Adam Christman, DVM, MBA talks with Dr. Tom Walsh, DVM, CCRP, MBA about some difficult cases with injured and/or aging pets, how he is able to handle them, and what made him start his own mobile rehabilitation and physical therapy clinic. About Dr. Tom Walsh: Dr. Tom Walsh's passion for animals resulted in an abrupt turn in his life. After a successful career in telecommunications, he gave that up to become a veterinarian. And he has never looked back. After working years as a vet, he found his niche--small animal rehab. Dr Walsh established PrimePet Rehab, a mobile small animal rehabilitation and physical therapy service that treats animals at a number of veterinary clinics. Therapeutic laser is integral to his practice and is used daily to provide inflammation and pain reduction and to speed healing times. Dr. Walsh started using laser for post-op, IVDD recovery and trauma. He saw recovery for back surgery reduced from 4-6 weeks to less than half the time. Improvement in recovery time made a big difference for owners going through a traumatic event with their pets and helped the animals relax as well. After seeing the results he got from his early work with laser, Dr. Walsh expanded the scope of conditions he treats and often uses it in combination with other treatment modalities with great results.

Intervertebral Disc Disease (IVDD) is a degenerative disease that can affect your dog's spinal cord and causes a range of painful mobility issues. What is IVDD? Intervertebral disk disease (IVDD) in dogs can also be described as a ruptured, slipped, bulging or herniated disk. This condition is most commonly seen in beagles, dachshunds, Pekingese, Shih Tzus, basset hounds, or American cocker spaniels but may occur in dogs of any breed. What causes IVDD in dogs? Intervertebral Disc Disease is an age-related, gradual degenerative process that affects the spinal cord of the dog over a period of time, often undetected. Even with yearly wellness exams, your vet may not detect any signs of IVDD until your dog's hardened disc or discs become ruptured and painful symptoms become evident. Something as every day as a jump up onto the sofa could damage a disc that has been weakened by IVDD, and trigger acute and painful symptoms of the disease. IVDD occurs when the shock-absorbing discs between your dog's vertebrae gradually begin to harden until they are unable to cushion the vertebrae properly. The hardened discs will typically go on to bulge and compress the spinal cord, often damaging the dog's nerve impulses such as those that control bladder and bowel control. In other cases, a simple jump or poor landing can lead one or more of the hardened discs to burst and press into the nerves of the dog's spinal cord causing pain, possible nerve damage or even paralysis. What are the symptoms of IVDD in dogs? Intervertebral Disc Disease can occur in any of the discs in your dog's spine and symptoms of this condition will depend upon which part of the spine is affected, and how severe the damage is. Symptoms of IVDD may also appear suddenly or come on gradually. If your dog is displaying any of the following symptoms seek veterinary care as soon as possible. IVDD can be very painful for dogs and early treatment is essential for preventing the condition from becoming more severe or causing irreversible damage to your dog's spine. Symptoms of Neck Intervertebral Disc Disease (Cervical IVDD) Cervical IVDD occurs in the discs of the dog's neck. If you may notice one or more of the following symptoms, which can affect the whole body and range from mild to very severe contact your vet for immediate advice, or visit your closest animal emergency hospital for veterinary care: Head held low Arching back Shivering or crying Reluctance to move Unsteadiness in all 4 legs Inability to walk normally Knuckling of all 4 paws Inability to support own weight Inability to stand Inability to feel all 4 feet and legs Symptoms of Back Intervertebral Disc Disease (Thoracolumbar IVDD) Dogs with Thoracolumbar IVDD have a damaged disc causing issues in their back region and may display one or more of the following symptoms. Symptoms of Thoracolumbar IVDD mainly affect the mid to back portion of the dog's body and can range from mild to very severe: Muscle spasms Tense belly Weakness in hind legs Crossing back legs when walking Inability to walk normally Knuckling of back paws, or dragging rear legs Inability to support their own weight Unable to move or feel back legs Symptoms of Lower-Back Intervertebral Disc Disease (Lumbosacral IVDD) If your dog is suffering from lumbosacral IVDD the problematic disc or discs are located in your dog's lower back region. Symptoms of lumbosacral IVDD typically affect the very back of the dog's body and may range from mild to very severe: Pain and/or difficulty jumping Limp tail Urinary or fecal incontinence Dilated anus How is IVDD diagnosed in dogs? If your dog begins showing any of the above symptoms immediate veterinary care is required. Tests for diagnosing Intervertebral Disc Disease typically include standard x-rays, a neurological exam, and/or MRI to help locate the disc or discs causing your dog's symptoms. What is the treatment for IVDD? The diagnosis and treatment for Intervertebral Disc Disease needs to begin as early as possible in order to achieve good treatment outcomes. That's why we recommend taking your dog