Today's guest has a unique perspective as she's both a high performance athlete AND a pelvic physio who is passionate about keeping girls and women involved in sport.Clodagh McIntyre is a rising star in physiotherapy who is both an inter-county camogie player for Tipperary and a pelvic health physiotherapist. Clodagh discussed her journey from UCD graduate to working in private practice, her experiences with stress incontinence as an athlete, and the lack of pelvic health education in physio training programs. We explored how pelvic health issues affect female athletes, with statistics showing 7 out of 10 athletes leaving sport due to bladder leakage, and discussed the taboo nature of discussing these topics within sporting teams. We discussed the importance of education and support around pelvic health issues for female athletes, highlighting the lack of standardization in GAA and potentially LGFA, as well as emphasising the need to normalize conversations about these issues and prevent them from deterring athletes from participating in sports.Clodagh shared her upcoming trip to Kenya with Warriors for Humanity and Self-Help Africa, where she plans to plant trees and help communities, for which she is fundraising. "Radiation therapy is often extremely well tolerated in colorectal cancer. Technology has really changed things. But location of the tumor can affect side effects, such as radiation dermatitis. If a patient has a low-lying tumor, if it's less than six centimeters from the anal verge, the patient is likely to have some skin reaction. Highlights From This Episode "In recent years, there has been more nonsurgical management of rectal cancer, especially in what we call the low-lying population. This is the population of patients who would likely end up with a permanent colostomy because their cancer is so low in terms of being close to or involving the anal verge. There is now a regimen where these patients can get their chemotherapy followed by their chemoradiation and then be monitored on close surveillance without surgery." TS 2:23 "Another assessment would be to assess what effects have they had from their chemotherapy that they're bringing with them. FOLFOX-based treatment is commonly used, and the platinum therapy oxaliplatin often causes peripheral neuropathy. What is the patient having? What are those symptoms like? Are they having peripheral neuropathy? If they are that is likely not going to get better or improve during their whole course of radiation. In fact, sometimes when oxaliplatin therapy stops, the peripheral neuropathy can get worse as patients are going through other treatments." TS 5:42 "If the patient has a low-lying tumor, if it's less than six centimeters from the anal verge, the patient is likely to have some skin reaction. It's good to be proactive if that's the case. And then proactively minimizing radiation dermatitis effects, such as keeping the area clean, good washing of the area, and prophylactically starting them on or having someone start them on steroid creams a couple of times a day to minimize that radiation dermatitis effect in the long run." TS 7:25 "I have a sexual health clinic for women with these effects. It's very important as nurses that if you can develop the comfort to ask patients about their sexual activity—it's hard, but it really needs to be done. And I will tell you that the healthcare providers are not doing it. They don't have time, and like us as nurses, we don't get this in school, and neither do they. The other providers don't get it in school either, but it's important. Patients are getting more and more worried about their sexual health. They're coming to us at a younger age, and this is really, really important to address." TS 15:35 "I would say that working with your advanced practice providers and education for advanced practice providers has definitely been focusing on [sexual health] more. Your PAs and your NPs—I think they're going to have the ears and the wherewithal to be able to be your allies and colleagues in this. By and large, it's my APP colleagues and nursing that I talk to the most about this. … Again, it's not an easy thing to bring forward, having dilators in place. But I will tell you in the department that I work in, it was me and couple of nurses who pushed this issue with the physicians for two years and finally got it put in place. It can be done. There's a lot more centers out there doing that." TS 21:51

Hello & Welcome to today's episode where we are focusing on new research that really helps us change the conversation about bladder leakage in runners, and why we need to move beyond isolated pelvic floor muscle exercises...I'm in conversation with Natalia Cardoso Campos, who published a really interesting paper in 2025 'Inefficient impact absorption and reduced shock attenuation in female runners with stress urinary incontinence'Natália is a physical therapist from Brazil, and in our chat we presented findings from her master's thesis showing that female runners with urinary incontinence have poor shock absorption capacity at the ankle and significantly reduced shock attenuation compared to continent runners. Her research revealed that while continent runners had shock attenuation values of around -29, incontinent runners had values of only -6, indicating much more energy reaching the pelvic region. We discussed how this biomechanical imbalance could explain why some runners experience urinary leakage despite having normal pelvic floor strength, suggesting that the problem originates in the lower kinetic chain rather than the pelvic floor itself. We explored:urinary incontinence research in runnersshock absorption & shock attenuation in runnersfoot strike patterns, ankle injuries and asymmetry & how conditions like REDS (Relative Energy Deficiency in Sport) and bone stress injuries can exacerbate these issuesSome key takeaways for me were the importance of looking beyond the pelvic floor muscles when we are working with women, especially athletes, with pelvic health issues - considering the ankle, knee and hip as well as systemic issues like hypermobility and REDs.I can't wait to see what Natalia's PhD research on this topic reveals!We also talked about a new paper she was a co-author on, 'Sexual Trauma and Pain: A Study of Women's Experiences With Vaginal Dilators and Perineal Massage in Trauma-Informed Pelvic Floor Physical Therapy' (Modesto et al 2026)Natália shared insights from this qualitative study on women's experiences with vaginal dilators and perineal massage in trauma-informed pelvic