Yesterday Health Minister Mark Butler was forced to clarify that there will still be some patients who are charged a gap fee at the GP. Meanwhile at a state level, the LNP government are scrambling to redraw Queensland's hospital building plan after another scathing review. Australian Medical Association of Queensland President Dr Nick Yim told Luke Bradnam on 4BC Breakfast, "when the government did first make their announcement at the very start of the campaign... we've always said that it's quite unlikely that all GPs or 90 per cent of GPs will be bulk billing." "When we surveyed our members, they've told us that if they choose to universally bulk bill, that's actually going to be a loss of 20 to 30 per cent of their revenue." Incentives worth up to $70,000 luring Queensland Health workers to remote and vacant roles have been axed by the Crisafulli Government. The scheme was launched on July 1 last year by the Palaszczuk Government to address the critical shortage of doctors and clinicians in regional areas. Health Minister Tim Nicholls, said the reason behind the decision was a projected overspend of $165.8m from the program. AMAQ President Dr Nick Yim told Peter Fegan on 4BC Breakfast, 'it was quite perplexing from my perspective.' 'We know that in Queensland, we have a workforce shortage, and that's not just doctors,' Dr Yim said. 'And we're competing, not just in Queensland, but with other states in Australia and also the world. The other big thing is, those people who do go to regional rural areas, they will also reinvest into that community as well, because they are delivering that healthcare service,' he said. Dr. Lindsey Hughey // #ClinicalTuesday // www.ptonice.com  In today's episode of the PT on ICE Daily Show, Extremity Division Leader Lindsey Hughey discusses the significance of addressing the underlying ecosystem challenge to achieve better outcomes for patients. She specifically highlights the prevalence of poor diet and obesity as contributing factors to this challenge. Lindsey points out that there is evidence suggesting a link between these factors and knee pain, as overweight and obesity are often observed in individuals experiencing knee pain. Lindsey emphasizes that focusing solely on physical therapy interventions, such as knee range of motion and strength exercises, is insufficient. Instead, she argues that healthcare professionals, including physical therapists, need to consider the broader ecosystem in which patients exist. This includes addressing mindset, mindfulness, exercise, diet, and sleep. To guide patients along this path, Lindsey  suggests that physical therapists can play a role by providing support and education. She compares physical therapists to shepherds, who can assist patients in navigating and making positive changes in their overall lifestyle. EPISODE TRANSCRIPTION 00:00 LINDSEY HUGHEY Good morning, PT on Ice Daily Show. How's it going? Welcome to Clinical Tuesday. I'm Dr. Lindsay Hughey coming to you live from Edgerton, Wisconsin. So good to see you all today. I am going to chat with you about playing offensive medicine in our folks with degenerative meniscal injury. Before I dive in to what that looks like, I'd love to share with you a little bit about courses Mark and I have coming up in extremity management. So we have a couple options in August and actually one of them, well we did have a couple options, we only have one now because all the tickets in Fremont, Nebraska August 19th and 20th are actually sold out. So our last ticket went I think yesterday. So the only option in August to check us out and learn all things best dosage and tendinopathy care of the upper and lower quarter is Rochester Hills. So August 12th and 13th I will be teaching there and so join me if you can. And then in September Mark has two options for you on September 9th and 10th out of Amarillo, Texas and then September 16th, 17th out of Ohio. So Cincinnati will be coming your way. And then some fall and winter courses but again opportunities are dwindling. We hope if we don't see you this summer to see you in the fall or winter. 01:48 STOP THE SCOPE But let's chat about how do we play offense for degenerative meniscal injury because today is really a call, another call to stop the scope. I've hopped on here before over a year ago, I'm kind of charging us with those folks that have that gradual onset of symptoms of pain in their knee, maybe a little bit of swelling but have no specific injury or twisting event that happened that's more related to a degenerative process or like or I would like to refer to as a living life process. They don't need arthroscopic meniscectomy. And so we had more literature just come out this year to really bolster that argument of why physical therapy is really the number one choice, exercise medicine is the way to go. But I would like to first highlight that new literature that came out in January of why it's not appropriate to have surgery for these folks and then to also take a moment to reflect on why are we still seeing the arthroscopic partial meniscectomies being done if we keep finding literature that says let's not do this. And then also reflect on how can we do better as a profession to stop this continued over medicalization. So I first just want to briefly review in January 2023 we had a systematic review and meta-analysis come out from the Osteoarthritis and Cartilage Journal and we actually did share that on hump day