BUFFALO, NY - February 6, 2025 – A new #casereport was #published in Volume 16 of Oncotarget on February 5, 2025, titled “Acquired RUFY1-RET rearrangement as a mechanism of resistance to lorlatinib in a patient with CD74-ROS1 rearranged non-small cell lung cancer: A case report." In this case report, Jenny L. Wu from Vanderbilt University School of Medicine and Wade T. Iams from Vanderbilt-Ingram Cancer Center describe a rare case of drug resistance in a patient with advanced non-small cell lung cancer (NSCLC). The patient, a 42-year-old man who had never smoked, initially responded well to lorlatinib, a targeted therapy designed to treat cancer driven by specific genetic alterations. However, after six months, his cancer began to grow again. Clinicians discovered that this was due to a new genetic change, known as the RUFY1-RET fusion. This finding highlights how cancers can adapt to treatment and the importance of ongoing genetic testing to guide therapy decisions. NSCLC is the most common type of lung cancer, and in some cases, it is driven by genetic changes that can be targeted with specific drugs. The patient's cancer originally had a ROS1 gene rearrangement, which made it responsive to lorlatinib. But as time went on, the cancer started to grow again, and tests revealed a new genetic alteration called RUFY1-RET fusion, which likely caused resistance to lorlatinib. This new genetic change was identified using RNA next-generation sequencing (RNA NGS), an advanced test that can find mutations that standard genetic tests might miss. After discovering the RUFY1-RET gene fusion, the patient was treated with a combination of lorlatinib and pralsetinib, a drug that specifically targets RET gene alterations. While this combination helped control the cancer for about four months, the patient's condition unfortunately worsened after four months. “This is the first reported case of a RET fusion as a potential mechanism of resistance to lorlatinib, it identifies a novel RET fusion partner, and it emphasizes the importance of testing for acquired resistance mutations with both DNA and RNA at the time of progression in patients with targetable oncogenic drivers.” Understanding cases like this can help clinicians and researchers develop more effective treatment strategies, including combination therapies that target multiple genetic changes to combat drug resistance. While the combined therapy in this case provided only temporary benefits, it offers important insights for future research and patient care, particularly for cancers that no longer respond to standard treatments. Almost 30 years ago a team of researchers banded together to investigate an aggressive form of stomach cancer that was responsible for early deaths in one Tauranga whānau. Together, Kimi Hauora Health and Research Clinic, whānau members, and University of Otago geneticists, found the cancer-causing genetic change, helping save thousands of lives worldwide. Justine Murray catches up with some of the team as they reflect on the journey they've been on.
Here we survey of the causes and consequences of genetic mutation, including a discussion of mechanisms of endogenous and induced mutations, rates of mutation, types of single nucleotide mutations, and the phenotypic effects of mutation. We also discuss various mechanisms for detecting and repairing genetic mutations, including base excision repair, DNA mismatch repair, nucleotide excision repair, double strand break repair. We conclude with an examination of large-scale chromosomal changes, including deletions, duplications, inversions, and translocations, unequal crossing over, with a brief look at polypoidy in plants and aneuploidy in humans. Many practitioners tell people to never supplement vitamin D. This was my belief system for several years because on the surface it made sense. When I dug deeper I found that there is something called a "biphasic dose response curve". That is a fancy way of saying: Take too much or too little and either way will increase calcification of soft tissues (Drueke, et al 2012). Dr Tyler Panzner has a PhD in Mollecular and Cellular Pharmacology and looks at nutritional supplementation through a genetic mutations lens. In this episode he gives a brief explanation of how genetics works, how he was deficient in vitamin D on the beach in the middle of summer, why he believes vitamin D signaling is more important than blood levels, what he thinks about lab reference ranges, an alternative vitamin D supplement to cholecalciferol that he thinks works better, and some of the genetic mutations to look for if you suspect you are deficient. He also talks about why he thinks NAC supplements are overrated, the demonization of melatonin supplements, borage oil, and gives a mini consultation looking at my genetic mutations with B12 and how supplementing lithium can help mitigate them. When Barb's father all but left, her mother turned inward, sitting quietly in front of the television, always smoking, often with a cocktail. Something had overtaken her, though it wasn't clear what.Six years later, Barb was 20 and in college when someone else in the family needed help. Her sister Christy was the second-born, 24 years older than Barb and the star of the family in many ways. But where once Christy was capable and professionally ambitious and socially conscious, now, at 44, she was alone, her clothes unkempt and ripped, her hair unwashed, her marriage over.Depression was the first suspected diagnosis, then schizophrenia, though neither seemed quite right. Christy wasn't sad or delusional; she wasn't even upset. It was more as if she were reverting to a childlike state, losing her knack for self-regulation. Her personality was diluting — on its way out, with seemingly nothing to replace it.What was left of Christy was chaotic and unpredictable. She refused to bathe and stopped bothering to make meals. She crashed a neighbor's party and made odd conversation with strangers. She clogged a toilet with tampons and flooded the house. She was gleefully impulsive, spending thousands of dollars a year on magazine subscriptions. That strange, reckless profligacy made Barb think of their mother, who in her final years sat at home, saying yes to every sales phone call.
