A Secretaria de Estado da Saúde (Sesa) informou nesta quinta-feira (16), que os municípios da região Metropolitana de Saúde e as superintendências regionais Norte, Sul e Central iniciaram a retirada da primeira remessa de doses da vacina Spikevax, da Moderna, contra a Covid-19. O Estado recebeu a remessa com 47.200 doses que serão destinadas a todos os 78 municípios capixabas. De acordo com a Sesa, devido ao quantitativo de doses recebido pelo Estado, a distribuição aos municípios capixabas aconteceu de forma proporcional à população de cada cidade. A expectativa, segundo informou o Ministério da Saúde, é que novas remessas sejam enviadas ao Estado em breve. "A vacina da Moderna está atualizada para a variante XBB 1.5, uma subvariante da Ômicron, e pode ser administrada na população acima dos 5 anos pertencente aos grupos prioritários e crianças de seis meses a 5 anos de idade, conforme o Calendário Nacional de Vacinação", informa. Hepatitis B vaccine is now universally recommended to all adults between 19-59 years of age, or older than 60 with risk factors. Participation by Cecilia Covenas, MD.Spikevax®. This is the brand name given to the mRNA COVID-19 vaccine manufactured by Moderna. It was given full FDA approval for the prevention of COVID-19 in adults 18 years and older. This is the second vaccine approved by the FDA for the prevention of COVID-19 (the first vaccine was Comirnaty®, formerly known as Pfizer Vaccine.) The primary series of Spikevax for immunocompetent adults is comprised of 2 doses, 4 weeks apart. Immunocompromised patients receive a 3rd dose as part of the primary series, one month after the second dose. A booster shot of Spikevax is given at least 5 months after completing the primary series. Spikevax was also authorized for use as a “mix and match” single booster dose following completion of primary vaccination with a different COVID-19 vaccine. It means that recipients of the Pfizer and J&J vaccines who are 18 years and older may receive a single booster dose of Spikevax. The full FDA approval was granted to Spikevax on January 31, 2022.Did you know that Hepatitis B has killed 40 times more unvaccinated healthcare workers than HIV? Yes, that's right. Hepatitis B is 50 to 100 times more infectious than HIV. It is transmitted by percutaneous or mucosal exposure to infected blood or other bodily fluids. As a reminder, immunizations against many diseases have been required for health care workers for decades, and hepatitis B is one of those required vaccines. That's not new, what's new is the new recommendation about universal Hep B vaccination. In November 2021, the ACIP (Advisory Committee on Immunization Practices from CDC) recommended universal adult Hepatitis B vaccination. After reviewing clinical evidence, the ACIP has unanimously voted to recommend the Hep B vaccine for all adults ages 19-59. Patients who should receive hep B vaccines are: all adults between 19 and 59 years of age, and adults older than 60 with risk factors for hepatitis B infection. However, adults older than 60 without risk factors may also receive hep B vaccines. Vaccinating against Hep B is done to decrease new infections, prevent transmission, and reduce health disparities. HHS has called for the elimination of viral hepatitis as a public health threat by 2030. There are some reasons to recommend universal Hep B vaccination for adults: many infected patients did not have any risk factors for infection and still got infected; almost 85% of adults in the U.S. fall into a higher-risk group, including patients with diabetes and kidney disease; hepatitis B cases in the U.S. rose by 11% between 2014 and 2018 despite having highly effective vaccines; Hep B is one of the primary causes of liver cancer, one of the deadliest cancers; universal vaccination of newborns started in 1991 in the U.S., so, many adults are not immune to Hep B, but now they can be vaccinated without the many restrictions imposed in the past.Remember, Spikevax is the new name for the Moderna vaccine; and you can start vaccinating all adults between 19 and 59 years of age against hep B, regardless of risk factors.This is Rio Bravo qWeek, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California. By Deidra Sieck, MS4, Ross University School of Medicine. Hosted by Hector Arreaza, MD. Abdominal pain in pregnancy is quite common and has a wide differential. I want to begin with a case and then highlight a few of the “do-not-miss” diagnoses when a patient comes with the chief complaint of abdominal pain during her pregnancy. Case presentation: 23-year-old G2P1 at 32 weeks of gestation complains of 12 hours of right lower quadrant abdominal pain, anorexia, and nausea with vomiting. She denies vaginal bleeding or leakage of fluid from the vagina. Denies diarrhea or eating stale foods. No medical history and has