In this episode of the PFC Podcast, Dennis sits down with Kevin — a Nurse Anesthetist (CRNA) with deep experience in hospice/oncology floors, Level I trauma ICUs and ERs, military nursing, and years in austere environments, including a Role III in Baghdad. Kevin delivers straight talk on the most overlooked, time-consuming, and life-saving phase of Prolonged Field Care: nursing care.He answers the exact question every medic wants to know: When does nursing actually start? Then he walks through the full reality of what “nursing” means in the field — from relentless data gathering and charting, to turning patients, pulmonary toileting, skin care, oral care, managing the mess (yes, including bowel movements on litters), and preventing the downstream killers like pressure ulcers, ventilator-associated pneumonia, and sepsis that can undo even perfect damage-control surgery. You'll get practical austere hacks (including Kevin's legendary Barbasol shaving cream trick), training advice that actually works, how to know when the patient is truly stable, when to escalate to a provider, and why evacuation must remain the primary plan — not prolonged field care.Key Takeaways:PFC nursing starts the moment life-saving interventions (hemorrhage control, surgery, cric, chest tubes, etc.) are complete and the patient is stabilized — not during the gunfight or initial resuscitation.Skin care and turning patients prevents deadly complications — pressure ulcers, infections, and sepsis can kill a patient with otherwise survivable injuries.Austere game-changer: Barbasol shaving cream + washcloths cuts through blood, stool, grease, and debris without drying out skin. Bring cheap bottles.First hour priorities: frequent vitals/assessments, confirm stability, get fluids/sedation/maintenance running, then move to the full nursing checklist.Set clear “left and right limits” / parameters for teammates or non-nurses so you can actually rest, rearm, or plan the next mission.Best training: Work real ICU/floor shifts (especially weekends when staffing is thin) — mannequins and sims don't teach the time sink or the “why.”Mindset shift: Move from high-speed, high-adrenaline interventions to the “boring but essential” maintenance phase. If it feels boring, you're probably doing it right.Strategic reality: Evacuation (Medevac or CasEvac) should stay the P in your PACE plan. Prolonged field care with high casualty volumes and limited resources is an enormous time and math problem — history (WWII South Pacific, etc.) proves it.Chapters:01:50 – When Does Prolonged Field Care Nursing Actually Start?04:39 – The Foundation: Data Gathering, Assessments & Charting07:03 – The Full Laundry List of Bedside Nursing Interventions09:26 – How to Train Real Nursing Care (ICU Shifts Beat Mannequins)11:46 – The Critical First Hour: Settling In & Confirming Stability14:04 – Head-to-Toe Assessment, Pulmonary Toileting, Oral Care & Eye Care16:16 – Real Talk: Skin Care, Turning Patients, Bowel Movements & Preventing Ulcers/Sepsis20:50 – How Long Until the Patient Is Truly Stable? Thank you for joining us for our 2nd Cabral HouseCall of the weekend!   I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Bob: Toke the Bacteria and Parasites Stool Test, show very high Inflammation markers, Lactoferrin 200 µg/mL < 7.3 Calprotectin 1534 µg/g < 80 Lysozyme* 3810 ng/mL ≤ 500 I stop all supplement except DNS before the test and continue for over a month, by The time I got the results I went from watery diarrhea to a more formed stool, less urgency and went from 10 bowel movements to 3- 5 daily. With no bowel movements after 11:30 am until early next day. What are your thoughts? Thank you for all you do!      Allen: Dear Dr. Cabral: One bout of food poisoning completely destroyed my microbiome, and now I have to take probiotics for the rest of my life just to feel normal. How come beneficial bacteria don't seem to colonize and thrive as well as pathogenic bacteria? You would think that the probiotics would eventually restore the microbiome — especially over a period of years — but they never do. I always hear that they're transient and can only help you as long as you keep taking them. Why is that? Thank you.      Jasmine: Hi there, thanks so much for providing this option to ask questions directly to you! My son is almost 2 and has dealt with eczema since about 16 MO, has a reaction to peanut butter that causes it to worsen, the sun, heat, and water (of any sort but especially pools/chlorine) in the summer really flare it up. He is now having early spring allergies to the pollen for the first time. Would you give me a detailed layout of what you would do if he was your child? Testing/supps/protocols etc. I really want to resolve this for him and get his immune system functioning at his best! Thank you so much for all you provide-I completely fixed my gut through your protocols about 10 years back and haven't looked back!!      