« Les ménages sont coincés dans des maisons qui ne pouvaient pas accueillir trois, quatre familles. Dans le quartier Mpoli, nous avons enregistré cinquante-quatre maisons et dix bureaux de l'État. Mais dans le quartier Basoko, c'est au-delà de cinquante ». Ce témoignage de Martin Suta, président  de la société civile de Kwamouth, dans la province du Mai-Ndombe, révèle l'ampleur des dégâts causés par les eaux du fleuve Congo, qui ont quitté leur lit ces derniers jours.Ce débordement a atteint jusqu'à  6,5 mètres au-dessus du niveau moyen de la mer, selon la Régie des voies fluviales (RVF). Il fait  des ravages dans toutes les régions côtières de Kinshasa,  Ituri,   Kongo-central, Tshopo, Equateur, Mai-Ndombe, etc.A Kinshasa par exemple,  le pont reliant le quartier Kinsuka à la carrière des grès (Carrigrès) dans la commune de Ngaliema est plongé sous les eaux tumultueuses du fleuve. Cette catastrophe naturelle a jeté de nombreux ménages dans la détresse, obligeant les riverains à payer 500 Fc par jour pour emprunter des pirogues afin de traverser le pont. Autrement, ils doivent entreprendre un long détour via Pompage, un trajet qui, autrefois, ne prenait que cinq minutes. A Mbandaka, dans la province de l'Equateur, les inondations, résultant des pluies incessantes, ont plongé les quartiers d'Ekundé, Basoko, Bongodjo et le territoire de Bikoro dans une situation critique. Les conséquences désastreuses de ces inondations sont particulièrement visibles avec plus de 100 familles se retrouvant sans abri, leurs maisons ayant été emportées par les eaux déchaînées. Ces familles endurent des conditions de vie précaires et inacceptables, dépourvues de tout moyen pour subvenir à leurs besoins fondamentaux. En outre, plusieurs activités commerciales ont été englouties, et le bon fonctionnement de certains ports a été sérieusement compromis.Cette situation dramatique était parmi les principaux thèmes évoqués par le Pape François dimanche 7 janvier 2024 après avoir récité la prière de l'angélus à la place Saint-Pierre à Rome en Italie.Comment expliquer le débordement des eaux du fleuve Congo ? C'était une idée un peu folle de vouloir créer un énième guide sur le vin, tant l'offre existante (Parker, RVF, Bettane, Guide Hachette, Glouguide…) semble déjà pléthorique et l'usage des guides à l'ancienne gentiment s'étioler. Rift Valley fever (RVF) is a major emerging, zoonotic and arboviral haemorrhagic fever that greatly affects public and animal health, primarily in Africa. The ability of RVF virus to persist within distinct environments has raised questions over its potential to spread and persist within currently disease-free settings. Viral persistence requires suitable meteorological factors for vector competence and breeding alongside sufficiently susceptible local host communities. Prior to our work, no study had formally estimated the importance of these persistence mechanisms from field data, preventing any formal assessment of disease control strategies in a real-world setting.

Peste des petits ruminants (PPR) and Rift Valley Fever are examples of transboundary animal diseases having major consequences in terms of health and economics. PPR is a highly pathogenic viral disease infecting principally sheep and goats, but also some wild artiodactyls, camelids and suids. This disease circulates in large parts of Africa, Asia, Eurasia and Middle-East and it has recently appeared at the gates of Europe in Georgia (2016) and Bulgaria (2018). A vaccine sxiste and the disease is the target of an eradication campaign by 2030. RVF is an arbovirus also affecting small ruminants and camelids, but also cattle and can be transmitted to human. The virus is transmitted among ruminants through the bite of infected mosquitoes (belonging to the Aedes and Culex species) and to human through the direct contact with infected blood or tissues. The disease circulates mostly in most countries in North, East, West and Sahelian Africa as well as in the Arabian Peninsula. Despite the different way of transmission, transboundary animal movement is the most likely route of the spread of the two diseases. Even though the livestock trade is strictly regulated between endemic regions of the two diseases and most of the Mediterranean countries, regional, locally adapted strategies are needed to control the diseases efficiently. The aim of this talk is to provide a panorama of the epidemiological situation around Europe and provide some preliminary scenario for the introduction of the diseases, through animal movement, and risk of transmission due to the presence of competent vectors (only for Rift Valley Fever).

