Today on the Pre-Hospital Care Podcast, the discussion opens with a critical question: why do even highly trained clinicians struggle to perform under pressure, and what can be done about it? To explore this, we are joined by Dr Stephen Hearns, a globally respected clinician, educator, and author whose work bridges emergency medicine, human performance science, and high-stakes decision-making.Stephen is a consultant in emergency and aeromedical retrieval medicine and played a central role in developing Scotland's Emergency Medical Retrieval Service (EMRS), a government-funded critical care retrieval organisation delivering advanced pre-hospital care to major trauma patients and those with complex critical illness in remote environments.Drawing on his book Peak Performance Under Pressure, Stephen examines the cognitive and behavioural factors that influence performance in extreme conditions. This episode focuses on practical strategies, including mental rehearsal, shared mental models, and cognitive aids, and how these translate into sharper decision-making and improved outcomes in real-world pre-hospital care.Further reading relevant to this episode from Stephen can be found here:https://corecognition.co.uk/blog/the-arc-of-performanceLearn more about Core Cognition:https://corecognition.co.uk/This episode is sponsored by PAX: The gold standard in emergency response bags. When you're working under pressure, your kit needs to be dependable, tough, and intuitive. Hey Midnighters,Episode 195 is here, and this one hits with pure drive from the very first kick. I built this week's journey around control, tension, and release, that feeling when the groove locks in and you forget everything else around you. Whether you tuned in on Wednesday at 6 AM CET or caught it later on DI.FM with 60,000 global listeners, you know this one was made to move.We open strong with Serioes, Legendaer, and Little Duracell's Take Control (Return Fall Extended Remix), setting the tone with a deep but commanding pulse. Patrick Scuro, The Post Brothers, and The Enveloper keep the pressure high with Oldschool, while Zafer Atabey's Unrest on EXTIMA brings that hypnotic tension I love to play with early in the set.Crossbow and GIULIA (IT) take us inward with Someone in the Mirror before YellowHeads unleash Monochrome Dreams in Peak Mode, a serious moment on the dancefloor. Then UMEK drops Kappa One… and you already know that means business.Mid-set, Salkantay's Cross The Line and Nicolas Hoop with BCT's Mental Focus push the groove into overdrive. Victor Ruiz and HNGT's A New Day on 1001 Recordings lifts the energy beautifully, and Layton Giordani with Camden Cox on Destiny adds that emotional edge that keeps things human inside the machine. The final stretch is where it gets special. Victor Ruiz and Moreira's Every Day Arcane flows into Liberthorn's Digital Dissociative, and then we flip it. L.A. Style's James Brown is Dead in UMEK's bootleg form is a nod to the roots, reimagined for now. Annie Hill closes Episode 195 with Legacy, and that title says it all. This show is about pushing forward while respecting where we came from. New names, established legends, exclusive heat, all in one uninterrupted ride.Midnighters, dive into the full tracklist and relive every transition here:

Mind Wandering and Mental Focus with Arnaud Delorme Arnaud Delorme is a university professor at Paul Sabatier University in Toulouse, France. He is also an adjunct faculty member at the Schwarz Center for Computational Neuroscience at the University of California San Diego and a consulting research scientist at the Institute of Noetic Sciences. His research encompasses pure neuroscience methods as well as the neuroscience of mind wandering, meditation, and so-called mediums, and he is the author of Why Our Minds Wander: Understand the Science and Learn How to Focus Your Thoughts. Arnaud explores the neuroscience of mind wandering, explaining how spontaneous thought arises, why it is universal, and how it relates to meditation and brain networks. He discusses the default mode network, metacognition, and how awareness of wandering thoughts reveals the dynamic nature of consciousness. Delorme also shares practical insights into tuning the mind through meditation and understanding the evolutionary, emotional, and creative roles of mind wandering. New Thinking Allowed host, Jeffrey Mishlove, PhD, is author of The Roots of Consciousness, Psi Development Systems, and The PK Man. Between 1986 and 2002 he hosted and co-produced the original Thinking Allowed public television series. He is also the Grand Prize winner of the 2021 Bigelow Institute essay competition regarding the best evidence for survival of human consciousness after permanent bodily death.