hustling a while back. But this systematic review and meta-analysis again let us know that degenerative meniscal injury, the scope is not the way. And so let me unpack a little bit about this study because it was pretty inclusive this systematic review and meta-analysis. They looked at tons of RCTs so that the pool data of all patients was 605 patients. The study populations in each of the RCTs ranged somewhere between 44 and 319 so decent size overall in each study. The mean age of these folks was about 55 with the standard deviation of 7.5 so kind of that middle age and then majority were female about 52.4 percent. So you also see an even distribution almost of males and females in this study and then mean BMI was 26.5 standard deviation 3.7 you know below or above that. And what they investigated was the effectiveness of using arthroscopic partial manisectomy and they compared that via non-surgical so either sham which was exercise treatment or some form of exercise program so every RCT they looked at had to have the comparator of exercise. And degenerative meniscal findings were confirmed on MRI in all of the studies. The primary outcomes were knee pain, overall knee function, and then health-related quality of life and they looked at outcomes for up to two years so we see again a long-term follow-up in these RCTs this collection of RCTs that they looked at. And so the conclusions January 2023 so we're you know over six months out over half a year through and the conclusion was for insidious onset of knee pain so non-traumatic with MRI confirming degenerative meniscal tear in adults arthroscopic partial manisectomy is not the answer. 05:15 "NO CLINICALLY RELEVANT EFFECTS OF PARTIAL ARTHOSCOPIC MENISECTOMY" Literally if I'm going to quote verbatim no clinically relevant effect of arthroscopic partial meniscectomy was detected for overall knee function health-related quality of life or mental health. They did find one small marginal difference in pain levels a couple points but there was no evidence that there was superiority in having surgery. In fact they even took a look to see are there subgroups of patients right that might have a greater benefit from APM that were just not recognizing and when they looked and compared again the non-surgical to sham exercise therapy they did not see a subgroup that existed. They made other conclusions to say most degenerative meniscal tears are going to improve over time without the need for that arthroscopic partial menisectomy. Other findings that I think are really important to point out before we kind of reflect on why if we have this evidence do we keep seeing surgeries being done is that when they looked at the individuals in the studies those with BMI over 30 so obese individuals compared to the healthy BMIs less than 25 they had a 4.7 fold increased risk of progressing to knee osteoarthritis whether they had surgery or not. It was really a call to action when they found this in this pool data of all these folks is that body weight reduction strategies need to be on board for pain and function effects. 07:28 "...NO SIGNIFICANT ADVANTAGE OVER NON-SURGICAL TREATMENT" So just to send it home about this study and what they said one of the final things that they wrote in their conclusion was and I'm going to read it verbatim we recommend that physicians minimize the use of arthroscopic partial mastectomy to treat patients with degenerative meniscal tears because there is no significant advantage over non-surgical treatment. This is the osteoarthritis journal right this is a pretty high tier journal osteoarthritis and cartilage journal making this statement. So why are we still seeing a ton of them? Why does this keep happening where we see patients I have one of my caseload right now right why is this happening? Well we're obviously not reading the literature as a health care team and as physicians right because patients still think this is a primary defense. I'd love to reflect on that even 10 years ago in 2013 we had a study from Yim et al where they compared meniscectomy versus non-operative strength care and this was in 103 patients them and the same exact message was there there are no significant difference between arthroscopic meniscectomy and non-operative management with strengthening exercises again when we look at knee pain function and satisfaction at the two-year mark. So even 10 years ago we had this evidence but yet it's not translating to practice that's a lot of surgeries a lot of over-medicalization so I we need to really step it up here in our not only in ingesting this information but advocating that this is not a new message. In this article they point out that in 2017 so the systematic review and meta-analysis that we just reviewed that in 2017 an expert panel that regarding the degenerative meniscal injury said that the use of arthroscopic partial meniscectomy in nearly all patients with degenerative knee disease that several guidelines do not support this procedure. They've literally made clear statements against it again yet we're still seeing it so we can do better here and that probably takes some building relationships with surgeons right and chatting with them and letting them know like PT first get them to us right but really advocating that message in the community because we know that's not always going to work talking to the health care team. I think this message needs to be broadcasted widely more widely than