Au fil des années, l'évenement s'est décliné en plusieurs salons, dont Beaujol'Art où nous nous sommes rendus cette année. Existent aussi dans la famille BBB les salons Beaujol'Wines, La Biojolaise, La Beaujoloise, Les Gamay Chics... Au somptueux Chateau de Pisay où se déroule l'évenement nous rencontrons d'abord Sylvan Tête, président de Beaujol'Art et vigneron au Domaine Le Clos Du Fief. Avec lui nous dégustons un Juliénas fruité et généreux, excellente entrée en matière dans ce voyage en 1001 nuances de Gamay. Claude-Emannuelle Desvignes lui succède à notre micro, elle incarne la 8eme génération du Domaine Desvignes, une des références de la région, comme en témoignent les 3* à la RVF, et ce superbe Morgon . Enfin, Mee Godard nous raconte son coup de coeur pour le Gamay et sa difficulté à juger de son propre travail... que nous trouvons magnifique! Dimanche 13 février 2022  Maxime Cottenceau  Né en 1996 et titulaire d'un BTS œnologie, il a appris pendant deux ans aux côtés de son mentor, l'excellent Vincent Dureuil à Rully, ce qui lui a permis de poser des bases solides pour se lancer à son tour. Maxime s'installe début 2018 avec un peu moins de 4 hectares en Montagny 1er Cru, Givry Village Blanc, côteaux Bourguignons et Mercurey Village Rouge.  Aujourd'hui, la superficie totale s'étend sur 5,5 hectares. Le respect et la protection de l'environnement font parties intégrantes des préoccupations de Maxime, c'est pourquoi il met dès le début une pratique biologique en place et lance la conversion en bio à partir du millésime 2020, les vendanges sont effectuées à la main et les travaux sont tous réalisés dans le respect de l'environnement. Toutes les étapes réalisées en cave suivent même le cycle lunaire. Les débuts de Maxime en tant que vigneron sont prometteurs : à noter que son talent et la précision de ses vinifications lui valent même de faire son entrée dans la RVF.   Pauline Lair  Après des études spécialisées dans les vins et spiritueux en école de commerce à Bordeaux, Pauline passe plusieurs années à vendre des Grands Crus de Bordeaux à l'export, marketer des vodkas de luxe en France et à l'export. Puis elle décide de tout plaquer pour devenir vigneronne. Au départ, son envie est de créer un domaine sur les grands terroirs d'Anjou. Mais Pauline reconsidère vite son modèle d'installation, notamment à la suite des dévastateurs gels de 2017 et 2019. C'est alors qu'une idée lui vient en tête : créer un négoce artisanal de raisins de qualité. Son goût pour la vinification, l'envie de partager sa passion des vins de Loire et sa vie urbaine s'expriment alors dans le chai urbain qu'elle installe dans le centre d'Angers. Pauline travaille en partenariat avec des vigneronnes et vignerons de talent qui, tous, suivent le cahier des charges bio. Elle vinifie les raisins de ses partenaires de manière naturelle. Ce qui l'anime est de façonner des vins digestes, sincères et reflétant leur terroir d'origine.

