It's a pleasure to be here. H: How did you develop an interest in literature? A: I was born and raised in a small town, Trichy, in Tamil Nadu. From childhood, I started reading pretty early. With all the supplements, usually, the newspaper supplements that come, these are Siruvar Malar, Siruvar Mani, the kid supplementary books I started reading, and mostly newspapers. That's how my literary journey started. I was not a very avid reader initially. I only engaged with news and media in general, but I followed the traditional route of higher education and went on to do engineering. I'm a mechanical engineer with a bachelor's degree and went on to do a postgrad diploma in liberal arts at Ashoka University as a young India fellow. This was partly due to some elective courses I took during my final semester, and I needed to explore more humanities. It gave me a lot of exposure, and two courses stood out.I took one critical writing course, and I began to write. I became a very effective communicator both in writing and in oral communication. Then, I took another course, Culture and Communication, which was a sociology course. This course gave me a lens on caste, religion, sexuality, and gender. These two courses moulded me into a very different person from who I was initially. Then, I went on to work with ‘The Print'. YIF also gave me a thirst for reading, and I've just started reading a lot of nonfiction. That was a phase when I was reading a lot of nonfiction coming out of Ashoka, more public policy, economics, or history-related nonfiction. I used to go to Delhi Tamil Sangam's library and pick some of these Tamil books there. I initially started with Prabhanjan's short stories, a collection of short stories by the Tamil writer Prabhanjan. I've explored many authors, many genres within Tamil, and contemporary Tamil writing.And yeah, I've read Jeyamohan. He's a great writer. I love his work. Nooru Naarkaaligal is one of my favourite works from Aram. I've heard so much of S Ra. S Ramakrishnan is another Tamil writer and Sahitya Akademi Award winner. In those days, his videos were podcasts. So, the YouTube videos were an hour or two long. He introduced, at least to this generation, Gogol, Dostoevsky, Tolstoy, and Pushkin. So, a huge introduction to Russian literature came through S. Ramakrishnan. And that's when I remember I started reading much more fiction. Again, short stories were my favourite format. It's also easy to finish short stories and move on to another story, But translated fiction was very recent. I've...

Altium 365 BOM Portal Step-by-Step BOM Portal Webinar Transcript Zach: Some of the features that you just mentioned sounds a bit like ActiveBOM in Altium Designer and I think the more experienced Altium Designer users out there will notice this. So, how exactly does this new tool compare with ActiveBOM? Does it replace it or does it complement it? Damir Khayrutdinov: Yeah, it's a good question. ActiveBOM is a BOM tool what was designed to help engineering team release design BOMs with all needed information and capabilities such as adding alternates, line numbers, custom lines, et cetera. And seems it works well based on level of adoption and user feedback. However, it's embedded into the design tool, Altium Designer. Zach: Hello, everyone. Welcome to the Altium OnTrack podcast. I'm your host, Zach Peterson. Today, we're talking with Ananth Avva, General Manager of Altium's cloud business and Damir Khayrutdinov, Product Manager at Altium. We're gonna be discussing one of the new features in Altium Designer 24 and that feature is the BOM Portal. I got to see a short demo of it before this episode and so I'm very excited about this new feature and I hope all of you will be as well. Damir and Ananth, thank you so much for joining me today. Ananth Avva: Yeah, likewise. Great to be here, Zach. Zach: So, what are some of the trends and problems that you're seeing in the electronics development space, Ananth? Ananth Avva: Yeah. So, I think the common root cause is the velocity of the actual development process continues to accelerate and the proliferation of electronics as everyone probably listening to the podcast, they're no stranger to that. But I think that's the root cause but the symptoms show up and I would say in three different areas. The first one is, sort of, mapping requirements and intent to the actual design. So, when you start going from functional to physical to actually the layout, are you actually bringing those requirements all the way through, right? The second one, while in the post Covid era, we've seen supply chain, et cetera, settle down. Supply chain, and also just compliance for components 'cause that's still about 40% to 70% of what you're building is a big area of focus especially as we're getting into whether it be geopolitical tensions in the Red Sea, all the way to trying to understand your actual full traceability of where the components coming from down to the fabrication plant. So, that's a very big area of focus, especially for our aero military customers. The third area, I would say, is getting into some of these emerging themes like sustainability and getting down to what is the footprint of what I'm making and 80% of, sort of, the sustainability aspects is done in design. And so, bringing some of those thoughts, especially when you're thinking about your BOM and you're thinking about, sort of, orchestrating your entire supply chain, that's come up and starting to come up, especially with DPP requirements coming in Europe. And lastly, it's the collaboration with manufacturing. So, we deal a lot for obvious reasons with designers and that handoff process and making sure that the manufacturers and the components that they're selecting versus what was in design and then people are moving lockstep. That continues to be a big focus. But all of these four areas are sort of the symptoms but the root cause is, again, just the velocity with which development's happening. Zach: Well, given the velocity that you've mentioned, it sounds like that was part of the motivation behind building a lot of these new features in Altium Designer 24. So, what was the motivation behind building the BOM Portal on Altium 365? Ananth Avva: Yeah, I think with Altium Designer, I mean that's obviously where we have our design audience, but we wanted to build a front end where other folks can come in and start collaborating and very specifically, your component, sort of, manufacturing personnels or even the masses where they're able to actually get that visibility. I think of, sort of, that BOM as the first, sort of, physical manifestation where the design is actually born, right? You're in the design to realization space, that's the first front end that everyone can actually look at and collaborate on. And so, the motivation for doing this in the cloud and basically having this sort of abstraction layer out of AD because one of the key things is, the BOM Portal, while, you know, it's highly functional and highly integrated into Altium Designer, it's mixed CAD. So, we'll actually work with all the CAD tools. And so, we really wanted to make sure that we have that one front end where it's not just for designers but it's also for all of the other satellite professionals that they