Dr. Cynthia Bulik is a clinical psychologist and one of the world's leading experts on eating disorders. She is the Founding Director of the University of North Carolina Center of Excellence for Eating Disorders and also the founder director of the Centre for Eating Disorders Innovation at Karolinska Institutet in Stockholm, Sweden. Dr. Bulik is Distinguished Professor of Eating Disorders in the Department of Psychiatry at UNC, Professor of Nutrition in the Gillings School of Global Public Health, and Professor of Medical Epidemiology and Biostatistics at Karolinska Institute.  Dr Bulik has received numerous awards for her pioneering work, including Lifetime Achievement Awards from the National Eating Disorders Association, the Academy for Eating Disorders, and the International Society of Psychiatric Genetics. She has written over 750 scientific papers, and several books aimed at educating the public about eating disorders.  Currently, Dr. Bulik's focus  is in the reconceptualization of eating disorders as being a metabo-psychiatric diseases. Rachel Presskreischer is a postdoctoral research fellow in the Psychiatric Epidemiology Training Program in the Department of Epidemiology at Columbia University's Mailman School of Public Health. She is a mental health policy and services researcher with primary interests in leveraging health and social policies to improve outcomes for people with mental illness, health insurance coverage and design, and access to treatment for people with eating disorders.Her work is informed by both her academic training and her prior experience as a social work intern at the New York City Department of Health and Mental Hygiene and employee of the National Eating Disorders Association. Rachel completed her PhD in health and public policy at the Johns Hopkins Bloomberg School of Public Health, MSSW in social policy from Columbia University, and BA in individualized study from New York University. Rachel is a member of the Academy for Eating Disorders (AED),serving on the research practice committee from 2018 to this year and as co-chair from 2021-2024. After competing at Tokyo 2020, Sammy's transition to non-competitive life has been a big adjustment, and in 2023 she sought professional help for an eating disorder. Samantha shares her journey and continued work in recovery with us and what she's learned from her experience as an elite athlete. But for decades, much of the criteria to diagnose one applied only to cisgender girls and those assigned female at birth – like a loss of menstruation. "The first line treatment for adolescents with anorexia now is family-based therapy typically, which involves helping the parents facilitate the refeeding of the adolescent. So, I was working with the patient in that way and found it to be helpful and useful, but was consistently struck by the neglect of the patient's inner life, and found, at least based on my experience with many patients, that while you could get some symptomatic relief, if you didn't, in some way, address the deeper dynamics, the aspects of the patient's personality organization that drove the disorder, that were implicated at the disorder, there was a way that the patient would snap back to their old behaviors over time, that deeper change and a deeper understanding of what was going on was really necessary; and so that's been kind of evolution from my work over the past ten years from  my first book, which was about anorexia in males, and tried to present a kind of Integrative understanding of that phenomena, increasingly over time I've become more and more interested in the deeper kind of analytic thinking that we can bring to bear on this kind of suffering.”    Episode Description:  We begin with a description of the common contertransferential pull to intervene behaviorally in the face of repetitive self-destructive eating disorder symptoms. This intention can inform but not compel the clinical decision as to the indicated treatment of choice for someone at any particular moment. Behavioral and pharmacologic treatments can be important in softening the pressure of eating disorder symptoms. They do not, however, give an individual access to their interoceptive life, from which these disturbing self-preoccupations emerge. We discuss the challenges of working with those who have limited capacities for mentalisation and as a result, live out their inner lives somatically and motorically. Immersive treatment leads the clinician to experience these proto-affects in one's own body and in one's own ruminations. Tom discusses alexithymia, typical family structures, and the presence of the 'abject' experience in the lives of these patients. He presents a disguised case of a patient who was able to work through both the early struggles and later neurotic aspects of these conflicts analytically. We close with his sharing with us his vision for the future which includes more integration between the dynamic and adynamic approaches to these challenging patients.   