Recently, the American Family Physician journal published a practice guideline update issued by the Global Initiative for Asthma (GINA). GINA now recommends against using short-acting beta2 agonist (SABA) as sole therapy for patients with mild intermittent asthma (Step 1). A low-dose inhaled corticosteroid (ICS) and formoterol combination used as needed is the preferred treatment in adults and adolescents in the Step 1 group. Some patients may not be cured, and that can be devastating for some physicians, but even when we cannot cure, we can often offer relief, and always provide comfort.  What a great teaching for us!Today we have Dr Verna Marquez. She is a faculty in our program who is always involved in new and exciting projects. Today, she will talk about POCUS. Dr Marquez, please introduce yourself.What is POCUS? Why is it important?POCUS stands for Point of Care Ultrasound. It is a goal-directed, bedside ultrasound examination performed by a healthcare provider to answer a specific question or to guide performance of an invasive procedure.  History: In 1940s Diagnostic ultrasounds was first developed and used in medicine, but POCUS has been integrated into diverse areas of clinical practice since the early 1990s. Impact in primary care: Many professional societies and national organizations nowadays have recognized the potent impact of POCUS and have endorsed its routine use in clinical practice. POCUS improves clinical outcomes, reduces failure rates during procedures, rapidly narrows differential diagnosis, shortens time to definitive treatment, lower costs, and reduces the use of ionizing radiation of CT imaging. It is especially empowering and critical for front line providers in rural, underserved, or resource-constrained environments where advance imaging and specialists are in scarce supply. Family physicians are often the providers in these key clinical contexts. Because Family medicine physicians have a strong background in obstetric ultrasound, family physicians are well positioned to learn other applications of POCUS. When can we use POCUS?POCUS can be used to assess most body systems. Generally, the rule is to “rule in” or “rule out” a specific condition or answer a “yes or no” question. Clinical applications:As a FM physician, we perform it mostly for diagnostics –and the most commonly performed ones are evaluation of GB, liver, kidneys, bladder, gravid and non-gravid uterus, joints, LE veins, breast, soft tissues, scrotal and since our program just started the curriculum, heart and lungs are other applications we can do as well. POCUS can narrow down our differential diagnosis based on the presenting signs and symptoms. It will guide additional investigations, especially in urgent or emergent situations.We can also use POCUS for Procedural guidance – it has been shown to reduce complications and improve success rates of invasive bedside procedures.We are also utilizing it for Screening such AAA. Screening with US is potentially advantageous because it is non- invasive and avoids ionizing radiation. Others are for Monitoring and resuscitation commonly performed in the hospital setting like in the ED and ICU. Example include monitoring for volume status on patients with CHF or dehydration so scanning for IVC distention and collapsibility, monitoring LV contraction in responses to inotrope initiation, and monitoring for resolution or worsening of pneumothorax or pneumonia on lung US.Bedside US can direct emergent interventions by rapidly detecting tension PT, cardiac tamponade and massive PE with acute RVF. What are important considerations when using POCUS?There are at least 3 things to consider while performing POCUS:Provider training- the amount of training required to achieve competency in POCUS applications varies by provider skill and exam type. Those with prior experience greatly facilitates learning new applications and no big deal. While those that are novice needs more exposure and more practice scanning to be comfortable with POCUS.  Also, the skills required relate to provider’s scope of practice. For example, a Rheumatologist may be proficient with MSK US but less proficient in cardiac or abdominal US, while the opposite maybe true for critical care physicians. For us as FM physician, we can do a lot which requires more time to practice scanning to be able to proficient. Patient factor – body habitus, positioning, and acute illness are important considerations when imaging patients. Similar to x ray, US waves are attenuated by adipose tissue, and US has limited penetration in morbidly obese patients. Lower frequencies US probe must be used for deeper penetration, resulting in lower resolution of images. Positioning can limit US examination; for example, apical cardiac US images is often limited in patients who cannot be placed in left lateral decubitus position. US equipment- Lack of familiarity with the equipment can present a barrier to its use. Fortunately, many machines nowadays are designed specifically for POCUS applications with ease of use as a primary feature. Providers must be familiar with basic operations including entering patient information, selecting the appropriate imaging mode, and adjusting the image depth and gain. Transducer availability is