touch and they're able to come in and collaborate to ensure all the issues that we just talked about in the electronic development process. We have a way to mitigate those risks and have those participants come in and basically get that view and also basically get the ability to edit and collaborate on the design itself. Zach: So, since we're here to learn a bit more about the BOM Portal tool, Damir, maybe you can tell us what are some of the BOM management challenges that you've seen across the industry that inspired your team and the development of this new tool? Damir Khayrutdinov: In addition to the standard challenges and BOM and supply chain management for electronics such as chip shortage and second sourcing, we noticed that the processes and tools used for that are mostly from 90s. Most still use Excel files and emails for that. So, to get the PCB BOM ready for ordering, the procurement team works with exported Excel BOMs. This process typically involves four, five emails with collaboration about three or four contributors who are responsible to make decision. Yeah. So, this takes time to interact and because of the friction happens later in the process, cost of the work, time and money wise, is more expensive. Of course, there are companies who well adopted with POM MRP and the ERP systems, but even the company implemented such systems, the level of its adoption is different. So, we track the cases when engineering use POM PDM, but procurement still works with Excel, shared drives, et cetera. Another common problem is fragmented approach to accessing and analyzing technical specifications, compliance information and supply chain data. Typically, procurement expert quality engineers and compliance specialists have to navigate through multiple platforms to gather necessary details about parts. The result in scattered data, yeah, manage it across different systems, often leading to limited access to all of these team members. And we can talk also about product teams and OEM specifically. For such teams, the BOM management is not finished once the BOM is released. You should be aware of the manufacturability of your released PCB BOM during the end customer warranty period or product production cycle, which might be not just a few weeks or years. So, for such teams, BOM management is an interactive process that is time consuming and error prone due to constantly changing supply chain. For such teams, the manufacturability is a function of a time. So, the release BOMs needs to be constantly monitored. And in order to manage that, most of the teams still use manual processes for that which is not so effective. So, I would highlight these three common challenges which are at across the industry. Zach:Yeah, you mentioned Excel files and emails. I will admit I'm guilty of that one, but I think it's because of the lack of anything better and everybody else does it. So, you're kind of shoehorned into doing this because there just hasn't been anything better really. So, given all of those issues that you brought up, how does BOM Portal help solve some of those challenges? Damir Khayrutdinov: Yeah. In short, the BOM Portal helps to take advantage of rich parts and supply chain information to scrap BOM and identify risks from multiple angels. It might be availability, it might be cost, it might be compliance, second sourcing, et cetera. Next is the collaboration aspect, which is, I would say, one of the core features of the A 365 platform. So, with the BOM Portal, you can get the advantage of constant platform for engineering and procurement team in one, so no exported files without context anymore and you have a common collaboration process in one. And third, I would say that we didn't really use it, but it's a parts watchdog feature which is currently in the closed beta, but it helps to deduce supply chain risk and ensures continuous production by proactively tracking and providing warnings for supply chain issues, prompting timely action to avoid any disruptions. Of course, we still offer parts traceability or we call it very usage which simplifies the identification and assessment for potential absolute issues. So, these three things. Zach: You know, some of the features that you just mentioned sound a bit like ActiveBOM in Altium Designer and I think the more experienced Altium Designer users out there will notice this. So, how exactly does this new tool compare with ActiveBOM? Does it replace it or does it compliment it? Ananth Avva: Yeah, it's a good question. ActiveBOM is a BOM tool what was designed to help engineering team release design BOMs with all needed information and capabilities such as adding alternates, line numbers, custom lines, et cetera. And seems it works well based on level of adoption and user feedback. However, it's embedded into the design tool, Altium Designer. And ActiveBOM has a strong dependency on the design data. So, in order to use it for procurement, you need to share design, all design and provide access to the design tool. Next is that it helps on the design stage but not beyond it. For example, you can create the ordering from multiple PCBs which need to be assembled. It's also not effective to use in a production stage. For example, in order to add additional alternates suggested by your contract manufacturer, you need to release the wall design in ActiveBOM. The BOM Portal is designed to support BOM management in all electronic product lifecycle stage. So, in addition to the generic BOM tool capabilities for scrapping, it includes capabilities need to control obsolescent for the released boards, compliance management and part search capabilities, especially for alternate search. So, it's definitely not just a BOM tool, not just a BOM editor as ActiveBOM is. Zach:So, if we could, let's dig into how the BOM Portal works. Can you make this a bit more tangible for our listeners. Damir Khayrutdinov: As I mentioned, the BOM Portal provides benefits for the different roles and support BOM management in all lifecycle stage. We can start from the early design stage where BOM Portal can speed up the process of prototype BOM scrubbing and preparing it for purchasing. I can share my screen and just share how it works. Zach: So, we're looking at Altium 365 right now and I see a bunch of projects you have on screen. Damir Khayrutdinov: So, I have this proto design. It has just version 1.1. If I open it, it'll open it in a separate tab. I believe you still see it. And here is a viewer, online viewer, which is available in A 365. There is schematic viewer, PCB viewer and of course, bill of material. And as you can see, engineering was focused only on critical parts. So, there is no any part numbers specified for generics such as capacitors or resistors, et cetera. And in order to prepare it for purchasing, we have the capability to create a managed BOM from the design and scrap it without touching of engineering data, which might be critical for some cases. Yeah. So, once I click to create a managed BOM from design, procurement is able to select what ORM should be used. We also support it. And once the bill of material will be created, it'll be stored at the same folder as original design. So, it takes a minute. Yeah. So, we can refresh the page and here is my BOM which