Dr. Costantino is the sole practitioner in her outpatient-based catheterization lab, and she finds that ultrasound provides an opportunity for her to connect with patients and communicate their diagnoses and options in a straightforward manner. She also recognizes the value of investing in advanced ultrasound training for her vascular technologists and for herself, so the practice can function cohesively and focus on providing patient-centered care. On the other hand, Dr. Montero-Baker speaks about the utility of ultrasound in practices across the globe, particularly in resource-limited settings. Additionally, he describes the need for the longitudinal follow up needed to fight this chronic disease process, as well as the need to manage holistic aspects of CTLI such as proper nutrition, wound care, and orthotics. He is currently building a clinical center that will provide these aspects. We take a moment to reflect on the power of social media as a unifying force for a like-minded “global tribe” of vascular specialists. All three guests can recall numerous instances in which they were inspired by another practitioner, or they were contacted and asked for advice by others. Dr. Costantino adds that social media has helped her realize that there is a strong need for collaboration between interventional radiologists and vascular surgeons, in order to generate treatment options and provide quality patient care. She speaks about the importance of learning from and trusting other specialties. To wrap up the episode, we examine how innovation in ultrasound technology has largely stagnated with the advent of other imaging modalities and opportunities to better incorporate ultrasound into CLTI care. Dr. Costantino encourages vascular specialists to seek out ultrasound training and let patient symptoms and quality of life guide treatment decisions. This week on The Podcast: In Episode 241 Jill Sommerset interviews Vascular Surgeon Miguel Montero Baker about how he learned advanced vascular ultrasound skills in his unique training path, and how important these skills are in his daily practice, including the pre-procedure planning and workup of chronic limb-threatening ischemia (CLTI) patients.
Pianista, director, compositor, fue uno de los grandes representantes del tango milonguero, popular, y falleció el 25 de julio de 1995, cuando tenía 89 años. Osvaldo Pugliese nació en una familia de músicos el 2 de diciembre de 1905, en el barrio porteño de Villa Crespo, aunque de chico se resistió a seguir esa tradición familiar. En 1918, dejó la escuela primaria para trabajar como gráfico de imprenta pero su padre lo convenció y comenzó a tomar clases de piano con el maestro Antonio D'Agostino. Desde entonces, su carrera fue imparable y llegaron los primeros tangos: Recuerdo, Retoños; el primer trabajo de músico en Café de la Chancha; su primer sexteto con Pedro Maffia, hasta su primera orquesta definitiva, en 1939. Debido a la aparición del cine sonoro y la gran crisis del año 1929, la desocupación empujó a muchos de su profesión a formar el Sindicato Argentino de Músicos del que fue el afiliado número 5, para promover las labores de la música. Las recurrentes alusiones a las condiciones de trabajo “feudales” que sufrían los músicos lo asociaron al lenguaje utilizado por entonces al Partido Comunista, al cual se afilió en 1936, lo que le trajo en los años 40 y 50 problemas, censuras e incluso días de cárcel. Crítico de las políticas culturales de las dictaduras de los `60 y `70, de los organismos como SADAIC, pero optimista en cuanto al futuro del tango, logró en 1985 tocar en el Teatro Colón ante un público que lo adoró y ovacionó. Por su orquesta, pasaron destacados cantante e intérpretes como Roberto Chanel, Alberto Morán, Jorge Vidal, Jorge Maciel, Miguel Montero, Alfredo Belusi, Adrián Guida y Abel Córdoba. Recibió innumerables distinciones tanto nacionales como internacionales, entre ellas, la otorgada por el gobierno de Cuba, la medalla “Alejo Carpentier”, la más importante distinción cultural de la Isla; el gobierno francés lo nombró Commandeur de L'Ordre des Arts et Letters y Buenos Aires lo declaró Ciudadano Ilustre. La Sociedad Argentina de Autores y Compositores de Música (SADAIC) y la Asociación de Coleccionistas de Tango colocaron una placa en la Av. Corrientes 960, en conmemoración y por sus 50 años de carrera y aporte a la cultura argentina y al tango. En 1990, recibió el título de Académico Honorario de la Academia Nacional del Tango. El 25 de julio de 1995, a los 89 años, falleció en la Ciudad de Buenos Aires. Lo recordamos con un informe elaborado por el Área de Contenidos y conservado en el Archivo Histórico de Radio Nacional. El cantante puertorriqueño Héctor Lavoe publicó en 1979 su álbum “Recordando a Felipe Pirela” en el que realiza un homenaje al gran intérprete venezolano y en el que incluyó el bolero “El retrato de mamá”. Así la escuché yo… El propio Felipe Pirela “El bolerista de América” grabó la canción cuando integraba la orquesta Billo's Caracas Boys. Una de las más recordadas interpretaciones de “El retrato de mamá” la realizó el ecuatoriano Julio Jaramillo “El ruiseñor de América” en ritmo de bolero. Lo que muchos desconocen es que esta canción no nació en ritmo de bolero, sino que originalmente es un tango escrito por Carlos Russo y Marujita Falero y cuya primera grabación la realizó el cantante argentino Miguel Montero en 1959. Como dato curioso, hay que decir que algunas fuentes afirman que