Amy Derksen, ND is a former competitive athlete and Naturopathic Physician with a full scope of practice in Washington State since 2003. She did her undergraduate studies in biochemistry and her medical post-graduate training with Dr. Dietrich Klinghardt, MD, PhD for over 2 years. She specializes in working with both Autism and chronically ill patients, including Lyme disease. Our guest this week is Dr. Anju Usman Singh, a board certified family practitioner and the medical director at True Health Medical Center in Naperville, IL, providing integrative and biomedical treatments that enhance traditional medical care. Dr. Usman is director and owner of True Health Medical Center and Pure Compounding Pharmacy in Naperville, Illinois. She also is the Chief Science Officer for True Healing Naturals. She specializes in biomedical interventions for children with ADD, Autism, PANDAS/PANS, Down Syndrome and related disorders. She has been involved in research regarding copper/zinc imbalances, metallothionein dysfunction, biofilm related infections and hyperbaric oxygen therapy. This episode is a conversation about our visit to Bryce's new autism doctor. This type of doctor is also referred to as a MAPS doctor (Medical Academy of Pediatric Special Needs), a DAN doctor (Defeat Autism Now), or a Functional Medicine doctor. Our first autism doctor experience helped us achieve our goals for Bryce. After a difference of opinion for the next steps and a bit of a plateua/regression, we felt led to make a change with a fresh perspective as we move into the future. It was a great decision! We are thrilled to see Bryce progressing again with some changes that we were advised to make with his supplements. Bryce had his first experience with a hyperbaric chamber. The autism doctor visits have definitely been "information overload" but each of them have been  well worth the investment due to the positive results we have seen with Bryce. Bryce is a funny, mechanical, HAPPY little guy who was diagnosed with autism at age two and is now eight years old. Autism, ADHD, OCD, and PANDAS/PANS are some of the challenging neuroinflammatory conditions in growing numbers of children these days. While the conventional treatment has typically been pharmaceuticals and behavioral therapies, we are truly in the midst of a paradigm shift aiming at root causes and asking 'why' and 'how' for both cause and treatment. Innovations in medicine are bringing us new evaluation tools, nuanced diagnosis capabilities, and treatment options. Dr Emily Gutierrez, a seasoned pediatric psychiatric doctorally-prepared nurse practitioner in Austin, Texas, prioritizes these kids who need new approaches. We begin the show by walking through the names, diagnostic criteria, and developmental progressions of some of these disorders like PANDAS (Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections), PANS (Pediatric Acute-Onset Neuropsychiatric Syndrome), and autism spectrum disorder, including their associated symptoms of anxiety, depression, and OCD (obsessive compulsive disorder). She delineates autism as kids with global delay who have never met milestones (consider genetic testing with chromosomal microarray and Fragile X testing) versus those with regression or later development of symptoms (consider environmental triggers). We review Dr Richard Frye's formula for regressive autism or PANS/PANDAS etiologies being 20% genetics + 80% environmental triggers, including oxidative stress (aka total toxic burden of metals, infections, mycotoxins, poor elimination), methylation deficiencies, and mitochondrial dysfunction. She notes, "Psychiatric symptoms can have an immunological basis that needs to be ruled out before sticking a kid on a medication. Maybe the med is amoxicillin instead of prozac." We get far into the details with these physiological processes. A few foods and toxins that negatively effect cortisol levels are:  sugars high fructose corn syrup red and yellow food dyes meats injected with hormones environmental toxins that disrupt hormone balance, nitrates pesticides Monosodium glutamate (MSG) imitation sweeteners such as aspartame When testing cortisol levels be sure to test with urine, or my favorite, saliva. The test must be done at least 4 times throughout the day, as cortisone levels change. If only one sample is taken then you do not have a clear reading. It is commonly low in the morning so you may notice it being difficult to wake. It can be high in the evening so it makes it hard to go to sleep. A few tips to help your cortisol levels throughout the day: In the morning solid licorice root extract can be helpful to give some energy. The adaptogenic herbs, ashwaganda and holy basil can be helpful to help calm in the evening before bed.  COQ10 can help increase energy  Ginseng can help give some energy in the afternoon but helps calm too. Deep breaths and exercising can be very helpful to teach your child to self regulate with also. Low dose immunotherapy (LDI) has become increasingly popular in the area of autism because it is known to reduce the inflammatory responses in the body and restore balance to the immune system. It works with the infections in the body. LDI has become recognized for  its ability to provide relief from symptoms and improving the health of people with multiple areas of disease and chronic illness such as Lyme disease, chronic fatigue syndrome, fibromyalgia, Hashimoto’s, multiple sclerosis, Crohn’s disease, ulcerative colitis, and more.Originally used to offer relief from allergies, it can also be referred to as low dose allergen therapy or LDA. LDI is often used along with LDA.  Autoimmune issues can be created by the harmful toxins released by bacteria in the G.I. tract. Children with autism are well known to have an ill gut. There is a direct gut to brain connection, so if the gut has inflammation, so does the brain. My guest this week is Tara Hunkin, Nutritional Therapy Practitioner, Certified GAPS Practitioner, Restorative Wellness Practitioner, mother and founder of My Child Will Thrive and host of the ‘Autism, ADHD and SPD Summit'.