it is currently. The other reason I think we keep seeing it besides like poor translation from what we're reading to the general public is there's this image mismatch so we see this a lot in the extremities and if you've been to our extremity of course you know we have a lot of conversations around this in different areas of the body shoulder hip knee but you see degeneration on the MRI right but there is no clear link that that's the cause of their pain symptoms it's an incidental finding but yet patients think oh you know my knee is really banged up right they leave hearing this message of harm rather than hearing you know I'm glad this is a normal age-related change so there's the image is linked inaccurately to pain and so again another opportunity to educate in this space and then the other reason I think that we keep seeing a ton of them being done regardless of what we know in the literature regardless of what we know that imaging doesn't tell the whole story is that there's this message put out about the fear of progression right if you do not get this meniscectomy you will go on to having knee OA or early onset knee OA which will lead to a knee replacement. 11:12 "IT'S DOING MORE HARM THAN GOOD" Let's stop allowing this message to be passed on it is harmful right it's doing more harmful than good and we don't actually know that right any fear-based messaging is not the way and so that message that is a thought virus and if our patients are coming into us or even like people in our community right our family or friends um we have to really um call BS on that right because we don't know that for sure and we're not seeing that link so finally kind of the background of the that we just had in January 2023 tell us that having surgery is not the way we've kind of reflected on why do we keep seeing this so what do we actually finally do about it well promote PT first faster right when someone's knee is starting to ache right stop ignoring it get into PT stop going to a medical provider even primary care orthopedic first come to physical therapy first so we can help you um with your hip and your knee pain and your um any associated muscle weakness or swelling so that we can get these healthy messages into our folks and into the community these folks get lost in the system letting them know that it is very common what they're experiencing and a plan for success that's our job that's our wheelhouse we need to manage expectation too so folks right some of our patients are going to want to do the surgery anyway right despite any of the things we can tell them about the evidence right they're set on it their belief and expectation it's going to help well i need you to manage those expectations as well because surgery after surgery i don't know about you all but all the ones i see doesn't actually take away their pain and swelling in fact the surgeons have actually told my patients you can expect swelling for up to six months which is literally the reason they came in there they want to feel better and they want the swelling to go away well guess what at least for six months it's not going to happen folks so letting them know that in a kinder less passionate way probably so while these folks might return to work or sport they're going to have ongoing symptoms and that's swelling so letting them know that that even if they opt for that it's still going to be a challenge they're still going to need pt so i tend to want to say why not play offensive time along those six months where you don't have to um respect healing time frames after surgery where we can really get after strength around that that knee and that hip the other thing we need to reflect on and how we can do better is that it's not just promote pt faster it's not just managing expectation but we have to understand the underlying ecosystem challenge that is present in a lot of these folks we see and especially in the systematic review and analysis that came out in january 2023 we see an underlying poor diet and we the reason we can know that it's related somehow to diet is it's we see overweight and obesity being precedent being present excuse me and so we have to understand that we have to intervene in these folks not just on knee range of motion and knee and hip strength and proprioception but we actually have to consider there's that underlying ecosystem piece and here's where pts can help too right we can help with mindset mindfulness exercise diet sleep and really guide them along that path as a shepherd we can help so we need to know that we can help right so some of us maybe don't even realize that our own you know 2018 cpg guidelines at the josp t let us know that exercise is medicine and whether patients do opt for surgery not that guideline really points out that supervised exercise so how many folks you see after arthroscopic partial metastatic go on they have the surgery and then the docs just give them a standard h.e.p. right so they go on having swelling quad like because they don't have an individualized program with progressive resistance exercise let those folks know too you need to be a part of their care in our own clinical practice guidelines say that it's not good enough to just do a an h.e.p. that's not tailored to the individual and then what that cpg highlights is we're always going to do a mix of hip and knee strengthening we will have manual therapy on board we will do proprioceptive activity and neuro