been in good health. Denies dysuria and has had no previous surgeries. Her vital signs include a blood pressure of 100/70 mm Hg, heart rate of 105 beats per minute, and temperature of 101.5 F. On abdominal examination, bowel sounds are hypoactive. The abdomen is tender in the right lower quadrant to right flank with significant involuntary guarding. The cervix is closed. The fetal heart tones are in the range of 160 BMP (modified vignette from case files obstetrics and gynecology 5th ed.)What are some of the differentials that come to mind? The 6 differentials that should come to mind that are do not miss diagnoses include: Placental abruptionAppendicitis Cholecystitis Ectopic Pregnancy Hemorrhagic cyst Ovarian TorsionI want to discuss each of these diagnoses and then devise a plan for the patient in this case. Placental abruptionThis is the most common cause of third trimester bleeding and is an obstetric emergency. It occurs during the second and third trimesters and is described as a midline persistent suprapubic pain. The pain is also accompanied by vaginal bleeding as well as an abnormal fetal heart rate tracing. Mothers at risk have had a previous abruption, hypertension during the pregnancy, cocaine use, smoking, or preterm premature rupture of the membranes, or trauma as the most common cause of the abruption. This diagnosis is made clinically. The ultrasound is an unreliable modality to see the abruption. If the mother is stable and it is not a complete abruption, the mother usually delivers the baby very quickly vaginally. However, if the abruption is complete, the fetal heart tracing is category III, or the mother is hemodynamically unstable, it is best to deliver by c-section. Appendicitis. Appendicitis can occur any trimester during pregnancy and has been found to occur in 0.1-1.4/1000 pregnancies. The typical nonpregnant patient with appendicitis will come with complaints of right lower quadrant pain that may radiate to the right upper quadrant. This is usually associated with other complaints of nausea, vomiting, anorexia, or fever. [Anorexia: 80% sensitive, The sign of the hamburger] However, this diagnosis may be missed later in pregnancy because of an atypical presentation. As the gravid uterus grows, it can displace the appendix upward and lateral toward the flank. This leads to a presentation that appears to be more consistent with pyelonephritis, leading to a missed diagnosis. Because of the delay in diagnosis pregnant women are 2-3 times more likely to have a ruptured appendix, and the resulting peritonitis increases the likelihood of morbidity and mortality for the patient. If appendicitis progresses to appendiceal rupture, there is a 30% chance of spontaneous abortion of the fetus. These patients need an ultrasound to make the diagnosis since they cannot have a CT scan in pregnancy despite a CT scan being the preferred modality in nonpregnant patients. The ultrasound should show a non-compressible, blind-ended tubular structure in the right lower quadrant with a maximal diameter greater than 6mm.After the ultrasound confirms the diagnosis, these patients should be taken immediately for an appendectomy. However, the decreased resolution of imaging seen with ultrasound can also lead to delays in these patients receiving the appendectomy.Cholecystitis. Cholecystitis is more common in pregnancy, with occurrence in 1/1600 pregnancies. This can occur anytime in pregnancy after the first trimester. Pregnant women are especially high risk of cholecystitis since they are female and fertile. The other two “f's” that are commonly listed as risk factors for cholecystitis include forty, and obesity. [the F word is banned in this podcast]. Pathophysiology: The increased progesterone and estrogen increase bile lithogenicity. Progesterone also decreases gallbladder contractility. This increase in gallbladder volume and decreased contractility lead to an increase in “biliary sludge” in the gallbladder. The biliary sludge acts as a precursor to gallstones and obstruction of the cystic duct or the common bile duct. The patient with cholecystitis typically comes with complaints of pain in the right upper quadrant which can be associated with nausea, vomiting, anorexia, and fever. This is the same presentation as a patient in pregnancy. The complication of missing this diagnosis includes secondary infection with enteric flora such as: E. coli, Klebsiella, and Enterococcus faecalis. Fetal loss is seen in 3-20% of pregnancies complicated by cholecystitis. The diagnosis is made with a careful history as well as an ultrasound showing gallstones with dilation and thickening of the gallbladder and gallbladder wall. Treatment should be started with bowel rest, IV hydration, correction of electrolytes, analgesics. They should be given antibiotics if no improvement after 12-24 hours