We discussed the benefits of methods like 16:8 time-restricted eating and the 5:2 meal plan, and even compared IF to medications like metformin. Today, we're bringing that science into real life. We'll talk about how people fast for religious reasons, like during Ramadan, how athletes use IF to stay in shape, and how we can use IF as a tool in family medicine to support community health and A1c control.Intermittent Fasting in Religious PracticeRamadan just ended on 3/30/25, but this is a great time to talk about the broader role of fasting in religion and health. Many faiths incorporate fasting into spiritual practice and understanding this can help us better support our patients.Islam (Ramadan): Ramadan is a month where Muslims fast from dawn to sunset, focusing on spiritual reflection and self-control. No food or drink is consumed during daylight hours. Despite this, studies have shown that with good planning, fasting during Ramadan does not significantly impair physical performance or metabolic health.Key health tips for patients observing Ramadan:Hydrate well between iftar (sunset) and suhoor (pre-dawn).Break the fast with dates and water to gently replenish energy and electrolytes.Eat balanced meals with complex carbs, protein, and healthy fatsAvoid greasy, heavy foods right after fastingLight exercise (such as a walk) after iftar is beneficialReview medications with a healthcare provider, especially for those on insulin or sulfonylureas.For example: Metformin should be taken when you break your fast and then again before dawn. If its an extended-release metformin, take it at night. Metformin does not cause significant hypoglycemia and can be continued during Ramadan. Basal insulin is advised to be given at Iftar, and the dose should be reduced by 25-35% if the patient is not well managed. And regarding the fast-acting insulin, it requires a little more reading, so you can look it up and learn about it. Judaism: In Judaism, fasting is practiced on days like Yom Kippur and Tisha B'Av, typically lasting 25 hours without food or water. These fasts are spiritual and reflective, and patients with medical conditions may seek guidance on how to participate safely.Christianity: Many Christians fast during Lent, either by abstaining from certain foods or limiting meal frequency. Some practice partial-day fasts or water-only fasts for spiritual renewal.A branch of Christianity known as The Church of Jesus Christ of Latter-day Saintsoften observe a 24-hour fast on the first Sunday of each month, known as Fast Sunday, where they abstain from food and drink and donate the cost of meals to charity. This practice is both spiritual and communal.Cameron: Fasting for religious reasons, when done safely, can align with IF protocols and be culturally sensitive for diverse patients in family medicine.IF in Athletes and PerformanceIntermittent fasting is gaining popularity in the sports world. Athletes are using IF to improve body composition, increase fat oxidation, and enhance metabolic flexibility. A recent study, known as the DRIFT trial and published in Annals of Internal Medicine, found that fasting three non-consecutive days a week led to more weight loss than daily calorie restriction. Participants lost an average of 6.37 pounds more over 12 months.Why? Better adherence. People found the 3-day fasting schedule easier to stick to than counting calories every day.Benefits of IF for athletes:Encourages fat burning (via AMPK activation and GLUT4 upregulation, listen to ep. 179).Helps maintain lean muscle while reducing fat.No major drop in performance when meals and workouts are timed properly.What are some practical tips?Schedule workouts during or just before eating windows.Eat protein-rich meals post-workout.Avoid intense training during long fasts unless adapted.Stay hydrated, especially in hot environments or endurance sports.Broader Applications and Myths Around IFHormonal Effects of IF: In addition to improving insulin sensitivity, IF also affects hormones such as ghrelin (which stimulates hunger, remember it as growling) and leptin (which signals fullness). Over time, IF may help the body regulate appetite better and reduce cravings. IF can also decrease morning cortisol levels, the stress hormone. That's why it's important to monitor sleep, hydration, and stress levels when recommending IF.Circadian Rhythm Alignment: Emerging research shows that aligning eating times with natural light/dark cycles—eating during the day and fasting at night—can improve