Safe Jabs Thanks to Horseshoe Crabs: Making Sure Your Injection is Free of Endotoxins Allen Burgenson, Lonza's expert for all things testing, speaks to us about the dangers of endotoxin contamination and the future of non-animal testing for it.   “Before testing for endotoxins in the 1940s, a physician literally had to gauge the risk to your life because of something called injection fever,” explains Allen Burgenson. Luckily, we've come a long way since then. Thanks to advanced testing methods, one can rest assured today that any sort of injection or implant is completely free of dangerous endotoxins. Currently, the predominant mode is Limulus Amebocyte Lysate (LAL) testing, in which scientists harvest the bright blue blood of American Horseshoe Crabs and use the animal's primitive immune system to look for clotting reactions that would indicate the presence of any endotoxins. The horseshoe crabs, Burgenson explains, survive the extraction unscathed and are safely returned to the waters in less than 24 hours. However, in a continual attempt to remove animals from the testing pipeline, Lonza's recombinant factor C assay known as PyroGene could eventually replace LAL testing. Curious to Know More? In Episode 2 of the new season of the podcast A View On, host Martina Ribarhestericova speaks with Lonza expert Allen Burgenson to discuss his close bond with the American Horseshoe Crab and the history of testing for endotoxin contamination.    KEY TERMS: Endotoxins are parts of bacterial membranes that could lead to a harmful reaction – or even death – if they enter a patient's bloodstream or spinal fluid. Surprisingly, we have kilograms of endotoxins in our stomachs, but even little more than a nanogram in the bloodstream could be deadly. Bacterial endotoxin tests, or BETs, is the general name for all assays used to detect endotoxins. Rabbit pyrogen tests are BETs that were developed in the 1940s using rabbits as test subjects. To ascertain the endotoxic danger to humans, the scientist observes a rabbit's reaction to an injection over a period of 3 hours. The European Pharmacopoeia Commission decided in June 2021 to completely replace the rabbit pyrogen test (RPT) within approximately 5 years. Limulus Amebocyte Lysate (LAL) is an aqueous extract of blood cells (amoebocytes) from the horseshoe crab, Limulus Polyphemus, that enables batch testing of vaccines and other drugs for endotoxins. The crab's extracted blood is a surprising blue color due to the crabs' copper-based Hamasyan. The obtained LAL is an opaque white-colored liquid that clots in the presence of any toxicity. PyroGene recombinant factor C is an animal-free way to test for endotoxins. It was initially developed at the National University of Singapore by Lin Deng and her husband Bo Ho to save money on testing at their relatively small lab. Lonza collaborated with Deng and Ho to become the first company to offer the test on a commercial scale.

A Welcome Virus: Cracking the Viral Code for the Battle Against Cancer Chairman and founder of PsiVac, Prof. Ghassan Alusi, and the chief operating officer, Imad Mardini, discuss how the company's proprietary oncolytic virus platform offers new hope for cancer patients. During a time when everyone actively fears viruses (especially THE virus) and their mutations, it is only cancer cells that have cause to worry about oncolytic viruses, and rightly so. These mutated viruses are administered directly into a tumor. Once inside, they crack open the tumor's cells in a process known as lysing that provokes a strong response from the body's immune system, which has, until then, ignored the cancerous cells. What's more, the therapy's attack doesn't stop at a single tumor. The replicating and lysing viruses release previously hidden tumor-associated antigens (TAAs) that alert the immune system about cancer cells to attack all over the body. The body's own immune system then goes on to destroy previously unrecognized tumors far removed from the initial injection point. The biotech company PsiVac advances this technology even further by creating a treatment platform that transforms the adenovirus, aka the common cold, into an especially powerful oncolytic virus. A precision modification in the virus's DNA improves its efficiency against cancer cells while making it harmless to other cells, rendering the treatment at once more effective and safer for patients. “Now that the technologies of other forms of immunotherapy are gaining ground, and as cancer remains a major cause of mortality, we now understand there is a huge need for oncolytic viral therapy,” says Prof. Alusi, whose company has planned to start Phase 1 clinical trials later this year. Unlike other immunotherapies, such as patient-centric CAR T-cell therapy, oncolytic viruses can be made in relatively large quantities once their efficacy and safety have been proved.   