el músico Luis Rafael Caruso es el verdadero coautor de la canción junto a Carlos Russo, pero que por cuestiones legales debió acreditarla a nombre de su esposa María Aurora Falero de Caruso. The year was 2001 when Miguel Montero, a 16-year-old, living with his family in Caracas, Venezuela, was signed into baseball's minor leagues. His contract was for $13,000, but the money was hardly what excited Miguel. “All I wanted was a plane ticket and put me in the Dominican and leave the rest up to me,” he recalls. But being away from home, he quickly found that it wasn't easy. He was fending for himself far from his family, and he wasn't playing his best ball. There were times, he remembers, when he thought of quitting. But Miguel found the willpower to stick around, and in doing so, he learned a lesson that he carried with him all the way to the World Series. “I was not the most talented guy,” he says. “I really worked hard. As you know, and as many of our guests on Just the Tips have said before, being a successful entrepreneur is difficult, consuming work. The same could be said about being in a successful relationship. That makes it all the more fascinating that the guests on today’s show are doing both of those things so well. Masa Cemazar and Miguel Montero have launched dozens of successful businesses and two years ago embarked on their latest and largest endeavor, Pyjama Bosses, their software and training company for network marketers. Dean and I had a lot of fun talking with this power couple, and you’re going to appreciate how open and honest they are not only about their business but how they make it work balancing startups and matrimony. How “Desperation” turned a pilot and a geneticist into an entrepreneur couple Dean and I enjoyed hearing how Masa and Miguel first met, on a plane ride to Australia. The international couple—she’s from Slovenia, he’s from Spain—decided to make that country their home. But Miguel had difficulty finding a job as a pilot due to visa issues, while Masa held down a six-figure job as a geneticist. But she told us that she was frustrated with her own job, too, discovering her work did more to benefit pharmaceutical companies than it did people. So the two of them decided to go into business by themselves, for themselves. On this episode of Just the Tips, You’re going to be inspired by how thoughtful and mindful they are about how they made that transition and how they’re always pushing themselves—and each other—to grow. In Australia, if you argue with your wife, you sleep with the snakes As I said, Masa and Miguel lay it all out for listeners of Just the Tips, so when I asked them what it’s like when they have a disagreement, Miguel didn’t hesitate to share a surprising story about how they make it work. One night, he and Masa were arguing, and Miguel felt he needed to take a breath and take a walk, so he ambled down the street to a park bench to cool off. He decided to lay down on the bench and, with the beautiful Australian weather providing a comfortable spot, decided to sleep off the fight. But then, he remembered that the poisonous snakes of Australia would have other ideas, so he hurried back to his wife, and made up. As Masa told us: “Certainly there have been very hard times. But you have to get your minds really clear and really together.” You’re going to want to hear how they do that. What is work/life balance when work is your life? Just like any entrepreneur, Masa and Miguel face the difficult task of separating their work from the rest of their life. It goes double for them since they are business partners and husband and wife. But you’ll be able to learn a lot from how they toss aside the conventional wisdom that says you need that bright line between your work and your home life. As you hear Masa explain their routine of starting the day, walking the dog, discussing what’s coming up for the day in their business and what they hope the outcomes will be, you’ll see that if you’re already living your passion, there’s no need to compartmentalize them. A big mid-season test for the Cubs came via the 4-game Washington series over this past week. Well, a couple of big tests, as it turns out. The most obvious was being pitted against a dangerous Nationals team that was playing really good baseball, and doing it while a good portion of the team's core was either on the DL or working through things in the minors. And then some apparent intra-squad friction reared its head when now-former Cub catcher Miguel Montero lamented Jake Arrieta's deliberate delivery to the plate as the Nats tied a club record for stolen bases. The two insisted the row was no big deal and that they hashed things out privately, but going 0-31 in catching base runners stealing this year for a backup catcher, albeit with last year's postseason heroics, became too much for the Cubs front office to bear. And with that, none of last season's three catchers on the Opening Day roster for the World Series champion Chicago Cubs are with the team. That shouldn't remain so for long, with Kyle Schwarber looking better already down in triple-A ball. Following the return of Addison Russell, the Cubs can look forward to getting Schwarber back along with the injured Jason Heyward and Ben Zobrist sometime soon. But, it wouldn't be the Cubs without Kris Bryant accidentally stepping on third base fielding a routine pop up and spraining his ankle in doing so. He's day to day, like we all are. After all, that's Cub.