re-ed for those joining this morning thank you to summarize where we are at when thinking about our degenerative meniscal care we need to advocate against surgery with that insidious onset of knee pain we need to share this evidence far and wide that it is not recommended as frontline defense we need to stop the fear messaging as a health care profession and let folks know that degenerative changes found on images are normal signs of living their life and that pain does not equate to imaging findings we need to dose hope and let folks know that at that two-year mark we can see just as great of improvements in pain function satisfaction of care with just p.t. right and i don't take the just p.t. lightly we don't need that overmedicalization p.t. first is the way i'd rather see a patient taking control of their ecosystem and knee health for two years rather than Juji shouts "I was right!" as Hooded Figure 1 dodges out of the way and stabs her three times with small daggers that drip with venom. Juji takes 22 points of piercing and poison damage.Reth, seeing this, charges forward through the crowd and shoves Hooded Figure 1 to the ground, getting a brief flash of a memory of Tish, Reth's mentor, cheering them on. Reth attempts to grapple the downed opponent, but the figure twists out of the way. Frustrated, Reth punches Hooded Figure 1 in the face. It's a crit! The face under the hood feels strangely hard as Reth punches it for 13 bludgeoning damage. Hooded Figure 1 uses a Legendary Action to shoot a web out from one of its hands and yoink itself 35 feet away, which doesn't incur opportunity attacks, landing directly next to Aboade.Aboade casts Branding Smite and shoots Hooded Figure 1 in the face, shattering its head and causing it to shrink down into a broken yellow stone. Hooded Figure 1 has been vanquished! Aboade scans the crowd, sees Hooded Figure 2 30 feet away, and shoots them for 10 piercing damage. The crowd starts to become alarmed by the sound of gunshots.Hooded Figure 2 ducks into the crowd, emerging next to Reth. Large, purple, wooden mandibles with yellow, dripping tips burst out of its hood, and it bites Reth in the shoulder for 10 points of piercing damage, forcing Reth to make a Con save. Reth succeeds and suffers no further effects. At this point, Hooded Figure 3 bursts out of the crowd and plunges its own set of mandibles into Juji for 13 points of piercing damage. Juji also succeeds on the Con save. As they're bitten, both Reth and Juji get disjointed flashes of memory: Reth leaving an arena in the Upper Realms with a pit in their stomach, and Juji climbing the Mahogany Tree in Wagadu, dreading what comes next. Hooded Figure 2 then uses a Legendary Action to thwip away into the crowd using its web.Juji activates her Star Chalice ability, which causes her markings to radiate like constellations and nebulae, boosting her healing abilities. She uses a 3rd level Cure Wounds on herself, regaining 14 HP, and also heals Reth for 12. She stands up, but stays put so she doesn't take an opportunity attack from Hooded Figure 3.Hooded Figure 2 then uses a Legendary Action to blast a cone of psychic energy at both Reth and Juji, forcing them to make a Wisdom saving throw. Juji succeeds, taking 5 psychic damage and suffering no further effects, but Reth takes 10 psychic damage and is stunned as they're trapped in a stupor of memories. A number of bystanders are caught in the blast, killing some and incapacitating others. Reth is forced to relive the memory of themselves walking through a seedy part of the Upper Realms city to see Faizah, who is behind a screen having a man tortured and maimed for having spoken against her in public.The crowd has fully started to panic at this point. Aboade sends a tiny metal plate from his clothing flying between Hooded Figure 3 and Juji, as he casts Sanctuary on Juji. He then shoots Hooded Figure 3 once for 13 piercing damage, and he misses his second shot. He tries to reassure the crowd, but his words don't have much of an effect on the panicked folks.Hooded Figure 2 comes running out of the crowd and stabs Reth 3 more times for 30 points of piercing and poison damage, but Reth is so deep in memories that they don't even feel it.. Hooded Figure 3 makes the Wisdom save to bite Juji again for 11 piercing damage. Juji again makes her Constitution saving throw and suffers no further effects. As Hooded Figure 2's turn ends, Reth comes out of their stupor. The last thing they hear before they come back to reality is Faizah walking up to them and saying "right, let's talk". Reth then uses their Bloodied but Unbowed feature to regain all their Moxie points and a bunch of temp HP.For her turn, Juji uses Healing Word on Reth for 10 HP, then uses her Chalice ability on herself for 5 HP. Juji then produces her holy symbol: a piece of the Mahogany Tree, and uses her Channel Divinity ability Twilight Sanctuary, giving 7 temp HP to every ally and bystander within 30 feet at the end of each of their turns.The crowd is now fully panicking and fleeing the area, forcing all combatants to make either Strength or Dex saves of DC 10. All our heroes succeed, but Hooded Figure 2 gets knocked prone. Everyone now has 1/2 cover against ranged attacks. One of the Hooded Figures uses the psychic blast Legendary Action again, dealing 6 psychic damage to Juji and 11 psychic damage to Reth, stunning Reth yet again. Reth returns to the memory of Faizah, who seems surprised