Curious to Know More? In the first episode of the new season of the podcast A View On, host Martina Ribar Hestericová discusses the current state of oncolytic viruses and their promising applications with Prof. Ghassan Alusi and Imad Mardini.    KEY TERMS: Cell lysing is the process of breaking down a cell's membrane, destroying the cell and releasing its contents into the body. If an oncolytic virus lyses a cell, it releases replicated versions of itself as well as antigens helpful in the immune system's fight against a tumor. Tumor-associated antigens (TAAs) are released once a cancer cell is lysed, setting the previously dormant immune system into action. The release of TAAs means that the tumor is no longer successfully hiding from the immune system, and the body can begin to fight the disease by its own means. The cytotoxicity of a virus is the extent to which a virus attacks and destroys cells, often an undesirable event. However, with oncolytic viruses, this cytotoxicity works in a patient's favor, thanks to gene editing, by being specifically designed to attack cancer cells. An agnostic oncolytic virus targets not only one or a group of cancers but is effective against all malignant solid palpable tumors. PsiVac's modified adenovirus has proven agnostic so far, making it a powerful weapon in the fight against cancer.

Devouring Bacteria: How Phage Therapy Is Shaping Antibacterial Treatments of the Future In this episode we speak with the CEO of BiomX, Jonathan Solomon, about producing and using phages to test and treat various diseases and conditions. Until very recently, treating a condition such as acne with an army of microscopic bacteria-destroyers known as phages—bacterial viruses that target and kill specific bacteria—would have seemed highly unlikely. However new research linking acne to an imbalance in the skin’s microbiome has opened the door to innovative treatment approaches. That’s where the biotech company BiomX comes in. Uniting powerful computational science with the inherent capacity of phages to destroy specific bacteria, BiomX creates natural and synthetic phage therapies for some of the most troublesome bacteria-related health issues: acne, atopic dermatitis, cystic fibrosis, inflammatory bowel disease (IBD) and even colorectal cancer. For acne the company has developed a successful cocktail of three different phages to treat the condition, with phase 2 testing close on the horizon. Phage cocktail: Since a phage targets and destroys only one type of bacteria, treatments for complex ailments necessitate a mixture, or cocktail, of different phages to be effective. Phage fermentation: Although destructive, unwanted phages can grow during fermentation processes for wine-making and milk production, fermentation is nevertheless the optimal way to produce phages for therapeutic uses. Computational (science): Computer modelling of the phage and its potential interaction with specific bacteria (known as in silico testing) allows researchers to develop phage cocktails more efficiently and with a greater chance of success.

Helping a Person With Alzheimer's Disease to Eat "Handfeeding is recommended over tube feeding in Alzheimer's disease. The handfeeding techniques offer an additional sensorimotor cue."  -Melissa Batchelor, PhD, RN, FNP-BC, FGSA, FAAN In the late stages of Alzheimer's disease, it's not unusual for older adults to have trouble eating.  Before the end of life, 85% of people living with Alzheimer’s disease will have trouble eating. There are many reasons for this, but many stem from the sensory changes that happen with the disease such as changes with smell, taste, vision, and hearing. Other contributing factors may involve chewing their food, swallowing, or they simply lose interest in food.  Ensuring that anyone with Alzheimer's eats a healthy meal or eats sufficiently turns into a real practical and emotional challenge for the caregiver.  In today's episode of This Is Getting Old, we'll talk about Alzheimer's Care: Making Mealtimes Easier. I'll share strategies that can help you help an older adult to eat more food. Part One of 'Helping a Person With Alzheimer's Disease to Eat. ' The first step in understanding is the sensory changes that impact an older adult's ability to feed themselves.  These sensory changes are briefly described below;  Smell. Smell is diminished with Alzheimer's disease. As this disease progresses, people lose their sense of smell. They can't smell their body odor, they can't smell if there's smoke in the house, so they certainly have a hard time smelling food. This change also increases their risk for food poisoning.    Taste. Have you ever wondered why an older adult with Alzheimer's disease loves sweet food? Sugar fires up a part of your brain that remains intact throughout the disease. So if we need to put a little bit of sugar on somebody's food (and adjust their medical regimen accordingly if they have diabetes), to get them to eat more food, we should do that.  "For an older adult with Alzheimer's disease, adding a little bit of sugar to their food could be a good thing because sugar fires up a part of the brain that remains intact throughout the disease." -Melissa Batchelor, PhD, RN, FNP-BC, FGSA, FAAN Hearing. If you only have Alzheimer's disease, you can still hear, but what happens is you lose the ability to use and understand language. It might seem like they can't hear us sometimes, but the reality is they can't understand us.    Vision. A person with Alzheimer’s disease will  lose peripheral vision. With Alzheimer's disease, vision diminishes from scuba vision, to binocular vision, down to monocular vision. Touch. The Palmer Reflex returns with late Alzehimer’s disease. The Palmar Reflex is similar to when you put your finger in the palm of a baby's hand, and they grab it. That reflex comes back very late in this disease, making it difficult for older adults to use utensils, handle cups, and all the things that it takes to feed themselves. Verbal and Visual Cues To Facilitate Ease in Feeding   When providing care to a person living with Alzheimer's disease, we already know to give people verbal and visual cues. However, late in the disease, they will have a hard time using and understanding language so what they pick up on is your nonverbal behavior.  So if you sit down and you're smiling, or you mimic chewing or whatever it is that you want them to do, that part of the brain remains intact even late in the disease (it’s called “Mirroring”). By using nonverbal cues, you're a lot more likely to get them to do what you want without using words.   Part Two of 'Helping a Person With Alzheimer's Disease to Eat.' Modified Hand-feeding Techniques In helping the older adult eat, the three hand-feeding techniques are Over Hand, Under Hand, and Direct Hand.  You should start with the least amount of assistance you need to provide and save using the Direct Handfeeding technique until last. Unfortunately, what we often see in practice is that people start and stay with Direct Hand. This can create unnecessary debility and dependence on others, rather than supporting self-feeding.    The first thing you need to know is if the person has“skill finger” ability (your thumb, first and second finger are your “skill fingers” and are needed to manipulate utensils when eating). Based on this information, you can tailor the level of assistance provided and which technique to use.  Over Hand  Let's say we have an older adult who's pretty much able to feed themselves that they have skill finger ability and need a little help getting the food from the plate to their mouths; start by offering overhand assistance. Place your hand over the person's and gently guide their hand with the utensil towards their mouth.    Be careful when you're doing this to make sure that you don't hurt their wrist or grab their hand too tight and always help them through the middle of their body.  "Hand-feeding techniques are helpful for other caregiving activities that you may be doing. You can help older adults brush their teeth, comb their hair, and even get dressed; because with the Under Hand technique, we're doing all the fine motor stuff that they can no longer do because of Alzheimer's disease." -Melissa Batchelor, PhD, RN, FNP-BC, FGSA, FAAN Under Hand  The Under Hand technique works the best for someone who's lost skill finger ability or doesn't seem to understand what to do with their utensils. At this point, we can use the Under Hand technique.  Start by getting into the Under Hand Shake position; what this technique does is it frees up is your skill fingers. The Under Hand technique provides a new, additional sensorimotor cue to maximize the signals that we're providing.  Modifications based on Range of Motion Ability One of the first modifications we can do with the underhand technique is if the person has a partial range of motion. In this case, we need to load the fork or the spoon for them because that's the part that they can no longer do. So if a person has very little range of motion, you may need to put their food item into a different container and get very close to them so that they don't have to move as far to participate in the act of eating. Even with a minimal range of motion, the underhand still provides the cues that might help them to understand that it's time to eat  Direct Hand  The third technique is the Direct Hand technique. You want to save this handfeeding technique for people who aren't able to participate in the act of eating. It is best practice to put your hand on their shoulder; because they've lost peripheral vision and they may not see you sitting there. Even with this technique, you need to make sure that they see the food before it comes towards their mouth. Lifting it into their visual field helps them to know that food is coming, and usually results in them opening their mouth. This often allows meal intake to go more quickly. Want to Know More? Exosomes Know Where To Go: Using the Body's Own Cell-to-Cell Communication Network for Diagnostics and Drug Delivery We speak with Uwe Gottschalk, the Chief Scientific Officer of Lonza, about how a better understanding of exosomes is leading to new treatments and diagnostic technologies.   