Nikomur, ki je pred tremi tedni sam ali s svojo družino zašel v decembrski mestni šunder, ne želim reči nič kritičnega. Vsakomur, ki mu prijajo mrzleči decembrski večeri v mestnem središču, kjer je zrak nasičen z vabljivim vonjem po kuhanem vinu in mesninah, iskreno privoščim dobro počutje in veselo druženje. Gneča na okrašenih ulicah in velike smreke z drobnimi lučkami ustvarjajo intimo, kjer bi se želeli zadrževati v neskončnost in bi stisnjeni k ljubečemu sozakoncu in otroku, obsijanemu s srečo, da je z nami, pozabili na vse tegobe iztekajočega se leta. Naše duše te decembrske večere napolnjuje skrivnostno pričakovanje. Toda, do kam sega hrepenenje, ki nas prežema, ko se sprehajamo ob stojnicah in brezskrbno pogledujemo, kaj od tega, kar je na njih, bi si glede na globino svojega žepa lahko privoščili. Kaj, razen tega, da bi ostali zdravi in z zalogo na bančni kartici, v resnici hočemo? O tem nas nihče nič ne vpraša, ne sozakonec ne otrok ne prijatelj. O tem najglobljem, kar bi želeli razumeti in občutiti sredi decembrskega šundra, vladata zadržanost in molk, ki se nadaljujeta tudi prek novoletnih praznikov in nato skozi vse leto. Ali je bila v to nerazložljivo hrepenenje, ki tiči globoko v nas, že kdaj izrečena beseda, ki bi dajala pomen življenju tudi potem, ko v poznih večernih urah zapuščamo mestno središče? Danes, na četrto adventno nedeljo, bodo verniki v svojih cerkvah slišali neobičajen pozdrav, ki ga je Mariji namenila njena teta Elizabeta, mati Janeza Krstnika: Blagor ti, ki si verovala. Ko si prestopila prag mojega doma, se je od veselja zganilo dete v mojem telesu. Obe noseči ženi sta v tistih zelo skromnih razmerah pred dva tisoč leti z očmi vere videli daleč prek sebe, v brezdanjost Božje ljubezni. Plamen vere je v njima oznanjal, da Bog ni pozabil nanju. Oni dve sta to verjeli. Ljudje našega časa pa imamo s tem težave. Ne zaupamo ničemur, kar presega domet naših oči in predvidevanja. Vsak dan se teže zanesemo celo na tistega, s katerim imamo otroke, in s katerim želimo biti družina. Kaj bi lahko povezalo naše skrhane odnose, zaradi česar nad vsem, kar delamo, visi senca dvoma in nesmisla. Marija in njena teta Elizabeta sta v vaški shodnici večkrat slišali preroka Izaija, ki je govoril: »Naj vriska puščava in goljava, naj poganja kakor narcisa.« Vedeli sta, da v puščavi ne bo pognala narcisa, sta pa v sinagogi doživeli, da se je v njuno dušo po veri prelivala Božja milina in njegova tolažba. In o tem danes znova beremo v cerkvi, da se v nas na kar najbolj preprost način ponovi vse tisto odrešujoče, kar je dajalo moč tudi Mariji in Elizabeti.

Poletno obarvana sobota je znova zgostila promet na slovenskih cestah. Zdaj so se razmere že umirile, zaradi konca praznikov v Avstriji so zastoji le še pred predorom Karavanke. Gnečo proti Hrvaški in v nasprotni smeri je pričakovati vse poletje, ob tem opozarja AMZS. Po ocenah evropskih avtoklubov pa bi bilo lahko tujih voznikov, ki potujejo proti turističnem krajem, letos poleti za 10 odstotkov več kot v rekordnem letu 2019. Drugi poudarki dneva: - Tadej Pogačar pred zadnjo etapo kolesarske Dirke po Slovniji zadržal zeleno majico. - Protestniki v Londonu zahtevali ukrepanje zaradi vse višjih življenjskih stroškov. - Poletna muzejska noč že dvajsetič brezplačno odprla vrata kulturnih ustanov.