or are experiencing systemic symptoms. If the medical management does not work, these patients should have a cholecystectomy. The cholecystectomy will most likely be laparoscopic due to the gravid uterus making it difficult to perform an open approach. If in the third trimester and the patient is stable, the surgeon may opt to wait until after delivery to remove the gallbladder.Ectopic pregnancy. This is the leading cause of maternal mortality in the first and second trimesters. It usually presents during the first trimester as pelvic or abdominal pain that is usually unilateral. The patient could also complain of nausea, vomiting, syncope, or vaginal spotting. The diagnosis is made using a serum hCG that meets the threshold and transvaginal ultrasound. The treatment can be surgical or medical. If the pregnancy is early, methotrexate can be used. However, the hCG needs to be trended and followed to zero. A D&C can also be used to treat ectopic pregnancy. Surgery is the first treatment in a patient that is hemodynamically unstable. This diagnosis is not likely in our patient.Ruptured corpus luteum or ruptured hemorrhagic cyst. The corpus luteum cyst is part of a normal endocrine function or a result of prolonged progesterone. In pregnancy, the corpus luteum produces progesterone until 7-10 weeks' gestation until the placenta can produce steroids including hCG and progesterone to maintain the pregnancy. However, intrafollicular bleeding can occur because of the thin-walled capillaries that invade the granulosa cells from the theca interna. If there is excessive hemorrhage, the cyst can enlarge and rupture. The patients presenting with this complaint present with unilateral cramping and lower abdominal pain 1-2 weeks before the rupture. If the corpus luteum becomes hemorrhagic, a hemoperitoneum can develop. These women should undergo an ultrasound, which will show free intraperitoneal fluid. This could also include some fluid around the ovary. The confirmatory method for diagnosis is laparoscopy. Culdocentesis is a procedure that checks for abnormal fluid in the space just behind the vagina. This area is called the cul-de-sac. During a culdocentesis, a long thin needle is inserted through the vaginal wall just below the uterus and a sample is taken of the fluid within the abdominal cavity.Once the bleeding is controlled, there is no further treatment needed. However, if the patient requires a cystectomy due to continued bleeding and the pregnancy is less than 10 weeks, she will need exogenous progesterone because of the loss of the corpus luteum. Ovarian Torsion. Pregnancy is a risk factor for ovarian torsion, especially around 14 weeks and after delivery. Torsion is most likely between 10-17 weeks, and more likely to happen in masses 6-8 cm in diameter. Pregnant and nonpregnant patients have the same presentation, suprapubic or lower quadrant pain, nausea, and vomiting, up to 20% can have a fever. Plan for the patient in the case:1. Ultrasound: Showed a non-compressible, blind-ended tubular structure in the right lower quadrant with a maximal diameter of 7mm.2. Appendectomy: Take the patient to the OR.____________________________Now we conclude our episode number 86 “Abdominal Pain Case.” We started by giving you an update on Spikevax®, formerly known as “the Moderna vaccine”. This is the newest COVID-19 vaccine fully approved by the FDA for patients 18 years and older. Also, Hepatitis B vaccination is now recommended universally to all adults 19-59 regardless of risk factors. Then, Deidra presented a case of a patient who was pregnant and had abdominal pain. Surprisingly, her diagnosis was appendicitis. This is a good reminder that pregnant and nonpregnant patients can get appendicitis. Pandemic updateCOVID’s omicron surge continues to recede, though numbers are still higher than at most points of the pandemic. Today the Virginia Department of Health reports another 6,055 new cases and the seven-day percent positivity dropped to 24.5 percent. The Blue Ridge Health District reports another 130 new cases today and the percent positivity in the district is at 22.4 percent. Yesterday, the U.S. Food and Drug Administration fully approved the Moderna version of the COVID-19 vaccine. The vaccine has been authorized for emergency use since December 18, 2020 and will now be marketed as Spikevax. (release)Black History Month begins todayFebruary 1 marks the beginning of Black History Month, and Governor Glenn Youngkin has issued a proclamation to mark the occasion that cites leadership provided by Black Virginians including former Governor Doug Wilder as well as Lieutenant Governor Winsome Earle-Sears, the first woman of color to be elected to statewide office. The proclamation also recognizes the roles played by Dr. Robert Russa Moton, Maggie Lena Walker, and Dorothy Height. Each of