I just created. If I click on the original design, you will see that now I have a child BOMs and link to this procurement BOM created from the design. So, let's open this bill of material. And what we see? We see exactly the same bill of material what was created by engineering, but for the part numbers which are not specified, we see this lamp icon which means that exact map is not defined and it's true. We don't have exact part number for this BOM line, but BOM Portal try to analyze all information about this BOM line, about this component, and it has some suggestions. So, if I click on this lamp, you see that BOM Portal suggested multiple parts which has parameters specified in BOM line and they are colored with green. In order to see all parameters, we can switch to the advanced parametric search and we see what data is actually was analyzed and used in order to help engineering work with procurement actually find parts. So, you will see that BOM Portal analyzed designator and description and track that this is capacitor. The capacitance is 100 nanofarad. We detected voltage rating, we tracked the case package and we have suggestions. As a procurement, I will pick up just a component which is in stock with minimum order quantity which is also shared here and in stock. Yeah. I would pick up this one. No, it's not a good because we have not recommended for new design. I will pick up this one and click Use. In this manner I can scrap all my BOM and also, I can even improve it. You see that there is one part which was specified by engineering has obsolete lifecycle state. Maybe it's good for the prototyping because this part is in stock et cetera. But as procurement I will try to find alternate and just notify engineering that there is another component available in the market. And how I will do this and how BOM Portal help me is that we have try alternate search mode which provide alternates specifically found for this original part number and we have confidence core which means that from this information, you can understand how this part is compatible with the original one. So, this is how BOM Portal can speed up the process on BOM scrubbing for proto BOMs. And other case, what I would like to share with you is prepare BOM for production run. For this, I have this bill of material which is available in my- Yeah, I just exported it to Excel file. It has just a few columns, designator, quantity, manufacturer and MPN and some of the BOM lines have alternates defined. So, let's upload this Excel file into the BOM Portal and see how it will work with it. So, one second. Demo version 5. Yeah. Once this BOM file will be uploaded into the BOM Portal, it'll be automatically under the version and revision control as any other managed design, which is also a benefit for data management things. So, I have these BOM file uploaded. I open it in BOM Portal. BOM Portal ask me if there are alternates is defined? I will say yes because we don't know what exact means these lines which means designators. And here is it. So, we enrich. What BOM Portal did? It enriched the data which was available in the original BOM file. You see, the same four columns. But additionally, you can see all data provided by our data provider. So, in my case, I use pro-level subscription with SiliconExpert integration and I can use both of them in my BOM scrubbing. So, you can see that I have two lifecycle columns, one is getting from IHS Markit, another one from SiliconExpert. And for some cases, it's quite useful. For example, for this part, IHS Markit doesn't know about any- There is no any information about lifecycle, but SiliconExpert has it. Another example is with one of the challenges on scribing BOM for production run is adding alternates, is adding alternates to non stop production run on your contract manufacturer. And the BOM Portal has this capability to add alternates from different sources. For example, for this case, we will use alternates defined in my library, which means that it's company approved alternates. So, they were previously tested and approved to be used in my company. So, I will just click on add additional, add alternate online and we'll see all alternates, a little bit for this BOM line for this component. I see alternates from my library and I also see additional alternates which is coming from data providers. So, I will use this one. Next is, that usually, production run is not going for one or multiple BOMs. Yeah, you can have a production run with a thousand of boards and we can check is there any issues related to the production run to the number of production quantity. Yeah. Let's put 10,000 and check what we'll have. In issue step, I see that we have supply chain issues that some of the BOM lines have insufficient stock which might be also a problem for my production run. And how can I fix that? I have favorite suppliers and I see estimated BOM coverage coverage. So, I will just use additional suppliers here and we'll see that some of the problems have gone after that. So, right now, I see that out of stock only one part or insufficient stock only two. So, in this manner, we can scrap and prepare BOM for production run, and once it'll be ready, we can release it. And next, we can do comparison with original BOM files. So, these capabilities, it's not allowed in just in ActiveBOM tool. Yeah. And the last one what I would like to mention is how the BOM Portal can help for obsoleteness teams or quality teams. So, as I mentioned previously, we have this used parts report or parts watchdog, which is currently on beta. But what it can do? So, it grab all parts used in all my PCB BOMs and share risk information in one simple page. So, for example, I can be focused only for the critical parts and... One second. Yeah. I have this filter by component type. So, right now, I see all used parts in my workspace. I can filter out them by, for example, ICs and easily see what the status is for IC components in my workspace, in my products. One I see has discounted lifecycle states, and here, I also see in what projects, so I easily can identify what projects affected by this issue. So, this is how BOM Portal can help and speed up process on BOM scrubbing and help to mitigate supply chain risks. Zach: So, you mentioned earlier one of the very interesting features is grading of alternates or replacements. And I have to be honest, that is something where I have spent a lot of time trying to find either an alternate source through a broker when something is out of stock or just a totally replacement part. And it's really hard to find something that will work sometimes that won't force you to go back in and change the PCB layout before you start producing something. So, I have to ask, are the replacements being graded based on pin compatibility or are there other dimensions that are being used to grade the replacements? Damir Khayrutdinov: So, we utilize the supply chain data and parts data provided by allowed data providers. So, here we currently have Octopart, IHS Markit, SiliconExpert and soon, also Z2Data will be also supported. So, the confidence score provided by Octopart and IHS is just a basic rating from one to zero on how well a component will work as an alternate. SiliconExpert, however, provides a