dans la lutte contre l'oubli du massacre de Thiaroye ».Le Soleil reprend les propos du porte-parole du gouvernement, Moutsapha Ndiekk Sarré, qui « considère que monsieur Diagne s'est lourdement trompé » et qu'il s'agit d'une « sortie malheureuse ». Dakar Actu partage l'indignation du musée d'Histoire du Djolof, qui condamne aussi les propos de Cheikh Oumar Diagne. « Les tirailleurs sénégalais sont un patrimoine commun, représentant un symbole de résistance, de loyauté et de sacrifice non seulement pour le Sénégal, mais également pour l'Afrique entière », s'émeut ce musée d'Histoire. La presse burkinabé s'en mêle aussi : « Un ministre ne devrait pas dire ça ! », titre Aujourd8 au Burkina Faso. Le journal, dans un édito, estime qu'il « contredit l'acte majeur et mémoriel posé par son patron, [le président Diomaye Faye, et] souille la mémoire de ceux qui ont versé leur sang, le sang africain ». La Fédération des descendants de tirailleurs a décidé de déposer plainte contre Cheikh Oumar Diagne. Tchad : l'armée française se retire de la base de Faya-Largeau« L'armée française cède sa base à l'armée tchadienne », titre Tachad qui rappelle que ce transfert « s'inscrit dans un contexte de tensions diplomatiques, après la suspension inattendue, il y a moins d'un mois, de l'accord militaire qui liait Paris à Ndjamena ». « La machine est lancée », écrit de son côté le burkinabè Le Pays. « Avec le départ de ses troupes du Tchad, poursuit l'édito, la France aura perdu totalement le contrôle du Sahel, où jadis, elle faisait la pluie et le beau temps ». Ce retrait dans le nord s'effectue juste avant les élections législatives, provinciales et communales au Tchad. Elles auront lieu le dimanche 29 décembre 2024. Naturally, as governments navigate the complexities of fast-paced technological advancements that actively impact society, the need for a robust digital public infrastructure (DPI) and the effective use of digital public goods (DPG) becomes crucial.In this last instalment of our Podcast before the upcoming e-Governance Conference 2024, we spoke to Nele Leosk, Ambassador-at-Large for Digital Affairs in the Estonian Ministry of Foreign Affairs. Travel was second nature as a child due her fathers work and lived in Hong Kong and Malaya as a child. Having concluded her education Sharon was working as a nanny to a family who were serving Police officers. After 4 years she decided, aged 22, that she would join the metropolitan police.After a couple of years she moved to the TSG and to progress her career she went on to join the DPG and latterly Traffic. During her time she was on duty when she saw royalty and presidents! On traffic she was deployed as a family liaison officer where she would deal with the families of those who were killed or seriously injured.The way of dealing with work place stress often took place in the canteen and Sharon recounts her experiences within the canteen and her memories of the “Polystyrene” tea cups that were the focal point of the officers who used the facilities.Sharon also discusses the events in 2001 when the real IRA detonated a car bomb. Sharon was with her police friends and walking past as the explosion took place. During her time on TSG she was introduced to Jackie Malton and she was asked to take Linda Le Plante out on patrol when she was writing her book Trial & RetributionSince retiring from the police Sharon has worked within hospitality and works at iconic sites in Scotland and she has met Scottish Rugby President Dee Bradbury and has enjoyed the benefits of working in the hospitality world. We sit down with Long beach, CA wordsmith "Kxng Crooked". We discuss his evolution from an 8 year old child rap phenomenon, to signing with a local label, getting picked up, and subsequently dropped from Virgin records, signing to Death Row records over Aftermath-which caused a major beef between him and DPG. We then go into the formation of the supergroup "Slaughterhouse" consisting of Crooked, Royce the 5'9", Joel Ortiz and Joe Budden. After releasing their debut album independently they were signed to Eminem's Shady Records, but never released their album because of alleged interference and sabotage by group member Joe Budden,  which leads to Kxng Crooked stating "Joe Budden is not a good person".