According to Uwe Gottschalk, the exosome revolution is already in full march. As researchers begin to identify how these cell-generated, nano-sized delivery drones function in the human body, novel drug delivery prospects are emerging, including applications for cancer, neurodegenerative diseases and spinal cord injury recovery. Perhaps even more exciting is the role exosomes will play in diagnostic applications in the near future, wherein a liquid biopsy, based on a blood sample, would detect cancer or other diseases both more easily and in a more timely fashion than traditional biopsies. One of the many challenges is the ongoing task of defining the manufacturing protocols and processes for this new biotechnological paradigm. Even so, the field is abuzz with new discoveries, trials and general optimism about the potential of these microscopic extracellular delivery vehicles. Curious to Know More? Lonza's Chief Scientific Officer gives us his expert insight into exosome research and application in this special, in-house episode of the podcast "A View On."   KEY TERMS: Exosomes are nano-sized delivery vehicles generated by all eukaryotic cells. They are between roughly 30 and 120 nanometers large and originate when endosomes, or intercellular vesicles, are released into the blood, milk or tissue. They then become messengers and surrogates for the original cell. Their surface markers represent a location code and spread through the extracellular space in the body to communicate with other cells and deliver packages. Extracellular vesicles (EV) are particles released from cells that cannot replicate but otherwise behave like the surrogate cells from which they originate. While there is some overlapping in definition between exosomes and EVs—all exosomes are extracellular vesicles but not vice-versa—exosomes are defined by their size (30 to 120 nm) and their biogenetic origin. Liquid biopsy: As with a traditional biopsy, a liquid biopsy is a test to diagnose and monitor diseases that uses a blood sample instead of a tissue sample. As a liquid biopsy is not restricted to one tissue or part of the body, the test is less invasive, cheaper and even more precise. Messenger RNA (mRNA) as a cancer biomarker:  Recent research has proven that mRNA from a blood test can be analyzed to find cancerous and pre-cancerous tumors throughout the body. As exosomes transport and stabilize the otherwise highly unstable mRNA, they could be targets for early detection and treatment in the near future.

Bugs as Drugs: Harnessing the Therapeutic Potential of the Microbiome Lukas Schüpbach, the CEO of Bacthera, and Gemma Henderson, Bacthera’s Head of Project and Portfolio Management, speak to Lonza about creating pharmaceuticals from the human microbiome.   The human microbiome, comprised of trillions of bacteria, fungi, viruses, and archaea, is unique to each individual and develops over the course of lifetime, stabilizing once we reach adulthood. Despite the widespread understanding that this microbiome is a key component to our health, there are currently no commercially available live biotherapeutic products (LBPs). There is, however, an increasing amount of scientific evidence that using live biotherapeutic products to promote a vigorous microbiome can improve general physical health and positively impact quality of life by targeting diseases such as obesity, diabetes, inflammatory bowel syndrome and cancer. The biopharma company Bacthera is manufacturing and testing these difficult-to-produce anaerobic bacteria treatments that could improve metabolic functions and have anti-inflammatory effects. Alongside manufacturing, Bacthera is meeting the challenging delivery process to harness the therapeutic potential of the microbiome through easily administered, encapsulated pills. Microbial biodiversity is key to a healthy microbiome. A poor microbiome is linked to diseases such as inflammatory bowel disease, cancer and possibly some central nervous disorders. Live biotherapeutic products: These pharmaceutical products, LBPs for short, are unique because their active substance is actually a living organism. that has been identified as showing promise in treating one or sometimes several diseases. Enclosed process: The manufacturing of LBPs necessitates special equipment and expertise since many of the microorganisms are anaerobes and spore-forming organisms. To ensure a robust process with high yields, the manufacturing must be entirely enclosed so that these strains are not exposed to oxygen. Entrinsic strict delivery: As some microbes would not make it to the intestine by way of stomach acids, Bacthera has access to a proprietary technology that encapsulates the microbe to ensure targeted and precise delivery.