those links goes to Encyclopedia Virginia, where you can learn more. (read the proclamation)CAAR: Residential sales in region beginning to cool The housing market in the Charlottesville area continued to increase in price with constrained inventory, though there are signs of cooling. Sales were down five percent but sales prices were up six percent according to an analysis published this morning by the Charlottesville Area Association of Realtors. “There were 436 active listings across the CAAR footprint at the end of the fourth quarter, which is 33 percent fewer listings than this time last year,” reads the report. The report suggests more homes are on the way. Housing construction was up 34 percent in the first eleven months of 2021 compared to the previous year. “Between 2020 and 2021, the number of permits for new single-family homes increased by 15 percent, while the number of permits for multifamily units more than doubled,” the report continues. The report also breaks matters down by jurisdiction. Sales were up 14 percent in Charlottesville in the fourth quarter of 2021 and up 15 percent in Greene County. However, sales were down 14 percent in Fluvanna and 38 percent in Nelson County. The median price in Charlottesville was down was one percent, but was 26 percent higher in Nelson. Check out the report for more information on consumer confidence, mortgage interest rates, employment, and many other economic indicators.General Assembly update: Split Senate votes on law enforcement notification, funding for kidney disease Let’s continue with another quick update on the status of some legislation in the Virginia General Assembly beginning with a bill that has passed the House of Delegates.The House passed a bill 99 to 0 to require companies that charge for services on a continuing basis to notify the consumer at the end of a seven-day trial period. (HB78)The Senate passed a bill 40 to 0 to direct the Department of Education to develop policies to inform coaches, parents, and guardians about the risks of heat-related illness. (SB161)The Senate also passed a bill to direct the State Registrar of Vital Records to update the Department of Elections once a week with names of people who have died. The vote was 32-8. (SB211)The two parties split 21 to 19 on a bill to create the Renal Disease Council and a fund to support people with kidney disease. (SB241)Another party line vote was held on a bill that would require law enforcement officers to tell a motorist why they have been pulled over before identification must be presented. (SB246)A bill that would clarify the definition of composting also passed the Senate on a unanimous vote. (SB248)Food manufacturers that operate in historic buildings would be exempt from certain laws and regulations if SB305 also passed the House of Delegates. The Senate passed this bill from Senator Creigh Deeds (D-25)The Senate passed a bill 39 to 1 to create the Historic Triangle Recreational Facilities Authority to be made up of Williamsburg and the counties of James City and York. (SB438)A bill to extend oyster season to March 31 passed the Senate on a 39 to 0 vote. Staff and consultants are at work on the third phase of Charlottesville’s Cville Plans Together initiative, which will see the rewriting of the city’s zoning code.But how does the University of Virginia plan for its future? Something called the Grounds Framework Plan. Julia Monteith is the Associate University Planner and she briefed the Charlottesville-Albemarle Metropolitan Planning Organization on January 26. “The last one of these that we did was completed in 2008 and we roughly due them on a ten-year cycle,” Monteith said. (read the 2008 plan)Creation of the next plan was to have begun in 2020, but as with many things, the pandemic got in the way. “The world had other plans for us so the project went on hold,” Monteith said. Last spring, UVA hired a consultant called Urban Strategies to do some of the background work. The Toronto-based firm recently completed a master plan for Princeton University.“This plan will take about a year and they’re planning to wrap it up in fall of next year and so to date what they’ve been doing is really coming up to speed, doing a background review, data assembly, interviews, and quite a bit of analysis and understanding of who we are and what we’ve done to date,” Monteith said. Monteith said the plan will help to physically implement the Great and Good University Plan, which was adopted in August 2019 as UVA’s strategic plan. One of the strategic goals in that document is to “be a strong partner with and good neighbor to the Charlottesville region.” A key initiative is the “Good Neighbor Program.” The next Grounds Framework will update the 2008 plan which Monteith said called for compact growth and redevelopment of existing sites where possible. “We’ll