an important consideration because certain exams can be performed with multiple transducer types, whereas others can be performed only with a single transducer type. For example, and curvilinear or phased array can be used to evaluate the abdomen but only a phased array can be used to evaluate the heart. How are we going to learn POCUS in our residency?Our program has incorporated a formal POCUS curriculum this academic year. We have been actually applying Obstetrical POCUS for the last 5 years to our pregnant patient using our first US machine. The PGY2 and PGY3 residents have 2 weeks POCUS rotation under my direct supervision. During this rotation, residents have one on one hands on training with image acquisition and interpretation with the actual patients. Our program also provides each resident the SonoSim probe and online courses while simultaneously learning the proper scanning method. We conduct POCUS lecture didactics every 2 weeks with US demo of actual patients or volunteers. All residents at all levels are encouraged to scan their own patients on their own continuity clinic time when time allows for practice under my direct supervision._________________________Espanish Por Favor: Hígadoby Hector ArreazaThe Spanish word of the week is hígado. The hígado is culturally relevant because people generally know that the hígado will get sick if you drink too much alcohol, and as doctors we know that the hígado once is lost, here is no way back. Some patients will die if their hígado does not perform its function. So, counsel your patients to drink with moderation to preserve their hígado. “Señor, evite tomar alcohol para cuidar el hígado”. Hígado de res (cow’s liver) is commonly used in folk medicine to cure anemia, yes, it´s iron rich, but not very tasty for most people. I’m afraid your DNA is backwards.”--Patient: “AND?” Now we conclude our episode number 27 “POCUS.” Dr Marquez explained how an ultrasound done in clinic may answer many questions to facilitate patient care. POCUS may give you the answers you are looking for in those patients with abdominal pain, pelvic pain, and more. [CONMEMORACION] A 34 años del asesinato de Miguel Leal Diaz, Villa Francia no olvida a quienes entregaron su vida por la libertad del pueblo. Miguel Ángel Leal, nació el 15 de junio de 1968. Menor de tres hermanos, durante la Unidad popular vio como sus padres formaban parte del cambio revolucionario que se vivía en esos años. Miguel Leal desde los 14 años militaba en las juventudes comunistas, a la edad de 16 años pasa a formar parte de las filas del FPMR y así siendo parte de heroicas acciones en el comienzo de este movimiento como brazo armado del pueblo. Así transcurren los últimos 2 años de su vida, Miguel se entregaba por completo a la lucha revolucionaria por la libertad de su pueblo. El 5 de Septiembre de 1986 Miguel encabeza junto a pobladores y a otros combatientes, una marcha por el interior de Villa Francia, cuyo destino es la recuperación de alimentos en la panadería, es así como ingresa a la lugar para ayudar a escapar a la gente que estaba en el interior, Miguel toma la decisión de contener la represión, para que escaparan los pobladores, fue en ese momento en dónde cae abatido, defendiendo la retirada. Las evidencia burda de la cultura patriarcal en la viralización de la fiesta camionera en la carretera. Reflexionamos sobre la cultura patriarcal y machista que se evidenció en numerosos comentarios en las redes, sin importar, la posición política o la edad. El anuncio de desconfinamiento masivo para el 18 de septiembre, con Toque de Queda extendido. La planeada extensión del Estado de Emergencia y su inutilidad sanitaria. No es normal, ni debemos naturalizar tener milicos en las Calles, siempre, que además no sirven de nada. Reaparecen los viejísimos estandartes de la Concerta (G. Martínez, Escalona, Toha, etc) postulándose a la constituyente. Las Ultimas Noticias de La Tercera relata el papel de Boric en el Acuerdo del 15-Nov-2019. Atentos, para relexionar haremos un programa “Si Apruebo, ¿qué apruebo?”. Las analogías del actual proceso con el plebiscito del 88. Sin organización popular la vieja casta concertacionista y la derecha acabaran con la esperanza de cambio, y reconstruirán el mismo sistema, con un nuevo nombre, la Constitución de Piñera. ¿Los caidos, los ojos de Gustavo y Fabiola, el Matapacos cabe en un voto?. Hoy en RVF livestram con Capucha Informa, después de la aprobación en segundo trámite en la cámara de diputados. No se aprobó el Fondo Solidario. No podrán retirar sus fondos los jubilados. Reportes de represión y provocaciones en horas de la tarde de los pacos y milicos en las poblaciones. El intento de la Clase Política de legitimarse por haber votado por primera vez algo de interés del Pueblo, después de 9 meses de aprobar todas las leyes represivas de la derecha. El sistema de AFPs roba legalmente a los trabajadores en favor solo de los grandes empresarios. El retiro del 10% no es un triunfo (solo un mordisco al sistema), el dinero que retiremos se lo devolveremos a los mismos empresarios pagando créditos, cuentas y comida. El Estado Policial, se vive todos los días con milicos en las calles, y aun así, los oportunistas que hablan de “re legitimización” de los políticos.