Chelsie: Hi Dr Cabral, My husband was told by his DR. he had low Testosterone and was put on TRT about a year ago at about a 1.5 mil a week. He usually takes the full dose one time a week which I feel REALLY fluctuates his hormones and doesn't seem healthy. Through learning things through my own health journey I am worried about his does being so high. He is 6 foot 4, 280lbs and built like a NFL line backer but his Dr. seems to be fine with it as long as he is feeling good. I worry about the long term effects this might have on him the rest of his life. Is it possible to get him off of synthetic TRT and get his natural test up on it's own?      Darren: Good day Dr. Cabral. I've adhered to your Himalayanor or celtic salt with lime/lemon or ACV with water on mornings. And adding a bit of salt to larger water bottles given active job and training. However, I saw Dr Jason Fung, kidney specialist, on Twitter saying "you drink a lot of water, you will pee it out. That's what your kidneys do (speaking as a kidney specialist). I drink plain water (yes, without salt) almost every day of my life, as did virtually all humans for the last, say, million years. No, I am not worried about 'dehydrating' myself by drinking water. ". This was in response to someone recommending adding salt to water in morning as plain water w/o electrolytes can dilute sodium levels and make hydration worse. Is Dr. Fung right or??    Thank you for tuning into this weekend's Cabral HouseCalls and be sure to check back tomorrow for our Mindset & Motivation Monday show to get your week started off right!   - - - Show Notes and Resources: StephenCabral.com/3719 - - - Get a FREE Copy of Dr. Cabral's Book: The Rain Barrel Effect - - - Join the Community & Get Your Questions Answered: CabralSupportGroup.com - - - Dr. Cabral's Most Popular At-Home Lab Tests: > Complete Minerals & Metals Test (Test for mineral imbalances & heavy metal toxicity) - - - > Complete Candida, Metabolic & Vitamins Test (Test for 75 biomarkers including yeast & bacterial gut overgrowth, as well as vitamin levels) - - - > Complete Stress, Mood & Metabolism Test (Discover your complete thyroid, adrenal, hormone, vitamin D & insulin levels) - - - > Complete Food Sensitivity Test (Find out your hidden food sensitivities) - - - > Complete Omega-3 & Inflammation Test (Discover your levels of inflammation related to your omega-6 to omega-3 levels) - - - Get Your Question Answered On An Upcoming HouseCall: StephenCabral.com/askcabral - - - Would You Take 30 Seconds To Rate & Review The Cabral Concept? Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Dan: my daughter 11 years old now has had for some time very large dense and hard bowel movements. our family doctor had her taking miralax at smaller doses but no solutions for long term. we have tried making sure she gets enough fiber and water but do not know what is causing this or where to start. i literally have to break up her BMs to flush the toilet . thanks for your help                                                                                                                                                                            Charlene: hello and thank you for all your help. my wife and i have been on a body transformation journey for about two years now. we have not reached our goals, our goal is overall health but are trying to build muscle and eventually lower our body fat percentages to a healthy number. we have been on a high protein diet about a gram per pound of body weight . i know this is not great for long term. how long is too long to be on this sort of diet and how should we best go about cycling our diet for best results. trying to get down to the 20 - 30 % body fat from 40 -50%. we also strength train regularly                                                                                                                                                                                               Dan: my teenage son has alot of acne. nothing seems to do any good for it and its much worse under his shirt sleeves. our doctor wanted him to take an antibiotic di something or other. we have tried a couple or topical treatments but nothing seems to work how do we get to the bottom of this?                                          Jean: I wake up too many days now with brain fog, extreme fatigue, no energy and headaches. Different parts of my body have discomfort. Thank you for answering my question.                                                                                                                          Sheena: Hi Dr. C! Hope you and your team are well. (This is the third time I've written in regarding this question). My liquid vitamin D says 1 drop equals 1000iu. . I was wondering if I can trust that? Because it seems soo little compared to a tablet. I end up consuming more drops then I need to, just in case. Our healthcare system treats us like it's one size fits all, yet that's not how true healing works, especially when it comes to chronic conditions. Today's guest will help us understand how to fill the gap we experience with Western medicine, including the difference between signs vs. symptoms, and empowers our individualized healthcare. On this episode of Salad with a Side of Fries, host Jenn Trepeck welcomes Functional Medicine Nutritionist and educator Andrea Nakayama to explore why chronic conditions require more than just a diagnosis and a prescription. They unpack Andrea's “three roots, many branches” model, revealing