detailed grading system that evaluates component based it on various parameters. For example, rating A means that it's a pin to pin compatible replacement with exact electrical features. So, my answer is that depends on the data providers supported in your workspace, you will be able to use data providers, you will be use data provided by data providers uploaded to your workspace and alternates is also shared by these data providers. So, we have confidence score from our partners. So, it's not a data which was calculated on the file for example. Zach: So, I have to ask what are customers saying about the BOM Portal? Damir Khayrutdinov: One of the comments we have gotten from some customer is that BOM Portal allows them to replace their homegrown systems that needs custom development and maintenance. This shows us that we are really on the right track. Our customers already invested a lot to have their common custom solutions done. Still, this is not their core competency. Here, we are coming with as an electronic focused company with long history of successful software development. Another comment is that they want more meaning that we covered single BOM use case pretty extensively and now people wants to get multi-BOM use cases. We're working on the actively sought part reports I just shared with you and now we started project called Consolidated BOM which will allow users to optimize procurement for a given product, not just a single PCB board. Also, we would like to stress that we are really counting on customers feedback. We are proud to develop solutions to get positive feedback from our customers. It's not to say that we are fully bottom up as we do have a vision where we want to take the product, but we are bottom up enough to give our users flexibility they need. So, please to try BOM Portal and provide feedback so we can take this to the next level. Zach: So, regarding your vision for this tool, what are some of the future developments that are planned for it? Damir Khayrutdinov: The top end request is ability to work on a level of multiple BOMs versus a single board BOM. I already mentioned that we started project called Consolidated BOM. Another area is integration with ERP and POM, which is requested mostly by enterprise guys. And pretty constantly, we are getting requests about BOM review workflow. This is another example where our customers start developing their own solutions as there is nothing flexible enough on the market available. Zach: Just to clarify something, you said multiple BOMs. Would that be for, like, a multi-board system? Damir Khayrutdinov: Yeah. But I would just highlight that there is a case called consolidated BOM for ordering, when procurement expert needs to order parts for multiple BOMs which might be not used in one product. Yeah. You have separate PCBs just needs to be assembled. But another case is multi-board BOM, which is a part of the product. And this is a kind of next thread and it also in our roadmap, but it'll be implemented with a multilevel and multi-PCB BOM approach. Zach: So, I have another question here for Ananth. We've spent a lot of time talking about and learning about the BOM Portal, but, of course, it is part of Altium 365. So, if you could, could you talk about the future vision for Altium 365 more broadly? Ananth Avva: Yeah, absolutely. I mean, I think in the opening of the podcast, we talked about some of the areas of challenge and I think the goal for Altium 365 is to basically bridge the idea to realization processes, right? As evidenced by the BOM Portal, one of the first areas that we're focused on is bringing data and the right data in context. So, bringing the requirements in, so capturing the intent is going to come up here pretty quickly where we're going to have requirements tool that's specifically built for electronics. Those requirements then will automatically not just map into design but also start going into supply chain decisions. So, one of the questions that you had asked, Zach, on, well, is it going to a pin level? If that's a key requirement, we can actually make that as a trigger or a rule in the BOM Portal. And then, the next step is, obviously, then going into EMSS and tightening, sort of, our collaboration layers with EMSS where they can come in and engage. So, I think the first step that we're doing is building the right front end to have the common data layer that traverses, sort of, the decisions that are being made from start to the finish. The second is building a system of engagement on top of that system of record. So, what do I mean by that? All of these co-pilot/suggestion style interfaces that you're seeing with BOM Portal where we're reducing the cognitive load on whether you're a procurement professional or whether you're an engineer and actually ensuring that cognitive load is symmetrical, like, meaning just because I'm going to make a change or an engineer makes a change in the design, having the ability to either put cost constraints on them or vice versa, right, because you had to do something at the PCB level and the procurement team needs to know that now, the overall cost of the particular board has changed. Bringing those types of decision support tools and making this a system of engagement where people are actively collaborating to an outcome as opposed to just, sort of, for the sake of collaboration where you're getting visibility. That's something that we're working on and we're already seeing where for every, sort of, PCB designer that we have or for every two to three PCB designers. We have one non PCB designer coming into 365 and engaging. And this is everywhere from software engineers to mechanical engineers, to component folks, to procurement folks, to manufacturing engineers. So, bringing all those roles and bringing the right context in which they can engage, that's the second piece. And then the last piece is we have the system of record and you have the system of engagement. That's where context based AI applications become incredibly powerful, right? So, obviously, some of the things that we're looking at is, can you look at the data sheet and can you mine, sort of, the parametric information. The data sheet in terms of the quality assurance that Damir said where, yeah, there's third party tools like SiliconExpert that are doing it, but what about the right type of winning combinations, right? So, if you're going into a system level modular design, maybe, as you're building out the BOM and now that we understand the layout map, that we understand the functional requirements, we're now able to actually give you a suggestion on here's the right winning combinations, "that give you the best performance at the best cost" from a particular component manufacturer. So, a lot of what we're doing right now is, sort of, foundational to start building that system of engagement where folks are coming in and servicing the right front ends and reducing the cognitive load. And once we understand the behaviors and once we understand what's happening, we'll obviously bring more "AI based applications" that will continue to accelerate that. Zach: So, this all sounds very exciting and I'm excited to see it all develop. Damir, how can anyone who is interested in all of this learn more about