to see them.Aboade keeps blasting away, completely shattering Hooded Figure 3 and turning it back into a yellow stone. He then commands Yim to use Thorn Whip on Hooded Figure 2, who shoots out a spiced yam-covered vine and pulls it closer to him, dealing 10 points of piercing damage. Hooded Figure 2 runs back to Juji and bites her again for 7 piercing damage. Yet again, Juji makes the Con save and suffers no additional effects. Hooded Figure 2 flees back into the crowd as Reth snaps out of being stunned. Hooded Figure 2 then uses a Legendary Action to web itself another 35 feet away. Juji, not wanting to let it escape, rolls a Nat 20 on a 2nd level Guiding Bolt and obliterates Hooded Figure 2 with 33 points of radiant damage. Juji then rolls another Nat 20 on her Withered Woman check, and the DC goes up to 2. The Hooded Ones are vanquished!Unfortunately, the crowd is still surging, and all our heroes have to make Str. or Dex. saves again, this time DC 12. Juji fails and is knocked prone, taking 4 bludgeoning damage as terrified citizens run over her. Reth picks through the crowd, runs over and picks up Adofo, who had been knocked over by the crowd, and sees Khadija lying prone, possibly unconscious. Reth uses the rest of their movement to run over and pick Khadija up as well. For their part, Aboade and Yim start pulling bystanders to their feet. The crowd surges again, knocking Aboade and Yim over, but Reth uses their movement to Dash through the crowd, set Juji, Adofo, and Khadija down safely, and then run back to help Aboade and Yim as the crowd runs out of the area. With our heroes and their allies safe, the crowd clears the area and the guards arrive. The battle is over! If they aren't appropriate for going deeper into determining why the patient is not well Dr. Yim has many tests he can draw from that relate to the individual. For example, if a welder comes in with signs of autoimmunity he will suggest a heavy metal hair analysis or urine toxic metal test to determine if heavy metals may be causing issues. After passing through a series of deeply unsettling memories, our heroes find themselves in a grove filled with even more mysteries. Something has brought them here, and whatever it is doesn't want them to leave... CONTENT WARNING: This episode contains a depiction of a parental death, as well as vague references to alcoholism. Post-production by Seth Leue and Daniel Ramos EPISODE SUMMARY: Our heroes find themselves in a grove surrounded by incredibly thick vines. The light is very low. There are items scattered all around them, mostly covered by the thick undergrowth. In the center of the grove stands a cracked idol, surrounding the idol is a beautiful arrangement of yellow stones, and kneeling at the edge of the arrangement an elderly Emere man. The man's body is connected to the idol by vein-like vines that are attached to each of his major articulation points (elbows, shoulders, knees, etc.). As Juji panics, and Reth is in shock, the party regroups. No one is quite sure of how they got here. They suspect that what they saw were illusions drawn from their memories. Juji reveals that the giant tree that she and Aboade fell down was from her past. She used to live in the tree, and they shared a powerful bond, but then things "went awry". Aboade and Reth note that the people they encountered in their memories all seemed to be made of some sticky substance. Aboade reveals that he has no memory of Dagbeor the automatons, although they did seem familiar and he did notice that the automatons bore a strong resemblance to Yim. Reth interrupts their speculation by going over to inspect the elderly Emere man, whom the group surmises is likely Jimoh. Reth walks over to Jimoh and touches him to test if he's sticky. While he thankfully isn't stick, Reth is drawn into his memories the moment they touch. They see how Jimoh fell to Wagadu as a child and was found by other Emere who were dressed very differently than the other Emere Reth has seen thus far. Among the Emere was another boy with whom Jimoh felt an immediate connection, like love at first sight. Reth relates this to the group, so Juji checks the vines and sees that they appear to have pierced into Jimoh's body and are forming some kind of vine network under his skin. The vines piercing Jimoh, as well as the vines covering all the objects spread around the room, all originate from the idol. Aboade decides to try and strike up a conversation with the old man, who has yet to react in any way to the party's presence. However, on his way over he trips on a vine and sees one of the overgrown items on the ground. It's an extremely old kalabash with an inscription in a language Aboade doesn't understand. Aboade rolls a 19 Calligrapher's Tools check to copy down the inscription, and then he decides to investigate the arrangement of yellow stones. He takes out his stone to compare, and immediately notices its similarity to the ones in the arrangement. He rolls a 27 Investigation check to notice that there are numerous, possibly hundreds of stones missing from the formation, which at one point might have covered the entire grove. The arrangement itself seems to be a 2-D rendering of a complex 3-D shape. Aboade decides to try and place his stone into the arrangement, but his task is made more difficult by the thick undergrowth among the stones. He accidentally knocks one of the stones out of place, and suddenly the stones all sprout legs and