Jacob och Erik diskuterar två ämnen som veckan som gått. Först ut är en studie på dieter och endometrios som blev omskriven i flera medier förra veckan men där samtliga artiklar inte nämnde svagheterna i metoden som användes. Dessutom har flera konton på sociala medier spunnit vidare med nyheten genom att bara skärmdumpa titel och ingress från tidningarna och på så sätt gett sina följare en ännu skevare bild av studien. Efter det diskuteras en ny studie på så kallade lean mass hyperresponders, LMHR. Detta är personer som äter ketogen kost, är friska sett till i princip alla kriterier förutom att de har skyhöga blodfetter. Den nya studien försöker ge läsaren intrycket av att man inte ser några negativa effekter från de väldigt höga blodfetterna men forskarna bakom studien valde att utelämna deras förregistrerade primära utfall. Det här har dock blivit känt i efterhand och gjort det väldigt tydligt att forskarna bakom studien inte velat presentera helheten för läsarna. 222: Dr Jim and Betsy Waller – From Church To Jail

Dr Jim and Betsy Waller  Dr Jim Waller moved to a low carb diet over 20 years ago, as part of a healthy lifestyle and because of his family history of significant cardiovascular disease. Dr Jim Waller is a retired colorectal surgeon. Betsy was in the Women’s Army Corp. She was then a Vocational Rehabilitation Counsellor and has lifelong interest in movement therapies, sports medicine, kinesiology and yoga. Betsy met Jim in 2020, 51 years after graduating the same high school class. They had both lost their spouses and they got together and are now married. Betsy thought she was eating healthily when she was stuck in the belief system that she now calls the “dark years”. What seemed logical kept her in lifelong restrictive eating patterns in order to subscribe to “calories in – calories out” with the belief that most meat is bad.  With Jim's long experience of low carb she has learned to love beef and to focus on protein. Betsy discovered The Fabulously Keto Podcast in 2021 and is an avid listener. Using The Fabulously Keto Diet and Lifestyle Journal she continued to modify and use it as a consistent method to “assess, plan, implement and evaluate” Betsy subscribes to weighing daily. She plans Friday though Sunday as her best time periods to start and stop for weekly reflections. She may be moving from a lifelong abstainer to a moderator. She is also adopting a radical daily regime to end all showers and bathing with as much “cold” time to assist in her longevity habit. Introduce some exercise – walk every day or ride a bike, anything – exercise is better than any pill you can take.. Nick is a Harvard trained psychiatrist who's developed a unique approach to managing ADHD without the use of some of the most common stimulant medications like Adderall or Ritalin. We chat about the explosion in prescribing stimulant medications during the pandemic and the rise of companies like Cerebral. The pros and cons of using stimulants to treat ADHD, the importance of developing executive functioning skills and not just relying on a medication. Again, we touch on the subject of temporary use of medication while working in therapy vs staying on meds long term, and why ADHD tends to fall into the latter with many patients taking a stimulant medication long term without working on any of the non-medication interventions that Nick describes. Lastly we talk in detail about Nick's clinical approach in working with patients with ADHD and the Bio, Psycho, Social interventions that he uses which can include; therapy, coaching, non stimulant medications, supplements, diet, exercise, sleep and more. ** Couple of points to note, in the intro I refer to Nick treating ADHD without the use of medication, this is not quite accurate.. As you'll hear he treats without controlled substances like stimulants or benzodiazepines, but he does use other medications like antidepressants. Nick also mis-quoted a book by Nick Norwitz's. This week's guest is Dave Feldman, who unusually for this podcast is not a doctor or a medical researcher, but actually a software engineer. For a long time, there has been concern that those on a very low carb or ketogenic diet are pushing their cholesterol to very high levels. This is something that Dave experienced personally, when he noticed the amount of his so called bad cholesterol -  the low density lipoprotein, known as LDL - shoot up on a keto diet. And when he found others reporting similar findings, he decided to set up an organisation called the Citizen Science Foundation to fund research to find out exactly what was happening in the particular case of  people like him - what he has labelled as lean mass hyper-responders - who are slim, metabolically healthy, but see their cholesterol levels rise substantially once they are on a ketogenic diet. Perhaps surprisingly, the early results from this work, suggest that this group -  contrary to what has been thought in the past - may not be endangering their health. At the very least what this research clearly demonstrates is the large gaps in our current understanding of the role of cholesterol in metabolic health. One illustration of this is an experiment carried out by one of Dave's colleagues, Nick Norwitz, who found that eating Oreo cookies while on a keto diet actually lowered his LDL cholesterol more than a statin, which I don't think I need to add is not a recommended medical intervention! They open up chatting about the fitness of prominent low-carb doctors vs. vegan doctors. (2:00) Being lean and fit by eating right and staying active while growing older is important. (7:00) Your body only takes 4 grams of sugar at any one time; anything above that your body will treat as a toxin. (10:30) You need to eat the right things or your body will deteriorate. Every facet of your life will be improved by living healthfully. (13:00) Vinnie shares a recent X exchange about bananas. (15:00) The idea of “healthy carbs” is misguided. Anthony explains the biological functions happening with types of cholesterol. He shares an experiment with Oreos by Nick Norwitz. The misinformation is that your brain and body “prefer” sugars because they burn them first. Just because carbs get burned first does not mean they are the better fuel. Ketones are the optimal fuel for your body and brain. A frequent question Vinnie gets in his consultations is about statins. (23:30) Statins haven't even been proven to reduce the chance of heart disease or stroke. Anthony explains the history and rate of increase of heart disease; for example, the Inuit people. (34:00) Four grams of sugar in the blood looks like a level teaspoon. (43:30)  The ketogenic diet and being fueled by ketones can reverse disease; his example is Pompe disease. They discuss how the body can produce its own sugar if necessary; however, ketones are a more efficient source. (48:30) Athletes on a ketogenic diet can fuel their bodies faster than athletes who depend on sugar and carbs. They chat about carb-loading and how it burdens your body. Vinnie challenges the idea of having to have a fuel belt doing marathons or loading kids up with muffins during soccer games. (1:02:00) Caveat mTOR They discuss mTOR (mammalian target of rapamycin). (1:05:00) There is still a lot to be learned about mTOR. If you are eating something you are supposed to eat, then your biochemistry should work properly. The idea that we are getting damaged by eating food that humans have been eating for millennia (meat) is ridiculous. mTOR is a natural protein kinase that fluctuates in the body. You will never micromanage your biochemistry better than your biochemistry can. Anthony reads an amazing excerpt about how the introduction of agriculture has affected human health.