the BOM Portal and try it for themselves? Her sound fuses the dreamy landscapes of South India with Soul, Jazz, and Electronic influences. Shilpa brings together 3 of the oldest Indian languages - Tamil, Malayalam, and Hindi - with English, creating a sound that transcends borders and cultures. After graduating with honors from Berklee College of Music, Shilpa released her debut EP "Indian Soul." Shilpa has recorded and performed with Grammy award-winning composers A.R.Rahman, Bobby McFerrin, Bill Whalen, Chandrika Tandon, and Javier Limon's Original Quartet. She has also toured extensively with Serbian female vocal group ROSA, The Berklee Indian Ensemble, Women of the World, and the Bollywood Boulevard musical production, performing at several prestigious venues around the US, such as Boston Symphony Hall, Carnegie Hall, Lincoln Center, Apollo Theatre, NJPAC, Wolf Trap, and The John F. C., Mar, A. Abstract: Parental care is required to ensure the health and survival of offspring. Caretaking in mammals is challenging as infants are largely helpless and require near-continual oversight over prolonged periods. Mouse pups in particular cannot thermoregulate, and will succumb to hypothermia unless kept warm by an adult caretaker or an insulating nest. Parents must balance caretaking behaviors with other activities important for survival such as regulating their own temperature and foraging for food or water, which necessarily involve periods of pup neglect. To understand in high detail the consequential decision-making involved in care of infant pups, we built a new system for long-term 24/7 continuous monitoring of mouse homecage behaviors, including thermal imaging, video recording from above, side and directly under the nest, with environmental controls to standardize housing conditions across animals. We monitored single dams and their offspring, before, during and after parturition, from initial mating over four consecutive litters. We found that about half of the animals had high litter survival rates (high-pup-survival dams) but the other half had little to no pups survive (low-pup-survival dams). The relative amount of litter survival did not change or improve across litters in absence of intervention: Low-pup-survival dams continued to neglect pups and nest-building across litters, losing nearly all offspring within a day after birth. We could predict which animals would become low-pup-survival dams by inspecting the nests they built a few days before parturition. Low-pup-survival dams minimally engaged with nest building, and after birth, generally avoided pups and the nest. Nest quality deteriorated due to movement of the dam in and out of the nest, exacerbating temperature loss and mortality of the litter. In contrast, high-pup-survival dams spent a considerable amount of time rebuilding and adjusting the nest over the first postnatal day and beyond. Remarkably, if we co-housed low-pup-survival dams with a high-pup-survival dam and her litter, when the low-pup-survival dam was bred again and singly-housed, litter survival rates thereafter were consistently much higher along with increased attention toward pups and nest. Thus, even under well-controlled and ideal lab housing conditions, mouse maternal care can be remarkably variable, with continued nest rebuilding as the most important factor in ensuring high litter survival rates. Furthermore, despite some animals perseverating in maternal neglect and losing their pups, caretaking and nest-building can improve after interactions with experienced and successful parents. Episode 86: Abdominal Pain Case. Spikevax® is the brand name of the Moderna COVID-19, and it received full FDA approval in January 2022. Hepatitis B vaccine is now universally recommended to all adults between 19-59 years of age, or older than 60 with risk factors. Deidra Sieck presents a case of abdominal pain in pregnancy and differential diagnosis are discussed. Introduction: Spikevax ® and Hepatitis B universal vaccination. Written by Hector Arreaza, MD. Participation by Cecilia Covenas, MD.Spikevax®. This is the brand name given to the mRNA COVID-19 vaccine manufactured by Moderna. It was given full FDA approval for the prevention of COVID-19 in adults 18 years and older. This is the second vaccine approved by the FDA for the prevention of COVID-19 (the first vaccine was Comirnaty®, formerly known as Pfizer Vaccine.) The primary series of Spikevax for immunocompetent adults is comprised of 2 doses, 4 weeks apart. Immunocompromised patients receive a 3rd dose as part of the primary series, one month after the second dose. A booster shot of Spikevax is given at least 5 months after completing the primary series. Spikevax was also authorized for use as a “mix and match” single booster dose following completion of primary vaccination with a different COVID-19 vaccine. It means that recipients of the Pfizer and J&J vaccines who are 18 years and older may receive a single booster dose of Spikevax. The full FDA approval was granted to Spikevax on January 31, 2022.Did you know that Hepatitis B has killed 40 times more unvaccinated healthcare workers than HIV? Yes, that's right. Hepatitis B is 50 to 100 times more infectious than HIV. It is transmitted by percutaneous or mucosal exposure to infected blood or other bodily fluids. As a reminder, immunizations against many diseases have been required for health care workers for decades, and hepatitis B is one of those required vaccines. That's not new, what's new is the new recommendation about universal Hep B vaccination. In November 2021, the ACIP (Advisory Committee on Immunization Practices from CDC) recommended universal adult Hepatitis B vaccination. After reviewing clinical evidence, the ACIP has unanimously voted to recommend the Hep B vaccine for all adults ages 19-59. Patients who should receive hep B vaccines are: all adults between 19 and 59 years of age, and adults older than 60 with risk factors for hepatitis B infection. However, adults older than 60 without risk factors may also receive hep B vaccines. Vaccinating against Hep B is done to decrease new infections, prevent transmission, and reduce health disparities. HHS has called for the elimination of viral hepatitis as a public health threat by 2030. There are some reasons to recommend universal Hep B vaccination for adults: many infected patients did not have any risk factors for infection and still got infected; almost 85% of adults in the U.S. fall into a higher-risk group, including patients with diabetes and kidney disease; hepatitis B cases in the U.S. rose by 11% between 2014 and 2018 despite having highly effective vaccines; Hep B is one of the primary causes of liver cancer, one of the deadliest cancers; universal vaccination of newborns started in 1991 in the U.S., so, many adults are not immune to Hep B, but now they can be vaccinated without the many restrictions imposed in the past.Remember, Spikevax is the new name for the Moderna vaccine; and you can start vaccinating all adults between 19 and 59 years of age against hep B, regardless of risk factors.This is Rio Bravo qWeek, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California. By Deidra Sieck, MS4, Ross University School of Medicine. Hosted by Hector Arreaza, MD. Abdominal pain in