Evergrande Group, China's second biggest property developer, managed to rack up staggering debts of more than $432 billion dollars, on the brink of collapse. However, at the very last minute managed to ink a deal to keep it afloat. News of this deal helped the iron ore price surge and Aussie materials stocks make steep gains, lifting the broader market by 0.32%. In the US, all three benchmarks closed in the green. This comes after the Fed announced it was not ready to remove stimulus yet. While no specific timeline was provided as to when it may begin moderating its purchases, in the Fed's post-meeting statement, they noted “if progress continues broadly as expected, the Committee judges that a moderation in the pace of asset purchases may soon be warranted.”Following a strong night of trade on Wall Street, the futures are suggesting the Aussie share market will open 0.19% higher this morning.What to watch today:In economic news, the Manufacturing PMI Flash data and the Services PMI Flash data will be released today at 9am AEST.Companies ex-dividend today include Eagers Automotive (ASX:APE), NRW Holdings (ASX:NWH), Cochlear (ASX:COH) and Genesis Energy (ASX:GNE).Suncorp Group (ASX:SUN) holds its AGM today and Brickworks (ASX:BKW) are set to report today.The most traded stocks by Bell Direct clients yesterday included Zip (ASX:Z1P). Its shares lifted 4.3% yesterday to $6.51 following investors responding positively to the company's strategic US$50 million investment in India-based BNPL operator ZestMoney. ZestMoney was founded in 2015 and is now one of the largest and fastest growing BNPL platforms in India. Another highly traded stock yesterday by Bell Direct clients was Fortescue Metals (ASX:FMG). Its shares lifted 4.2% yesterday, bouncing off 14-month lows following the news that China's embattled real estate developer Evergrande will meet its bond interest payments due today. The oil price climbed more than 2%, following a greater than expected drawdown of US supplies. The gold price fell after the Fed signalled a sooner than expected interest rate hike, while the palladium price lifted over 6%.  Trading ideas:Bell Potter has maintained its BUY recommendation on iron ore company Fortescue Metals Group (ASX:FMG), however have decreased its price target to $20.87 (previously $22.52). At its current share price value of $15.37, this implies about 36% share price growth.Bullish charting signals have been identified in De Grey Mining (ASX:DEG), Aussie Broadband (ASX:ABB) & REA Group (ASX:REA), according to Trading Central.

It's a great time to talk about how you can improve your English fluency with the top 10 phrasal verbs. So, I'd like to share these with you. I'm not holding back any secrets. These are the top ten phrasal verbs that you need to know and examples of each. Now, first of all a phrasal verb is a verb plus a preposition. And often in English, that preposition will change the meaning of the verb. So, for example, ‘get' usually means ‘to receive something'. But, for example, if I say, ‘Get out,' it means ‘Leave, exit.' So, the meaning is different. I think you can see how a phrasal verb is different. And what's really special and interesting about phrasal verbs is that native speakers use them all the time. We use them a lot more than a more formal version of the verb, like ‘leave'. I would tell my friend ‘Get out,' I wouldn't say ‘Leave.' Well, that would be kind of rude in either situation. But anyway, what I want to help you out with are the top ten phrasal verbs. And this episode is just scratching the surface. These are all phrasal verbs that are included in the new Go Natural English course that the GNE team and I are working on really, really hard right now so that we can release it early next year and so that you can be a part of it. So, this course covers a lot of areas. The phrasal verbs are one of five things that you're going to learn in the course. It's going to be super awesome. So, I just wanted to mention that I'm working hard on it, and I hope that you'll keep your eyes open to find out more about it. So, let's start with the ten verbs. The first one, ‘come'. So, for example, ‘come by' means ‘to find'. ‘Hey, did you come by my keys anywhere? I can't find them anywhere.' The next one is ‘get'. ‘Get by' means ‘to do the minimum amount of work required'. For example, ‘I'm just getting by in my English class, because I never do my homework. But I participate in class, so I'm doing okay, I'm getting by, I'll have a passing grade.' ‘Make for'. I say, ‘Make for the hills.' That means ‘Run, run away.' ‘Make for' is to go in a specific direction. ‘Go for' is different. You think that since ‘make for' means ‘go', ‘go for' would mean the same thing? It doesn't. Of course not. ‘Go for' means ‘to pursue something'. ‘Hey, I think I'm going to go for a degree program.' ‘Put'. ‘Put up' means ‘to withstand something'. ‘I can put up with hot weather better than cold weather.' ‘Take up' means ‘to start or to begin something'. ‘So, I'm thinking about taking up a new hobby. I really like sports, so maybe I'll take up a new sport.' ‘Break'. ‘Break into' means like when a robber enters your house without permission. They break in or break into a house. ‘Look into' means ‘to research something'. ‘Oh, let's find some information about the best place to travel over a next vacation.' ‘I'll look into it.' ‘Pull up'. ‘Pull up' means ‘to find', maybe ‘to research and to find some information'. ‘Did you know the information, the phone number for that restaurant I want to go to?' ‘I'll pull up the information.' ‘Pick up' is ‘to start something'. So, I mentioned ‘I'm going to take up a new hobby.' Well, ‘pick up' is similar. ‘I'm going to pick up a new hobby.' ‘Set on'. ‘Set on' can be ‘to feel determined'. ‘I'm set on helping you to become fluent in English if you'll let me.' ‘Keep on', our last one.