begin scuttling toward Aboade. Time to roll initiative! As the stones come to life, 8 thick vines rise up around the idol and begin lashing out at the party. One hits Aboade, sprouting smaller vines that wrap around his arm and grapple him, but two others miss Reth and Juji. Unfortunately, a fourth vine manages to whip around and successfully grapple Reth as well. Both Reth and Aboade are then dragged forward toward the idol. Aboade, who was closer to the idol, is dragged all the way to it and is forced to make a Wisdom saving throw, which he succeeds. Reth successfully tears away from the vine, breaking the grapple. The rocks swarm over Aboade, but their attacks are unable to break through his protection. Aboade uses his Ironmaster fetish charm to completely sever the vine that was grappling him, leaving it a smoking stump. He uses his second attack to swing at the spiders, but he fails to hit their AC as they dance over the blade of his weapon. Aboade then turns and tries to run, making it through the difficult terrain to the edge of the formation. Seeing all of this, Juji bends down, collects some of the twisted old vines, and casts Wither and Bloom, restoring HP to Aboade, deactivating a huge portion of the spidery rock swarm and damaging the vines. After dealing this damage, she has to make another one of her mysterious, unexplained rolls, which she succeeds on. At the top of the next round, the vines attack again, successfully grappling all 3 PCs. They dig into Reth's temple, forcing Reth to make a Wisdom save, which they fail. Upon failing the save, Reth is forced to relive a traumatic childhood memory of their parent, implied to have been an alcoholic, passing away in their sleep next to Reth when Reth was very young. Reth remembers staying in the room with them for a full day before leaving home into an uncertain future in the big city. As a result of this horrific memory, Reth gains another level of "psychic exhaustion", which essentially places them under the effects of the Slow spell. Their speed is halved, they suffer a -2 penalty to AC and Dex saves, they can't use reactions, and on their turn they can either use an action or a bonus action, but not both. Reth is completely demoralized by this vision, and their half-hearted attempt to break the grapple fails as a result. The spidery rocks swarm over them, dealing 5 points of piercing damage. Seeing Reth in distress, Aboade pulls out his pistol and begins blasting at the swarm, hitting both times and dealing 24 damage. Aboade's first shot deactivates the entire swarm, and his second shot shreds the vine, freeing Reth once more. With his bonus action, Aboade commands Yim to charge in and attack the idol itself. Yim bounds springily through the undergrowth and tears into the idol, dealing 6 points of force damage. Juji fires a 2nd level Guiding Bolt at the idol, but misses. She narrowly succeeds on another mysterious roll, and the air around her grows bitterly cold as the mysterious "she" approaches. On the vines' next turn, one successfully re-grapples Reth. Then, it's Juji's turn to fail a Wisdom save, which plunges her into the memory of a traumatic family dinner. Her entire family (a mother, father, two older sisters, and two older brothers) looks down on her for her lack of academic acumen. They devalue her again and again throughout her life, negatively comparing her to her siblings and constantly being disappointed in her. Juji takes a level of psychic exhaustion as well, giving her disadvantage on ability checks. Reth tries to kick out at the idol, but they miss yet again and stub their toe. Reth is feeling broken, lost, and hopeless as a result of their latest memory. Some of the spidery stones reactivate and begin attacking Aboade, but once again completely fail to harm him. Aboade shoots his pistol at the idol twice, doing 11 damage and blasting off a chunk, then Yim runs over to attack the spidery rocks, dealing 10 force damage and deactivating almost all of the stones once more. Aboade disappears under a flurry of coils as Yim goes to town. Juji uses her inspiration (gained for making a pun in a previous episode) to fire a 3rd level Guiding Bolt at the idol, but rolls two 4s and misses. She then rolls a Nat 1 on her mysterious roll, and even more chaos breaks loose. From Juji's perspective, a piercingly cold squall surrounds her as shriveled, gray hands grasp her shoulders. From Reth and Aboade's perspective, Juji is sucked screaming into a black hole that opens up inside of her her, then a shriveled, dessicated, terryfying woman drags herself out. The Withered Woman has arrived! Her eyes are so dark that they seem to swallow light rather than reflect it. From inside the black hole, Juji can see the flesh on her shoulders begin to decay where the Withered Woman is touching her. The Withered Woman is chaos incarnate, and her necrotic energy is lashing out, threatening to harm Juji's friends. Juji rolls a 19 on a Wisdom Save to divert the energy away from her friends and towards the idol, dealing 24 necrotic damage to it and causing it to begin to crumble. At the top of the round, Aboade succeeds on a Wisdom Save, but he, Reth, and the Withered Woman are dragged in towards the idol, which begins to glow. On Reth's turn, they remember Nzito's words and call out to the Forest Father for help. They roll a pivotal a Religion check and, bolstered by Nzito's love, succeed! The ground rumbles, the wall of vines that