how digestion, inflammation, and genetics lie at the heart of chronic illness—and how nurturing the "soil" around them can lead to true healing. From nutrition habits to the importance of bio-individuality, this conversation empowers you to become an informed partner in your health care. The Salad With a Side of Fries podcast is hosted by Jenn Trepeck, who discusses wellness and weight loss for real life, clearing up the myths, misinformation, bad science & marketing surrounding our nutrition knowledge and the food industry. Let's dive into wellness and weight loss for real life, including drinking, eating out, and skipping the grocery store. IN THIS EPISODE:(00:00) Intro: Our healthcare system is one size fits all(05:08) Andrea shares her motivation for her journey into functional nutrition, and a discussion of Western medicine(07:41) Should every person receive the same treatment for the same chronic condition(11:45) How to choose a guide in the health equation(16:40) Knowing the difference between your signs vs. symptoms(21:59) Andrea walks us through the “three roots and many branches” model(28:36) The roles digestion and inflammation play in our health(33:37) Tiers to nutrition mastery. Tier one is our non-negotiables(36:40) Don't neglect your passion and purpose(38:48) Andrea's non-negotiable trifecta: Sleep, poop, and blood sugarKEY TAKEAWAYS:Chronic health conditions must be understood through a systems-based lens, not just symptom management. Every chronic issue has three core roots—genes, digestion, and inflammation. Healing begins by nurturing these “roots” and the surrounding “soil,” rather than chasing individual symptoms alone. Western medicine excels in acute care but often falls short in managing chronic conditions. Patients are frequently seen through the lens of their diagnosis rather than as whole individuals, resulting in treatments that overlook their unique health histories and personal needs. One of the most powerful tools patients can use is specific, emotion-free communication based on personal health tracking. By clearly distinguishing between signs (measurable indicators like fever or rash) and symptoms (subjective experiences like fatigue or pain) and offering detailed timelines of when and how issues arise, patients empower providers to better understand and address their unique health needs.Supporting foundational health can start with three simple, non-negotiable nutrition habits: include fat, fiber, and protein at every meal or snack to help stabilize blood sugar; eat the rainbow to boost fiber and phytonutrient intake while feeding your microbiome; and know your yes-no-maybe list to understand better how specific foods impact your body. Tracking what makes you feel great, what doesn't, and what you're unsure about builds awareness and equips you to have more productive, personalized conversations with your healthcare provider.QUOTES:         (07:41) “The first two gaps I encountered were that people are treated like their diagnosis. So, when you are in the medical system, you are your endometriosis, your fibroids, your breast cancer, your PMS. You are your symptom. The other gap was that everybody with the same symptom or diagnosis was treated the same.” - Andrea Nakayama(09:34) “A chronic condition means you're sick and you're not getting better. There's things going on for a long time, and our medical system isn't trained to address chronic conditions.” Andrea Nakayama(25:32) “The three roots of any chronic condition are always our genes, digestion and inflammation.  All three need to be tended to for the branches to express themselves more fully.” - Andrea Nakayama(36:53) “We have an episode called the Longevity Equation, and one about Blue Zones and one of the big factors there that everybody seems to overlook is community and connection.” - Jenn TrepeckRESOURCES:Become A Member of Salad with a Side of FriesJenn's Free Menu PlanA Salad With a Side of FriesA Salad With A Side Of Fries MerchA Salad With a Side of Fries InstagramNutrition Nugget: Genes and EpigeneticsSalad With a Side of Fries: The Longevity EquationSalad With a Side of Fries: Blue Zones: A Blueprint for Physical and Mental HealthSalad With a Side of Fries: The Down and DirtyGUEST RESOURCES:Andrea Nakayama - WebsiteFunctional Nutrition Alliance  - WebsiteAndrea Nakayama - InstagramFunctional Nutrition Alliance - YouTubeAndrea Nakayama - PinterestAndrea Nakayama, Functional Nutritionist & EducatorAndrea Nakayama - Functional Nutrition Alliance | LinkedInGUEST BIOGRAPHY:Andrea is a Functional Medicine Nutritionist and educator who has led thousands of clients and teaches even more coaches and clinicians worldwide. Encouraging physicians to make a diagnosis of irritable bowel syndrome and prescribe medication for it is big business. I'm not saying that medication for IBS is never a good idea or never effective, but I am saying that it is often an overly expedient solution that doesn't usually lead to lasting relief for the simple reason that IBS consists of multiple different conditions. Since there is no one unifying diagnosis of "IBS" there can't be one unifying pharmaceutical solution. We'd love to hear from you - send us a text!Thank you for