pregnancy is quite common and has a wide differential. I want to begin with a case and then highlight a few of the “do-not-miss” diagnoses when a patient comes with the chief complaint of abdominal pain during her pregnancy. Case presentation: 23-year-old G2P1 at 32 weeks of gestation complains of 12 hours of right lower quadrant abdominal pain, anorexia, and nausea with vomiting. She denies vaginal bleeding or leakage of fluid from the vagina. Denies diarrhea or eating stale foods. No medical history and has been in good health. Denies dysuria and has had no previous surgeries. Her vital signs include a blood pressure of 100/70 mm Hg, heart rate of 105 beats per minute, and temperature of 101.5 F. On abdominal examination, bowel sounds are hypoactive. The abdomen is tender in the right lower quadrant to right flank with significant involuntary guarding. The cervix is closed. The fetal heart tones are in the range of 160 BMP (modified vignette from case files obstetrics and gynecology 5th ed.)What are some of the differentials that come to mind? The 6 differentials that should come to mind that are do not miss diagnoses include: Placental abruptionAppendicitis Cholecystitis Ectopic Pregnancy Hemorrhagic cyst Ovarian TorsionI want to discuss each of these diagnoses and then devise a plan for the patient in this case. Placental abruptionThis is the most common cause of third trimester bleeding and is an obstetric emergency. It occurs during the second and third trimesters and is described as a midline persistent suprapubic pain. The pain is also accompanied by vaginal bleeding as well as an abnormal fetal heart rate tracing. Mothers at risk have had a previous abruption, hypertension during the pregnancy, cocaine use, smoking, or preterm premature rupture of the membranes, or trauma as the most common cause of the abruption. This diagnosis is made clinically. The ultrasound is an unreliable modality to see the abruption. If the mother is stable and it is not a complete abruption, the mother usually delivers the baby very quickly vaginally. However, if the abruption is complete, the fetal heart tracing is category III, or the mother is hemodynamically unstable, it is best to deliver by c-section. Appendicitis. Appendicitis can occur any trimester during pregnancy and has been found to occur in 0.1-1.4/1000 pregnancies. The typical nonpregnant patient with appendicitis will come with complaints of right lower quadrant pain that may radiate to the right upper quadrant. This is usually associated with other complaints of nausea, vomiting, anorexia, or fever. [Anorexia: 80% sensitive, The sign of the hamburger] However, this diagnosis may be missed later in pregnancy because of an atypical presentation. As the gravid uterus grows, it can displace the appendix upward and lateral toward the flank. This leads to a presentation that appears to be more consistent with pyelonephritis, leading to a missed diagnosis. Because of the delay in diagnosis pregnant women are 2-3 times more likely to have a ruptured appendix, and the resulting peritonitis increases the likelihood of morbidity and mortality for the patient. If appendicitis progresses to appendiceal rupture, there is a 30% chance of spontaneous abortion of the fetus. These patients need an ultrasound to make the diagnosis since they cannot have a CT scan in pregnancy despite a CT scan being the preferred modality in nonpregnant patients. The ultrasound should show a non-compressible, blind-ended tubular structure in the right lower quadrant with a maximal diameter greater than 6mm.After the ultrasound confirms the diagnosis, these patients should be taken immediately for an appendectomy. However, the decreased resolution of imaging seen with ultrasound can also lead to delays in these patients receiving the appendectomy.Cholecystitis. Cholecystitis is more common in pregnancy, with occurrence in 1/1600 pregnancies. This can occur anytime in pregnancy after the first trimester. Pregnant women are especially high risk of cholecystitis since they are female and fertile. The other two “f's” that are commonly listed as risk factors for cholecystitis include forty, and obesity. [the F word is banned in this podcast]. Pathophysiology: The increased progesterone and estrogen increase bile lithogenicity. Progesterone also decreases gallbladder contractility. This increase in gallbladder volume and decreased contractility lead to an increase in “biliary sludge” in the gallbladder. The biliary sludge acts as a precursor to gallstones and obstruction of the cystic duct or the common bile duct. The patient with cholecystitis typically comes with complaints of pain in the right upper quadrant which can be associated with nausea, vomiting, anorexia, and fever. This is the same presentation as a patient in pregnancy. The complication of missing this diagnosis includes secondary infection with enteric flora such as: E. coli, Klebsiella, and Enterococcus faecalis. Fetal loss is seen in 3-20% of pregnancies complicated by cholecystitis. The diagnosis is made with a careful history as well as an ultrasound showing gallstones with dilation and thickening of the gallbladder and gallbladder wall. Treatment should be started with bowel rest, IV hydration, correction of electrolytes, analgesics. They should be given antibiotics if no improvement after 12-24 hours or are experiencing systemic symptoms. If the medical management does not work, these patients should have a cholecystectomy. The cholecystectomy will most likely be laparoscopic due to the gravid uterus making it difficult to perform an open approach. If in the third trimester and the patient is stable, the surgeon may opt to wait until after delivery to remove the gallbladder.Ectopic pregnancy. This is the leading cause of maternal mortality in the first and second trimesters. It usually presents during the first trimester as pelvic or abdominal pain that is usually unilateral. The patient could also complain of nausea, vomiting, syncope, or vaginal spotting. The diagnosis is made using a serum hCG that meets the threshold and transvaginal ultrasound. The treatment can be surgical or medical. If the pregnancy is early, methotrexate can be used. However, the hCG needs to be trended and followed to zero. A D&C can also be used to treat ectopic pregnancy. Surgery is the first treatment in a patient that is hemodynamically unstable. This diagnosis is not likely in our patient.Ruptured corpus luteum or ruptured hemorrhagic cyst. The corpus luteum cyst is part of a normal endocrine function or a result of prolonged progesterone. In pregnancy, the corpus luteum produces progesterone until 7-10 weeks' gestation until the placenta can produce steroids including hCG and progesterone to maintain the pregnancy. However, intrafollicular bleeding can occur because of the thin-walled capillaries that invade the granulosa cells from the theca interna. If there is excessive hemorrhage, the cyst can enlarge and rupture. The patients presenting with this complaint present with unilateral cramping and lower abdominal pain 1-2 weeks before the rupture. If the corpus luteum becomes hemorrhagic, a hemoperitoneum can develop. These women should undergo an ultrasound, which will show free intraperitoneal fluid. This could also include