surround the grove shredded, and a glowing manifestation of the Forest Father bursts into the grove. He stands 10 feet tall, his entire body covered by hundreds of upside-down palm fronds. He glides into the room on rapidly dancing feet, draws a bow made of two rainbows tied together, and fires a glowing arrow into the heart of the idol. The idol shatters, the yellow stones go flying, and all the attacking vines shrivel and dissolve into light. The Withered Woman is also banished back into the black hole by this incredible power, and Juji reappears, still bearing faint marks where the Woman had touched her. The Forest Father says nothing, but gestures warmly to the party, shoots them a thumbs up, and then dances into the sky. Reth kneels as he leaves, Aboade offers a meek wave and a "er, thank you!", and Juji sits in a catatonic state. Yim also waves at the retreating Forest Father, looks around at the group and says "well, snap!" Our guest today, Roger Yim, has over 430 weeks of leading and instructing wilderness-based expeditions, focusing on leadership, wilderness skills, personal development, environmental stewardship, and expedition behaviour. He started working in outdoor education in the early '90s. In 1996 Roger began working for the Canadian Outward Bound School, and it 98; Roger took his NOLS River Instructors course and work 18 years with the school. He mostly worked on ski, mountaineering, rock climbing, avalanche, and whitewater courses but has led several rock climbing caving and sea kayaking courses.  Now, Roger lives in Nelson, British Columbia and works as an ACMG ski guide in the winter. This summer, Roger is excited to return to NOLS, leading course in Alaska after a few years hiatus from the school.  In this episode: (02:38) Roger talks about the day of the helicopter food drop on a Yukon mountaineering course and how the weather took a drastic turn, leading them to have no food for two days. But since desperate times call for desperate measures, before they completely ran out, the group shared whatever food they had left and ate it together, forming a strong bond between the members and help them survive during the difficult times ahead. (20:40) Roger shares the story about running out of coffee mainly to share the  “real truth.” This may not be a strange scenario in most cases, and everyone runs out of caffeine eventually but what makes it funny is that the rumour wasn't even the real truth. While leading a hiking course, Roger explains that he had his final paperwork flown in with the last food ration. Everything seemed perfect until he realized that he forgot to include every evaluation. Coming closer to the pickup date, Yim had no choice but to compromise his pride to do a call from the sat phone, and instead of stating the ‘real deal' he ended up complaining about the shortage of coffee and requesting them to send a few packs along with the folder he left in the filing cabinet – It's even more hilarious in his narration. (26:11) Roger shares yet another funny story about his companion KG and the giraffe. KG being the most incredible and fantastic instructor, had an entertaining way to educate the students by stating facts about every big animal that walked the forest of Kenya – starting from giraffe all the way to rhinoceros. Not only leaving students hysterical but also the other instructors in the fit of laughter because this class on a NOLS course in Chile! (28:44) Roger takes us back to his experience of Denali Highway and some dog mushers. While his students were on their small group expedition without Instructors on an Alaska Mountaineering NOLS course, he and his pal Andy stumble upon a dog musher company owned by an ex-model. Not able to contain their excitement to meet a model in real life, they paid all their savings for the tour to meet the beauty – but end up disappointed when the model never shows up, and the short-lived tour turns out to be just visiting dogs. (38:35) In the end, Yim shares a story behind “Runaway in Vegas.” He tells us when one of the students was kicked out of the course for breaking the no drug rule, and he was left to take the responsibility to take the student back to the NOLS branch. He further explains how the rebellious kid runs away when hiring a rental car and was nowhere to be found. The story is filled with more thrill and suspense, but as they say, hard work only pays off when it meets the right plan of action – the same goes with this one.

Abstract: Cannabidiol (CBD) and {Delta}9-tetrahydrocannabinol (THC) are two major constituents of cannabis with contrasting mechanisms of action. THC is the major psychoactive, addiction-promoting, and psychotomimetic compound, while CBD may have somewhat opposite effects. The brain effects of these drugs alone and in combination are poorly understood. In particular the striatum is implicated in the pathophysiology of several psychiatric disorders, but it is unclear how THC and CBD influence striato-cortical connectivity. Across two placebo-controlled, double-blind studies, we examine the effects of THC, CBD, and THC+CBD on the functional connectivity of striatal sub-divisions (associative, limbic, and sensorimotor) using resting-state functional Magnetic Resonance Imaging (fMRI) and seed-based functional connectivity analyses. Study 1 (N=17; inhaled 8mg THC, 8mg THC+10mg CBD, placebo) showed strong disruptive effects of both THC and THC+CBD conditions on connectivity in the