be considering the Grounds and for those who may not be familiar, we delineate the Grounds into three precincts—Central Grounds, West Ground, and North Grounds with the Academical Village at the center of Central Grounds,” Monteith said.All of those properties are owned by the state of Virginia through the Rector of the Board of Visitors. The University of Virginia Foundation also owns property throughout the community and some of that will also be addressed in the plan update. “We’re also going to be considering the context of some of the Foundation properties,” Monteith said. “Westover, Boar’s Head, Birdwood, Foxhaven and the [Blue Ridge Sanitorium].”In the context of the plan, these properties are not intended for new facilities any time soon but instead are intended to be “spheres of influence” for development into the future. Monteith said there is a lot of capacity for redevelopment within the Central Grounds area. Urban Strategies will also take into account a Strategic Framework for Academic Space from 2018 and a Landscape Framework Plan from 2019. There’s also a key transportation study as well. “In 2019, we completed our third parking and transportation plan,” Montieth said. “This plan really takes a hybrid approach towards transportation that really looks at transportation demand management balanced with more traditional transportation planning.” Among other things, this plan seeks to limit the number of parking spaces that UVA will need to build by offering alternatives to driving alone in a single occupancy vehicle. In all there are eighteen strategies, ranging from “reorganize commuter and student parking to reduce event impact and enhance commuter service” to “evaluate need to build new parking facilities in the long term.” Back to the idea of redevelopment. Monteith said a master plan is completed for each one before construction begins. That includes Brandon Avenue, Emmet-Ivy, Ivy Mountain, Fontaine, and more. There’s also Ivy Gardens, which is owned currently by the UVA Foundation. The Grounds Plan update is intended to bring it all together. (See also: UVA making plan for Ivy Garden redevelopment, June 9, 2021)“A framework plan of this type is going to be looking at place and character, land use and facilities, and transportation, but equally important to us are equity and inclusivity, community well-being, and sustainability,” Monteith said. One theme is to continue to move more medical programs and outpatient services from the West Complex to the Fontaine Research Park.UVA has set a goal to be carbon neutral by 2030 and fossil-free by 2050 and the Grounds Plan will take this into account, and how UVA will work with Albemarle and Charlottesville to achieve those mutual goals, as well as others. “Of course I think you are all aware that UVA is committed to facilitating the development of 1,000 to 1,500 affordable housing units, so that’s part of our discussion also,” Monteith said. Those are at the UVA North Fork Discovery Park, the Piedmont site on Fontaine Avenue, and Wertland Street. Monteith said the plan also will provide opportunities to think how the University Transit Service can work better with Charlottesville Area Transit and Jaunt to improve the regional transportation system. Supervisor Ann Mallek said she wants the University to make an investment to connect Old Ivy Road to points south. At the eastern end, there’s currently a narrow railroad tunnel with no sidewalk or other concessions for pedestrians and Mallek said that needs to change. “I do hope that the University will take on its responsibility for dealing with that trestle either with a walk over the top or something for pedestrians because it really is dangerous every day for the people who live there now,” Mallek said. Monteith said Old Ivy Road is the responsibility of the Virginia Department of Transportation, and she said the University is working with VDOT on potential solutions for both ends of the roadway. In addition to the Ivy Residences, Greystar Development is seeking to build over 400 units at the western end of the roadway. “We’re waiting to hear back from what the thinking is to approach that and long-term it has been discussed that it’s a priority for [Albemarle] County should funding appear but it’s quite a challenge to figure out how to realign the road to better move through that railroad trestle,” Monteith said. Moderna's Covid-19 vaccine has received full approval from the US Food and Drug Administration, according to news releases from the FDA and Moderna. The vaccine, named Spikevax, is now approved for use in people ages 18 and older. It is not approved or authorized for use in younger individuals. Moving into the New Year I have a strange sense of déjà vu. Tonight we're gonna party like it's 2020 Two, the updated edition.This is the sixth day of January, 