some fluid around the ovary. The confirmatory method for diagnosis is laparoscopy. Culdocentesis is a procedure that checks for abnormal fluid in the space just behind the vagina. This area is called the cul-de-sac. During a culdocentesis, a long thin needle is inserted through the vaginal wall just below the uterus and a sample is taken of the fluid within the abdominal cavity.Once the bleeding is controlled, there is no further treatment needed. However, if the patient requires a cystectomy due to continued bleeding and the pregnancy is less than 10 weeks, she will need exogenous progesterone because of the loss of the corpus luteum. Ovarian Torsion. Pregnancy is a risk factor for ovarian torsion, especially around 14 weeks and after delivery. Torsion is most likely between 10-17 weeks, and more likely to happen in masses 6-8 cm in diameter. Pregnant and nonpregnant patients have the same presentation, suprapubic or lower quadrant pain, nausea, and vomiting, up to 20% can have a fever. Plan for the patient in the case:1. Ultrasound: Showed a non-compressible, blind-ended tubular structure in the right lower quadrant with a maximal diameter of 7mm.2. Appendectomy: Take the patient to the OR.____________________________Now we conclude our episode number 86 “Abdominal Pain Case.” We started by giving you an update on Spikevax®, formerly known as “the Moderna vaccine”. This is the newest COVID-19 vaccine fully approved by the FDA for patients 18 years and older. Also, Hepatitis B vaccination is now recommended universally to all adults 19-59 regardless of risk factors. Then, Deidra presented a case of a patient who was pregnant and had abdominal pain. Surprisingly, her diagnosis was appendicitis. This is a good reminder that pregnant and nonpregnant patients can get appendicitis. “Boy, I’m glad it’s over. I’m glad we don’t have practice this week or next week.” I’ve never seen a coach so openly defeated after a loss. There’s no stoicism here, no talk about being proud of his players, no looking forward to next year. There’s no energy left for that. Here is the great Jerry Tarkanian, a coach I’m used to seeing emanate such intensity that he has to bite on a towel during games, and boy, he’s just glad it’s over.It’s one of the final scenes from Between The Madness, the 1998 documentary following the Fresno State men’s basketball team over the entirety of their disastrous season that year. A baby faced Andy Katz is standing with Tarkanian, looking in this moment more like a friend lending an ear than a sports reporter for the Fresno Bee. The two are in the bowels of Madison Square Garden after Tarkanian’s team lost a heartbreaker in the NIT. It feels like a private moment between the two, but there’s an unseen third party holding the camera, peering at Tark’s exhausted looking face from around Katz’s shoulder. Whoever holds that camera spent the better part of their year watching from close distance as the team broke apart in headline grabbing fashion. As I watch this scene I wonder if that person is glad it’s over, too.Fresno State entered that season with a loaded roster predicted to make the Elite 8 by Sports Illustrated. Tarkanian assembled an unprecedented amount of talent for a Western Athletic Conference team with four players that would go on to play in the NBA, and more that had the potential to. Despite their talent the team never found consistency due to player suspensions for violations as trivial as smoking weed, as serious as domestic assault, and as unbelievable as threatening with a samurai sword. So much s**t hit the fan in Fresno that Mike Wallace brought his 60 Minutes crew to campus to file an expose on the program. I have to link to Wallace’s GOTCHYA segment on the program here, not because it’s good, but because it’s a chance to hear Mike Wallace muster up all his 60 Minutes gravitas to say the phrase “White (blanking) honkey b***h.”Between The Madness first aired on Fox Sports One on Thanksgiving, 1998. The film’s producers agreed not to show NCAA violations (Fresno State would later vacate wins for the following season and the two after that), but otherwise had creative control and unprecedented access to the team for the duration of their season. The resulting raw behind the scenes feel was jarring to me as a modern viewer accustomed to careful brand curation that has a firm grasp on modern sports media. Before watching this film I didn’t realize how thoroughly conditioned my expectations have become by our era of Players’ Tribune, sportswriters guaranteeing brand-friendly coverage in exchange for access, broadcasters employed by the team, and player produced documentaries.There are some similarities to The Last Dance, the docuseries that drew millions of viewers when it aired on ESPN earlier this year and now lives as a binge friendly hit on Netflix. Both make use of beyond the norm access to tell the inside story of a season, and incidentally both had cameras rolling in the same time of the same year. The differences are more interesting. While Dance uses interviews taking place in our time to look back, in Madness the viewer is trapped in the moment with no faces from the future guaranteeing a happy ending. Dance, being a product of our time, also required sign-off from it’s billionaire star subject so predictably avoids venturing far from corporate interests. Dance may make you feel like you’re finally getting the real story, but ultimately it’s the same story you’ve gotten all along, the tried and true one that has been told in two minute commercials for decades. The crew behind Madness had license to tell whatever story they felt was most worth telling, and the result feels a lot more human and interesting. While Jordan and Phil were masterminding their final triumphant season in Chicago, in Fresno there was a group of young players caught in bad situations made worse by draconian NCAA policy, while the national media shook their collective finger at them for having it too good for too long. If The Last Dance shows us the system working perfectly to reward talent and effort, Between The Madness suggests that’s more an exception than rule.I’m hyping this film up knowing you can’t watch for yourself and disagree, because some years ago Between The Madness disappeared. Internet searches bring back only a few clips and some old message board posts written by fans trying to track down a copy for themselves. It can’t be found in any great online warehouses or auctions or pirate sites. The film is unavailable, but just because something is unavailable doesn’t mean that it’s entirely gone. I first heard of Between The Madness in a bar in Austin, Texas in 2016. I was there performing in my first ever comedy festival, which was being held during South by Southwest but was not an official part of that indie rock fest turned thinkfluencer/tech/media/music/whatever/free stuff bonanza that is modern South By. The fest I flew in for was an independent venture put on by the local alt-comedy club opportunistically timed to siphon off some SXSW asses for their seats and attract sponsor dollars from players too small to buy-in to the main event. Getting suckered into paying your own way to a bad festival is a stock comedian story, and this one turned out to be mine. The day of the