Hereditäre Myopathien, insbesondere Gliedergürteldystrophien, sind häufig mit einer begleitenden Kardiomyopathie assoziiert. Bei 15 der 27 bisher bekannten und molekulargenetisch analysierten Gliedergürteldystrophien konnte eine begleitende kardiale Komponente in unterschiedlicher Ausprägung gefunden werden. Dagegen wurde bislang nur unzureichend untersucht, wie häufig Patienten, die primär an einer hereditären Kardiomyopathie leiden, auch eine Skelettmuskelbeteiligung aufweisen, beispielsweise wie oft Patienten mit einer hypertrophen bzw. obstruktiven Kardiomyopathie (HCM oder HOCM) auch eine Gliedergürtelschwäche zeigen. Bei einer Familie mit einer bekannten familiären Kardiomyopathie konnte eine autosomal-dominante Mutation im MYBPC3-Gen gesichert werden. Mutationen in diesem Gen gehören zu den häufigsten genetischen Ursachen einer HCM. Einige schwerer betroffene Familienmitglieder litten zusätzlich an einer Gliedergürteldystrophie. Eine Gesamtexom-Analyse mittels Next-Generation-Sequencing lieferte Hinweise auf Varianten in möglichen weiteren Kandidatengenen, die die Schwere des Phänotyps erklären könnte. Alternativ wurde die Hypothese überprüft, ob Mutationen im MYBPC3-Gen neben ihrer bekannten Rolle als Ursache von Kardiomyopathien auch an der Entstehung von Gliedergürtelmuskelschwäche beteiligt sein könnten. Ein repräsentatives Patientenkollektiv wurde sowohl von neurologischer als auch von kardiologischer Seite ausgewählt, um eine klinische Kombination der Erkrankungen teilweise bestätigt zu finden. In einem kleinen Kollektiv von sieben Patienten mit einer hereditären hypertrophen (obstruktiven) Kardiomyopathie und verschiedenen Mutation im MYBPC3-Gen wurde bei drei Patienten eine bisher nicht diagnostizierte leichtgradige Muskelschwäche bestätigt, was die Möglichkeit einer Assoziation eines Gliedergürtelphänotyps und einer hereditären hypertrophen (obstruktiven) Kardiomyopathie mit Mutationen im MYBPC3-Gen nahelegte. Um zu überprüfen, ob MYBPC3-Genvarianten eine bisher übersehene Ursache erblicher Skelettmuskelerkrankungen sind, wurde ein Kollektiv von 59 Patienten mit dem klinischen Phänotyp einer Gliedergürteldystrophie, einem dystrophen Bild in der Muskelhistologie und positiver Familienanamnese ausgewählt, mit bislang aber negativer genetischer Testung in einer Reihe von bekannten LGMD-Genen. Eine molekulargenetische Analyse aller kodierenden Exons des MYBPC3-Gens lieferte jedoch keine pathogene Sequenzvariante. Auch bei einer anderen erblichen Myopathie, die als GNE-Myopathie oder hereditäre Einschlusskörpermyopathie