A book club is a great way to build community—bringing people together around shared interests, while also introducing them to new perspectives and ideas.Today, Bill Tishler hosts his inaugural episode centered on community. Tishler, who is also a local elected official, has been hosting book clubs in his district. On today's episode, four area residents join him in the WORT studio to share their thoughts on recent book clubs they participated in this year and how the books they read raised awareness about issues in our city.Those issues range from pedestrian and bicycle safety to the health effects of loneliness and social disconnection, to the dangers of too much road salt and PFAS contamination in our area lakes and drinking water. The books discussed included The Art of Community: Seven Principles for Belonging by Charles Vogl (Oakland, CA: Berrett-Koehler Publishers, 2016); Silent Spring by Rachel Carson (Houghton Mifflin, 1962); and Killed by a Traffic Engineer: Shattering the Delusion that Science Underlies Our Transportation System by Wes Marshall (Washington, D.C.: Island Press, 2024).

I don't normally cover a project twice but this one is pretty special as it extends beyond the medium of comics. Don't worry, it was a comic first. We're revisiting the Mad Cave Studios project The Last Wardens with writers Amit Tishler and Elliot Sperl. They have been busy expanding the scope of their IP and developing a TTRPG as well as an audio drama based upon the world created in the comic. Pretty amazing right? We touch on the principle theme of failure which drives the characters in the story, especially the main protagonist Dani, and how an open mind towards collaboration has opened doors to new possibilities for their world. Amit discusses his philosophy with embracing failure as a learning tooland how it can be applied to all creative endeavors. Dr. Jordan Tishler, MD, is a cannabis specialist with a practice in Boston. Dr. Tishler graduated from both Harvard College and Harvard Medical School. Now he helps patients around the world. All of his appointments are virtual.   His website has lots of helpful information. Soon Dr. Tishler will publish his randomized, controlled study with one thousand females, looking at situational orgasmic disorders. Most women can bring themselves to orgasm by masturbating, especially using a vibrating toy. Orgasms with a partner are more difficult for many women. Cannabis helps with orgasms!   “Evidence Mounts: Cannabis Enhances Lovemaking for Most” is the title of Michael Castleman's Psychology Today Blog post (Oct. 15, 2022). Dr. Diana and Dr. Tishler spoke about the key points:   As cannabis becomes increasingly legal, many studies show that most users report better sex. A 2020 report from Stanford researchers revealed that cannabis increases desire and arousal for most women and improves orgasm and satisfaction, and for most men, cannabis improves their erections and increases orgasms and satisfaction. Cannabis doesn't improve sex for everyone, but the research shows that two-thirds of users report sexual benefits.   Cannabis helps with pain and healing. Dr. Diana shared about her abdominal surgery in February. Cannabis helped with the pain … much better than the opioids! Two weeks after the surgery, she felt ready to resume sex with her husband. Smoking pot made a difference! More healing ensued! (Dr. Tishler also commented on the problem with opioids is that they can cause constipation; in some cases, addiction.)   Another cannabis benefit is that with lowered inhibitions, laughter and play may be more accessible! A good laugh—like a good cry or good sex—is a natural tranquilizer. Turns out, laughter is a natural stress-buster! Plus, remember that couples who laugh together, last together!   On the topic of cannabis vs alcohol, Dr. Tishler observed that alcohol is essentially yeast poop … it's been through fermentation. It is waste.  The phytocannabinoids in cannabis come from a plant and work on the human body's own endocannabinoid system. The term “disorder” is discussed in relation to female orgasmic disorder, in that a woman must be experiencing distress in order to be diagnosed with female orgasmic disorder. Dr. Tishler discusses the lack of research on male orgasm in comparison to female orgasm.  The value of cannabis is discussed in creating an altered state and being used in a thoughtful manner where one can distance oneself from inner voices, such as self-judgment, that stand in the way of orgasm. Dosage is discussed as being more complicated for men than for women. Frequency of cannabis use is discussed as it relates to research on the topic and the discussion broadens to look at cannabis as a therapy, a medicine, to be used overtime, where the cannabis guides us to know what issues need to be addressed and resolved within oneself.Suggestions are made if one is starting to use therapeutic cannabis to enhance sexuality and orgasm. Cannabis for Couples   Jordan Tishler, MD, is a cannabinoid specialist physician based in Boston (although he sees patients from anywhere via telemedicine visits). He's the perfect guest for today—April 19th—one day short of the annual cannabis celebration of “4/20”! Dr. Tishler graduated from both Harvard College and Harvard Medical School. Bryan Brewer, Dr. Diana's husband, joined as the cohost.   