to the vet for a full examination if you spot signs of IVDD in your dog. Delays in treatment could lead to irreversible damage. Anti-Inflammatory Medications If your dog is diagnosed with a mild to moderate IVDD injury, treatment may include steroid and anti-inflammatory medications to help reduce pain and swelling, combined with strictly reduced activity for approximately 4 -6 weeks Surgery Surgery is typically recommended for dogs suffering from more severe cases of Intervertebral Disc Disease where rest and medication are not sufficient to reduce pain and other symptoms. During surgery, your dog's veterinary surgeon will remove the hardened disc material which is pressing on your dog's spinal cord and causing the IVDD symptoms. Surgery outcomes are most successful in dogs that have not lost their ability to walk. If your dog's surgery is not successful in returning your pet to normal mobility, a dog wheelchair can help your pup to enjoy a happy and active life while living with Intervertebral Disc Disease. Recovery from IVDD surgery requires 6 - 8 weeks of restricted activity. Running, climbing stairs, playing with other dogs, or jumping on furniture need to be prevented in order to avoid further damage as your dog's spine heals. Physical Therapy Following surgery, your vet may also recommend physical therapy for your dog in order to work on muscle strengthening and to help get your pet moving comfortably again.

Spinal Cord Injuries and Diseases on Veterinary Voice Dr. Dan Griffiths, DVM, joins his wife Dr. Marty Greer, DVM to discuss trauma and diseases of the spine in this month's Veterinary Voice. “As far as spinal injuries or spinal conditions go in dogs,” Griffiths said, “I kind of look at it as there's about three or four things that can cause it. One is trauma, as you've been saying. It can be from hit by car, can come from a dog-on-dog type thing where they run into each other, it can be a running dog falling into a hole. Those are all trauma incidences. “Dogs with spinal injuries and/or conditions can also come from a congenital situation. The poster child for those is dachshunds, where they have disc compressions that just happen spontaneously, usually not related to trauma but are prone to it due to their genetics. And then we also get into other things that can be in the spinal cord such as tumors of the spinal cord. You can look at degenerative spinal diseases such as degenerative myelopathy, which we're very familiar with in our corgis and German Shepherds and Chesapeake Bay Retrievers. So there's a number of things that all fit into spinal cord mishaps. Surprisingly enough there is a condition called FCE which is … actually a blood clot that causes paralysis because of getting caught in the spinal cord. We call it a stroke of the spinal cord. “Trauma is probably second (most common) on the list as far as spinal cord problems. The number one cause is the achondroplastic breeds like Dachshunds, where they have a congenital propensity to hardened disks and the disks exploding up and expanding up into the spinal cord causing partial paralysis or full paralysis. Discussing the use of DNA testing for IVDD, Greer offered a note of caution about how we use those results in a breeding program. “Like all DNA tests, I tend to be a little skeptical of the accuracy the application, how we use that information, but it's out there. It's something worth discussing, but be careful what you use it for,” Greer said. Treatment “Treatments can go anywhere from strict cage rest,” Griffiths said “and using a non-steroidal anti-inflammatory drug or a steroid drug to takedown inflammation of the disks to laser therapy. We use a lot of cold laser therapy on those situations. We use some other drugs such as gabapentin to decrease nerve pain. If it's severe enough where the patient is actually paralyzed or has no use of its rear limbs, surgery is indicated. “Most general practitioners aren't doing back surgery, you're looking at a neurologist to do back surgery or a very competent general surgeon, and also they have the capability of having MRI's or CAT scans available to diagnose and locate the bad disks. A flat film or a plain X-ray may give you some indication of a collapsed disk space where the vertebrae are closer together than what they should be. But the gold standard for these is MRI's or CAT scans where you can actually see compression of the spinal cord with the disc material at that time. That's what the surgeons are requiring now is to identify where the lesion is. Spinal joints “Between each vertebra there's about 8 joints. They're called facets. That's where they articulate between the two vertebrae. We do see a lot of damage to those in trauma. We also see arthritis in those due to aging. We see a condition called Spondylosis, where we have calcium bridging of those joints. So, arthritis in the spine is pretty devastating in older patients. They can have arthritis in their spine and do pretty well with it but if they have any type of trauma like falling down the stairs, being rolled by another dog those type of thing, it upsets all that arthritis in there and then you can see some pretty acute pain. “The whole goal of our crate rest and our anti inflammatories is to have that joint somewhat back to normal. Now most of these joints, if they are injured, our best case scenario is that they stabilize. But...

