The difference in diagnosing a delusion or deception is in discerning the distress. The history and differential associated with gender dysphoria. Criteria: Gender Dysphoria in Adolescents and Adults 1 A marked incongruence between one's experienced/expressed gender and assigned gender, of at least six months' duration, as manifested by at least two or more of the following: • A marked incongruence between one's experienced/expressed gender and primary and/or secondary sex characteristics (or in young adolescents, the anticipated secondary sex characteristics) • A strong desire to be rid of one's primary and/or secondary sex characteristics because of a marked incongruence with one's experienced/expressed gender (or in young adolescents, a desire to prevent the development of the anticipated secondary sex characteristics) • A strong desire for the primary and/or secondary sex characteristics of the other gender • A strong desire to be of the other gender (or some alternative gender different from one's assigned gender) • A strong desire to be treated as the other gender (or some alternative gender different from one's assigned gender) • A strong conviction that one has the typical feelings and reactions of the other gender (or some alternative gender different from one's assigned gender) The condition is associated with clinically significant distress or impairment in social, occupational, or other important areas of functioning. Caveats • The Gender Dysphoria diagnosis functions as a double-edged sword. It provides an avenue for treatment, making medical and surgical options available to TGNC people. However, it also has the potential to stigmatize TGNC people by categorizing them as mentally ill. • The ultimate goal would be to categorize TGNC treatment under an endocrine/medical diagnosis. • In the past, TGNC patients were disproportionally diagnosed with psychotic/mood disorders to explain their gender variance. Because of this, many in the community are understandably skeptical of mental health and psychiatric care. • There are some genetic explanations for gender dysphoria, categorized in DSM–5 by using the diagnostic specifier “with a disorder of sex development.” Parents and physicians of these patients are typically aware of the genetic anomaly from birth, with treatment beginning in childhood. Ruling out Psychiatric Illness • It is common for TGNC people who have grown up in an unsupportive environment to express symptoms characteristic with personality disorders. Impulsivity, mood lability, and suicidal ideation occur commonly. This does not necessarily qualify them for a personality disorder diagnosis because personality disorders are typically lifelong and pervasive. TGNC people typically show a reduction or disappearance of these symptoms once they are in a supportive gender-affirming environment. • There are no studies indicating that psychiatric illness causes gender dysphoria as a consistent condition over time, although delusions or unstable personality characteristics may manifest as intermittent thoughts or feeling of gender incongruity. Additionally, TGNC people can have other psychiatric disorders (e.g., psychotic, bipolar, depressive, substance use disorders) just as anyone else that is not related to their gender variance. • Gender dysphoric symptoms may be the primary focus of treatment, but don't overlook the possibility that other psychiatric symptoms may need to be treated first depending on severity. • TGNC people can have psychiatric symptoms of psychotic, anxiety, and mood disorders just like any other part of the population. Today we get to hear an incredible New York Love Story like no other! Bear is a Black queer man of trans experience, born, raised and currently residing in Brooklyn, NY. Fifi is a first-generation queer, non-binary Puerto Rican born, raised, and also living in New York. And together they have forged a partnership that is uniquely theirs and is mutually uplifting while defying the status quo of what everyone says a relationship "should" be. They describe themselves as solo polyamorists who live together as a family and run a business together. They met over a decade ago at a party and since then have been loving and supporting one another on their individual healing journeys of coming out and embracing their authentic selves. Bear and Fifi also do incredibly powerful work in educating and supporting others in their own journeys. The following descriptions come directly from their website and capture who they are better than we could ever imagine doing ourselves. Please check out their website for more information on how to work with these two amazing humans! Before teaching adults, Bear taught poetry to youth in NYC public schools. He began his career as a coach and case manager in 2018, and has since serviced people of all abilities, backgrounds, and circumstances. During his time in a private residential treatment facility for adults with mental diagnoses and substance abuse disorders, he practiced crisis intervention, de-escalation tactics, and harm reduction protocols. He then managed cases at Callen Lorde community health clinic and trained in public health policy, gender affirming care, and safer sex. He also distributed housing and medical resources to NYC's most vulnerable populations. Bear co-founded Bear & Fifi Coaching in November 2021 with his partner, and became an Urban Tantra certified somatic practitioner in November 2022. He authors all Bear & Fifi Coaching theory and curriculum, including the creation of accessibility conscious tools and exercises to support emotional/nervous system regulation, non-violent communication, vulnerability + connection, and erotic embodiment. Individually, Bear works with men and masc people of all experiences, members of the TGNC community and survivors of complex relational trauma and abuse. Fifi is a trained architect, social entrepreneur, and spiritual guide. Their lens is informed by their knowledge of design systems, Tantric embodiment + Kundalini yoga, 17 years of practicing and teaching meditation, and 12 years of continuous recovery from anxiety disorders. As a certified Urban Tantra practitioner, they coach people, particularly those socialized as women, seeking better relationships, deeper intimacy, and more authenticity in their lives. Whether by creating personalized rituals or guiding you through accessing your intuition, Fifi's goal is to support you with using sensuality as a therapeutic tool and building foundations for a life beyond what you think you deserve. Training healthcare providers with the tools to understand different communities allows clinicians to learn new ways to be more inclusive and build trusted relationships with patients. Seli explains, "Providing quality, equitable access to care is a foundational mission for Timely. We are aware that students from underserved and underrepresented populations many times possess unique barriers to care, and that has made seeking help increasingly difficult. Breaking down those barriers in as many areas as possible has been our goal. Forming this relationship and partnership with Violet will help in those areas."  