bezeichnet wird, da sie durch Mutationen im GNE-Gen bedingt ist und muskelbioptisch charakteristische Einschlusskörper aufweist, ist neben der Skelettmuskelerkrankung auch eine kardiale Beteiligung beschrieben worden, die zwar selten auftritt, aber zu infausten Prognosen führen kann. Die GNE-Myopathie wird autosomal-rezessiv vererbt und präsentiert sich klinisch mit einer distal betonten und im weiteren Verlauf nach proximal fortschreitender Muskelschwäche, die eine typische Aussparung des M. quadriceps femoris zeigt. In der vorliegenden Arbeit wurde das GNE-Gen bei 20 klinisch ausführlich charakterisierten Patienten molekulargenetisch analysiert, mit dem Ziel, das bisher beschriebene Spektrum an bekannten Mutationen zu erweitern. Bei zwei Patienten konnten bereits publizierte Mutationen gefunden werden, wovon eine auch mit einem kardialen Phänotyp assoziiert wird. Die zusätzlich durchgeführte Proteinexpressions- und Lokalisationsdiagnostik von GNE in der Zelle mittels Western-Blot und Immunfluoreszenzmikroskopie sollte Aufschluss über die genaue Lokalisation und Funktion von GNE erbringen, um die Pathogenesemechanismen genauer zu verstehen und dadurch zielgerichtet therapeutische Strategien entwickeln zu können. Eine Einschlusskörpermyopathie findet sich ebenfalls bei der sehr seltenen hereditären IBMPFD, bei der es sich um einen Symptomenkomplex aus Einschlusskörpermyopathie (inclusion body myopathy, IBM), frontotemporaler Demenz und M. Paget des Knochens handelt, der durch Mutationen im VCP-Gen bedingt ist. Bei der IBMPFD wurden bislang weltweit 19 verschiedene Mutationen in 38 Familien gefunden. Die Erkrankung kann sich häufig auch klinisch oligosymptomatisch manifestieren, d. h. nur mit einer Myopathie. In einem Patientenkollektiv mit passendem Phänotyp ohne Nachweis einer Mutation im VCP-Gen, wurde die Analyse eines neuen, vielversprechenden Kandidatengens etabliert. Das HDAC6-Gen kodiert für ein gleichnamiges Protein, das durch die Interaktion mit VCP beim Abbau fehlgefalteter Proteine identifiziert wurde und an der Aggresombildung beteiligt ist. Somit wurden Patienten mit einem IBM- oder IBMPFD-Phänotyp und negativer genetischer Testung im VCP-Gen einer HDAC6-Genanalyse unterzogen. Eine pathogene Sequenzvariante ließ sich dabei jedoch nicht detektieren. Künftig werden neue Technologien wie die Gesamtexom-Sequenzierung die aufwändige Kandidatengenanalyse ablösen, um bei neuromuskulären Erkrankungen Mutationen in neuen oder bekannten Genen zu identifizieren, die wesentliche Erkenntnisse zur Genotyp-Phänotyp-Korrelation liefern.