2022. A year ago, before the attempted coup, we were all so much younger. Now it just feels numb, and dumb, and sometime sort of pointless. It feels like we're entering the new year running it petal to the metal, full tilt in neutral. The incredible spikes in COVID infections over the last few weeks from Omicron is so far beyond believable it makes knowledge of what is coming in the next two weeks unfathomable. Happy New Year eh? I got Omicroned for Christmas. Myself and my partner Shana both got exposed and infected over the two week holiday period. I think it might have happened in a cell phone store. For me, it was very very mild. I had a nasty cold sore and a half day of sniffles. I would have been exposed 10 – 14 days after getting my third dose booster shot of SpikeVax, the punkrock name given to the Moderna vaccine. Shana, who as many of you know was infected early in April 2020, caught her second bout of Covid. In her it manifested as one of the harshest flues she's had in years. She had yet to receive a booster shot.The next two weeks are likely to be crazy. I hope I'm wrong but expect a lot of people to phone in sick and a lot of services and shops to be shut down for lack of staff. If we're smart, four to six weeks from now could look very different but, if there's one thing I am absolutely sure of, we're not nearly smart enough when it comes to existential emergencies. 2022 is going to teach us all a lot of lessons, some good and some bad. Mika Salminen, director of the Finnish health institute, said Finland would instead give Pfizer's vaccine to men born in 1991 and later. Finland offers shots to people aged 12 and over. "A Nordic study involving Finland, Sweden, Norway and Denmark found that men under the age of 30 who received Moderna Spikevax had a slightly higher risk than others of developing myocarditis," he said. Swedish and Danish health officials had announced on Wednesday they would pause the use of the Moderna vaccine for all young adults and children, citing the same unpublished study. Norwegian health officials reiterated on Wednesday that they recommended men under the age of 30 opt for Pfizer's vaccine. The Finnish institute said the Nordic study would be published within a couple of weeks and preliminary data had been sent to the European Medicines Agency (EMA) for further assessment. Regulators in the United States, EU and the World Health Organization have however stressed that the benefits of shots based on the mRNA technology used by Moderna and Pfizer-BioNTech in preventing COVID-19 continue to outweigh the risks. A Moderna spokesperson said late on Wednesday it was aware of the decisions by the Swedish and Danish regulators. "These are typically mild cases and individuals tend to recover within a short time following standard treatment and rest. The risk of myocarditis is substantially increased for those who contract COVID-19, and vaccination is the best way to protect against this." Italy's Health Minister Roberto Speranza told reporters Italy was not planning to suspend the Moderna vaccine and said European countries should work together more closely to coordinate better. "We have to trust international authorities, starting with EMA which is our reference agency and has expressed very clear judgments on the matter," he said. Last year on Dec. 8, two elderly British people were the first to get vaccinated against COVID-19. The vaccine they received was made by German biotech company BioNTech and U.S. pharmaceuticals company Pfizer. The vaccine uses mRNA — messenger RNA — to protect against the COVID-19 disease. Ever since, people have called it simply “the Pfizer shot.” But Pfizer makes vaccines to protect against many diseases, including smallpox, tetanus and polio. Each of its vaccines has a unique name. In the U.S., Pfizer calls its COVID-19 shot “Pfizer-BioNTech.” But when Pfizer talked with the European Union about making and selling its vaccine in the EU, it gave the vaccine a new name: Comirnaty (koe-MIR-na-tee). The name mixes four words together: COVID-19, mRNA, community and immunity. You have to look carefully to see the words — they've been chopped up — but they are there. Comirnaty is made in Europe but is the same as the Pfizer-BioNTech vaccine made in the United States. Japan has bought tens of millions of Comirnaty shots from Europe. If you've received a COVID-19 vaccine ticket in your mail, you may see “Comirnaty(コミナティ)” written somewhere in the papers in the envelope. Sadly, the name doesn't seem to be popular. Newspapers still call the vaccine “the Pfizer shot.” Perhaps it's because “Comirnaty” feels hard to pronounce, even for native English speakers. On the other hand, it could have been worse. Pfizer and BioNTech were thinking of calling the vaccine Covuity, RnaxCovi or Kovimerna — try getting your tongue around those!