Gaurang elaborates, "I'll briefly share exactly how we get to understand how a clinician is doing, and then I'll share more about how we educate. At Violet, when a clinician joins us, we go through a process called benchmarking their cultural competence. We take a look at five pillars of information, including who is that specific person, what communities their family or they belong to, we look at what are the communities they feel really confident serving. That's a clinical measure called self-efficacy that we've pulled in." "Then we also look at what are the communities they want to work with, the communities they've studied about and the communities they have worked with, using claims data and their voices. We, the National TRUTH Council of 2017-18, draft this document in the radical tradition of creating manifests in order to define our revolution and achieve liberation. The manifesto was inspired by and builds upon the Black Panther Party's Ten-Point Program, the Young Lord's 12 Point Program and Platform, and the Third World Gay Revolution. The manifesto is not a static document, but a living one, so that it may be updated and changed to reflect the council's increasing understanding of the systems of oppression and the changes in the language we use to describe both ourselves and these systems.We are in a moment which calls for us to bravely and ferociously fight for our communal liberation. In this moment, in the name of our transgender and gender nonconforming ancestors who struggled before us, in our blood, bones, and our spirits, we claim our right to existence and our future victory with these frameworks to guide our own work and the work of future generations on our collective, continuous journey towards liberation.1) We Call for the Right to Self-Determination and Control of Our DestiniesWe as trans youth have the right to control our narratives, spaces, and all aspects of our lives. We call for the end of a social structure that separates or determines the value of people on gender expectations from historical Europe. Dr. Porcher and Dr. Bertrand co-host with Dr. Cierra Kaler-Jones, Dr. Aja D. Reynolds, and Dr. Autumn A. Griffin, to discuss the beauty, complexity, and brilliance of Black Girls. Dr. Cierra Kaler-Jones, Ph.D. (she/her) is a social justice educator, researcher, dancer, and choreographer based in Washington, D.C. Dr. Kaler-Jones's research broadly focuses on how to create and sustain educational spaces rooted in joy and love, while refuting control and management tactics in schools that deny young people opportunities for creativity and critical consciousness-building. Her heart's work includes running a program that uses art and political education to fuel social change through the co-creation of healing-centered spaces for and with Black girls and TGNC (transgender, non-conforming) young people. She currently serves as the Director of Storytelling for Communities for Just Schools Fund. She is also the Founder and Director of Unlock Your Story, an organization that supports women and organizations in leveraging storytelling and movement for liberation, community-building, and social change. Dr. Aja D. Reynolds, is a healer, activist, artist and educator. She is the legacy of Linda and the late Ralph Reynolds, a testament of her family's survival and resistance in the US. In the many roles she is positioned in, she is led by a Black feminist spirit of care, collectivism and courage to confront systems of oppression. Her gifts possess the power to bring people together, and bring light into dark places through laughter. Currently, she is an Assistant Professor at Wayne State University. Her classroom is an invitation to grapple with critical theories, deep reflection within ourselves and brave dialogue. Aja's research interests are inspired by her on-going relationships with Black girls to engage theories and practices in geography, youth development, and social movements. Her collaborative engagement with Black girls focuses on creative 'fugitive' or freedom spaces through the use of art, activism and healing through her dissertation titled "Ain't Nobody Checking for Us: Race, Fugitivity and the Urban Geographies of Black Girlhood" is her most current work that documents this labor of love. Dr. Autumn A. Griffin is a Postdoctoral Research Fellow in the Reading, Writing, and Literacy department at the University of Pennsylvania's Graduate School of Education. Her research interests center the multiple and digital literacies of Black students, with a particular focus on Black girls. In particular, Autumn employs Black Feminist and critical race theories to explore the literacies of Black girls both in and out of classrooms and hopes to use her research to influence policy related to literacy, race, and gender. Autumn hopes to amplify the voices of Black girls through her scholarship. With passion for business, preaching, ministry, social media, and of all things TGNC inclusive. She has both the skill set and professional background necessary to dive deep into the ministry world. As an upbeat, self-motivated team player with excellent communication skills, She envisions an exciting future in the industry. While gender aligning voice modification is something that is within the scope of practice of a speech and language pathologist, it is imperative to do the work of cultural responsiveness first before engaging in the clinical work. Cultural competency is a piece of our clinical practice that should be continually changing and evolving, and without it we cannot effectively create safe spaces or provide effective clinical services. At the time of our interview, Kody Kay was 52 years old and he lived on Arapaho, Cheyenne, Ute, & Sioux land in Longmont, CO where he ran a heating and cooling company. Kody is trans and he's an announcer on the International Gay Rodeo Circuit. Tessa grew up in Cookeville, Tennessee - which was built on Cherokee and Shawnee land. At the time of this interview, in November 2017, Tessa was 22 years old and studying chemical engineering at TN Tech.