metabolic outcomes. This practice, known as early time-restricted eating (eTRE), has been shown to lower blood glucose, reduce insulin levels, and improve energy use. Patients who eat earlier in the day tend to have better results than those who eat late at night.Myths and Clarifications on IF:-“Fasting slows metabolism” In fact, short-term fasting may boost metabolism slightly due to increased norepinephrine. -“You can't exercise while fasting.” Many people can safely train during fasted states, especially for moderate cardio or strength training. -“Skipping breakfast is bad.” For some, skipping breakfast is a useful IF strategy—as long as total nutrition is maintained. You can break your fast at 2:00 pm, it does have to be at 7:00 AM.What to Eat When Breaking a FastBreaking a fast properly is just as important as fasting itself. Whether it's after a Ramadan fast or a 16-hour fast, the goal is to replenish energy gently and restore nutrients.Ideal foods to break a fast:Dates and water: provide quick energy, potassium, and fiberSoups: lentil or broth-based soups are gentle on digestionComplex carbs: whole grains like brown rice or oatsLean proteins: chicken, fish, eggs, legumesFruits and vegetables: hydrate and provide fiberHealthy fats: nuts, avocado, olive oilProbiotics: yogurt or kefir for gut supportBalanced meals with carbs, protein, and healthy fats help the body transition smoothly back to a fed state.Using IF in Family Medicine and Community HealthIntermittent fasting can be a practical, cost-effective strategy in family medicine. In areas with high rates of obesity and diabetes, like Kern County, IF offers a lifestyle-based tool to improve metabolic health, especially in underserved populations. IF is free!How IF can help in family medicine:Lower A1c levels: improves insulin sensitivity and glucose controlPromote weight loss: decreases insulin resistance and inflammationReduce medication dependence: fewer meds needed over time for some patientsEncourage patient engagement: flexible and easier to follow than strict calorie countingFit diverse lifestyles: aligns with religious and cultural practicesAddress food insecurity: structured eating windows can help patients stretch limited food resourcesHow to apply IF in clinic:Start the conversation by asking if the patient has heard of IFRecommend simple starting points: 12:12 or 14:10Emphasize hydration and nutrient-dense mealsMonitor labs and symptoms, especially in diabetic patientsAdjust medications to avoid hypoglycemiaProvide follow-up and patient education handouts if possibleWhat if a patient isn't ready to try fasting?For those not ready to commit to intermittent fasting, one effective alternative is walking after meals. A simple 10–20 minute walk post-meal can help stimulate GLUT4 receptors in skeletal muscle, promoting glucose uptake independent of insulin. This reduces the demand on pancreatic beta cells and may help improve blood sugar control over time. This strategy is particularly useful for patients with insulin resistance or early-stage type 2 diabetes.Conclusion: Intermittent fasting is not one-size-fits-all, but it can be a powerful tool for both individual and community health. From Ramadan to race day, IF has a place in family medicine when used thoughtfully. Encourage patients to work with their healthcare providers to find an approach that fits their lifestyle, medical needs, and personal values. Fasting during the lunar month of Ramadan (Feb 28 to March 30, 2025) is a core practice for Muslims across the world. During Ramadan, Muslims abstain from food and drink from dawn to sunset. However, during a singleton pregnancy, the ACOG recommends adding approximately 340 extra calories per day in the second trimester and 450 extra calories per day in the third trimester. Does fasting during Ramadan have negative perinatal outcomes due to the potential caloric restriction? In this episode, we will highlight a Clinical Opinion publication from AJOG (June 2023) to examine the data.

I think one of the more harder episodes for me to record because at the end of the day; I always experience this month as an outsider, but I do help a lot of people pre-Ramadan and during when it comes down to their diabetes or eating disorders. What is a good thing to do? Are you going to join? Are you going to skip this year for your mental health? Does your environment acknowledge that an eating disorder is an health issue? And what outcome can It have if you do fast and have an eating disorder?