being a part of our community! Welcome back to our weekend Cabral HouseCall shows! This is where we answer our community's wellness, weight loss, and anti-aging questions to help people get back on track! Check out today's questions:    Snaz: Hi Dr. Cabral. I am located in Southern CA in an area impacted by fires. The air has been very ashy. When the farmers market reopened after the initial crisis had passed, I had gone shopping at the market and then I was washing my veggies and started to think about how this toxic ash may end up in our food being grown in the region. What if anything can or should we do to prevent the body from absorbing these additional toxins? I can remember "smoky" vintages of wine from previous fires. This obviously ends up in our food system through direct settling on the produce or absorption in the soil. Is there anything extra we can do to protect our health from this additional source of toxins? Or just all of the usual things you talk about? Thank you for your thoughts.                           Gemma: Has the Dr Cabral practice ever worked with Milky Oats (avena sativa)? I see that it is often used for anxiety (amongst other things such as alcohol or drug recovery) but I would like to understand if you have seen this work in practice and if its worthwhile as its quite an expensive herb. Pub med is hinting that it got good results but theres a lot of science in that report! It behaves differently from other herbs for anxiety as other herbs such as passionflower seem to make you sleepy and switch off your brain. I dont quite understand the science behind this one but you take it every day, no sleepiness, no withdrawal symptoms if you forget….seems like a wonder herb in theory. And apparently can also lower cholesterol as an added bonus! Why isn't half the population on this?                        Michael: Hi Dr. Cabral, Thank you for everything you do. My question is regarding cone beam CT scans at dental offices. What is your recommendation for having this imaging done? I'm 38 and had all 4 of my wisdom teeth pulled about 5 years ago. I've had a history of bad oral health that I've been working on. I've also had some intermittent jaw/facial symptoms on one side of my face for the last year or so. I recently started to see a holistic dentist and the CBCT is an option. My worry is the radiation with CT scans.                                                                                                                                                             Michael: Hi Dr. Cabral, I have a question regarding regular sauna use. I've read a lot about the benefits of multiple sauna sessions per week. I have used a dry sauna at a local gym regularly for a couple of years. I recently purchased a sauna blanket from Higher Dose for the convenience of not having to travel to the gym every day. I do sweat a lot using the sauna blanket especially when I use it after a workout. Is there more of a benefit from a sauna that you're inside of and breathing the warm air, or does the benefit simply come from warming the body to the point of sweating which is still provided by the sauna blanket? Thank you for taking the time to answer everyone's questions!                                                  Belinda: Hi doctor Cabral, I was wondering if you could help me with the following: every morning, I have a large, wel-formed, normal bowel movement, but in the afternoon, I feel the urge to go again, but I'm only able to pass a small amount of stool. I'm experiencing bloating and gas, and I feel like I should be able to have a more complete bowel movement. I'm eating a healthy diet with plenty of fiber, drinking enough water, and I recently completed a cleanse, but my symptoms haven't improved. Could you please provide some insight into what might be causing these issues? Really grateful for your help here. Thanks so much! Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Ales: Dr. Cabral, what do you think of Palmitoylethanolamide (PEA) as a natural help with severe histamine intolerance? If everything else fails, would you recommend this compound to help with mast cells a and supress their excessive production of histamine? Is it even natural suplement like compound or a drug? Please help me understand. Thank you   Sarah: I've had reoccurring UTIs and always have to take antibiotics to clear it despite knowing how destructive they can be. This has led to reoccurring yeast infections. I'm about to start the CBO protocol for the yeast, how do I tackle the UTIs? I've tried ALL the natural remedies (dmannose, uva ursi, oregano, probiotics etc) and still suffering and feel my only resort is antibiotics which I want to avoid, as I'm now dealing with yeast overgrowth too. Thank you!!   Sarah: My boyfriend has a staph infection that is showing up as rash/pimples all over his body. Dermatologists are telling him the only option is antibiotics. He also has done lab testing and is showing an overgrowth of candida krusei. Trying to avoid antibiotic use as much as possible especially when he has an overgrowth of candida. He is about to start the CBO protocol. Is there anything that can be done for staph naturally/preventing it, other than washing with ACV, tea trea, coconut oil as we've tried that.   