No cuadran los datos de Paris y su “Paso a Paso” con las cifras de disminución de casos del Covid. El desmentido del Colmed sobre el Plan de Desconfinamiento. ¿Es seguro reiniciar la Economía por presiones de los empresarios?. Los últimos planes del Mineduc. El detalle del “Paso a Paso”. Los especialistas advierten que no es una política sanitaria correcta. ¿De lunes a viernes se reiniciaran las protestas? Se avizoran rebrotes. Cuando aparezca la vacuna, y llegue al tercer mundo, Chile estará a la cola, para recibirla. La manipulación de cifras, que ya lleva rato, podrían ser mayores a las reportadas por el DEIS. Proyecto retiro se vota el miércoles 22 temprano. El gobierno dio por perdida esta “batalla” y vetaría o recurrirá al TC. Para la verdadera Oposición, para el Pueblo, no hay que perder de vista que la verdadera batalla es NO+AFP. La “oposición” del Congreso, sólo ha votado leyes represivas para el Pueblo y económicas de “Doctrina de Shock”. Pa' los sapos de la Yuta, RVF es un medio de opinión desde la Pobla, no es académico, no es sociológico, es un medio de comunicación popular, aquí se habla de nuestra clase, los pobladores, no de Jadue, Jackson, Bachelet, etc.

Earlier this week, senior members of former Sudan President Omar al-Bashir’s regime were reportedly diagnosed with the coronavirus and transferred from the notorious Kober prison to a hospital in Sudan’s capital city of Khartoum.It has put a spotlight on how the country’s health care system was laid to waste under Bashir’s regime, which was toppled and replaced by a civilian-led government last year.“Our public hospitals in Sudan over the past 30 years have deteriorated to the extent that they are hardly providing the regular health services to patients.”Nahla Gadalla, Sudanese American Medical Association“Our public hospitals in Sudan over the past 30 years have deteriorated to the extent that they are hardly providing the regular health services to patients,” said Nahla Gadalla, executive director of the Sudanese American Medical Association. “There’s no clear flow of information. Qui se met comment au service du vin ? Comment donner sa meilleure chance au vin ? Comme je préfère poser des questions que finir en réaaaa.... je pars donc à la rencontre de ceux qui donnent le sourire au vin ! Aujourd'hui, épisode inédit, interview par téléphone avec Vianney, du Château Canadel à Bandol. Une conversation précieuse et nourrissante qui nous permet de nous évader au SUD... Nous avons parlé, dans le désordre, d'enfants et de vie, de la cave idéale, d'opportunité, d'oenologie, de vision de vinification, des restanques et de la brise marine, d'ugni blanc, de clairette, de cinsault et de mourvèdre évidemment, d'engagement en bio, de la juste maturité, de jeux de société et de chaussures, bien sûr ! Une heure d'échange avec un Vianney, qui a fait le choix de s'installer avec Laure, sa femme, après une première vie de baroudeur. Un amoureux de la vie (ça s'entend !), qui transmet sa passion au son de savoix, qui chausse du 48, écoute Jain et enchaîne les partie de Dixit. Je pars donc à la rencontre de ceux qui donnent le sourire au vin... comme l'agent très très spécial Romuald Cardon.  Cette conversation a filé, vite trop vite, et j'ai regretté d'y avoir mis un terme. Il aurait fallu au moins 2 épisodes tellement j'avais de questions pour cet agent de vignerons amoureux de montagne, de sport... mais aussi de bande-dessinée et de gastronomie. Dans le désordre, nous avons parlé de son métier, bien sûr : un métier de rencontres, où l'humain est au centre de tout. Un métier d'assiduité, d'endurance ! Nous avons aussi parlé de l'évolution de la cuisine, de liberté, de l'émotion (rare !) que l'on peut rencontrer à l'occasion d'un accord mets & vins, d'ultra-trails, d'ivresse et de soirées qui se terminent à l'aube, de confiance, de respect de l'autre, de chaussure bien sûr ! C'est un véritable entrepreneur que vous allez écouter, et son histoire est riche d'enseignements : celle d'un enfant qui n'a jamais réussi à l'école, et qui se retrouve aujourd'hui à la tête d'une entreprise florissante. Son expérience, il ne l'a pas apprise dans les livres, mais sur le terrain, et surtout au contact des vignerons.  Une conversation précieuse, inspirante et pleine d'espoir. N.B.: Nous n'avons pas parlé de Vendanges Solidaires, une association qui aide les jeunes vignerons à traverser les aléas des épreuves climatiques, une association d'utilité publique (qui accepte les dons !), et que Romuald a fondée il y a 4 ans. Pour l'anecdote, dimanche dernier au Resto-Zinc, j'ai eu en direct le témoignage d'un vigneron avec qui je travaille, Sylvain Liotard du domaine des 13 Lunes. Sylvain a insisté : l'association a permis à son tout jeune domaine de survivre après une saison dramatique ! N'hésitez pas à faire un tour sur leur site Internet. Guidelines. Algorithms. Evidence based medicine. These all play a significant part in the safe and effective management of the majority of our patients. As a result there is a danger that treatment pathways are followed blindly without critiquing their