Dr. Tishler was quoted in a recent New York Times article titled “Cannabis for Better Sex? Here's What the Science Says.” Although the research has room for improvement, some studies indicate that using cannabis can help with sexual concerns such as low desire, inability to reach orgasm, erectile and ejaculatory dysfunctions, and others. Dr. Tishler recommends trying cannabis alone for the first few times and masturbating, in order “to understand what it does to the body and sensations.” Then introduce its use with a partner.   A key piece of his advice is to “start low and go slow.” Finding the right dosage is critical. Too little and you won't feel much of the effects. Too much and you can become overwhelmed, which can eventually lead to the development of tolerance. (Don't worry—you can't die from an overdose of cannabis!)   To find the right dosage, Dr. Tishler advises patients to start with inhaling one puff from a cannabis flower vaporizer (not cannabis oil), which would typically deliver about 5mg of THC, the main psychoactive molecule in cannabis. He suggests that most people will benefit from a dosage ranging between 5mg and 20mg of THC. (Edibles and tinctures are too slow to take effect for sex, and smoking cannabis is not good for the lungs.) He also maintains that there is no particular “strain” of cannabis that works best for sex (or any other purpose).   There are many pleasures to be found by combining cannabis and sex. It can help with orgasms. The more intense orgasms you have, the more it strengthens the bond between the partners and motivates you to have even more sex!   Dr. Tishler is an articulate and tireless advocate for patients seeking to take advantage of the benefits of medical cannabis. Cannabis for Couples on 4/20   Dr. Jordan Tishler, MD, is a cannabis specialist. Today is 4/20 – so he is the perfect guest! Dr. Tishler graduated from both Harvard College and Harvard Medical School. What are some of the positive effects of using cannabis to enhance sex? There is a heightened sensitivity on emotional and physical levels. What about pitfalls? A small percentage of users consume too much and become dependent. When compared to overuse of alcohol, cannabis offers many health benefits. Alcohol can cause bleeding ulcers and cognitive decline, just to name two negatives. Dr. Tishler claims that cognitive function can get better with cannabis use, and there aren’t all the health problems associated with abuse of alcohol.   Any medical contraindications for cannabis use? Not many. If a man has heart disease and has pot and sex with someone other than his regular partner, he puts himself at a heightened risk for a heart attack. We discussed recommended methods and dosage for beginners. Also, how important is “set and setting” when using cannabis? A warm, safe place provides comfort because everything is amplified when high. Have you ever felt lonely and overwhelmed in a clinic room with a patient whose needs are far beyond your skills and ability to meet? I have, many times, and so has our guest this week, Dr. Lori Tishler. Dr. Tishler is the Vice President of Medical Affairs at Commonwealth Care Alliance, a not-for-profit organization that cares for more than 20,000 of the most vulnerable patients in Massachusetts, duals, or individuals who have both Medicare and Medicaid. We talk about how CCA’s member-centered approach (as opposed to a physician-centered approach) has helped her feel more effective in caring for these vulnerable patients. We talk about the range of services that CCA offers, the role of their care partners, and the freedom that their financial model permits – for example, they provide 90% of care to patients in their homes. Her leadership roles have extended outside of the clinic and outside of primary care. She served on the Partners Healthcare Board of Directors, the Board of Directors for the Office for Women’s Careers, and the Board of the Schwartz Rounds while at the Brigham. At Commonwealth Care Alliance, she teaches and presents nationally about our care model and innovations. A proposed bill PB317 has been written by CT State Senator Eric Berthel which proposes to require insurance companies to cover bariatric surgery benefits. Despite coverage by Medicaid and Medicare in our state, the majority of private insurance plans do not offer bariatric surgery as a benefit. When the ACS was passed by Obama, the essential benefit in Connecticut was based on the current coverage by our biggest HMO: Connecticare. As a result Obamacare has NOT covered bariatric surgery. Most insurers do not and have not been forced to offer the benefit. Many of our patients have switched to Medicaid for coverage despite having good jobs. This has caused a burden to taxpayers. State employees also have bariatric surgery coverage. Darren Tishler spoke to Brad and Paul about it

The International Studies Association (ISA) is one of the oldest interdisciplinary associations dedicated to understanding international, transnational and global affairs. Its more than 7,000 members span the globe, comprising academics, practitioners, policy experts, private sector workers and independent researchers, among others. The Association has long served as a central hub for the exchange of ideas and for networking and programmatic initiatives among those involved in the study, teaching, and practice of international studies. This past February, Analysis Bureau Chief Dean Baratta and Intelligence Analysts Paige Schilling and Angie Gad took Intelligence. Unclassified. on the road with them to the 2017 ISA annual conference.