Jim: Hello Dr. C! I can't thank you enough for all of the hard work you put into these podcasts. I binge listen to them when I travel. I've searched but can't find anything on Post Exertional Malaise. I've been dealing with a variety of health issues from Hashimoto's to, most recently, toxic mold poisoning. I'm currently doing your mold protocol and am having some really bad flare-ups that are very similar to ones I've been experiencing for a few years. To describe what happens, it's as if I am a battery powered robot and my battery go dead. I can literally feel my body shut off, like my battery has been drained. I've had "the virus" twice so I looked into Long Covid and ran across PEM. I checked all the boxes for the symptoms but how can I verify it is this and not the mold?   Amanda: Hello! Just wondering what to do for a 3 year old with very inconsistent bowel movements/pellets. Tried high dose vitamin C/magnesium citrate, oxypowder, higher fiber diet, castor oil packs at night, energy work, osteopath, Epsom salt baths, prunes, pears etc. Diet is on point, only choline and ghee butter trigger bowel movements, but if I stop these then it gets irregular. Any advice on supplements/tinctures or foods that can support bile flow? Any advice on how to correct these bowel issues for a toddler, or where to start? Thank you very much for all your help !!!   Thank you for tuning into this weekend's Cabral HouseCalls and be sure to check back tomorrow for our Mindset & Motivation Monday show to get your week started off right! - - - Show Notes and Resources: StephenCabral.com/3068 - - - Get a FREE Copy of Dr. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Kelly: Hi Dr. Cabral I have always had very regular digestion and eat a healthy Whole Foods plant-based vegan diet. However, after having my baby nine months ago, everything changed. I went through natural childbirth and the babies head size was in the 99th percentile I had some pelvic floor issues after delivery And part of that was bleeding after bowel movements. The therapist attributed it to structural changes in pelvic floor. I have worked on pelvic floor exercises to remedy this, but currently the only thing that prevents me from bleeding when having bowel movements is drinking one “smooth move” tea per night, which features the herb Senna. I understand that you should never take laxatives on a daily basis, but this is the only thing that helps! Is senna ok?   Kenzie: I saw you recommended fecal immunotherapy test. If you sometimes have blood from hemmorhoids would that show in test/ effect results? Thank you   Anonymous: Hi, dr. C., I’m tracking my cycle with Oura.My follicular phase is usually between 17-21 days long,my luteal phase is around 10 days. What does that mean? My periods are not strong but they start with spotting a few days before the actual VERY PAINFUL first day of my period. The cramps/pain come in 3 or 4 intervals that last around 2h each, usually only on that first day but lately even on the second day once or twice. The pain is excruciating & unbearable, but I don’t take any pharmaceuticals. I use supplements, not sure about their bioavailability. I have fibromyalgia & I see with Oura that I get a lot of REM sleep but not 90min of deep sleep (usually between 1h or 75min, rarely 85). My stress levels are also very high throughout most of the day. Testing is not an option, please help   Anonymous: Hi ! I started taking parasite protocol last month and 2 weeks later , i began seeing white specks(that look like sesame seeds), and black flat hard shells(the size of a small bean) in my stool. I have been seeing so may of them since as well as a lot of mucus in the stool, and I am wondering if you have any insight on what kinds of parasite or parasite eggs these could be? If these are eggs and not fully grown parasites, would that mean I got the parasitic infection recently? I haven't seen any grown worms yet. Also, what can I do so I never get parasites again? It is so traumatic.   Lindsay: Hi Dr Cabral. I have self diagnosed myself with (R-CPD). I have all the symptoms. Mainly I can burp and get very gassy in my stomach because of it. I have no pain like other thank god. In my research the only thing I've found people do for relief is botox on their throat. Is there any functional medicine practices to help with the extended stomach issue from this?   Hannah: Hi Dr.Cabral. I would first off like to thankyou for all of the information you provide on these podcasts, it has changed my life in so many ways for the better. Im 17 and have been listening for 2 years now every since i started getting sick, i have had amenorrhea for two years that was followed by losing 17 kg unexpectedly in 6 months and alot of other issues with my gut even eating over 3000 calories a day (but whole food) . However the issue i have now is my thyroid keeps getting lower (currently at 3.74) and i cannot stop binge eating after a TBI in a horse riding accident and have gained 5+ kg in 1 month and it keeps going up. I cant afford lab tests (i live in australia) as i have looked into it and it just isnt possible, do you have any other recommendations?