We get Tyler back this week but open up with the World Series and check in with Pops to see how he's feeling at this historic championship. Did Joe Maddon over manage game 7? There were plenty of times for the Chicago Cubs to revert back to the choker persona, but this team is different. What was the turning point in the World Series? The Kris Bryant home run in game 5? Should Ben Zobrist have been the MVP? Are the Indians and Cubs the favorites for next year? Tyler takes on Cleveland as a team that was going to change the logo and then slapped it everywhere after getting to the Series. We aren't asking the D question, but when it comes to the Cubs' talent how about the simple question of where does Zobrist play? Chapman is a free agent, does he come back? Pops tries to blow up the entire Cubs pen. We were expecting a down season for the Blackhawks and they really are streaking right now. Are they better than we thought? Vey little to go off for the Bulls, but what do we make of the team on the short season? What do we make of the villains this season, the Golden State Warriors? Steve Kerr's squad may have a bit of an attitude change after going all in to chase 72-10 last season. LSU and Alabama starts of college football. It was tough, low scoring and physical. What do we make of the game and some missed calls? What do we take away for Bama? Is it an impressive win? Everyone freaked out about Texas A&M being in the first Playoff poll but they handled it themselves by getting upset by Mississippi State. So who do we think are actually the four best teams? If Washington wins the PAC12 will they be in? Pops is still riding high on Louisville, can they jump the conference champion? From Saban we trust to Belichick we trust, what does Pops make of the Jamie Collins trade? It looks very much like another possible Patriots' Super Bowl year, could this damper that? I mean who stands out in the NFL, let alone the AFC to challenge New England? Who would think with a triple side of beef Tyler doesn't have any. For his beef Pops is beefin on Miguel Montero complaining during the World Series parade. For Blair's beef he is taking on delusional quaterbacks. Ryan Fitzpatrick and Jay Cutler speak out about not having support of their teams...after being bad. If you don't play well you lose your job. Tyler jumps in to beef on some of Pops' arguments against non-SEC playoff teams.
Pops was on the wait and see train last week for Purdue who got dominated by Iowa at half and now Darrell Hazell is fired. Do we give Mike Bobinski credit or was his hand forced? The Boilermakers are finally getting lights so they can host night games, one of the last programs to get them. The Hazell hire was un-Purdue move and canning him before the season is over is a move in new territory. We give a little update on the possible coaching candidates Purdue could be looking at. We get an updated on college football, starting with Notre Dame losing again and Alabama trouncing Tennessee. Houston lost which opens up the BCS Invitational picture. Are we buying Washington, Baylor or West Virginia? Does a one loss Michigan, Ohio State or Louisville get in instead? Pops is 100% against that school up North this season but his normal punching bag, Nebraska, is still undefeated. Mark Dantonio and Michigan State are really having a down year, will he leave because of it? We move over to baseball where Pops said he wasn't worried when they got ready to face off against the Giants but what about being down 5-2 going into the 9th of Game 4? How about that decision to load the bases in Game 1 before Miguel Montero hit a Grand Slam to win the game. Do we have any worries over the pen with Aroldis Champan's issues to stop inherited runners from scoring? Especially with the new habit being to use closers for more than just an inning. The ALCS match up is Cleveland and Toronto, the exact opposite of what we thought. Who do we think is going to the World Series? How crucial is Game 2 against Clayton Kershaw? Can we count on John Lackey for Game 4 in the NLCS? Hockey and basketball season are around the corner and we kick off the previews with the Chicago Blackhakws. This may be a season unlike fans have seen in awhile with the all the youth and depth depleted from the cap issues. Is a salary dump coming finally? You have the core of Kane, Toews, Panarin and Crawford so Stan Bowman has tried to stay in the hunt for the Stanley Cup, but it may cost your core's prime with just first or second round exits. Are we starting to lose faith in Stan Bowman? Pops also gives us his Top 4 team in the West and East, though really it seems like either Pittsburgh or Tampa. We break the NBA in half and knock out the Western Conference preview this week. Is the Northwest division still OKC's to win? Does Minnesota break the drought and get into the playoffs? What does Damian Lilliard and Portland do this season? Utah is another young roster as well, do they take a step forward? In the Pacific Division of course we take Golden State. The Clippers seems likely to get into the playoffs, but anyone else? How long does it take the Warriors to gel? In the Southwest Division it's still clearly the Spurs, but do they drop off? Memphis spent a bunch of money but are still getting pretty old, does Anthony Davis stay healthy, Dallas lost their off season again and Houston added more jump shooters so that should work right? We pick our eight Western playoff teams. Who is the biggest threat to the Warriors in the Western Conference? Pops has beef with the Bears who are quickly trending downwards.