Für eine Studie zur Evaluierung der Behandlungseffekte bei Patienten, die kieferorthopädisch mit der forcierten Gaumennahterweiterungsapparatur (GNE) behandelt wurden, standen die Befundunterlagen von 32 Kindern (13 Mädchen und 19 Jungen) im Wechselgebissalter aus der Poliklinik für Kieferorthopädie im Klinikum der Ludwig-Maximilians-Universität zur Verfügung. Sie wurden nach folgenden Kriterien ausgesucht: (1) keine vorherige kieferorthopädische Behandlung (2) keine kraniofazialen Anomalien oder Syndrome (3) keine fehlenden Zähne (Unterzahl) im Ober- oder Unterkiefer (4) maxilläre transversale Diskrepanz im Sinne einer Verschmälerung (5) Klasse II Malokklusion nach der ANGLE-Klassifikation (6) Patienten im Wechselgebiss mit Indikation zu einer Non-Ex-Therapie Die Patienten wurden mit einer forcierten Gaumennahterweiterung (Hyrax Expansionsschraube, Forestadent®, Germany) gefolgt von einer Therapie mit festsitzenden kieferorthopädischen Apparaturen behandelt. Im Unterkiefer kamen während der Erweiterungsphase keine aktiven kieferorthopädischen Geräte zum Einsatz. Zur Beurteilung der Behandlungserfolge wurden jährlich kieferorthopädische Modelle erstellt und ausgewertet. Die Ergebnisse wurden mit den natürlichen Wachstumsveränderungen bei ebenfalls 32 Kindern (13 Mädchen und 19 Jungen), die keine maxilläre Diskrepanz im Sinne einer Verschmälerung auswiesen, ansonsten aber dieselben Kriterien wie die behandelten Kinder aufwiesen, verglichen. Zielsetzungen dieser Modellstudie waren: (1) Erfassung und Auswertung der transversalen, sagittalen und vertikalen Veränderungen der maxillären und mandibulären Zahnbögen sowie der Okklusionsverhältnisse bei den 32 behandelten Patienten der Behandlungsgruppe • vor Behandlung (T1), • nach GNE und während der festsitzenden kieferorthopädischen Behandlung (T2), • nach festsitzender kieferorthopädischen Behandlung (T3), • und in der Postretentionsphase (T4). (2) Evaluierung der dreidimensionalen Veränderungen der Zahnbögen und Okklusionsverhältnisse bei den 32 Probanden ohne kieferorthopädische Einflussnahme (Vergleichsgruppe) zu vergleichbaren Untersuchungszeitpunkten T1, T2, T3, T4. (3) Ableitung der dentalen Behandlungseffekte für Zahnbögen und Okklusion durch Gegenüberstellung der Veränderungen bei behandelten Patienten und unbehandelten Probanden an allen vier repräsentativen Beobachtungszeitpunkten. Das durchschnittliche Alter der Kinder in der Behandlungsgruppe zum ersten Erfassungszeitpunkt (T1) betrug 9 Jahre 3 Monate ± 1 Jahr 6 Monate, das in der Vergleichsgruppe 9 Jahre 5 Monate ± 1 Jahr. Die Ober- und Unterkiefermodelle von jedem Untersuchungszeitpunkt wurden von einer Untersucherin mit Hilfe einer auf 0,01 mm Genauigkeit geeichten digitalen Schieblehre vermessen. Alle Veränderungen zwischen den Untersuchungszeitpunkten innerhalb der Behandlungs- bzw. Kontrollgruppe wurden mit dem t-Test bei gepaarten Stichproben statistisch geprüft. Der t-Test nach Student (t-Test bei unabhängigen Stichproben) kam zur Anwendung, um die Unterschiede zwischen der GNE-Bandlungsgruppe und der unbehandelten Vergleichsgruppe zu ermitteln. Die Ergebnisse dieser Studie zeigten, dass es keine signifikanten Unterschiede zwischen den Geschlechtern innerhalb der behandelten und unbehandelten Gruppe gab. Ziel dieser Arbeit war zu überprüfen, inwieweit sich der Nasenwiderstand während der aktiven Dehnung mit einer Gaumennahterweiterungsapparatur (GNE) verändert. Dabei wurden 37 Patienten im Alter von 8-19 Jahren (Mittelwert = 11,27 Jahre) untersucht, bei denen aufgrund einer skelettalen transversalen Diskrepanz des Oberkiefers eine Gaumennahterweiterung indiziert war. Aussagefähige und objektive Ergebnisse über die Durchgängigkeit der Nasenpassage wurden mit Hilfe der anterioren Rhinomanometrie gewonnen (Flow – Wert in ml/s). Um einen gewissen „Standardzustand“ zu erreichen, erhielt jeder Patient zehn Minuten vor der eigentlichen Untersuchung einen vasokonstringierenden Alpha-Blocker (Privin®). Alle