floor physical therapy, underscoring the significance of patient rapport and validation in treatment.Want to learn more? Hello & welcome to today's episode where we're talking about a new paper looking at menstrual health in female rowing - I'm in conversation with lead author, Dr Ciara Everard, who is an assistant professor at Trinity College Dublin's physiotherapy programmeIn our chat, we discussed her recent research paper focusing on menstrual health in female rowers, from Ireland & the US, highlighting how sociocultural and gendered norms influence how female athletes experience and manage their menstrual cycles. The research, involving ex-international rowers, explored menstrual cycle pain and symptoms but found that gendered norms in the male-dominated sport played a significant role in the findings. Ciara explained that female rowers often feel illegitimized within the rowing culture, as they are perceived as weaker than their male counterparts.Key findings included that 70% of respondents experienced irregular cycles, 80% reported menstrual symptoms affecting performance, and 70% had fears about leakage, though only 50% discussed these issues with their coaches.The conversation highlighted that while some athletes showed interest in tracking their cycles, there was a gap between existing knowledge in the wider literature and implementation in sub-elite contexts.Ciara & I discussed findings from a research paper on female athletes, highlighting issues around education and misinformation regarding menstrual health. We noted that harmful discourses, such as the "slim-to-win" ideology where missing periods is valorized, still prevail among younger athletes. We also talked about the silencing of symptoms related to conditions like endometriosis and PCOS, which can delay athletes from seeking help due to fear of being perceived as weaker or affecting their selection for teams. Despite increased awareness, athletes still face issues related to fueling and period-related pain. There's a huge need for continuous conversations and education at all levels of sport, including the importance of sharing personal stories to help athletes relate to the information. Why do so many women with chronic pelvic pain get told "this is just motherhood" or "it'll get better when you go through menopause"? What if the heaviness, the aching, the constant discomfort isn't something you have to live with?In this episode, I sit down with Dr. Julie Baron, a pelvic floor physical therapist and director of Pelvic Health and Performance Center in Bellevue, Washington. Dr. Baron blew our minds at ISSWSH this year with her groundbreaking lecture on pelvic venous disorders a condition that's massively underdiagnosed and often dismissed as "just in your head."Dr. Baron shares her own lived experience with pelvic venous disease. For years, she couldn't sit or stand for more than 10 minutes without needing to lie down. She saw urologists, gynecologists, GI docs, colorectal specialists, and pelvic PTs and everyone told her she was normal. She was working as a pelvic floor PT herself, helping other people with pelvic pain while feeling like a fraud because she couldn't solve her own.She finally diagnosed herself, pushed her way into getting the imaging no one wanted to order, and finally felt validated after the report came back showing renal vein obstruction, iliac obstruction, gonadal vein reflux, and 12-millimeter varicose veins across her uterus. After gonadal vein embolization and sclerotherapy, her life changed completely.We discuss the classic presentation of pelvic venous disorderschronic non-cyclical ache or heaviness that gets worse throughout the day, urinary urgency, postcoital pain, and varicose veins. We talk about why pregnancy is one of the biggest risk factors and how hypermobility disorders, MCAS, and POTS all connect. Dr. Baron explains the imaging process and why a normal ultrasound doesn't always mean you're fine.She also explains the five functions of the pelvic floor and how pelvic venous disease impacts everything from bladder support to sexual function. We get into the sump pump concept, why belly breathing can actually make things worse for this patient population, and how compression shorts can be life-changing. Today's conversation is with Lydia Durkin George, all about pelvic health in women's rugbyIn our chat, we discussed Lydia's journey from being an elite kickboxer to becoming a physiotherapist, driven by a desire to support female athletes. She highlights her research on pelvic floor dysfunction in elite women's rugby players and her collaboration with advocates for female athletes. We discussed the prevalence of pelvic floor issues among elite women's rugby players, noting a high incidence of urinary and anorectal symptoms despite a lack of childbirth history. We also highlighted the need for better awareness and education about pelvic health, as only 30% of the athletes knew what a pelvic health physiotherapist was and only 10% engaged in pelvic health exercises. I'm so pleased to share this conversation with high performance rehab specialist, SJ McDonnell, who is leading the charge to integrate pelvic health into sports medicine at Sport Ireland.Our paths crossed many (many) years ago but we reconnected when SJ and I started having conversations about how to bring pelvic health into high performance sports - how to bring expertise into this field in a thoughtful way.We discussed lots of different ways to empower and educate athletes, coaches & medical staff about assessing and managing pelvic health conditions, including:the importance of the MC in sports & the challenges of bringing a nuanced approach to menstrual healththe level of information about pelvic health that is available to athletesthe difficulty with the diagnosis and management of LEA or dysmenorrhea in athletes who have limited access to medical appointmentsthat athletes may not seek help for a variety of reasonsand how a one size fits all approach to menstrual health is not a good strategy & how more training & understanding are required to improve the skills practitioners need to better support female athletesUltimately, there is a definite need to do a better job at integrating pelvic health into sport at every level, with the inclusion of psychological health into this domain as well, as there is a complex interplay between physical performance and mental wellbeing. SJ is leading a great team at Sport Ireland and is driving the conversation about creating a safe space for athletes to discuss menstrual & pelvic health.