Our Guest: Tom Wooldridge, PsyD, is Chair in the Department of Psychology at Golden Gate University as well as a psychoanalyst and board-certified, licensed psychologist. His first book, Understanding Anorexia Nervosa in Males, was published in 2016. His second book, Psychoanalytic Treatment of Eating Disorders: When Words Fail and Bodies Speak, an edited volume in the Relational Perspectives Book Series, was published in 2018. His third book, Eating Disorders (New Introductions to Contemporary Psychoanalysis), was released in 2022.  His fourth book, co-edited with Burke, Michaels, and Muhr, is entitled Advancing Psychotherapy for the Next Generation: Rehumanizing Mental Health Policy and Practice. He has also written a novel about the process of psychotherapy, Ghosts of the Unremembered Past, additionally released as an audiobook. He is a Personal and Supervising Analyst at the Psychoanalytic Institute for Northern California and a Training Analyst at the San Francisco Center for Psychoanalysis. Anne is a nationally recognized Certified Intuitive Eating Counselor, author of “The Body Joyful”, a Certified Self-Talk Trainer, eating disorder specialist, and life coach. She is the leader of the body joyful movement and does a lot of body image and body neutrality work. She shares her own lived experiences with disordered eating with me in this discussion and we talk a lot about diet culture. We get vulnerable in this episode and we also address why we don't talk about this subject enough. While this may in part be true, for many people complex trauma is often at the root. Our guest today, Katie Kilbourn, co-founder of a dance company helping people heal through expressive arts, courageously shares about many aspects of eating disorders that are silenced, including the links between addiction, trauma, and eating disorders. We discuss the adaptive coping strategies we develop as a result of trauma, and the double-edged sword of escapism, perfectionism, and control. She also shares about what it's like to navigate pregnancy while recovering from an eating disorder, and why it's critical for mothers to stay authentic and boundaried in caring for their own mental health. From a young age Katie realized there is a generic pressure within the dance community to maintain a specific body type and she realized that many people, herself included, engage in unhealthy behaviors to achieve a certain look. In 2015 she co founded Borne Dance Company with Kianna Moye and together they created a company dedicated to mental health and eating disorder awareness. As someone who has dealt with mental health concerns her entire life, Katie strived to create a company that would inspire body positivity, self appreciation, and the idea of recovery through art. She was a volunteer at the National Eating Disorders Association for over four years and has personal history with mental health and recovery. For many providers, the term “eating disorders” suggests a couple of somewhat familiar disorders like bulimia or anorexia. But, like with many things, the category is more complex and varied than that, not only in the types of disordered eating that can happen, but also the complex contributions from environment, social influences, physiology, crises such as the pandemic, and many other influences. So many providers, especially those of us that specialize in other things or are more generalist in approach, feel ill-prepared to assess or even ask about eating disorders in their clients, much less how to address these issues. And yet, eating disorders can be very disruptive to military readiness, ability to accomplish the mission, and function effectively in various domains of their lives. Fortunately, Dr. Deborah Glasofer shares with our hosts information about training opportunities being developed to help fill some of these skills gaps. Join us for a fascinating discussion about eating disorders, leveling the playing field, and increasing access to care for clients with ED challenges. She was also expected to recruit new coaches, which put a strain on her personal relationships. As a result, Heidi found herself in debt and struggling with her mental health. Heidi's story is a reminder that MLM companies are not always what they seem. They can be predatory and exploitative, and they can take advantage of people who are looking for a way to make money. Iskra Lawrence is a British model, activist, and founder of Saltair, a modern bodycare line. She first landed in the spotlight in the aughts when she was a model and spokesperson for the lingerie line brand Aerie, which prominently featured unretouched photos of diverse women. To this day, Iskra does not retouch the images she posts on her social media platforms and is also a brand ambassador for the National Eating Disorders Association. At the beginning of this year, Lawrence started her first business as the founder of Saltair, a modern bodycare company branded as an everyday luxury with scents such as Santal Bloom, Black Tide, Exotic Pulp, and Seascape.