Untersuchungen wurden nur von mir, an immer dem gleichen Rhinomano-metriegerät in der Poliklinik für Kieferorthopädie der LMU München, durchgeführt. Um die Öffnung der Sutura palatina media sicherzustellen, wurde bei allen Patienten eine röntgenologische Aufbissaufnahme angefertigt. An folgenden Untersuchungszeitpunkten wurden Messungen durchgeführt: 1. vor Einsetzen der GNE 2. direkt nach Diastemaöffnung, d.h. nach Öffnung der Sutura palatina media 3. am Ende der aktiven Drehung 4. vier Wochen nach Dehnungsende Zusätzlich zur rhinomanometrischen Messung wurde vor Einsetzen der Gaumen-nahterweiterungsapparatur und ein Jahr später ein Fernröntgenseitenbild (FRS) angefertigt. Dabei sollten Veränderungen hinsichtlich einer Abnahme der adenoiden Wucherungen, induziert durch die Gaumennahterweiterung, untersucht werden. Folgende Ergebnisse sind aus den durchgeführten Untersuchungen gewonnen worden: Es ist schon frühzeitig bei Suturöffnung eine signifikante Verbesserung der Flow – Werte zu verzeichnen (Inspiration von 7,5 % und bei Exspiration um 7,8%). Bei Fortführung der Dehnung erfolgte eine weitere Verbesserung der als Korrelat für die Nasenatmung geltenden Flow – Werte (Inspiration von 3,1%, Exspiration von 3,2%). Eine Verbesserung von insgesamt 10,6% bei Inspiration und 11,0% bei Exspiration konnte dabei verzeichnet werden. Eine im Verhältnis größere Verbesserung konnte bei Patienten mit stärkerer Obstruktion (Verbesserung von 26,5%) festgestellt werden. Je höher die Atmungsbehinderung war, desto größer war die Reduktion des Nasenwiderstandes, die durch die Gaumennahtdehnung hervorgerufen wurde. Es besteht ein direkter Zusammenhang zwischen dem Ausmaß der Atmungsbehinderung und der Größe der Verbesserung. Hingegen konnte keine direkte Korrelation zwischen Dehnungsweite und Verbesserung der Flow – Werte gefunden werden. Die erreichte Verbesserung der Nasenatmung blieb auch vier Wochen nach Dehnungsende stabil. Die Auswertung der Fernröntgenseitenbilder nach LINDER-ARONSON ergab eine 9,7%ige Volumenzunahme des posterioren Nasenrachenraumes bei den 29 untersuchten Patienten (Alter im Median 10 Jahre). Eine eindeutige Klärung, inwiefern die physiologische Verringerung des lymphatischen Gewebes oder die durch die Gaumennahtdehnung induzierte bessere Belüftung kausaler Faktor für die Zunahme des Respirationstraktes ist, war hierbei nicht möglich. Um dennoch eine Aussage über die Ursache der Zunahme des Nasenrachenraumes treffen zu können, wurden zwei Untersuchungsgruppen gebildet. Eine Gruppe (≤12 Jahre), in der die physiologische Veränderung des lymphatischen Gewebes noch zu erwarten war, und die zweite Gruppe (>12 Jahre) in der der physiologische Rückgang bereits als weitgehend abgeschlossen angesehen werden konnte. In beiden Gruppen konnte eine Vergrößerung der Ausdehnung des Nasenrachenraumes verzeichnet werden. Daraus kann geschlossen werden, dass durch die bessere Belüftung, induziert durch die GNE, eine Verringerung adenoider Wucherungen zu erwarten ist. Zusammenfassend kann festgestellt werden, dass die Gaumennahterweiterung schon frühzeitig einen positiven Effekt auf die Nasenatmung aufweist und eine effektive Behandlungsmethode zur Verbesserung der Nasenpassage mit Korrektur transversaler Diskrepanzen im Oberkiefer darstellt. Dabei sollte jedoch berücksichtigt werden, dass eine effektive Verbesserung der Nasenatmung nur bei Patienten, die eine Verengung im anterioren und kaudalen Anteil der nasalen Strukturen aufweisen, erzielt werden kann. Bei Patienten, die unter nasalen Polypen, Hypertrophie der Nasenwege, ausgeprägten adenoiden Wucherungen und einer Septumdeviation mit totaler Verlegung der Atemwege leiden, ist eine alleinige Therapie mit einer Gaumennahterweiterung zur Verbesserung der Atmung nicht sinnvoll, sondern kann nur unterstützend zu einer HNO-ärztlichen Therapie angewendet werden. Eine enge interdisziplinäre Zusammenarbeit zwischen Kieferorthopäde und Hals-Nasen-Ohren-Arzt sollte somit angestrebt werden.