use and there is real risk we can loose sight of what’s best for the patient in front of us. Guidelines encourage inflexible decision making, which creates further challenge when we are met by patients who do not fit standard treatment pathways. If this is the case then the management of cardiac arrest, which is taught and delivered in a didactic and protocol driven fashion, is surely the pinnacle of the problem. Standard Advanced Life Support (ALS) is totally appropriate for the majority of cardiac arrests, but what happens when it fails our patients? Refractory ventricular fibrillation (rVF) is, by its very nature, defined by the failure of ALS, but frustratingly there is very little evidence, or guidance, surrounding how to manage this patient group. I was faced with this situation when called to support an ambulance crew who were resuscitating an out-of-hospital VF arrest. When benchmarked against the ALS guidelines their management had been exemplary, but the patient remained in VF after eight shocks. So what now? The UK Resuscitation Council doesn’t specifically discuss rVF, but offers the advice that it is “usually worthwhile continuing” if the patient remains in VF. Not a particularly controversial statement, but not much help either. They do discuss the potential for ECMO use, but this is currently a very rare option in UK practice, or thrombolysis for known or suspected pulmonary embolism. Other potential interventions not in the guidelines include IV magnesium and placing defibrillator pads in the anterior-posterior orientation. PCI can also be considered if there is a suitable receiving centre available and the patient can be delivered in a safe and timely fashion. A final option is the use of double sequential defibrillation (DSD), using two defibrillators, charged to their maximum energy setting, to deliver two shocks in an almost simultaneous fashion.DSD was first described in human subjects in 1994 when it was used to successfully defibrillate five patients who entered rVF during routine electrophysiologic testing. These patients were otherwise refractory to between seven and twenty single shocks. Looking at the available literature there has been little interest in DSD since then, until the last two to three years when it appears to have undergone a small revival. Sadly, there is no evidence for its use beyond case reports and small case series. The case reports appear to show good results with four in the last two years reporting survival to discharge with good neurological outcome. There are also a handful of other cases discussed in online blogs and articles with good outcomes. But these case reports and articles almost certainly represent an excellent example of publication bias. The most recent case series reviewed the use of DSD by an American ambulance service over a period of four years. During this time DSD was written into their refractory VF protocol, with rVF defined as failure of five single shocks. The study included twelve patients, three of whom survived to discharge with two of these demonstrating a cerebral performance score of 1. Despite appearing to demonstrate reasonable outcomes for DSD, sadly this study has a number of significant limitations. One important point the authors fail to discuss is that the two neurologically intact survivors received their DSD shocks after two and three single shocks respectively, not after five shocks which would have been per-protocol and consistent with the authors definition of refractory VF. This highlights a further problem with analysing the use of DSD. Not only is there a dearth of high-level evidence, but the literature that is available is highly inconsistent. There are a range of definitions for refractory VF, different orientations for the second set of pads, variable interventions prior to using DSD, a variety of timings between the shocks and so on. This means that comparison between studies and drawing meaningful conclusions is nearly impossible. Given these challenges, what are the explanations for why DSD might work? The first theory is that by using DSD the myocardium is defibrillated with a broader energy vector compared to a single set of pads resulting in a more complete depolarisation of the myocardium. A second theory is that the first shock reduces the ventricular defibrillation threshold meaning that the second shock is more effective. A third explanation may simply be the large amount of energy delivered to the myocardium. These theories should be tempered by the fact that studies have demonstrated increasing defibrillation energy to result in increased defibrillation success, but only up to a plateau. After this the success of defibrillation drops sharply. Increased energy use has also been demonstrated to cause an increase in A-V block but without an associated increase in shock success or patient outcome. The use of double sequential defibrillation is clearly an area that would benefit from further research, but despite this it is interesting to note that London Ambulance Service have enabled their Advanced Paramedic Practitioners to use DSD and some American EMS systems have written DSD into their protocols. So returning to the case in point what did I choose to do with my patient? After changing the pad position, administering magnesium and continuing defibrillation they remained in VF.  