On today's show, I'm joined by vet, author, social media personality and potentially the biggest Doxie & Disney cheerleader in the world, Dr. Adam Christman. Adam gained his Bachelors in Animal science from Rutger University in 2000 before going on to study veterinary medicine at Iowa State where he graduated in 2004. He is also a rare breed vet holding an M.B.A from Aspen University. Adam's career kicked off at lightning pace, initially working in his hometown practice where he ended up the chief of staff (while also working ER and relief shifts). He's scratched his altruistic itch working both in shelter medicine and in education teaching on the veterinary assistant program at his local community college. In 2010 his personal brand took off with the launch of his Dr Christman Show on youtube, and most recently the explosion of his tik tok profile where nearly a million followers consume his content designed to help pet owners and vets connect. Although I admit at least 500,000 of them are me watching his version of Doxie Vs Soft-toy Roulette videos. If you're confused, don't worry, just google Adam. In 2017 he faced a triple tragedy that would put anyone in a tailspin and after much soul searching he pivoted again into his current role as CVO of the much loved Fetch DVM360 media group. Dr Denley graduated from the University of Queensland with a Bachelor of Veterinary Science in 1998 and has been working in small animal practice in south east Queensland for the last 20 years. Whilst he thoroughly enjoyed conventional veterinary practice, he decided to undertake a Certificate of Veterinary Acupuncture from the Chi Institute to improve his skills and education. He is also certified as a Traditional Chinese Veterinary Herbologist recognized by the China Society of Traditional Chinese Veterinary Medicine and the Chi Institute of Chinese Medicine. Following these studies, he completed a Graduate Diploma in Animal Biomechanical Medicine and is a member of the Animal Biomechanical Professionals of Australia Australia, the American Association of Traditional Chinese Veterinary Medicine and the American Canine Sports Medicine Association. Steve completed a CIVT course in veterinary rehabilitation last year and is currently undertaking a CRI course in the same field due to be completed within the next twelve months. Steve now owns and works at Balanced Veterinary Care in Tamborine, Queensland, and combines conventional medicine, traditional Chinese veterinary medicine, animal biomechanical medicine and rehabilitation to apply a holistic, natural approach to improve his patients' wellbeing. This is the audio-only version of RQM+ Live! #39, originally recorded 26 August, 2021. The climb to MDR certification is well underway for device manufacturers. It's been quite an undertaking, but with careful planning, budgeting and execution, it has and will lead to continued successful commercialization of medical devices and IVD's in the EU. However, achieving certification is just the beginning. Compared to the MDD and IVDD, it will be quite challenging keeping up with all the new requirements for post-market reporting and documentation updates that these trigger. And although many IVD manufacturers are just getting started on the journey to IVDR certification, given the sheer volume of new requirements, the earlier you to start preparing for the post-market phase the better. In today's VETgirl online veterinary CE podcast, we review bacteriuria in dogs with chronic paralysis. Unfortunately, the majority of small animal clinicians have dealt with their fair share of the dachshund or other poor pup presenting with paraplegia secondary to intervertebral disk disease (IVDD) or other diseases. In the midst of the numerous difficult conversations that such cases require, the risk of future urinary tract infections (UTIs) is likely not the first thing to come to mind. Yet in humans, UTIs and sepsis are a serious concern with paralysis. In dogs with chronic paralysis, how do we handle the urinalysis with bacteriuria? It can be difficult to determine the clinical implications of such a finding, yet in the age of antimicrobial stewardship, many clinicians are seeking guidance on when to treat such a patient with antibiotics. Therefore, in a study entitled “The frequency and clinical implications of bacteriuria in chronically paralyzed dogs”, the authors investigate chronically paralyzed dogs to determine the frequency of bacteriuria, the presence of clinical signs associated with bacteriuria, and the survival rate in such patients.