associative and sensorimotor networks, but a specific effect of THC in the limbic striatum, which was alleviated in the THC+CBD condition such that it did not differ from placebo. In Study 2 (N=23, oral 600mg CBD, placebo) CBD increased connectivity in the associative network, but relatively minor decreases/disruptions were found in the limbic and sensorimotor. In conclusion, THC strongly disrupts striato-cortical networks, and this effect is selectively mitigated in the limbic striatum when co-administered with CBD. When administered alone, 600mg oral CBD has a more complex effect profile of relative increases and decreases in connectivity. The insula emerges as a key region affected by cannabinoid-induced changes in functional connectivity, with potential implications for understanding cannabis related disorders, and the development of cannabinoid therapeutics. Abstract: Many digestive disorders have prominent sex differences in incidence, symptomatology, and treatment response that are not well understood. Irritable bowel syndrome (IBS), for example, affects approximately 10% of the population worldwide and tends to have different manifestations in males and females. Androgens are steroid hormones present at much higher levels in post-pubertal males than females and could be involved in these sex differences, but their normal functions in the bowel are largely unknown. Here, we show that gonadal androgens are required for normal gastrointestinal motility in vivo. In the healthy mouse gut, we detected androgen receptors in smooth muscle cells and a subset of enteric neurons. Surgical or genetic disruption of androgen signaling in adult mice selectively and reversibly altered colonic motility by affecting neurons rather than smooth muscle. To determine if androgens also influence human bowel function, we measured androgen levels in 208 adults with IBS. Free testosterone levels were lower in patients with IBS compared to healthy controls and inversely correlated with symptom severity. Taken together, these observations establish a role for androgens in the regulation of colonic motility and link altered androgen signaling with a common digestive disorder. These findings advance the fundamental understanding of gut motility, with implications for normal aging and disorders involving the gut-brain axis. Copy rights belong to original authors. This episode is one that is very close to my heart. Due to MCO, most musicians/performers are not able to perform at gigs/shows/concerts. So I figured why not have them on the podcast to not only promote themselves but their craft as well. The very first name that I thought of was Bazli, he is a long time friend of mine and was one of the reasons why I got into performing as well. I honestly wouldn't be able to play the guitar well enough if it wasn't for him. He used to play in a band called One Night Service when he first started gigging back then but now he sessions for notable names like Bil Musa and occasionally perform with his blues band. I have not caught up with Bazli for years but every now and then I'd still rock up to his shows. This is probably the most we've spoken over the years of not seeing each other and this podcast is pretty much where we relive our old days, his music, what's happening with our lives, a impromptu jam session, him playing the blues and a free guitar lesson. He is the CEO and executive creative director of Trigger Global, the mixed reality agency. He has creatively led over 150,000 hours of development in mixed reality, including as a Snap Lens Studio partner, preferred developer for Facebook, and showcase developer for Euphoria and Google, as well as an early adopter and early developer for Magic Leap. Jason, welcome to the show. Jason: Alan, thanks for that kind introduction. Alan: It’s amazing, just that introduction; you think “Holy crap, you’ve done work with Honda. You’ve done work with Lego. You’ve done work with Snapchat, and Facebook, and Google.” It’s crazy, the things that you’ve done. And you joined us on stage at AWE this year, to talk about supercharging your marketing. Tell me about some of the things you guys are working at right now. Jason: Yeah, I think for us on the marketing side it’s actually quite an interesting time. We’re seeing basically the market maturing a little bit and then kind of dividing into two big chunks of work. On the introductory to AR side of things, we have the social lenses. So that’s the Snap/Facebook/Instagram approach, where it’s a small experience for a smaller budget and it’s going through someone else’s app, but it’s a much larger user base, which is a good way to start it off. And then the other group of projects that we work on are kind of larger development, where the brand can own their own app or they have an existing app and we’re pushing an AR module into that existing app. Alan: Let’s break those into pieces, here. The first one you mentioned is smaller ones with social lenses. Can you maybe talk about some of the work you’ve done in that? Jason: Sure. We were one of Snap’s first agencies — the Lens Studio partners. We actually were kind of a guinea pig as they were developing the Lens Studio itself. I believe we’re probably one of the first people outside of Snap to actually use the tool. On the client side, we’ve worked with everywhere from Adidas, Pepsi, the NFL, all from sports and brands on the lens side. On the Snap side, typically they are coming to us.