I considered transport to a hospital with interventional cardiology but the patient was several stories up in a property with an inherently complex extrication. So I chose to use DSD because I felt that all other avenues had been explored. The patient had suffered a witnessed arrest, received bystander CPR immediately and throughout the resuscitation they had maintained a high end tidal CO2 and a coarse VF. I felt that this was a patient who could still respond to non-standard cardiac arrest management in the absence of a response to guideline directed treatment. After two DSD shocks a return of spontaneous circulation was achieved and the patient survived to hospital admission, but sadly didn’t survive to hospital discharge. We’re left with a even bigger question: if we accept that DSD is a potentially useful intervention in rVF, when should we consider using it? Would the outcome for this patient have been different if DSD had been used earlier? The European Resuscitation Council states that the use of double sequential defibrillation cannot be recommended for routine use. But treating rVF is not routine and the guidelines have otherwise failed our patient. It is said that insanity is defined as doing the same thing over and over again, without changing anything, and expecting a different result. Is this not what the guidelines preach in rVF? It is up to you, the clinician, to determine whether DSD is appropriate for each rVF case you encounter. But I urge you to consider the patient in front of you and tailor your resuscitation to their needs, whether that includes DSD or an alternative option. Personally, I believe DSD does have a place in the management of rVF patients, after considering the other interventions previously discussed. Given that shock success declines over time, DSD could be used as early as the sixth shock, because at this point the guidelines have nothing further to add. Or maybe it’s me who’s insane… James Yates (Critical Care Paramedic GWAAC) References Double sequential Defibrillation therapy for out-of-hospital cardiac arrests: the London experience. Emmerson AC, et al. Double Sequential Defibrillation Double sequential defibrillation is a procedure that may be helpful in the treatment of refractory ventricular fibrillation (RVF) in the out of hospital environment. This module demonstrates the procedure of double sequential defibrillation which is included as a medical control consideration for instances in which ventricular fibrillation persists after five shocks. Further research will be required to determine the optimal treatment strategies for RVF. Double sequential defibrillation is not considered part of the minimum standard of care at this time. Nonetheless, providers will be instructed on how to properly perform this procedure in the event that a medical control order is granted for this treatment option. Background: The Rift Valley fever virus (RVFV) is an arthropod-borne phlebovirus. RVFV mostly causes outbreaks among domestic ruminants with a major economic impact. Human infections are associated with these events, with a fatality rate of 0.5-2%. Since the virus is able to use many mosquito species of temperate climates as vectors, it has a high potential to spread to outside Africa. Methodology/Principal Findings: We conducted a stratified, cross-sectional sero-prevalence survey in 1228 participants from Mbeya region, southwestern Tanzania. Samples were selected from 17,872 persons who took part in a cohort study in 2007 and 2008. RVFV IgG status was determined by indirect immunofluorescence. Possible risk factors were analyzed using uni-and multi-variable Poisson regression models. We found a unique local maximum of RVFV IgG prevalence of 29.3% in a study site close to Lake Malawi (N = 150). The overall seroprevalence was 5.2%. Seropositivity was significantly associated with higher age, lower socio-economic status, ownership of cattle and decreased with distance to Lake Malawi. A high vegetation density, higher minimum and lower maximum temperatures were found to be associated with RVFV IgG positivity. Altitude of residence, especially on a small scale in the high-prevalence area was strongly correlated (PR 0.87 per meter, 95% CI = 0.80-0.94). Abundant surface water collections are present in the lower areas of the high-prevalence site. RVF has not been diagnosed clinically, nor an outbreak detected in the high-prevalence area. Conclusions: RVFV is probably circulating endemically in the region. The presence of cattle, dense vegetation and temperate conditions favour mosquito propagation and virus replication in the vector and seem to play major roles in virus transmission and circulation. The environmental risk-factors that we identified could serve to more exactly determine areas at risk for RVFV endemicity.