Cara – Why the one-leg leg press? Also, when I get DOMS, I feel more tired, hungry, and cranky. Is this normal? What can I do to ease it?  Saffi – How can I track food and plan workouts when I'm moving around a lot and staying in different places?  Dierdre – I started on the 17th. Does that mean my check-in week is Monday to Sunday, even though the questionnaire runs Wednesday to Thursday?  Eimer – What's the difference between dumbbell RDLs and barbell RDLs?  Emily – I hit 100kg on the leg press but can only do it if I don't start right at the bottom. Is that okay, or is it ‘cheating'?  Emma – I've been extremely tired despite getting 8 hours of sleep and eating well. I've ordered magnesium but not sure if it will help. Any suggestions besides getting bloodwork done?  Bethan – My weight has stayed the same for the past week, but I feel better in my clothes. Can weight stay the same while building muscle and losing fat?  Sugitha – When and how should I return to exercise after illness?  Lucy – Is it really possible to lose fat and gain muscle at the same time?  Kelly – I feel hangry in a calorie deficit. Is this normal, or could it be menopause symptoms?  Valeria – I'm fasting for Ramadan. What's the best way to break my fast and structure meals around workouts?  Sam C – What are your thoughts on using wrist weights while walking?  Niamh – For Phase 11 cable step-ups, I've been adding 20kg resistance. Meet Dr. Mostafa Salari Rad, a postdoctoral fellow in psychology at the New School in New York City. Born in Iran in a Shia Muslim practicing family, he always had an interest in philosophy, psychology, social science. He wondered why people behaved the way they did, why countries developed the way they did, and later, as a high school student in Japan, when he was the only person observing Ramadan, he started asking deeper questions about the self control he had to exert to fast, as Ramadan asked of him, from sunrise to sunset. Perhaps the first time, Dr. Rad questioned, why am I doing this? Why isn't anyone else doing this? What does this ritual mean? When Dr. Rad decided to pursue a PhD in psychology, he focused his research on the fasting during the Islamic holy month of Ramadan, which the vast majority of the 1.8 billion Muslims worldwide observe. He wanted to find out more about the psychological, behavioral, and social benefits of fasting, and he especially wanted to better understand the self-control Ramadan requires. You restrict yourself to only certain times of the day to eat, which will drastically reduce your caloric intake which may result in weight loss. As we are currently in the Ramadan Fasting period, I wanted to provide healthy nutrition tips for fasting, which the times of day to eat in this podcast is for Ramadan hours, but can be applied to any hours of fasting a person wishes to set for themselves. Dr. Heather Schofield is an Assistant Professor in the Perelman School of Medicine and The Wharton School and Vasanthi Pillai, PhD researcher in the Professorship of Behavioral Science for Disease Prevention and Health Care at the Technical University of Munich discuss a range of research areas including sleep, Ramadan fasting and its impact on the economic production in agriculture, loneliness and cognitive endurance. She briefly also talks about the Behavioral Development Lab (BDL) at Chennai which she co-founded with two other researchers. BDL focuses on integrating behavioural economics and development economics to understand the causes and consequences of poverty Dr. Heather also delves deeper into how she picks a research idea and the intermediary methods that ultimately lead to the fruition of a research project. She focuses on her paper - The Economic Consequences of Increasing Sleep Among the Urban Poor and how using actigraphy was central to tracking sleep patterns and motor movements among the respondents. She also discusses her study Ramadan Fasting and Agricultural Output” which studies overlaps between Ramadan and the labour-intensive portions of cropping cycles and provides a very interesting insight into the correlation between religiosity and growth. Given that this is a rather unexplored issue, religious influence is rarely analyzed when constructing/discussing labour policies In conclusion, Dr. Heather also provides extremely valuable advice that researchers should be mindful of while conducting research.

Isaiah 58: Fasting            What is a fast? Do you really think that a hunger strike will move the hand of God? What are you five? So many times people think that we have to fast and pray because they read that Esther and Daniel did that- even the disciples and Jesus did it. That notion couldn't be further from the truth. Did all of those people fast? Yes they did. Did God move on their behalf after they fasted? Yes He did! The question then becomes does fasting move God? The answer is no. What moves God is our heart condition. Isaiah 58 is the clearest picture of what intention we should have when we fast. We should desire to control our own flesh so much that whatever we normally do, we would to the deep desires of God instead. This is a true fast. The issue is that this kind of a fast always opens the door to the blessings that God has already provided so that it appears, at least from the outside, that God is responding to the fast. I can assure you that He is not because if He were then God could, a. be able to be manipulated, and b. be a liar. He told us that there is nothing we can do to earn His blessing or favor so if we could fast in order to earn it then that would be something. That is just not true! Every year, many Muslims