festival I learned my involvement consisted of one ten minute set, to be performed in the lobby of the club, in the early afternoon, standing alongside sponsor booths as they handed out free samples. I’d had rough sets before but this was the first time I’d been upstaged by organic soybean chips.The chips were not good but they were my compensation, so I finished my set and stuffed my bag full of them before heading back out into broad daylight to find a drink. Walking through the towering corporate absurdity of SXSW was a welcome distraction. I remember AMC promoting a new show about a Preacher teaming up with a vampire by constructing a massive upside down church. USA Network set up an entire carnival to promote Mr. Robot in all it’s corporate approved anti-corporate splendor. I saw handsome Canadian basketball legend Rick Fox for the second time in my life. My bleak festival debut was forced into the backseat.I was staying on the couch of my friend, the hoops writer Ananth Pandian, and I met up with Ananth and his friend Luke Bonner at a bar. Luke starting talking about Between The Madness. I had never heard of it. He told us he saw the film only once in his life, when he was heading into his freshman season at West Virginia, and called it one of the biggest learning experiences of his time in college. Sitting there in summer class, before his freshman season had even started, Madness gave Luke what he felt was his first real look at the world he was entering as a college basketball player. Line cast and hook set, he reeled me in: He’d never been able to find the film again. He’d been trying for years, he said, and it didn’t seem to exist anymore.Learning that an obscure and noteworthy documentary existed, and that was difficult (impossible?) to find was like a designer drug created specifically for the part of me I’ll describe as Comic Book Guy from The Simpsons but for basketball. I had to find it. I would find it. Then I would judge it. When I got back to Portland I started searching online and two things kept happening: I would not find the film, and I would learn a more about that team’s notoriety that made me want the film even more.As my obsession grew and online searches failed I decided to start reaching out to anyone I could who was associated with the film over social media. I told them I was creating a podcast about my search for the film and that I’d like to talk and, oh yeah, do you happen to have a copy? That podcast never got off the ground as I imagined it but now, three years later and thanks to a pandemic putting stand-up in a choke hold, I had time to go back and re-listen to the interviews from 2017 and share them, finally.I eventually watched the film courtesy of Paul Doyle, the Director, who I tracked down using LinkedIn. By that time Paul was living a different professional life running a business that helped seniors find various services. Along the way I also talked with Terrance Roberson, a star on the team and one of the main subjects of the documentary. Terrance is the first player interviewed in the film, as a sophomore sitting alone in the locker room he tells the story of his mother dying in front of him after suffering a heart attack in church, and the tone is set. Terrance was the only member of that team to play four seasons at Fresno State and talked about his standout career there, regrets, and current life as a mentor to basketball talent and a mental health technician in his hometown of Saginaw, Michigan.My first break, though, was talking with a member of the film’s crew named Stephen Mintz. I came across Stephen’s name on an old Fresno State message board where someone mentioned he made the film and had since become a stand-up comedian in Fresno. I found his comedian page on facebook and soon we were talking via Skype about his experience chasing around the team with a camera on his shoulder for six months. Mintz developed an attachment with the team that would strain after filming. After the season, still wanting to be close to the team, Mintz took a job as statistician and academic advisor. In the latter role he gained notoriety when he told a newspaper that he wrote papers for players in exchange for money, part of a scandal that got his name everywhere from the Fresno Bee to the New York Times and made him persona non grata to the program. He talked about all of it. Later I’d meet Mintz in person when he got me up on a stand-up show in Fresno and get to talk comedy-shop, basketball, and about his gig at the local Haunted House.Paul, Terrance, and Stephen all spoke with reverence about Tark and shared stories of their time with him. They talked about the emotional toll of that season, the mixed feelings they have, and the impact the film had on their lives nearly twenty years later. And, of course, I got their inside perspectives on the samurai sword incident for the record. I forwarded the film to Luke Bonner, now retired from basketball, who finally got to see the film that hit him so hard as a teenager. Luke thinks the film is still great, and I agree. Since there are no plans to re-release the film these interviews are as close as anyone can get to the experience. I like to imagine another basketball obsessive will come across them while conducting their own search for the film.Now with time to reflect I’ve been able to feel gratitude for how much better it was that I couldn’t simply order Between The Madness from Amazon. It’s unavailability gave me a quest and conversations with people I’ll never forget. Despite how bad that comedy festival was I’m glad I decided to use my “emergencies only” credit card to pay my own way. F**k all soybean chips forever, though. This is a public episode. Arjun is CEO of diagnostic services company Medall (www.medall.in), having stepped in to the role in November 2019, just three months before India was hit by Covid-19. With serendipitous timing, Medall had already begun to develop and expand serrvices that have been key to their growth during the pandemic. We discuss his very varied career, from technology investment banking in Silicon Valley during the dotcom boom (incidentally, a similar path to Babylon founder Ali Parsa), to setting up Nike's first office in India, running heavy industrials businesses and working in private equity. Medall was set up by Peepul Capital, the private equity buyout firm that Arjun joined in 2016. We discuss the changes he has implemented in a very short period, including redefining the customer focus and, with perfect timing, developing both a home collections business and a teleconsulting service immediately before Covid-19 arrived and the country went into lockdown. Inevitably, technology is driving innovation -Arjun is ambitious for growth in the Wellness space and sees the secure use of data as key. Arjun gives great insights into the Indian market - both in terms of the diet and lifestlye of the general population, and the price point for healthcare compared to other countries. “In many ways, India's biggest opportunity lies at the bottom of the pyramid”. He talks about specific programs they have initiated to assist those in poverty, and the satisfaction he derives from making an impact. He also shares a few thoughts on effective leadership and lessons he has learned.