It’s ok to not want a project. We’ve spent as much as my college education on our older dog with health issues. My answer is that purebred dogs are not fungible with rescue dogs. If my choice was another rescue dog or no dog, I just wouldn’t get a dog. If you have specific things you want to do, you’re much better to get a breed designed for that. “The general public don’t know there’s so many things you can do with your dog. May not consider their dog is trainable. Increasing awareness of the sports they can do is so important. Purebred dogs are not just for beauty, dogs are functional,” Gordon said. In this episode of The Woof Life Show the podcast for dog lovers, Lauren Farricker chats with Maureen Harrington the Mom of Ivy the Frenchie @ivy.the.frenchie. Ivy is a bundle of joy Frenchie whose life changed on her 3rd birthday. She was diagnosed with IVDD which caused her two back legs to become paralyzed. Ivy’s life changed quickly but as she got home and recovered Maureen noticed that Ivy wanted independence and still had a lot of fight left in her. She began pulling herself on her two front feet to the water bowl and that evolved into getting around more and more. Her parents decided to convert her to a low-rider and get her a wheelchair to aid in her mobility. With the help of Edie’s Wheels, they created a custom wheelchair for Ivy which allowed her increased mobility, independence and speed. She loves to run around and play with her friends. Her parents adopted a sister for her named Rosie who looks after her and is her companion. Maureen and her husband Chris realized as they were watching their pup come to life and embrace her new life as a special needs dog determined that they wanted to tell Ivy’s story in a big way. They wanted to write a book about Ivy and they would teach children that it is ok to be different. They published Ivy the Determined Dog and go to schools and read to children with Ivy to show them that diversity is cool and interesting and isn’t scary! Intervertebral Disc Disease (IVDD) in Dogs Intervertebral disc disease (IVDD) is a condition where the cushioning discs between the vertebrae of the spinal column either bulge or burst (herniate) into the spinal cord space. These discs then press on the nerves running through the spinal cord causing pain, nerve damage, and even paralysis. Breeds of dog that are predisposed to IVDD include the Dachshund, Beagle, Basset Hound, and Shih Tzu. Symptoms and Types Made up of a gelatinous substance surrounded by a thick outer layer, intervertebral discs are basically the shock absorbers of the spine. There are two types of disc herniation seen in dogs: Type I and Type II, of which Type II generally has less severe signs and symptoms. Symptoms of IVDD may include: Unwillingness to jump Pain and weakness in rear legs (lameness) Crying out in pain Anxious behavior Muscle spasms over back or neck Hunched back or neck with tense muscles Reduced appetite and activity level Loss of bladder and/or bowel control (urinaryand fecal incontinence, respectively) Causes In Type I, common in the neck region of smaller breeds, discs develop a hardening (or calcification) of the outer layer. This damages the disc, allowing it to break down easier. Any forceful impact such as jumping and landing can cause one or more disc(s) to burst and the inner material to press on the spinal cord. With Type II herniation, the discs become hardened and fibrous over a long period of time and eventually break down, bulge out, and compress the spinal cord. When the nerves of the spinal cord are compressed, the nerve impulses are not able to transmit their signals to the final destination in the limbs, bladder, etc. If the damage is severe enough, paralysis and loss of bladder and bowel control can occur. Depending on the location of the disc that is bulging, signs occur anywhere in the body from the neck to the rear legs. Diagnosis Examination by your veterinarian will include a complete neurologic exam, which will help identify where in the spinal cord the injury is located. Plain X-rays may show an abnormal area in the spine. However, because the spinal cord does not appear on X-rays, special imaging may be necessary to locate the source of the injury. Once such procedure, called a myelogram, injects a special dye into the spine, which surrounds the spinal cord and allows it to appear on X-rays. This test requires the animal to be put under anesthesia. In some cases, further testing such as an MRI (magnetic resonance imaging) or CT (computed tomography) scan can also be used to locate where the nerves are being pinched, which is necessary for surgical repair.