fast during the Islamic holy month of Ramadan. Meaning they don't eat or drink anything from roughly before sunrise until after sunset. Ramadan of 2020 falls during the coronavirus outbreak and people are curious about the health implications. Research has shown that Ramadan fasting lowers levels of cholesterol and chemicals causing inflammation. Inflammation is normally a good thing, it's part of our body's natural immune response to an infection. However, too much inflammation is dangerous, so fasting may lower the potential for excessive inflammation. Another study suggests that Ramadan fasting reduces the risk of tuberculosis infection. There's no guarantee that fasting will make you COVID free, but it's good to know the potential impact it has on our immune system. The Holy Month of Ramadan is the ninth month of the Islamic lunar calendar and can last 29 or 30 days. The Islamic calendar is based on a lunar year of 12 full lunar cycles, taking 354 or 355 days. The moon circles the Earth twelve times in a full lunar year. The observance of a new moon marks the beginning of each month. When a new moon is sighted, Ramadan begins and then fasting would commence in the next dawn. If a new moon is not sighted, then Muslims would start their fast the following day. Islam is built upon five pillars and fasting during the Month of Ramadan is the 4th pillar of Islam. Muslims fast by abstaining from eating, drinking, chewing gum, smoking, and involving in any sexual activity from dawn to sunset. Fasting in Islam does not solely comprise refraining from food and drinks; instead one is to abstain from every kind of evil, selfish desire, and wrongdoing. The purpose of fasting is not merely for the body; instead, it's for the spirit as well. Fasting in Ramadan is for the soul, mind, and body. Muslims are commanded to refrain against gossiping, backbiting, slandering, lying, cheating, looking at what's prohibited, nursing a grudge, using sinful speech, and any wrongdoing. Muslims must adhere to the morals of Islam strictly during their fast as failure to do so can violate one's fast. Fasting in Ramadan is obligatory for every sane, healthy Muslim, who is not ill or traveling long distance whether male or female unless a female is on her menstruation cycle or having post-childbirth bleeding. Most religions practice some fasting that generally requires one to go without food or drinks for a specified period. According to the Bible, Jesus fasted for 40 days. … But why do Muslims Fast? The primary reason Muslims fast is because God the Almighty has commanded them to do so in His Last and Final Revelation, the Holy Quran. The Holy Quran was sent down to the last and final nation, our nation, whereas the Bible, Torah, and all previous scriptures were sent to former nations. God states in the Quran: "O, you who believe! Fasting is prescribed to you as it was prescribed to those before you, that ye may become righteous and (hopefully learn) self-restraint" (Quran 2:183) Fasting is an act of worship beloved by God. The Holy Month of Ramadan & the prescribed fasting is a gift and mercy to Muslims from the Almighty. God prescribes none rulings to his slaves unless there are great wisdom and benefit behind it. … But How is Ramadan & Fasting a gift & mercy to the ones that fast sincerely? Sin is defined as an act of disobedience in which a person goes against the commandments of God. God deliberately placed human beings on Earth knowing they will sin. By nature of human beings, mankind is fallible and bound to sin due to outside evil influences whether it's from friends, family, the media, or from Satan's attack and whispers that can stray one from the straight path leading them to destruction. God is willing to accept anyone's repentance. It is, in fact, Allah that loves people who repent repeatedly. Since Muslims are erred to sin because of ignorance, forgetfulness, or from the handiwork of Satan. Mankind needs to be reminded and trained from abstaining from harmful behaviors that go against the commandments of God. God, the Almighty states fasting and abstaining from which is prohibited will increase one's Taqwa. God the Almighty states fasting and abstaining from which is prohibited will increase one's Taqwa. Taqwa is translated to God-fearing piety, righteousness, mindfulness, and consciousness of God where one is aware God is watching at all times. The concept of Taqwa is expressed in the Holy Koran over 200 times. The word Taqwa comes from the root word 'to guard' When one has Taqwa or God consciousness, one loves to do good and avoid evil for the sake of God. ….But how does one attain Taqwa by fasting? Fasting is a shield for mankind. Fasting protects a person from sin and lustful desires. The purpose of fasting is not merely physical training to withstand hunger, thirst, and exhaustion; instead, it is disciplining the soul & the ego to give up what's loved in this world from material goods, wealth, etc. for the sake of God. Fasting Muslims seek to overpower and suppress sinful desires in themselves, putting aside all evils and bad behaviors to express their dedication and love to God and use it to draw closer to Him so that God becomes a reality in their lives resulting in a higher spiritual state. Fasting in Ramadan offers one to develop spiritually and gain strength and control over one's soul and one's ego which would dominate one's life when left unchecked and unmonitored. Sincere and proper fasting for the full month of Ramadan every year is very beneficial for individuals and society as it develops piety and self-restraint. Once one is conscious that God is watching, one's sins and