Prayer to Know More of Jesus and These Blessings Ephesians 1:15-23 August 16, 2020 I. Introduction: A. We live in a day and age where out of the sincerity of our hearts to live out God’s truth and not just study it the first question we ask ourselves is, ”What do I need to do with this truth?” 1. We have been learning some of the most amazing truths in the Bible over the past number of months, blessing upon blessing that God has given to us in Jesus Christ! 2. This morning Paul answers for us the first thing we need to do with these truths, and what he does with them may surprise many of us. Actually too often, we skip over this crucial first step in the process as we rush to put God’s Word into practice in our lives. B. We find the answer to this starting in v15 of chapter 1 as he says, “for this reason” 1. Simply he is connecting what follows with what has gone before. He tells us the very reason he is going to do what he does next is because of all these blessings in Christ we just learned about in v3-14. 2. Just like v3-14 was all one long sentence closely tying it all together so v15- 23 is also just one sentence closely tying it all together. C. Before I read, let me tell you what the heart of this section says so you can watch for it as I read – simply he prays for them to know more of Jesus and these blessings! 1. The structure to the prayer is as follows: a) V17 he prays that God would give them a fuller, more practical and more intimate knowledge of Jesus b) In v18 -19a he prays that God would enlighten our hearts to fully see and grasp three specific blessing God gives us – His calling, His inheritance and His power c) Then in v 19b-23 he shows us three ways in which His power towards us right now is manifested and in agreement with the power that was manifested in Jesus when He was resurrected, when He was ascended into heaven and seated at God’s right hand and finally when He gave Him as head over all things pertaining to the church! 2. Watch for these as I read D. Paul’s first response to all these amazing blessings, the first thing he does is to pray! Pray that we would know more of Jesus the one whom all these blessings are in and that we would know more of the blessings themselves! • Before I jump into the text I just want to say a few things about II. Prayer in general A. We have learned before that prayer is the means to receiving in God’s kingdom. 1. Often as a last resort we say to people in tough times, when there is nothing else we can do we say, “All I can do is pray!” 2. Actually prayer should be the first thing we do and the best thing we can do as prayer allows us to put one hand on the person and their situation and the other hand on God and bring the two together! 3. Prayer is the first thing that Paul does with these amazing life-changing truths! It really is the first thing we should do B. We have been talking lately in staff about the restarting of our ministries here at MVC in this unique day of restrictions and MVC scattered all over the place. 1. As we look at the way that Jesus did ministry we have been reminded how Jesus started His ministry and daily met with the Father in prayer. 2. Jesus said He did nothing of Himself; His teachings and words were not His own, His works were not His own, His will was not His own but they all were from and initiated by the Father. 3. If we do not go to the Father first and regularly in prayer then at best we will be working out of our best ideas or copying and pasting what others are doing rather than hearing directly in a life changing and empowering way what God wants us to do. 4. What we learn from this passage is that God does not want His truths from the Bible just to go from our heads to our feet and hands but rather from our heads, through our hearts where it is anointed by the Spirit then out through our hands and feet! C. Finally, we see in this passage that Paul prays God’s word and truth. 1. So often, we will read God’s word when we end with our prayer time, when we begin to pray for our needs, friends, and ministry but never breathe a prayer about what we just learned in God’s word. 2. It is like sitting at the breakfast table with Kim and she talks to me about something on her heart she wants me to know then I respond by telling her about what happened in the ballgame yesterday! There is no connection and no two way conversation 3. Here Paul is praying about what we just learned in God’s word – to know more about Jesus, the one in whom are all these blessings and to know more of the blessings themselves in a life changing way! 4. This is the prayer we all should be praying for each other and ourselves in these days. • So the rest of today my goal is simply to introduce you to this prayer and then the next two weeks we will look into it more specifically. In v17 Paul prays that we would III.Know more of Jesus A. The key word here is knowledge – it is