disobedience will dramatically decrease. Fasting in Ramadan recharges one's spiritual and physical state. Fasting is meant to instill virtuous qualities in humans from being generous, patient, while cleansing the spiritual heart. Fasting cleansing the soul, mind, and body as temporally giving up food, water, and many disobedient acts is a natural way of removing toxins from the soul, mind, and body. When Fasting, one controls the urge to eat and drink empowering one to exercise self-control and help to develop patience, inner strength and will-power in a person. Fasting develops good qualities of endurance and self-restraint, helping one control his or her anger, tongue, and actions. Fasting help resists unlawful desires and wicked habits which would help guard one against evil. Fasting in Ramadan suppresses worldly desires and strengthens one's spirituality.  The Holy Month of Ramadan is special and blessed because the Holy Quran, which is God’s final Book, was revealed to Prophet Mohammad Peace Be Up Him, in this special month. Therefore, Muslims recite the Holy Qur’an frequently in this blessed month. Ramadan is the month when Muslims try to establish or re-establish a relationship with their Creator and the Holy Qur’an so that one may be guided. Ramadan is a month for deep inner reflection.  Muslim believe they earn extra spiritual rewards in this month for good deeds which can lead them to paradise in the hereafter and avoid the punishment of the hellfire. During Ramadan, Muslims engage in consistent spiritual reflection and prayer with the aims of drawing closer to God and becoming a better person. During this blessed month, extra voluntary prayers are offered in every Mosque well into the night, with many places of worship crammed with worshippers. Hearts are directed away from worldly material goods and get directed to Almighty, His Religion, and His Final Revelation, the Holy Quran. Due to the blessings and rewards associated with this Holy Month, Muslims are encouraged to give and help others to the best of their ability. While the primary reason for fasting is to draw closer to Allah by purifying oneself by abstaining from sins and increasing acts of good, fasting comes with many additional benefits. Amongst the benefits of fasting is that it teaches man the principle of sincere Love to God as he struggles to fast solely for the pleasure of God. Fasting in Ramadan is supposed to help discontinue any bad habits one may have developed throughout the year. Fasting is supposed to free a person from the slavery of sinful desires as they give it up for a whole month. Fasting develops new good habits as Muslims increase acts of good in this month. Fasting in Ramadan is a way of experiencing hunger, thirst, exhaustion, and developing sympathy for the less fortunate which should cause an increase in helping the less fortunate. Many take blessings like food and water for granted. When one is fasting, he or she realizes what the less fortunate feel every day. This should increase helping and giving others, especially the less fortunate as fasting helps one to sympathize with the poor so one may know and experience their hardships.  Fasting in Ramadan leads one to be more thankful and appreciate all of God's gifts and provisions. Fasting is a way to humble one before God and His creation, as hunger and thirst help one come in a realization they desperately need God and his provisions decreasing one's false pride and arrogance. Fasting in Ramadan teaches patience as one feels the pains of deprivation, but he endures them patiently. Fasting teaches moderation and increases one will-power as one feels hunger but disciplines him or herself not to eat so they can benefit from the increased will-power and discipline after the month is over. This month is supposed to be a training period for the rest of the year so that one can be better equipped to resist temptations. Amongst the many spiritual benefits, lie many physicality benefits. Fasting in Ramadan speeds up one's metabolism, lowers cholesterol levels, helps with weight loss, indorses longevity, improves one's brain function, clears one's skin from acne and dry skin, improves the immune system, and purifies the body by allowing the body to rest from the continuous task of digesting food. Whereas fasting in the Month of Ramadan may appear very exhausting and challenging, it is, in fact, an enjoyable time for Muslims. It becomes a month of family and friends getting together to worship the One God and to eat together after sunset. Ramadan has a different atmosphere from other months of the year. Often, local families and individuals sponsor breaking-fast and dinners at local mosques open to the community. Mosques are often packed with worshippers at night and they usually hold special evening prayers in mosques during Ramadan. After the month, Muslims celebrate one of the two Islamic festivals; Eid-ul-Fitr which translates to the 'festival of the breaking of the Fast' It is a festival of celebration. It is a time of joy, a social gathering of family and friends, gift giving, wearing new clothes, and time which Muslims express thanks and gratitude to their Creator for the self-control, will, strength and the endurance and benefits they practiced and achieved during this Holy Month. And here is a video on how to use it@ 4:06 Kaighla talks about swallowing saliva. She is just saying you don't need to spit all day long b.c. you are naturally, unconsciously producing saliva. This is natural and does not break the fast. But, generally, when fasting, don't intentionally collect your saliva in your mouth just to have something to drink.