the Greek word “epignosis” which indicates a personal, intimate, and progressively growing knowledge of Jesus! B. As I reread this verse note three things: 1. This is something that God gives, something that originates with God, not something we work out by our efforts 2. He wants them to know more in a practical way – that is what wisdom and revelation basically means 3. Then finally that growing, practical knowledge is in the particular area it is in. The area of a personal, experiential and intimate relationship with Jesus 4. Read C. This same idea is pictured and the same word is used in Matthew 16 for us to get a picture of what this is like. 1. Context, Jesus had been on the scene and doing amazing miracles and saying things that amazed some and angered others. 2. Those whom He angered said He was from the Devil and those who were amazed knew He was someone very special as they thought he was John the Baptist raised from the dead or Elijah or Jeremiah or one of the other prophets raised from the dead. 3. Then Jesus said this to Peter a) Read Matthew 16:15-17 4. Like we see in this passage a growing more intimate, experiential and practical knowledge of Jesus is not something that comes from man’s efforts, observations, or even being around Jesus, like so many of them were. Rather it is a supernatural revelation – an unfolding to us by God where God discloses this to us in the core of our being! D. This is not just head knowledge or even just Bible knowledge about Jesus but it is the truths of the Bible coming to life within my heart by the power of the Holy Spirit! 1. This is not just reading, studying, memorizing and knowing the big idea the twenty third psalm that teaches us that God, specifically Jesus, is our personal shepherd but rather it is the daily experience in the grind of real life on the streets where we experience Jesus shepherding us through every situation we face. 2. It is not just knowing about Jesus in my head but rather knowing Jesus deep within the core of my being in a life giving, life changing way! • The second part of this prayer is that we would IV.Know more of these blessings A. The key word here is know – it is the Greek word “oida” which indicates an I see something and I get the significance of what that is. It is the knowing from observation but knowing in such a way that you grasp the significance of what you know! B. Let me reread v18-19a C. Again, we see these are things he does not just want us to have head knowledge about but he wants it to go to the core of our being, so they flow out of our lives as naturally as blood flows out of our hearts! When the Bible speaks of the heart it is speaking basically of the inner person, that part of us that lies deep within us, the immaterial part of our lives 1. In the Bible, it speaks of the heart as a place where we imagine, think and reason, a place where we have desires, affections and grief, a place where our conscience and will is, a place where faith resides. 2. Here it talks about the heart having eyes – figuratively speaking that means the ability to see. D. But like everyone who sees we need to have light to see – so his prayer is that the eyes of our hearts would be enlightened. That the Holy Spirit would shine light down in our hearts so we can see and know these wonderful blessings that God gives us! V. Application A. So everything that follows are works of God deep within our hearts that come in the context of prayer so that God’s Word would become a living reality in and through us! B. So from the core of our beings we would know and experience deeply the hope that comes from His calling, the riches of the glory of His inheritance in the saints and what is the surpassing greatness of His power towards us. It is story time! Plans were made, forms were formed, rock was moved, compactors were used, rain came, snow came, everything became muddy, even more rock was moved, concrete was poured, and in the middle of the night the concrete was covered with plastic/tarps/straw. Actually it sounds pretty easy when it is summed up in one long run-on sentence! Honestly though, it was a big week on the farm to get everything ready for concrete and then eventually for the pour. And while there is still plenty of work to do in order to get everything finalized I thought it would would be fun to share the story of the big cement project. The most important thing though is to recognize the awesome help that I had! We dug in the water lines into the hoop building, but I am beginning to realize that the concrete is not going to happen this year (that is my fault). To top it all off though the tractor started acting up again, and this time it seems that the probably actually may be caused by a much larger problem! Mentally it was a daunting week to so the least. All of this is happening though right before what I consider to be my favorite holiday ... Thanksgiving.
